Pesticide Exposure During Pregnancy and Low Birth Weight

Background: Limited epidemiologic studies have investigated the effects of pesticide exposure during pregnancy on low birth weight in offspring in rural China.Methods: A survey of a total of 503 women was conducted in Ling county of Shandong Province of China following delivery from 1 November 2009 to 8 February 2010.Results: After adjustment for confounding and compared with no pesticide exposure, multiple logistic regression showed a non-significant increased likelihood of low birth weight for both children of mothers exposed to pesticides when not pregnant (OR = 1.80, 95% CI: 0.62, 5.22) and mothers exposed to pesticides during pregnancy (OR = 2.42, 95% CI: 0.73, 8.08); multiple linear regression showed a non-significant reduced birth weight for both children of mothers exposed to pesticides when not pregnant (β=–0.59, p=0.28) and mothers exposed to pesticides during pregnancy (β=–0.89, p=0.15).Conclusions: Exposure to pesticides during pregnancy was associated with a non-significant increase in low birth weight in this rural Chinese population. Future studies using larger sample sizes and longer follow-up periods are warranted.

Cesarean Section and the Risk of Overweight in Grade 6 Children

We examined the relationship between cesarean section (C-section) and the risk of overweight and obesity in children in grade 6 (mean age, 11.92 years; standard deviation = 0.34). Data from phase I through phase III of the National Institute of Child Health and Human Development Study of Early Child Care and Youth Development were used. Children with complete data from 1991 through 2004 were included in this study (n = 917). Multiple logistic regression analyses were used to adjust for potential confounding and to evaluate the association of C-section and childhood overweight and obesity. Compared to children delivered vaginally, children delivered by C-section had approximately twice the likelihood of being overweight (odds ratio (OR) = 1.86, 95 % confidence interval (CI) = 1.27–2.73) or obese (OR = 1.87, 95 % CI = 1.19–2.95). However, when examined according to sex, males delivered by C-section had an increased risk for being overweight (OR = 1.78, 95 % CI = 1.01–3.12) and obese (OR = 2.58, 95 % CI = 1.36–4.88), while females had an increased risk only for being overweight (OR = 1.99, 95 % CI = 1.17–3.39). Conclusion: C-section was associated with an increased risk of overweight and obesity in children in grade 6, but the relationship differed according to gender. Further longitudinal studies are warranted to examine the long-term effect of delivery mode on the risk of childhood overweight.

Tennessee Stroke Registration Report, 2015

Background: Stroke is the fifth leading cause of death in Tennessee. The Tennessee Stroke Registry (TSR) Act requires East Tennessee State University’s College of Public Health to maintain a stroke database of participating hospitals and produce an annual report. Currently, twelve hospitals submit data to the TSR. Objectives: The TSR seeks to provide stroke information to Tennessee residents, policy makers, and health-care professionals. This presentation will highlight the key findings of the 2015 TSR report and will serve as an update to the 2014 report presented at the 2015 Tennessee Public Health Association conference. Methods: Data for the TSR was collected via Quintiles, American Heart Association’s online database. Microsoft Excel and ArcMap 10.3.x were used to conduct a descriptive analysis of stroke across Tennessee, observing characteristics of both stroke overall and of individual subtypes. Results: Seventy-nine percent of strokes in Tennessee in 2015 were ischemic. Some subtypes showed significantly different proportions of males versus females. For instance, females made up 63% of subarachnoid hemorrhage patients. The average age of stroke patients in Tennessee was 67.1 years, but varied for subtypes, ranging from 58.2 to 69 years. The majority of stroke patients (75.1%) had a recorded history of hypertension. Counties with high stroke mortality rates tended to cluster in areas of few certified stroke centers. Conclusion: Stroke subtypes exhibited different characteristics than stroke overall and geographic disparities were highlighted through the descriptive mapping. Identifying and understanding these differences and disparities can help in addressing ways to improve stroke care in Tennessee.

Twin Faces! Vitamin E and Respiratory and Atopic Diseases

Purpose To estimate susceptibility to smoking among never-smoking youth globally and identify factors associated with such behavior. Methods Cross-sectional data for 168 countries were obtained from the Global Youth Tobacco Survey. Simple and multiple logistic regression analyses were conducted. Frequencies and proportions for descriptive statistics, and adjusted odds ratios with 95% confidence intervals for logistic regression models were reported. Results Approximately 12.5% of never-smoking youth worldwide were susceptible to smoking worldwide, of which 7.2% were males and 5.3% were females. Compared with youth in the Americas, those in other WHO regions were associated with decreased susceptibility to smoking. Regardless of gender, exposure to parental or peer smoking, secondhand smoke inside or outside home, and tobacco industry promotion was associated with increased smoking susceptibility. In contrast, support for smoke-free policies and school antismoking education was associated with decreased susceptibility to smoking among females. Moreover, exposure to antismoking media messages was associated with increased susceptibility to smoking among never-smoking youth. Conclusions Approximately 1 in 8 never-smoking youth worldwide was found to be susceptible to smoking. A comprehensive approach involving parental and peer education, smoke-free policies, ban on tobacco advertising and promotions, and antismoking education in schools should be developed by policy makers and public health professionals to protect never-smoking youth from being susceptible to smoking and transforming into future regular smokers.

The Effect of Cigarette Smoke on the Microbiota or the Normal Flora of the Nasal Cavity

Introduction. The adverse health effects of cigarette smoking are well established, including the direct effects of nicotine on human endothelial cells and fibroblasts. However, the direct effects of nicotine in the nasal cavity remain uninvestigated. The study aim was to ascertain the direct effect of chemical components in cigarette smoke on the microbial flora or microbiota of the nasal cavity. Methods. We enrolled 40 participants from the Benson Idahosa University’s Health clinics to conduct this study. Information on demographic characteristics including age, sex, and smoking duration was obtained at baseline. We collected nasal swabs of 20 smokers and 20 non-smokers. The wool content of the swap stick was immediately stored in mac Cathy bottles containing 9 ml of normal saline and transported to the laboratory in less than 45 minutes after collection for microbiological analysis under aseptic condition. The enumeration of the microorganisms was carried out using the streak and pours plate method on Nutrient agar, Blood agar, Chocolate agar, Mac coney agar and Potato Dextrose agar (PDA). A two-sample t-test was used to determine differences in enumeration of microorganisms and isolates between smokers and non-smokers. Results. Total bacterial counts for the smokers ranged from 177 - 307 x 102 cfu/ml and 223 - 551 x 102 cfu/ml for non-smokers with (p = 0.046 Nutrient agar, and p = 0.011 PDA). The bacteria isolates were identified as Staphylococcus aureus, Escherichia coli, Klebsiella pneumonia, Proteus vulgaris and Streptococcus pneumonia, and the fungi isolates were identified as Aspergillus niger, Rhizopus stolonifer, Tricoderma viridae, respectively. The mean of nutrient agar in smokers is significantly lower (12.10) than that in nonsmokers (19.35), and the mean of PDA in smokers is significantly higher in smokers (5.75) than in nonsmokers (2.35). Higher percentages of gram-negative rods including Klebsiella pneumoniae (20%), Escherichia coli (20%) and Proteus vulgaris (20%) were identified in the smokers, which is in contrast to higher percentage of gram-positive cocci including Staphylococcus aureus and Streptococcus pneumonia identified in non-smokers. The most commonly isolated fungus in smokers was Aspergillus niger (45%) and nonsmokers was Rhizopus stolonifer (60%). Conclusions. Study findings demonstrated increased bacterial count and isolates in nasal cavity of smokers than non-smokers. Future studies should be warranted to understand the mechanistic role of nicotine in influencing microbiota of the nasal cavity.

Calcium Intake and Hypertension among Obese Adults in United States: Associations and Implications Explored

The relationship between calcium intake and hypertension is receiving increased research attention. The prevalence of hypertension is high among the obese populations. Calcium is a mineral that influences blood pressure. The aim of the study was to examine the association between calcium intake and hypertension in a large nationally representative sample of obese American adults. A total of 14 408 obese adults aged 20 years or older were obtained from the 1999–2010 National Health and Nutrition Examination Survey. Analysis of variance and linear regression models were used to examine relationships between calcium intake and systolic blood pressure (SBP) as well as diastolic blood pressure (DBP). Multiple logistic regression models were used to examine the association between calcium intake and hypertension after adjusting for potential confounders and interactions, including: age, race, education level, alcohol use, smoking, diabetes status, sodium intake and potassium intake. Calcium intake was significantly lower for the hypertensive group compared with the normotensive group (P < 0.0001), especially among those obese female young adults aged 20–44 years and among non-diabetic obese adults. Based on ordinary linear regression analysis, a significant inverse relationship was detected, SBP and DBP decreased if calcium intake increased (SBP: regression coefficient estimate=−0.015, P < 0.0001; DBP: regression coefficient estimate=−0.028, P < 0.0001). Multiple logistic regression showed that calcium intake was negatively associated with the probability of hypertension (odds ratio (OR)=0.81, 95% confidence interval (CI): 0.74–0.87, P < 0.0001). In stratified analysis, calcium intake in youngest adults (age 20–44 years) had the lowest likelihood of hypertension (OR=0.77, 95% CI: 0.64–0.93, P < 0.0001), the inverse relationship between calcium intake and probability of hypertension was stronger among females (OR: 0.68, 95% CI: 0.55–0.84, P < 0.0001), when compared with the whole sample including all of 14 408 obese adults. The protective effect of calcium intake and hypertension was found significantly in obese non-diabetic adults (OR: OR=0.77, 95% CI: 0.67–0.89, P < 0.0001) not in obese diabetic adults. SBP, DBP and calcium intake were log transformed for both ordinary linear regression analysis and logistic regression analysis. Our study findings underscore the need to explore the physiological mechanism between calcium intake and hypertension. In this study, increased calcium intake was associated with the lowest risk of hypertension. Future studies utilizing longitudinal research designs are needed to quantify therapeutic levels of calcium for control of hypertension among obese adults. Increasing calcium intake among American adults may offer promise as a cost-effective strategy to improve hypertension among obese adults; however, further scientific exploration is warranted.
