The impact of pharmacist provision of medication therapy management (MTM) on medication and health-related problems, medication knowledge, and medication adherence among Medicare beneficiaries

This study used the Andersen Model for Health Services Utilization to examine a pharmacist-provided telephone MTM program among Medicare Part D beneficiaries. Predisposing (age, gender, race) and need factors (number of medications, number of chronic diseases, medication regimen complexity) were assessed. The health behavior, MTM utilization, distinguished the intervention and control groups. The health outcomes were change in number of medication-related problems, change in medication adherence [using the medication possession ratio (MPR)], and change in total drug costs. Medication knowledge, medication adherence (using the Morisky Scale), and patient satisfaction were also measured in the intervention group. The intervention and control groups were not significantly different in age (71.2 ± 7.5 vs. 73.9 ± 8.0 years), number of medications (13.0 ± 3.2 vs. 13.2 ± 3.4), number of chronic diseases (6.5 ± 2.3 vs. 7.0 ± 2.1), and medication regimen complexity [21.5 (range 8 – 43) vs. 22.8 (range 9 – 42.5)], respectively. For the subset of problems that was evaluated in the intervention and control groups, 4.8 (± 2.7) and 9.2 (± 2.9) problems were identified at baseline and 2.7 (± 2.3) and 8.6 (± 2.9) problems remained at the 3-month follow-up, respectively. Cost-related and preventative care needs and drug-drug interactions were the three most common problems identified. Multivariate regression analysis revealed that the intervention group had significantly more problems resolved (p < 0.0001) when compared to the control group, while controlling for predisposing and need factors. Significantly fewer problems were resolved (p = 0.01) as number of diseases increased and significantly more problems were resolved (p = 0.01) as medication regimen complexity increased. There were no significant predictors of change in MPR or total drug costs from baseline to the 3-month follow-up. Medication knowledge and medication adherence measured by the Morisky scale did not change significantly from baseline to the 2-week follow-up. However, patients were very satisfied with the service. A pharmacist-provided telephone MTM program was an effective method for identifying and resolving medication and health-related problems. A longer follow-up period may be necessary to detect the impact of pharmacist provision of MTM on adherence, total drug costs, and knowledge. / text

Contribution of land reform programmes to beneficiaries on Rocky Drift Farm, Muden, KwaZulu-Natal.

This study seeks to examine the contribution of land reform programmes to beneficiaries and focuses on post settlement development for beneficiaries once they have resettled on the farm. The contribution of land reform programmes is examined in this study through the investigation of the Rocky Drift Farm, a rural area in Muden, KwaZulu-Natal Province, This case study has been chosen because it became one of the first Land Reform Pilot Projects transferred in 1997 to a Trust, representing a large number of owners (232 in total), under the government assisted land redistribution programme. It is now 12 years since the residents have returned to Rocky Drift Farm and this period is regarded as sufficiently long enough to enable the critical examination of development progress relating to land reform since 1997. This research has hypothesized that "whilst the Land Reform Programme does generate some benefits for beneficiaries, it has proved to be an inadequate mechanism for improving the quality of life for beneficiaries, and thus needs to be supplemented by a number of other key support programmes in order to fully contribute to the lives of the poor". While this investigation has proven beyond reasonable doubt that the lives of of the beneficiaries have not fully improved as a result of Land Reform Programmes at Rocky Drift, improvement in infrastructure and service provision such as upgrading of 3 roads. construction and upgrading of 5 schools, building of 1 creche and the supply of portable water, has been noticed. Furthermore, benefits such as a sense of belonging, identity, trust, hope and reconciliation, resulting from the implementation of the Rocky Drift Project have also not gone unnoticed. However, despite the benefits mentioned above, it is strongly argued by this dissertation that these benefits are inadequate to boldly conclude that land reform programmes have improved the standard of life for Rock Drift residents after 12 years of resettlement. Therefore, this study recommends, inter-alia, the following supplementary measures as boost for land reform programmes to enable the beneficiaries to fully benefit from these programmes, during post settlement; • improving communication, co-operation and co-ordination among the government departments, • maintaining the continuity of the key stakeholders, • establishment of agri-villages; • link land reform projects with lDPs: • re-establishing the Rocky Drift community structures and the • letting of unused portions of Rocky Drift Farm, to mention a few. Lastly, since the Rocky Drift Project piloted other farms, this study also recommends that a further investigation needs to be conducted. Drought and famine in Somalia : an evaluation of the effectiveness of the international community's response

This study aimed at evaluating the effectiveness of the 2011 to 2012 famine response in Somalia in saving lives and livelihoods, offering livelihood opportunities, supporting quick recovery and complying with accountability standards in the delivery of humanitarian assistance. Understanding the performance and effectiveness of the humanitarian response to emergencies such as the 2011 to 2012 famine in Somalia is important for managing similar large-scale disasters which tend to be more frequent than ever before. It is also vital for addressing the chronic food insecurity in sub-Saharan Africa. The study was conducted in the Dolow district, Gedo region, Somalia. The study was largely rooted in the qualitative paradigm even though quantitative methodologies were employed to explain certain aspects in order to adequately answer some of the research questions. The case study approach was employed to conduct this study and achieve the research objectives. Despite the need for more evidence-based and local community-driven response to droughts, the 2011 to 2012 famine response was largely relevant to meet the priority needs of affected populations. The response was too late to prevent the death of hundreds of thousands and the suffering of millions of people and the response had to struggle for quite some time to stabilise and reverse a devastating situation. Although the response was not adequate to cover the needs of all affected populations, it had a commendable impact by saving the lives of malnourished children and mothers; minimising suffering from lack of food, water and shelter; restoring livelihoods for host communities; and creating livelihood opportunities for internally displaced families. Compliance with the NGO Code of Conduct and the application of Sphere standards were fair. Strong accountability mechanisms are required to ensure effective beneficiary targeting and curb aid diversion. The beneficiaries of the response stated that they are equally vulnerable to droughts despite an increase in income and agricultural production as a result of the response. Long-term livelihood projects that address structural vulnerabilities and create multipleincome sources are essential for strengthening resilience to droughts. / Development Studies / M.A. (Development Studies)

Land reform policies to promote women’s sustainable development in South Africa

This study focuses on the role that land reform policies have played and are continuing to play in promoting women’s sustainable development in South Africa. The Land Reform Policy and its related Programmes which aim to provide equitable access to land have gathered strength after 1994 and intend to promote participation and involvement of women, youth and people with disabilities. To this effect the study has investigated the effectiveness of the Land Reform Policy and the Programmes with specific reference to women beneficiaries. The topic of the thesis refers to the Land Reform Policies as opposed to only one Land Reform Policy (The White Paper on South African Land Policy of 1997) which was introduced with the aim of integrating the then three existing policies which were later renamed programmes; Redistribution, Tenure Reform and Restitution. The Land Redistribution Programme is based on the Provision of Certain Land Rights for Settlement Act, 1993 (Act 126 of 1993) and Section 25 (b) (5) of the Constitution, 1996. The programme provides for the description of land for settlement purposes and financial assistance to people acquiring land for agricultural purposes. The Land Tenure Reform Programme is based on the Upgrading of Land Tenure Rights Act, 1993 (Act 112 of 1993) and Section 25 (b) (6) of the Constitution, 1996. This programme is aimed at availing and providing security of tenure in different ways to its beneficiaries. The Land Restitution Programme has its basis on the Restitution of Land Rights Act, 1994 (Act 22 of 1994) and Section 25 (b) (7) of the Constitution, 1996. The Restitution of Land Rights Act, 1994 (Act 22 of 1994) provides for the restitution of rights in land to those dispossessed of land in terms of racially based policies of the past. This thesis will therefore make reference to the main Land Reform Policy (WPSALP, 1997) and the Redistribution Programme and its sub-programmes. The role of government has been quite critical in the establishment and implementation of the Land Reform Policies. This is fundamental especially because land reform is a national mandate. There is proven necessity that each government sphere must play its role and support each other towards achieving this call. There is potential to achieve beyond what has been achieved to date if all spheres of government and the relevant stakeholders put enough effort into this and land reform is placed centrally in all government spheres. Formal agreements are necessary between the three spheres of government on ensuring that land reform is implemented successfully and benefits its targets beneficiaries. The study revealed that previous policies and legislations purposefully neglected and isolated women as beneficiaries of any developmental initiatives. The study proved that women are still regarded as the main provider of support to their families and yet have little say in land matters. The way in which the inferiority of women was inculcated was evident in the manner in which women were complacent with their lifestyles and ability to still take care of their families irrespective of the lack of resources. The study mainly discusses the White Paper on South African Land Policy of 1997 in isolation of the other policies even though there is strong mentioning of the Constitution, 1996, the land related legislation which the programmes are based on and the RDP document as some of the policies supporting land reform. The other beneficiary categories as mentioned are equally important but the focus herein is on women. The study applied the qualitative research method towards addressing the study focus challenges. A qualitative study is defined as an inquiry process of understanding based on distinct methodological traditions of analysis that explore a social or human problem. The ability of women to own and utilise land is the focus of the study particularly in terms of promoting sustainable development for women. The desired result out of this study is an enquiry on how has the land reform empowered women towards their sustainable development, as well as what has been the process of ensuring that women have equal access to land. The qualitative research types that have been applied are a combination of case studies, literature review and unstructured interviews. An analysis of the relationship between Public Administration and land reform is undertaken by the study. Within the analysis of the relationship between Public Administration and land reform, focus is on the origin of Public Administration and sustainable development’s perspective to Public Administration. The need to maintain a balance between economic development and environmental protection to ensure generational equity is highlighted as critical for sustained livelihoods. The use of natural resources such as land in an environmentally friendly manner will ensure that future generations can utilise land for their livelihoods as well. The enquiry has revealed that there is a lot of literature on the subject of land reform. Further on, through the unstructured interviews and cases used within the study, women have made a success of the land they received even though support to utilise land was not timeously provided by the relevant Department of Agriculture. The level of education, presence of women on farms, the ability to make decisions and knowledge of business concepts contributed to the women’s success. The study revealed that previous policies and legislations purposefully neglected and isolated women as beneficiaries of any developmental initiatives. The study has proven that women were and still are regarded as the main support for their families and yet do not have any say in what happens on land matters. The way in which the inferiority of women was inculcated was evident in the manner in which women were complacent and satisfied with their lifestyles and ability to still take care of their families irrespective of the lack of resources. The study further emphasises the important role of government in the establishment and implementation of the Land Reform Policies. There is proven necessity that each government sphere must play its role and support each other towards achieving this call. There is potential to achieve beyond what has been achieved to date if all spheres of government put enough effort into this and land reform is placed centrally in all government spheres. Cooperation between the three spheres of government and the beneficiaries of land is necessary towards ensuring that land reform is implemented successfully. The thesis proposes that women should not be regarded as recipients of land only but also as contributors towards the government’s land reform. The lack of support that has been observed in the early years of land transfers to women has only delayed success of women but it did not hamper their continuous production on land. Implementation of these Land Reform Policies has supported women towards their sustainable development and improved livelihoods. The study has also revealed that some of the international countries still face challenges towards the inclusiveness of women in land related initiatives and can exchange success cases and learning with the South African models. The systems model that has been proposed by the study is based on the contribution of all the affected stakeholders and not government alone.
