Biomarker of Long-Chain N-3 Fatty Acid Intake and Breast Cancer: Accumulative Evidence From an Updated Meta-Analysis of Epidemiological StudiesYang, Bo, Ren, Xiao L., Wang, Zhi Y., Wang, Liang, Zhao, Feng, Guo, Xiao J., Li, Duo 28 October 2019 (has links)
Objective: We aimed to summarize the up-to-date epidemiology evidence on biomarkers of long-chain (LC) n-3 fatty acid (FA) intake in relation to breast cancer (BC). Methods: Epidemiology studies determining FA levels in biospecimen (circulating blood or adipose tissue (AT)) were identified from PubMed, EMBASE, and Cochrane Library databases until March 2018. Multivariate-adjusted risk ratios (RRs) with 95% confidence intervals (CIs) were pooled using a random-effect model. Difference in biospecimen proportions of LC n-3 FA between BC cases and non-cases were analyzed as a standardized mean difference (SMD). Results: Thirteen cohort and eleven case-control studies were eligible for the present meta-analysis. The estimated SMD was -0.14 (95% CI: -0.27, -0.11) for LC n-3 FA and -0.27 (95% CI: -0.42, -0.11) for LC n-3/n-6 FA ratio. When comparing the top tertiles with the bottom baseline levels, circulating LC n-3 FA was significantly associated with a lower risk of BC (RR: 0.84, 95% CI: 0.74, 0.96), but not AT (RR: 1.02, 95% CI: 0.70, 1.48). Significant inverse dose-response associations were observed for each 1% increment of circulating 20:5n-3 and 22:6n-3. Conclusion: This meta-analysis highlights that circulating LC n-3 FA as a biomarker of intake may be an independent predictive factor for BC, especially 20:5n-3 and 22:6n-3.

This systematic review examined the shifts in undernutrition and overnutrition in Nepal during the past two decades. We searched PubMed for studies and reports published between January 1, 2000, and June 30, 2018. Publications with a sample size greater than or equal to 500 that reported prevalence of nutritional status were included. Six large national reports and 36 studies met study inclusion criteria and were included. Overall, available nationally representative data remained limited. The Nepal Demographic and Health Survey 2001 to 2016 showed that underweight prevalence decreased from 26.7% to 17.2% and prevalence of overweight/obesity increased from 6.5% to 22.1% among women of reproductive age (15-49 years). In preschool children, prevalence of stunting, wasting, and underweight decreased from 57.2% to 35.8%, 11.2% to 9.7%, and 42.7% to 27.0%, respectively. Prevalence of overweight/obesity was low among children and was higher in higher socio-economic status (SES) groups. The overweight-obesity/underweight ratios indicate a shift from undernutrition to overnutrition problem; it was more evident in urban areas and higher SES groups. In conclusion, Nepal is experiencing a nutrition transition. More research is warranted to address this shift, and well-tailored public health efforts need to combat the double burden of overweight/obesity and undernutrition.

To determine whether behavioral factors differ among metabolic conditions and self-reported health, and to determine whether self-reported health is a valid predictor of metabolic syndrome (MetS). A total of 2997 individuals (≥ 40 years old) were selected from four biennial U.S. National Health and Nutrition Examination Surveys (2007–2014). A set of weighted logistic regression models were used to estimate the odds ratios (ORs) and 95% confidence intervals (CIs)Individuals with light physical activity are more likely to have MetS and report poor health than those with vigorous physical activity with OR = 3.22 (95% CI: 2.23, 4.66) and 4.52 (95% CI: 2.78, 7.33), respectively. Individuals eating poor diet have greater odds of developing MetS and reporting poor health with OR = 1.32 (95% CI: 1.05, 1.66) and 3.13 (95% CI: 2.46, 3.98). The aforementioned relationships remained significant after adjustment for demographic and socio-economic status. A potential intervention strategy will be needed to encourage individuals to aggressively improve their lifestyle to reduce MetS and improve quality of life. Despite the significant association between self-reported health with MetS, a low sensitivity indicated that better screening tools for MetS, diabetes and cardiovascular disease are essential.

Background Both depression and metabolic syndrome (MetS) confer an increased risk of developing type 2 diabetes (T2D) and cardiovascular disease. Accumulating evidence suggests healthy behaviors are crucial to maintain, improve and manage chronical disease and mental health; and unhealthy diet and sedentary behavior were found two major risk factors of MetS. The objective of this study was to investigate whether health behaviors (alcohol consumption, smoking, diet and recreational physical activity) are associated with depression and metabolic syndrome simultaneously. Methods This study included 1300 participants aged 20 years and over who had answered mental health-depression screener questions (PHQ-9) and finished examinations and laboratory tests related to five risk factors of MetS during the U.S. National Health and Nutrition Examination Survey (NHANES) 2007–2014. A set of series of weighted logistic regression models were used to investigate the aforementioned relationship. Results The prevalence of depression among U.S. adults is 15.08%. The two most often reported depression symptoms were “Trouble sleeping or sleeping too much” and “Feeling tired or having little energy”, with rates of14.68% and 13.09%, respectively. Participants who engaged in only light physical activity were more likely to have been identified as experiencing depression and MetS than those who engaged in vigorous physical activity with odd ratios 3.18 (95% CI: 1.59, 6.37) and 3.50 (95%CI: 2.17, 5.63), respectively. Individuals in the study having poor diets were more likely to suffer from depression than those eating good diets (OR=2.17, 95%CI: 1.47, 3.22). Conclusion Physical activity is strongly and inversely associated with depression and MetS. Diet is significantly associated with depression rather than MetS in this study.

Speed of Human Tooth Movement in Growers and Non-Growers: Selection of Applied Stress MattersIwasaki, L. R., Liu, Y., Liu, H., Nickel, J. C. 01 June 2017 (has links)
Objectives: To test that the speed of tooth translation is not affected by stress magnitude and growth status. Setting and Sample Population: Advanced Education Orthodontic clinics at the Universities of Nebraska Medical Center and Missouri-Kansas City. Forty-six consenting subjects with orthodontic treatment plans involving maxillary first premolar extractions. Materials and Methods: This randomized split-mouth study used segmental mechanics with definitive posterior anchorage and individual vertical-loop maxillary canine retraction appliances and measured three-dimensional tooth movements. Height and cephalometric superimposition changes determined growing (G) and non-growing (NG) subjects. Subjects were appointed for 9-11 visits over 84 days for maxillary dental impressions to measure three-dimensional tooth movement and to ensure retraction forces were continuously applied via calibrated nitinol coil springs. Springs were custom selected to apply two different stresses of 4, 13, 26, 52 or 78 kPa to maxillary canines in each subject. Statistical analyses (α=0.050) included ANOVA, effect size (partial η2) and Tukey's Honest Significant Difference (HSD) and two-group t tests. Results: Distolateral translation speeds were 0.034±0.015, 0.047±0.019, 0.066±0.025, 0.068±0.016 and 0.079±0.030 mm/d for 4, 13, 26, 52 and 78 kPa, respectively. Stress significantly affected speed and partial η2=0.376. Overall, more distopalatal rotation was shown by teeth moved by 78 kPa (18.03±9.50º) compared to other stresses (3.86±6.83º), and speeds were significantly higher (P=.001) in G (0.062±0.026 mm/d) than NG subjects (0.041±0.019 mm/d). Conclusions: Stress magnitude and growth status significantly affected the speed of tooth translation. Optimal applied stresses were 26-52 kPa, and overall speeds were 1.5-fold faster in G compared to NG subjects.

Jaw Mechanics in Dolichofacial and Brachyfacial Phenotypes: A Longitudinal Cephalometric-Based StudyIwasaki, L. R., Liu, Y., Liu, H., Nickel, J. C. 01 June 2017 (has links)
Objectives: To determine whether dolichofacial (Frankfort horizontal mandibular plane angle (FHMPA) ≥30°) vs brachyfacial (FHMPA ≤22°) phenotypes differ in temporomandibular joint (TMJ) loads and whether these differences correlate longitudinally with mandibular ramus height (Condylion-Gonion, Co-Go). Setting and Sample Population: Lateral and posteroanterior cephalographs from ten dolichofacial and ten brachyfacial individuals made at average ages of 6 (T1), 12 (T2) and 18 (T3) years and available online (http://www.aaoflegacycollection.org/aaof_home.html) were used. Materials and Methods: Three-dimensional anatomical data were derived from cephalographs and used in numerical models to predict TMJ loads for a range of biting angles on incisors, canines and first molars. Two criteria were used to define clinically important between-group TMJ load differences: statistical significance was defined with a two-group t-test, and where differences were also ≥20%. A statistical approach called response surface analysis was used to assess correlation between TMJ loads and its predictors considered in this study. Results: The two phenotypes had significantly different FHMPA at all ages (P<.05). No differences in TMJ loads were found at T1. Ipsilateral and contralateral TMJ loads at T2 and T3 were significant and ≥20% larger in dolichofacial than brachyfacial phenotypes for specific biting angles (all adjusted P<.05). Regression analysis indicated age and ramus height contribute 53% of the variability in normalized values of TMJ loads. At higher ages, dolichofacial phenotypes had significantly higher TMJ loads which were correlated with shorter ramus heights compared to brachyfacial phenotypes. Conclusions: Craniofacial mechanics may explain, in part, mandibular growth differences between dolichofacial and brachyfacial phenotypes.

Background: The present study examined the effects of breastfeeding and its duration on the development of childhood obesity from 24 months through grade 6. Methods: U.S. longitudinal data collected from 1234 children were analyzed using logistic regression models and generalized estimating equation (GEE). Child height and weight were measured six times at ages of 24 months, 36 months, 54 months, grade 1, grade 3, and grade 6. Results: During the early 1990s, prevalence of breastfeeding was low in the United States, 60% and 48% at 1 and 6 months, respectively. Nonsmoking, white, married mothers with both parents in the household, and with income above the poverty line, were more likely to breastfeed at 1 month of age of their babies. Obesity rate of the children increased with age from 24 months to grade 6. Logistic regression showed that breastfeeding at month 1 was associated with 53% (odds ratio [OR]: 0.47, 95% confidence interval [CI]: 0.30-0.73) and 47% (OR: 0.53, 95% CI: 0.36-0.78) decreased risks for childhood obesity at grades 1 and 6, respectively. GEE analysis showed that breastfeeding at 1 month reduced risk for childhood obesity by 36% (95% CI: 0.47-0.88) from ages 24 months through grade 6. Regarding breastfeeding duration, more than 6 months (vs. never) was associated with a decreased risk for childhood obesity by 42% (OR: 0.58, 95% CI: 0.36-0.94). Conclusions: Breastfeeding at 1 month and more than 6 months reduced the risk of childhood obesity. Rate of breastfeeding was low in the United States in the 1990s, which may have had long-term implications on children.

Objectives: To test whether mechanobehaviour (temporomandibular joint (TMJ) loads, jaw muscle use) was different between facial types and correlated with ramus height (Condylion-Gonion, mm). Setting and Sample Population: University of Missouri-Kansas City (UMKC) Orthodontic Clinic. Ten dolichofacial and ten brachyfacial adolescents (Sella-Nasion–Gonion-Gnathion (SN-GoGn) angles ≥37° and ≤27°, respectively) consented to participate. Materials and Methods: Numerical models calculated TMJ loads for a range of static biting based on subjects’ three-dimensional anatomy. Subjects were trained to record jaw muscle electromyography (EMG) over 2 days and 2 nights in their natural environments. Laboratory EMG/bite-force calibrations determined subject-specific EMG for 20 N bite-force (T20Nave). Jaw muscle use via duty factors (DF=muscle activity duration/total recording time, %) was determined from day and night recordings for muscle-specific thresholds from ≥5% to ≥80%T20Nave. ANOVA and Tukey's HSD post hoc tests assessed for group differences in mechanobehaviour (TMJ loads, DFs). Regression modelling correlated subjects’ normalized TMJ loads, DFs and ramus height. Results: Dolichofacial compared to brachyfacial subjects produced significantly higher (P<.05) TMJ loads, where ipsilateral loads were ≥20% larger for some biting angles, but had significantly less (all P<.05) masseter (day, night) and temporalis (night) DFs. Regression analysis showed a significant relationship amongst normalized TMJ loads, masseter DF and ramus height (R2=.49). Conclusions: Mechanobehaviour showed significant differences between facial types and was correlated with ramus height.

Poor sleep quality is an important symptom of many medical or psychiatric disorders. However, the impact of cooking oil fumes (COFs) on sleep quality has not been studied. This population-based cross-sectional study was conducted to examine the association between COFs of Chinese household cooking and sleep quality. Individual sleep quality assessment was completed in 2197 participants with an average age of 37.52 years, through Pittsburgh Sleep Quality Index (PSQI). Information about their cooking practice were also collected by self-reported questionnaire. As an internal biomarker of COFs, urinary 1-hydroxypyrene (1-HOP) (n = 562) was further measured using high-performance liquid chromatography. Binary logistic regression models were performed to evaluate the association between exposure to COFs and individual sleep quality. We found that, subjective poor kitchen ventilation, preheating oil to smoking, and cooking for over 30 minutes were positively associated with overall poor sleep quality (global PSQI score >5) [odds ratio (OR) = 1.75, 95% confidence interval (CI) = 1.43–2.16; 1.25, (1.03–1.52); 1.42, (1.15–1.76), respectively]. After adjusting for potential confounders, subjective poor kitchen ventilation still tend to increase the risk of long sleep latency, sleep disturbances, and daytime dysfunction [OR = 1.37, 95% CI = 1.09–1.73; 1.91, (1.39–2.61); 1.54, (1.23–1.93), respectively]. Similar results were observed in participants who preheated oil to smoking [OR = 1.36, 95% CI = 1.08–1.72; 1.55, (1.14–2.14); 1.25, (1.02–1.55), respectively] and cooked for over 30 minutes [OR = 1.34, 95% CI = 1.05–1.72; 1.46, (1.03–2.06); 1.36, (1.08–1.72), respectively]. Furthermore, high urinary 1-HOP level was also positively associated with overall poor sleep quality (OR = 2.30, 95% CI = 1.31–4.05). The results indicated that exposure to COFs from Chinese household cooking may be a risk factor for poor sleep quality among middle-aged Chinese population.

The Effectiveness of a Merit- and Productivity-Based Teaching Incentive in a College of Public HealthYoung, Dara, Stork, Janet, Quinn, Megan, Wykoff, Randy 01 January 2021 (has links)
In 2017, the College of Public Health implemented a “teaching incentive” by which faculty would receive extra pay if the composite score of teaching effectiveness on the student assessment of instruction (SAI) was in the top third for all scores in the college in that same semester, and at least 40% of the students in the class completed the SAI. In the first 4 years, 53 incentives were awarded to 25 different members of the faculty, for a total of $206,772. The college documented an increase in the composite score of teaching effectiveness and in percent of students completing the SAI. Significant increases in the composite score were seen for all faculty (3.47 to 3.59 out of 4.00, p =.02) and for those faculty in the college for the entire period (3.47 to 3.57, p =.0189). Over the 4 years, the percent of classes that had at least a 40% response rate increased for all faculty (41.2% to 55.7%, p =.0447) and for faculty who were in the college for the entire 4 years (39.6% to 51.9%, p =.1373). A teaching incentive that includes a significant salary supplementation appears to be associated with an increase in both student response rates and overall assessment of teaching effectiveness.
