VALIDATION OF THE MILLER FORENSIC ASSESSMENT OF SYMPTOMS TEST (M-FAST) IN A CIVIL FORENSIC POPULATION

The Miller Forensic Assessment of Symptoms Test (M-FAST) is a relatively new measure shown to be a valid and effective tool for screening psychiatric malingering in criminal forensic and psychiatric inpatient settings. The present study attempts to cross-validate the M-FAST in civil forensic and neurologic samples. Three-hundred-eight civil forensic patients referred by their attorneys for neuropsychological testing were studied. Assessment batteries administered included tests of both psychiatric and neurocognitive feigning. Based on these gold standards, 4 sets of contrasts were formed in order to examine how the M-FAST performs in identifying psychiatric malingering, neurocognitive malingering, any malingering (including either or both types of malingering), as well as any malingering among a neurologic subset of this sample. At the level of group discrimination, the M-FAST Total score performed well in all contrasts. However, at the level of individual classification rates, although the M-FAST Total score was well supported for identifying psychiatric feigning, when neurocognitive malingering was present, performance dropped considerably. Thus, using the M-FAST recommended cutting score of 6, the M-FAST was able to successfully identify psychiatric malingering; however, the M-FAST is not an appropriate measure to use for identifying neurocognitive malingering within this sample.

Development of a computerized assessment tool for hand-arm function after stroke- test-retest reliability and convergent validity

Many performance-based assessment tools are available to measure upper extremity function. Most define hand function by the time taken to complete a task. A new computer game-based hand-arm function evaluation (GHA) tool has been developed to quantify fine and gross object manipulations skills. Objective of this study was to evaluate the test-retest reliability and convergent validity of the GHA assessment tool. Thirty stroke clients with the mean age of 68.41 years were recruited. Test-retest reliability was assessed by Intraclass correlation coefficient (ICCs) and t-test. Convergent validity between GHA and the Wolf Motor Function Test (WMFT) was determined using the Pearson’s correlation coefficient. Most of the GHA outcome measures had moderate to high ICC (0.5-0.9). With scant exceptions, low correlations were observed between GHA measures and WMFT score. The ICC values reflected the complexity of the tasks, more complex task showed lower ICCs values. / October 2016

Use of the King-Devick test as a concussion assessment tool in the pediatric emergency department: a pilot study

In the United States, an annual estimate of 1.36 million traumatic brain injuries present to the emergency department (ED), of which approximately 75% are concussions. Proper and timely treatment of concussion is especially important in pediatrics as children and adolescents under the age of 19 are at a higher risk for sustaining more severe and longer-lasting consequences. However, due to the wide range of symptoms at presentation, or to the potential lack of obvious symptoms, concussion can be especially difficult to diagnose in the ED setting. Neurocognitive tests provide a valuable supplement to the clinical diagnosis of concussion by objectively identifying aberrant brain activity. However, many of these tests are often too lengthy and impractical for use in the ED setting. The Immediate Post-Concussion Assessment and Cognitive Test (ImPACT) is a 20-minute computer test that is considered to be one of the gold-standard neurocognitive tests used to diagnose concussion and track recovery. The King-Devick test (KD) is a 1-2 minute test that uses saccadic eye movements to detect suboptimal brain impairment associated with concussion. To date, there have not been any studies that analyzed the relative usability of the KD and the ImPACT in the pediatric ED (PED). The present prospective pilot study investigates the use of the KD as a neurocognitive tool for concussion assessment in the PED and at a post-ED visit, relative to the ImPACT, the gold standard tool for concussion diagnosis. We hypothesize that the change in performance in the KD will correlate with the change in the ImPACT results. To date, 20 subjects between the ages of 11-18 years old presenting to the PED within 72 hours of sustaining a head injury have completed the study. The mean age of our study population was 13.6 years. The average change in test scores between PED and follow-up were: 7.2 seconds in the KD, 0.03 points in the ImPACT reaction time, 1.8 points in verbal memory, 8.3 points in visual memory, 0.8 points in visual motor speed, and 14.9 points in post-concussion symptom scale. Analysis of the correlation of the change in the KD scores to the change in the ImPACT measures revealed that the change in the KD was significantly correlated with the change in the ImPACT reaction time (p < 0.01), and with the change in the ImPACT verbal memory (p < 0.05) in the subjects that presented with LOC, 80% of whom were male. In conclusion, our findings report that the correlation between the results of the KD and the ImPACT is more pronounced in patients presenting with more severe head trauma, such as those leading to LOC. The usability of the KD as a reliable concussion assessment tool in the PED would require further investigation with a larger sample of participants. / 2017-04-30T00:00:00Z

Effects of Five Substances with Different Modes of Action on Cathepsin H, C and L Activities in Zebrafish Embryos

Cathepsins have been proposed as biomarkers of chemical exposure in the zebrafish embryo model but it is unclear whether they can also be used to detect sublethal stress. The present study evaluates three cathepsin types as candidate biomarkers in zebrafish embryos. In addition to other functions, cathepsins are also involved in yolk lysosomal processes for the internal nutrition of embryos of oviparous animals until external feeding starts. The baseline enzyme activity of cathepsin types H, C and L during the embryonic development of zebrafish in the first 96 h post fertilisation was studied. Secondly, the effect of leupeptin, a known cathepsin inhibitor, and four embryotoxic xenobiotic compounds with different modes of action (phenanthrene—baseline toxicity; rotenone—an inhibitor of electron transport chain in mitochondria; DNOC (Dinitro-ortho-cresol)—an inhibitor of ATP synthesis; and tebuconazole—a sterol biosynthesis inhibitor) on in vivo cathepsin H, C and L total activities have been tested. The positive control leupeptin showed effects on cathepsin L at a 20-fold lower concentration compared to the respective LC50 (0.4 mM) of the zebrafish embryo assay (FET). The observed effects on the enzyme activity of the four other xenobiotics were not or just slightly more sensitive (factor of 1.5 to 3), but the differences did not reach statistical significance. Results of this study indicate that the analysed cathepsins are not susceptible to toxins other than the known peptide-like inhibitors. However, specific cathepsin inhibitors might be identified using the zebrafish embryo.

In this perspective, this study aims to contribute by analyzing reviews of multiple choice and open questions, submitted on paper and computer. Four tests in four different models (multiple choices on your computer, multiple choices on paper, open questions on the computer and open questions in the paper) were performed. The focus of the research was analysis of the notes in each model, the duration of the test and student satisfaction at the end of the tests. The results were presented and discussed considering a context of controlled assessment, interpreting the conditions under which the models can be equivalent, advantages and disadvantages. A test of multiple-choice paper presented better results in terms od test notes. / A avaliação evolui constantemente em busca de melhores práticas educacionais mediante o desenvolvimento da educação. Nesta perspectiva, este estudo propõese contribuir através da análise de avaliações de múltipla escolha e questões abertas, apresentadas em papel e computador. Foram realizados quatro testes em quatro modelos diferentes (múltipla escolha no computador, múltipla escolha no papel, questões abertas no computador e questões abertas no papel). O foco da investigação foram análises das notas em cada modelo, do tempo de duração do teste e da satisfação do aluno ao término dos testes. Os resultados foram apresentados e discutidos considerando o contexto do teste controlado, interpretando as condições sob os quais os modelos podem apresentar equivalência, vantagens ou desvantagens. O teste de múltipla escolha em papel apresentou melhores resultados em termos de nota no teste.

This master thesis deals with static load test and static assessment of selected segments of reinforced concrete ceiling construction of the Hennebique system at the Faculty of Arts at Masaryk University in Brno. This thesis contains: the execution of the static load test, the data evaluation of the static load test and the data evaluation of the complementary laboratory testing, geometric orientation of bearing elements, the checking of the ceiling placement in the peripheral position, and the diagnosis of the reinforcing of the construction. This thesis also includes the static assessment according to various standard norm rules and the numerical analysis in ATENA software program.

En el presente trabajo de suficiencia profesional se ha analizado dos expedientes, en cuanto al expediente de derecho privado versa sobre otorgamiento de escritura pública referido a un contrato de compraventa en el cual la parte demandada alega que no se ha cumplido con el pago total del precio y que a la fecha dicho contrato se ha resuelto mediante resolución por cláusula resolutoria expresa. Así, la controversia es si el juez en el proceso sumarísimo puede hacer un control de eficacia del contrato materia de solicitud de otorgamiento de escritura pública.
