Assessment of injury risks associated with wearing the enhanced combat helmet and night vision goggle - driver: frontal vehicle collision study

The requirement to operate vehicles in low light and/or night environments whilst wearing night vision goggle (NVG) systems has become increasingly common during military operations. There is very limited research investigating injury risks associated with these systems during ground vehicle collisions. This study examined the injury risks associated with wearing the Australian Defence Force - Enhanced Combat Helmet (ECH) and NVG system, in frontal vehicle collisions. This project consisted of two components: (1) crash tests using a sled and (2) numerical simulations of impacts. Four dynamic sled tests were conducted using a 50th percentile, male, Hybrid III dummy positioned on a rigid seat. Frontal impact tests were performed at a 40 km/h change in velocity (*v) and 20 g deceleration. The test configurations were as follows: (a) Base; (no helmet or additional equipment); (b) ECH; and, (c) ECH and NVG. Condition (c) was carried out twice, to determine repeatability. The sled test protocols were reconstructed precisely with the numerical simulation package MADYMO and the simulations were shown to correlate well with the experimental results. Using this validated model, four parametric studies were undertaken to assess the influence of counterweights, seat cushion, seatbelt pre-tensioner, and the vehicle's *v and acceleration on injury risks. The study found that neck loads were within acceptable limits, with the exception of the neck extension moment, which was exceeded for all NVG conditions. Based on the parametric studies, no major improvements were observed in the neck extension moments with the use of counterweights or a seat cushion. In contrast the use of a seatbelt pre-tensioner was observed to decrease greatly this neck injury risk in certain scenarios. The study also identified that a *v of 15 km/h and peak acceleration of up to 14 g were required to keep the neck extension moment below the prescribed injury criteria. However, the high neck extension moment values may have been partially attributable to the stiff Hybrid III neck. This study identified a possible injury mechanism for soldiers using the ECH and NVG system during specific impact scenarios. The method applied in this project was designed to be repeatable.

Flight deck engineering : impact of flight deck crew alerting and information systems on English as a second language flight crewmembers performance in airline flight operations

There are many pieces of flight deck research on general use of written English language technical information and problem solving using technical documentation. Contributory causes of aircraft accidents have been due to misunderstandings of crew alerts and procedural divergence by English as-a-second language flight crewmembers (ESL). Research was conducted to understand impact of written English language technical information on ESL flight crewmembers’ performance. Two types of systems were evaluated, technical documentation and crew alerting systems that contain technical information, with respect to their impact on ESL flight crewmember performance. Preliminary analysis results indicated written English language technical information can be confusing, difficult to read and interpret, and leads to misunderstandings by ESL flight crewmembers during aircraft nonnormal conditions. English as-a-second language flight crewmembers indicated they often experience problems executing written English language technical procedures after outset of crew alerts. Conversely, experimental trials revealed ESL flight crewmembers did not experience many cognitive performance issues with use of crew alerting systems and technical information designed with an English language emphasis. English as-a second language flight crewmembers’ English language proficiency, background knowledge, and use of use of metacognitive strategies to read and comprehend written English language on crew alerting and information systems, indicated they utilized written English technical information with ease. Particularly, ESL flight crewmembers’ workload was low, they had fast response times to system faults, and they experienced minimal procedural deviations. On the contrary, when ESL flight crewmembers utilized written English language technical procedures translated into their native language during non-normal conditions, they experienced several cognitive performance challenges. English as-a second language flight crewmembers’ background knowledge of written English language technical information translated into their native language, use of metacognitive strategies to read and comprehend written English language translated into their native language, indicated they experienced difficulties with reading and comprehending translated technical information on information systems. Particularly, ESL flight crewmembers were challenged cognitively when they responded to crew alerts through execution of decision-making processes. They indicated translation of written English language technical information into their native language was a pre-cursor to procedural deviation, long response times to system issues, as well as high workload during experimental trials. It is recommended that further research focus on design and use of written English language technical documentation by ESL flight crewmembers during non-normal conditions. It is also recommended that if deemed practical by the aviation industry, further research should focus on design, integration, and utilization of technical documentation in a language(s) other than English, and measurement of ESL flight crewmembers performance on the flight deck.

Die vorliegende Diplomarbeit enthält eine Analyse von Untersuchungsberichten der deutschen Eisenbahnunfalluntersuchung sowie den darin betrachteten Unfallereignissen. Als Grundlage der Überlegungen wird dargestellt, welche Kriterien bei der Einstufung von gefährlichen Ereignissen zur Anwendung kommen. Anhand der festgestellten Unfallursachen erfolgen eine Kategorisierung von Ursachen und eine Zuordnung aller untersuchten Ereignisse gemäß den erarbeiteten Definitionen. Die Analyse befasst sich neben einer quantitativen und qualitativen Auswertung der Ursachenkategorien mit vielen weiteren berichts- sowie ereignisbezogenen Aspekten von Eisenbahnunfällen. Dabei werden auch mögliche Ansätze zur Verbesserung der Betriebssicherheit genannt. Anhand eines Beispiels werden abschließend die Zuständigkeiten in der Umsetzung von Sicherheitsempfehlungen erarbeitet. / This thesis analyses reports on accidents by the German railway accident investigation body as well as the investigated accidents themselves. Criteria for the assessment of railway incidents are presented. Corresponding to the identified causes, a categorisation of the accidents is made and all investigated events are classified according to definitions developed in advance. In addition to evaluating categories of accident causes both by quantity and by quality, this thesis addresses additional reporting-related and event-related aspects of railway accidents. Possible approaches to improving railway safety are proposed. Finally, following a case model, different jurisdictions for implementing safety recommendations are elaborated on.

Die vorliegende Studienarbeit befasst sich mit Berichten und Statistiken zu Daten der Eisenbahnsicherheit sowie Untersuchungsberichten von Eisenbahnunfällen. In diesem Zusammenhang werden sowohl deutsche Behörden und Institutionen als auch EU-weite Entwicklungen beschrieben. Es folgt eine Erläuterung und Abgrenzung zweier möglicher Kategorisierungen von gefährlichen Ereignissen im Bahnbetrieb. Daten ermittelter Quellen werden im Anschluss statistisch analysiert und Unterschiede dieser herausgearbeitet. Weiterhin wird die Durchführung einer ereignisbezogenen Unfalluntersuchung beschrieben sowie auf veröffentlichte Untersuchungsberichte eingegangen. Nach Bewertung der Berichte werden abschließend Vorschläge zur Verbesserung von Unfallanalysen und Berichten der Eisenbahnsicherheit unterbreitet. / This paper deals with reports and statistics on railway safety data and with investigation reports on railway accidents. Relevant German authorities and institutions as well as EU-wide developments are described. This is followed by an explanation of and dissociation between two possible categorisations of dangerous events in railway operations. Data from selected sources are then analysed statistically and differences within these are ascertained. Furthermore, the implementation of individual accident investigations is described and published investigation reports are outlined. After evaluating these reports, improvements for accident analyses and railway safety reports are suggested.

Influence Map Methodology For Evaluating Systemic Safety Issues

Raising the bar in safety performance is a critical challenge for many organizations. Contributing factor taxonomies organize information on why accidents occur. Therefore, they are essential elements of accident investigations and safety reporting systems. Organizations must balance efforts to identify causes of specific accidents with efforts to evaluate systemic safety issues in order to become more proactive about improving safety. This research successfully addressed two problems: (1) limited methods and metrics exist to support the design of effective taxonomies, and (2) influence relationships between contributing factors are not explicitly modeled within a taxonomy. The primary result of the taxonomic relationship modeling efforts was an innovative "dual role" contributing factor taxonomy with significant improvements in comprehensiveness and diagnosticity over existing taxonomies. The influence map methodology was the result of a unique graphical and analytical combination of the dual role taxonomy and influence relationship models. Influence maps were developed for several safety incidents at Kennedy Space Center. An independent assessment was conducted by a team of experts using the new dual role taxonomy and influence chain methodology to evaluate the accuracy and completeness of contributing factors identified during the formal incident investigations. One hundred and sixteen contributing factors were identified using the influence map methodology. Only 16% of these contributing factors were accurately identified with traditional tools, and over half of the 116 contributing factors were completely unaddressed by the findings and recommendations of the formal incident reports. The new methodology is being applied to improve spaceport operations and enhance designs of future NASA launch systems.

Uppsatsen är därför ett led i myndighetens utveckling av stödet för datainsamlingen vid kommunala olycksundersökningar i syfte att förbättra förutsättningarna för goda lärdomar vid olyckor.</p><p>Syfte: Syftet med uppsatsen är tvådelat. Att först analysera vilka erfarenheter som samlas in av de kommunala olycksutredarna och därefter verifiera resultaten utifrån tidigare forskningsresultat.</p><p>Metod och material: Med metoden kvalitativ innehållsanalys har ett urval av kommunala olycksundersökningar analyserats (n=21), där insamlade erfarenheter har kategoriserats i ett antal gemensamma teman. General Analysis is carried out within all industrial sectors, Mining sector, and Coal Mining sector. Detailed analysis is carried out within Western Lignite Company (GLi). Occupational accidents have vital importance for the mines from legal, human and economic aspects. The goal of every mine should be to minimize occupational accidents. The top management of every mine should prove their commitments to the occupational health and safety policy to carry out this goal by means of preparing and implementing an accident preventing program.
