Three Essays in Health Economics: The Role of Coordination in Improving Outcomes and Increasing Value in Health Care

Indiana University-Purdue University Indianapolis (IUPUI) / Hospital costs are the largest contributor to US health expenditures, making them a common target for cost containment policies. Policies that reduce fragmentation in health care and related systems could increase the value of these expenditures while improving outcomes. Efforts to address fragmentation of health care services, such as Accountable Care Organizations, have typically been enacted at the scale of health systems. However, coordination within health care facilities should also be explored. In three essays, I analyze the role of coordination in several forms. First, I examine the introduction of interdisciplinary care teams within a hospital. This analysis features care coordination within a health care facility with the potential to reduce resource utilization through improved communication between team members and between patients and their care providers. I find that care coordination reduced length of stay for some patients while maintaining care quality. This combination results in higher value care for patients and hospitals. Second, I explore whether these interdisciplinary care teams impact resource utilization and patient flow throughout the hospital. The primary outcome is reduction in patient transfers to the ICU. Here, care coordination includes interdisciplinary teams as well as coordination between interdisciplinary teams and intensivists in ICUs. Findings from this analysis suggest that ICU transfers were unaffected by care coordination. Finally, I examine coordination on a larger scale. I leverage data from a national database of trauma patients to compare mortality among adolescent patients with isolated traumatic brain injury between adult trauma centers and pediatric trauma centers. Previous work has shown that younger pediatric patients with this injury benefit from treatment at pediatric trauma centers. However, it is unclear whether this benefit extends to older pediatric patients on the cusp of adulthood. I find that, after adjusting for differences in injury severity, adolescent patients have no difference in mortality risk when treated at adult or pediatric trauma centers. This finding supports the current regionalized model of trauma care where severely injured patients are taken to the nearest trauma center, regardless of designation as pediatric or adult. / 2023-07-01

Industry platforms, particularly open-source based platforms are emerging as the tipping point of a new trend of interoganizational relationships among firms. They are characterized by a large number of actors with different objectives that come and go. However in order to reap the benefits of network effects, reduce fragmentation and get access to a large pool of resources, coordination dynamics within the different actors to create and innovate the platform are needed. As opposed to traditional literature where a single firm leads the evolution of the platform, a more democratic approach based on the institutionalization of coordination, the implementation of coordination processes and mechanisms is proposed. A study in the form of interviews and interactive forums was carried in the Symbian Platform, specifically in the Symbian Foundation to identify the main coordination dynamics. The results showed that in the case of the Symbian Platform, firms´ first step towards coordination was to establish the formal structure of coordination, in this case the Symbian Foundation. Consequently the Symbian Foundation established the processes and coordination mechanisms by which all of the actors participate and access to a pool of resources. The study describes the evolution from democratic coordination to an increasing selfcoordination promoted by the Symbian Foundation within its members.

Research on organizational coordination forms in China from middle-line managers' perspectives

Middle-line managers (MLMs) are becoming increasingly important for an organization. Coordination forms (immediate coordination and in-advance coordination) are used in the organization. In this thesis, four variables: organizational type, the degree of decentralization of organization, qualification and uncertainty are analyzed for investigating what are main factors influencing the use of coordination forms in Chinese organizations from MLM’s perspective.The empirical data for the study was gathered by a survey questionnaire, which involved a quota sampling of 158 MLMs in Chinese organizations. The interesting results justify the tendency to use in-advance coordination is greater for organizations that are characterized by a high level of uncertainty, especially learning and goal-setting; and the tendency to use in-advance coordination is greater for organizations that are characterized by a high level of decentralization.

In this industry, buyers and suppliers work in collaboration with each other to achieve superior supply chain performance. To achieve this, they have to find ways and methods to improve it. The purpose of this study was to investigate supply chain coordination and firm performance in the construction industry in the Gauteng Province. This study considers three drivers: supplier coordination, customer coordination and coordination effectiveness to achieve the performance of the construction industry. In most previous research studies, researchers have overlooked the important considerations and variables of supply chain coordination and firm performances as environmentally-friendly exercises in the construction industry. As a result, there was an abundant necessity to examine how the construction industry should implement a supply chain coordination programme and firm performance as environmentally-friendly practices in the organisational activities to ensure the sustainability of the construction industry. The effective application of SCC and firm performance in the construction industry will benefit all stakeholders. A quantitative approach was adopted in which a survey questionnaire was used to collect data from 414 construction supply chain managers and professional employees in the Gauteng Province. The study used a non-probability convenience sampling technique to select respondents. Data were analysed with the aid of two software packages: the Statistical Package for the Social Sciences (SPSS version 27.0) and the Analysis of Moment Structures (AMOS version 27.0). A confirmatory factor analysis was applied in examining and testing the relationships between observed constructs and their causal latent constructs while structural equation modelling was used to test the hypothesised relationships between constructs. The results revealed that supplier coordination, customer coordination and coordination effectiveness positively and significantly influence firm performance in the construction industry. Further, the study determines that to improve firm performance, the construction industry should advance the stages of trust, guarantee and sustainable relationships with their stakeholders. The results provide useful insights into how the construction industry could benefit from cultivating trust, assurance and sustainable relationships between supply chain partners as well as on how to improve firm performance. Therefore, this study is not only important for construction companies but also for manufacturers, suppliers and customers since most of them will collectively achieve the goal of performing purposeful activities to boost construction performance and sustainability.

Ce mémoire a pour problématique la coordination des services éducatifs complémentaires dans une école secondaire. Il interpelle plus précisément le champ de l'éducation à l'égard des besoins que posent la multiplicité et la spécialisation des services offerts aux élèves vivant des difficultés personnelles tant au plan scolaire que social. Dans la mesure où l'offre des ressources éducatives et socio-éducatives constitue un facteur de protection pour les élèves à risque de mésadaptation, la coordination des services éducatifs complémentaires peut être perçue comme un élément clé afin de contribuer à la persévérance scolaire. Cette recherche pose donc la question suivante: Quels sont les besoins d'une équipe de services éducatifs complémentaires en regard de la coordination dans le contexte scolaire secondaire ? Cette question nous a permis d'explorer la coordination de ces services, sous les angles de la continuité et de la collaboration. Le présent mémoire a poursuivi trois objectifs qui nous ont conduits à réaliser une étude de cas auprès d'une équipe constituée d'intervenants scolaires. Le premier objectif vise à connaître le fonctionnement des services complémentaires dans le cadre de la démarche d'intervention menée dans une école secondaire de la commission scolaire de la Région-de-Sherbrooke. Le deuxième objectif vise à repérer la présence de modalités de continuité et de collaboration au sein de cette démarche d'intervention. Le troisième objectif cherche enfin à contribuer à une réflexion, quant à une façon de coordonner les services complémentaires d'une équipe d'intervenants, en fonction de certains dispositifs comme celui de la gestion de cas. Ces objectifs ont structuré l'ensemble de nos opérations de recherche afin de documenter le processus de coordination des services éducatifs complémentaires à partir de renseignements documentaires et d'entrevues conduites auprès d'acteurs ciblés dans une école secondaire.
