Isolation and identification of a novel anti-diabetic compound from Euclea undulata thunb

Four plant species traditionally used for the treatment of diabetes by South African traditional healers and herbalists were investigated for hypoglycaemic activity. Species included Schkuhria pinnata (Lam.) Cabrera, Pteronia divaricata (P.J. Bergius) Less Elaeodendron transvaalense (Burtt Davy) R.H. Archer and Euclea undulata Thunb var. myrtina (Burch.) Hiern. Acetone and ethanol plant extracts were prepared and tested in vitro, for glucose utilization, at concentrations of 12.5 µg/ml on three cell lines namely; Murine C2C12 myocytes, Chang liver cells and 3T3-L1 preadipocytes. Metformin, at a concentration of 1 µM (0.166 µg/ml) was used as positive control for hepatic cells and insulin at a concentration 1 µM (5.7 µg/ml) for myocytes and preadipocytes. Toxicity tests were done for all extracts on preadipocytes and hepatic cells, but not on myocytes as these cells were exposed to the extract for only a short period (1 hour) during the hypoglycaemic bioassay. Preadipocytes and hepatic cells were exposed to the plant extracts for 48 hours. The four plant extracts were further investigated for hypoglycaemic activity by evaluating inhibiting effects on carbohydrate-hydrolising enzymes alpha-glycosidase and alpha-amylase. In vitro hypoglycaemic analysis revealed that acetone and ethanol plant extracts of S. pinnata, E. undulata and E. transvaalense, displayed hypoglycaemic activity in one or more of the various cell lines, whereas, P. divaricata showed no hypoglycaemic activity. The best results were obtained with the ethanol and acetone extracts of S. pinnata in preadipocytes with a glucose uptake of 148.2% and 79.6% respectively, above control (100%). However, about 50% preadipocytes survived on exposure to the extracts of S. pinnata at 12.5 µg/ml indicating significant cytotoxicity. Glucose uptake of 63.3% was observed by the ethanol extract of S. pinnata on hepatic cells. E. transvaalense showed hypoglycaemic activity on preadipocytes exhibiting glucose uptake of 38.6% above control 100%. Glucose uptake of 62.2 % were obtained by the E. undulata extract in C2C12 myocytes, with 100% cell viability. E. undulata scored a +3 and was chosen for further analysis. Antidiabetic activity and toxicity of the plant extracts were taken into consideration when scoring was applied. Alpha-glucosidase and alpha-amylase results indicated that P. divaricata extract inhibited alpha-glucosidase (IC50 31.22 µg/ml) whereas E. undulata (IC50 2.80 µg/ml) and E. transvaalense (IC50 1.12 µg/ml) extracts inhibited alpha-amylase. Results obtained indicated that all four plant extracts tested have the ability to lower blood glucose levels to some extent and in different manners and therefore corroborate the ethnomedicinal use of these four species in the treatment of diabetes. Phytochemical studies of a crude acetone extract of the root bark of E. undulata var. myrtina produced a new á-amyrin-3O-β-(5-hydroxy) ferulic acid compound (1), and three known compounds; betulin (2), lupeol (3) and epicatechin (4). The chemical structures were determined by spectroscopic means. In vitro assays on C2C12 myocytes revealed that (2) (21.4%) and (4) (166.3%) were active in lowering blood glucose levels whereas (1) (IC50 4.79 µg/ml) and to a lesser extent (4) (IC50 5.86 µg/ml) and (3) (IC50 6.27 µg/ml) inhibited alpha-glucosidase. These results indicated that the crude, E. undulata acetone extract does contain compounds that display hypoglycaemic activity. The Contribution of African Traditional Medicine For a Model of Relational Autonomy in Informed Consent

The Western liberal approach to informed consent defines autonomy as an independent choice or decision made by an individual as the final authority in medical decision-making. This approach is so dominant that one can fail to see the merits of other traditional and cultural perspectives. In this dissertation, another approach to informed consent is considered -that of communal culture of Africa, a process used in African traditional medicine (ATM) wherein patients make medical decisions and give consent in consultation with the members of their community and the ATM doctor. Often, but not necessarily, the final consent rests on the consensus reached in consultation with the group rather than on that by the individual patient alone. <br>This dissertation argues that the ATM form of consent constitutes legitimate informed consent based on the concept of relational autonomy, commonly called relational autonomy in informed consent (RAIC) in this dissertation. To interpret and enlighten the significance of the ATM approach to RAIC, the analysis in this dissertation examines the ethics of care movement in Western bioethics which also advocates a relational approach to informed consent. This movement presents a concept of persons as relational beings who are socially embedded, thereby interpreting patient autonomy in their relationships with others. This movement provides the hermeneutic to enlighten the significance of the ATM approach as a legitimate model of RAIC. <br>By comparison of the ethics of care approach with that of ATM to RAIC, this dissertation further provides a relational approach to informed consent in order to inform the restrictive emphasis on individual autonomy practiced in Western bioethics, all with a view towards fostering current discourse on global bioethics. The dissertation also provides an applied example of the contribution of ATM's RAIC approach to ethics committees in Africa, focusing in particular, on the decision-making process for patient informed consent. <br>This dissertation is distinct insofar as it focuses on informed consent in ATM, employs ethics of care as a hermeneutic to interpret ATM, advocates integrated model of healthcare ethics committees in ATM. Paving a way to effectively regulate African traditional medicines in South Africa

>Magister Scientiae - MSc / BACKGROUND: Following the historical suppression of traditional medicines in South Africa, the government published their National Health Plan in 1994 which made provision for traditional healing as an integral and recognised part of the health care system, inter alia, to establish a regulatory body for traditional medicines. Traditional medicines were included in the National Drug Policy for South Africa in 1996. A policy on traditional medicine was only drawn up in 2008 and is currently still in draft form. Some progress was made towards regulating traditional health practitioners; but very little towards regulating traditional medicines after its adoption into the National Health Plan over a decade ago. The aim of the study was to investigate how traditional medicines in South Africa can be effectively regulated with specific focus on the current status of traditional medicine regulation in South Africa; to highlight the challenges which have impeded progress towards regulating traditional medicines; and a review of regulatory strategies for traditional medicines in Ghana, India and the People’s Republic of China in an attempt to identify viable solutions to pave an effective way for regulating traditional medicines in South Africa.METHODS: The research design was exploratory and qualitative in nature following a deductive thematic analysis of data collected using a traditional literature review process. RESULTS: The delay in regulating traditional medicines in South Africa is due to a number of challenges where the most prominent is due to a lack in national priority to categorise African traditional medicine and to include it into the country’s national health system. Traditional medicines according to the World Health Organisation can be classified for inclusion into national health systems either as integrative or inclusive. Each of these inclusion strategies were presented by the chosen jurisdictions studied. An integrative health care system such as that used by China was found to be resource intensive in nature. An inclusive health care system as used in Ghana and India did not require traditional medicines as an integral part of the health care system and there were no expectations for the same requirements for regulating traditional medicines and allopathic medicines. What became apparent from the study was that irrespective of the inclusion strategy followed, there are still a number of challenges that obstruct the existence of an effective regulatory framework for traditional medicines with an overwhelming common factor of the instrumental role government play.

Identifying and finding the impact of Grade 8 learners' alternative conceptions of lightning

The study wais prompted by the fact that most learners have an African conception of lightning which in most cases is not in agreement with the conventional definition of lightning. The focus of this study was to identify the learners’ conceptions of lightning. Secondly it attempted to elicit, describe and assess the learners’ process of learning Western conception of lightning. And finally, it developed a model of teaching that can be used to help African learners accommodate the two conceptions without contradiction or hindrance. This research utilized qualitative research design to a large extent. A total of 33 participants responded to a questionnaire and 16 were interviewed. In response to question 1, learner’s mentioned that they believe that lightning is a result of witchcraft, it demonstrates the anger of ancestors or it is used by god to demonstrate his existence. These findings suggest that the learner’s African conception is a hindrance to the learning of science because learners’ cultural identity is often very different from the culture of conventional science. Learners experience a type of cultural clash whenever they attempt to learn science meaningfully. A substantial number of learners indicated that the African conception prohibits them from learning the western conception. Further findings suggest that learners do not challenge the validity of the conventional explanation of the origins of lightning. But they believe it is different to the African conception. In summary this research revealed that there is a strong need for educators to be sensitive and knowledgeable about the African learners’ way of thinking. It suggests a teaching model that is aimed at helping educators to deal with misconceptions instead of attempting to change a belief system.

Assessing the level of participation in the regulation of African traditional medicines in South Africa: focus on African traditional health practitioners in the villages of Thembisile Hani local municipality in Mpumalanga

This mini-thesis provides empirical evidence on the level of public participation and its correlation with trust, reciprocity, class and power amongst African traditional health practitioners (ATHPs) in the regulation of African traditional medicines (ATMs) in South Africa. Data on public participation of ATHPs was collected and analysed, using a social capital theoretical model created from an adaptation of Putnam’s (2000) concepts of trust and reciprocity as central to social capital theory. The analysis further employed Bourdieu’s (1968) thinking to link power and class to the concept of social capital and borrowed from feminist and historical materialism theories. The thesis sought to test how the analysis of power and class, once politicised, could be used to convert Putnam’s notion of trust and reciprocity into "critical trust” and “critical reciprocity”, based on hyper-reflexive engagement of social agents with their own reality within social networks. ATHPs were requested to rank themselves from a scale of high to low to determine their level of involvement. Public participation (the dependant variable) was tested against acts of reciprocity, trust, power and income or class (the independent variables) to ascertain correlation. Data was analysed using STATA, a computer-based programme for analysing quantitative data. The findings of the study showed that income and power play a significant role in determining public participation. The level of one’s income and power determines whether one is active or passive in participation. The high levels of trust and reciprocity demonstrated by ATHPs towards government had no particular significance in facilitating active participation instead trust, in particular, played a negative role, in that it demobilised ATHPs from active participation since they trusted that government will protect their best interest in the regulation process. The argument in this thesis is that trust and reciprocity has to be converted into ‘critical trust’ and ‘critical reciprocity’ through active questioning and analysis of power and class dynamics for it to activate and invoke conscious active participation. A follow-up study is necessary to test the possibility and success of such a conversion for public participation. / Magister Artium - MA

Interpreting the Culture of Ubuntu: The Contribution of a Representative Indigenous African Ethics to Global Bioethics

Ubuntu is a worldview and a way of life shared by most Africans south of Sahara. Basically Ubuntu underlines the often unrecognized role of relatedness and dependence of human individuality to other humans and the cosmos. The importance of relatedness to humanity is summarized by the two maxims of Ubuntu. The first is: a human being is human because of other human beings. The second maxim is an elaboration of the first. It goes; a human being is human because of the otherness of other human beings. John Mbiti combines those two maxims into, "I am because we are, and we are because I am." Ubuntu worldview can provide insights about relationships with communities and the world that contribute to the meaning of Global Bioethics. <br>Ubuntu can be described as involving several distinct yet related components that can be explored in relation to major strands of discourse in contemporary Bioethics. The first component of Ubuntu deals with the tension between individual and universal rights. The second component of Ubuntu deals with concerns about the cosmic and global context of life. The third component of Ubuntu deals with the role of solidarity that unites individuals and communities. Ubuntu has a lot in common with current discourse in bioethics. It can facilitate global bioethics. It can inspire the on-going dialogue about human dignity, human rights and the ethics that surround it. It can inspire and be inspired by global environmental concerns that threaten the biosphere and human life. Ubuntu can critique the formal bioethical principles of autonomy, justice, beneficence and non-maleficence. Above all, Ubuntu can create a basis for dialogue and mutually enlightening discourse between global bioethics and indigenous cultures. Health beliefs and HIV risk among employees : a comparative study.

The aim of this study was to investigate and compare health beliefs among employees. The study looked at the African Traditional Model and the Medical Model in relation to HIV risk among employees in the Eastern Cape, South Africa. The study sample was made up of 96 participants, all African, and predominately female. The study established employees` health beliefs by asking them to indicate their health beliefs on the Biographical Questionnaire provided in the study. HIV risk among employees was assessed through the use of HIV Risk-Taking Behaviour Scale (HRBS), Attitude to Condom Use, a sub-scale of the Sexual Risks Scale (SRSA) and Sexual Risk Cognitions Questionnaire (SRCQ). A comparison of HIV risk between the two groups of employees was achieved by performing ANOVA. In addition, the study conducted secondary analyses using correlations to establish relationships between variables. The results obtained in this study showed a statistically significant difference in HIV risk between employees who believed in the African Traditional Model and those who believed in the Medical Model in HIV prevention. Secondary correlational analyses between variables showed a significant, negative relationship between participants` attitude toward condom use and their sexual risk cognitions. The results indicated that employees who had a positive attitude towards condom use reported low sexual risk cognitions towards unsafe behaviour. In contrast, employees who had a negative attitude towards condom use showed high sexual cognitions towards unsafe sexual behaviours. Directions for future studies could focus on education and training strategies to reduce negative health beliefs among employees in the fight against HIV and AIDS in South Africa.
