Efeito da severidade da doença periodontal sobre a expressão do Receptor Ativado por Protease do Tipo 2 (PAR-2) / Effect of the severity of periodontal disease on the expression of Protease Activated Receptor Type 2 (PAR-2)

Pesquisas recentes investigando o receptor ativado por protease do tipo 2 (PAR-2) sugerem uma associação entre este receptor e a inflamação periodontal. Além disso, é sabido que a gingipaína, protease bacteriana secretada por um importante periodontopatógeno, Porphyromonas gingivalis (Pg), tem a capacidade de ativar o PAR-2. Ademais, um estudo anterior do grupo, verificou que quanto mais profunda a bolsa periodontal, maior era a expressão do receptor PAR-2. No entanto, não é sabido se a expressão de PAR-2 é proporcional a severidade de doença periodontal e a quantidade de gingipaína expressa na bolsa periodontal. Desta forma, o presente estudo, verificou no fluido gengival a correlação entre a expressão gênica de PAR-2 (Real Time-PCR) com os parâmetros clínicos periodontais, e a expressão gênica da protease gingipaína em pacientes com periodontite crônica severa e moderada, antes e após o tratamento periodontal não-cirúrgico. A expressão de PAR-2 e da protease gingipaína foi estatisticamente maior nos pacientes do grupo periodontite crônica severa (PS) em comparação com os pacientes do grupo periodontite crônica modera (PM) e controle (C). Além disso, o tratamento periodontal levou à redução significativa (p<0.05) da expressão de PAR-2 nos pacientes com periodontite crônica moderada. Em conclusão, dentro dos limites do presente estudo, nós demonstramos que a severidade da doença periodontal e a expressão de gingipaína influenciaram a expressão de PAR-2 no fluido gengival de pacientes com periodontite crônica. / Recent studies investigating the protease-activated receptor type 2 (PAR-2) suggest an association between the receptor and periodontal inflammation. In addition, it is known that gingipain, a bacterial protease secreted by an important periodontopathogen, Porphyromonas gingivalis (Pg), has the ability to activate PAR- 2. Furthermore, a previous study from our group found that the deeper the periodontal pocket, the higher the expression of the PAR-2 receptor. However, it is not known whether the expression of PAR-2 is associated to the severity of periodontal disease and the amount of gingipain expressed in the periodontal pocket. Thus, the present study verified, in the gingival fluid, the correlation between the PAR-2 gene expression (Real Time-PCR) with the clinical periodontal parameters, and the gene expression of the gingipain protease in patients with moderate and severe chronic periodontitis before and after non-surgical periodontal treatment. PAR-2 expression and gingipain protease were statistically more expressed in patients of the severe chronic periodontitis group (PS) compared with those in the moderate chronic periodontitis group (PM) and control group (C). Furthermore, periodontal treatment led to a significant reduction (p <0.05) in the expression of PAR-2 in patients with moderate chronic periodontitis. In conclusion, within the limits of the present study, we demonstrated that the severity of periodontal disease and the expression of gingipain influenced the PAR-2 expression in the gingival fluid of patients with chronic periodontitis.

O receptor ativado por protease do tipo 1 (PAR1) parece estar associado ao reparo periodontal, enquanto o tipo 2 (PAR2) com a inflamação periodontal. Esses receptores podem ser ativados pelas proteases gingipaina, uma protease secretada pela Porphyromonas gingivalis, um importante periodontopatógeno, e pela proteinase-3 de neutrófilos (P3), que é liberada por neutrófilos quando expostos a um estímulo inflamatório. Uma vez que o diabetes é reconhecido como um fator de risco importante para a doença periodontal, o objetivo deste estudo foi investigar a expressão de PAR1 e de PAR2 e de seus ativadores, gingipaina e P3 no fluido gengival (FG) de pacientes diabéticos com periodontite crônica, antes e após tratamento periodontal não cirúrgico. Amostras de FG e os parâmetros clínicos, como profundidade de sondagem (PS), nível clínico de inserção (NCI), sangramento à sondagem (SS) e índice de placa (IP) foram coletados de pacientes sistemicamente saudáveis e de pacientes com diabetes mellitus do tipo 2 (DMT2) com periodontite crônica , no baseline e após o tratamento periodontal não cirúrgico. As expressões gênicas de PAR1, PAR2, gingipaina e P3 no FG foram quantificadas por qPCR. Os parâmetros clínicos melhoraram significativamente após a terapia periodontal (p <0,01). O diabetes levou ao aumento da expressão de PAR1 no fluido gengival e na presença da periodontite crônica diminuiu significativamente a expressão de PAR1, PAR2 e P3 (p<0,05). Além disso, o tratamento periodontal não cirúrgico em diabéticos resultou no aumento da expressão de PAR1 e de PAR2 (p<0,05). Dentro dos limites do presente estudo, sugerimos que os PARs podem estar associados com a inflamação periodontal em diabéticos. / Protease activated receptor type 1 (PAR1) seems to play a role in periodontal repair, while PAR2 is associated with periodontal inflammation. These receptors can be activated by gingipain, a protease released from Porphyromonas gingivalis, an important periodontal pathogen, and neutrophil proteinase-3 (P3), which is released by neutrophils when exposed to an inflammatory stimulus. Since diabetes is known risk factor to periodontal disease, the aim of this study was to investigate PAR1 and PAR2 mRNA expression at the gingival crevicular fluid (GCF) in diabetic patients with chronic periodontitis, before and after non-surgical periodontal treatment. GCF samples and clinical parameters consisting of measuring probing depth (PD), clinical attachment level (CAL), bleeding on probing (BOP) and plaque index (PI) were collected from systemically healthy patients and patients with type 2 diabetes mellitus with chronic periodontitis, at baseline and after nonsurgical periodontal treatment. PAR1 and PAR2, as well the expression of the activators gingipain and P3 at the GCF were quantified by qPCR. The clinical parameters improved significantly after periodontal therapy (p <0.01). Diabetes led to increased expression of PAR1 in the GCF, and the presence of chronic periodontitis significantly decreased the expression of PAR1, PAR2 and P3 (p <0.05). Moreover, non-surgical periodontal treatment in diabetics resulted in increased expression of PAR1 and PAR2 (p <0.05). Within the limits of this study, we suggest that PARs may be associated with periodontal inflammation in diabetics.

Adult Oral Health Programme: The Effect of Periodontal Treatment and the Use of a Triclosan Containing Toothpaste on Glycaemic Control in Diabetics Abstract Aim: The aim of the research study is to establish an adult oral health programme for diabetics in Majuro, Republic of the Marshall Islands in order to determine the impact of non-surgical periodontal treatment followed by the use of a triclosan containing dentifrice on the maintenance of periodontal health and glycaemic control in type 2 diabetic patients. Hypothesis: Non-surgical periodontal treatment results in improved periodontal health and better glycaemic control in diabetics and use of a triclosan containing toothpaste is effective in maintaining this improvement in diabetics. Methods: An adult oral health programme was created, within which was conducted a two-group randomised clinical trial to address the hypothesis that non-surgical periodontal treatment results in improved periodontal health and better glycaemic control in type 2 diabetics and that the use of a triclosan containing toothpaste is effective in maintaining this improvement in diabetics. In this double blind controlled trial, sixty adult patients (aged 35 to 65 years) with type 2 diabetes mellitus having a minimum of 16 teeth received non-surgical periodontal treatment. Half of the patients were randomly assigned to use a triclosan containing toothpaste, Colgate Total, and the other group a non-triclosan toothpaste, Colgate Fluoriguard. The study evaluated the improvement in periodontal health by recording Probing Pocket Depth (PPD) on 6 sites of each tooth, and the number of sites bleeding on probing (BOP) at baseline, and at 6 months and 12 months after treatment. The second part of the study evaluated the impact of improvement of periodontal health on glycaemic control in type 2 diabetics by measuring HbA1c and RBS, and also assessing the levels of C-Peptides and CRP at baseline, and at 6 months and 12 months after treatment. The study also evaluated the effectiveness of a triclosan containing toothpaste in maintaining the improvement in periodontal health after non-surgical periodontal treatment. Results: The results showed that it was feasible to establish an oral health programme for the diabetics and could improve their periodontal health, and that toothpaste containing triclosan is effective in maintaining the improved periodontal heath in type 2 diabetics. Mean PPD dropped from 2.35mm to 1.95mm in the triclosan group and from 2.49mm to 2.24 mm in the non-triclosan group and the mean number of BOP sites dropped from 4.9 to 2.8 in the triclosan group and from 4.7 to 3.2 in the fluoriguard at 12 month visits. However, the results did not show improvement of HbA1c nor RBS levels in either group. C-Peptide levels increased and C-Reactive Protein levels decreased in both groups, however, not to significant levels at 12 month visits. Conclusion: The results of this research study lead to the conclusion that treating periodontal infection has effect of periodontal health of type 2 diabetic patients and following-up with simple personal oral hygiene of regular tooth-brushing helps maintain their periodontal health. This programme also proved that this type of oral health programme is feasible and valuable for diabetics in isolated places like the Marshall Islands, where infrastructure, personnel and resources are limited to treat microvascular and macrovascular complications of diabetes. As for the effectiveness of treating periodontal infections on glycaemic control of diabetics, this study failed to support the hypothesis that non-surgical treatment plus triclosan containing toothpaste would lead to better glycaemic management through improved periodontal health.

Qualidade de vida relacionada à saúde bucal após o tratamento de gengivite e da periodontite / Oral health related quality of life after gingivitis and periodontitis treatment

O objetivo do presente estudo foi verificar o impacto do tratamento periodontal não cirúrgico na qualidade de vida relacionada à saúde bucal de pacientes com periodontite de moderada à avançada. Quarenta e cinco pacientes foram submetidos aos exames físicos periodontais e à entrevista utilizando-se o instrumento OHIP-14, em três momentos experimentais: antes do tratamento (T1), após o tratamento da gengivite (T2) e um mês após o término do tratamento da periodontite (T3). A análise dos dados foi feita comparando-se as médias dos indicadores físicos periodontais (IFP), escore total (ET) do OHIP-14 e de seus domínios (D), separadamente. Além disso, foi avaliada a diferença de mudança do ET-OHIP-14 entre T1-T2 e T1-T3 e sua correlação com os IFP e o com o ET -OHIP- 14 em T1. Uma redução significante foi observada para o ET-OHIP-14 entre T1 (17,6±11,3) e T2 (9,1±8,0) e T3 (9,5±7,7). Porém, a mudança observada para o ETOHIP- 14 foi maior entre T1-T2, quando comparado a T1-T3. O tamanho deste efeito foi de 0.75 e 0,72 respectivamente. À exceção do domínio incapacidade física e limitação funcional, todos os demais apresentaram redução significante entre T1 e T2. Este resultado foi mantido em T3, exceto para o dor física que apresentou incremento positivo (de 2,8±2,0 para 3,7±2,5). Foi observada uma correlação entre a mudança do OHIP-14 com o ET e Perda de Inserção em T1. Conclui-se que a percepção de qualidade de vida melhora após o tratamento periodontal, principalmente após o tratamento da gengivite. Sendo assim, sugere-se o tratamento da gengivite, expressivo na sua capacidade de melhorar os impactos relacionados à qualidade de vida, como essencial em periodontia. / The aim of this study was to evaluate the impact of non-surgical periodontal treatment on oral health related quality of life in patients diagnosed with moderate to advanced chronic periodontitis. Forty five patients were examined for clinical periodontal parameters and for the assessment of oral health related quality of life using the OHIP-14 questionnaire in three different moments: before treatment (T1), after gingivitis treatment (T2) and after periodontitis treatment (T3). Means of periodontal clinical parameters, the OHIP-14 total score and its domains were compared separately at all moments. Change of means (between T1-T2 and T1-T3) and its correlation between initial (T1) clinical parameters and OHIP-14 total score was calculated. A significant reduction of OHIP-14 total score was observed between the T1 (17.6±11.3) and T2 and T3 (9.1±8.0 and 9.5±7.7). However, the change of OHIP-14 means was higher for T1-T2 when compared to T1-T3, effect sizes calculated for this comparisons were 0.75 and 0.72 respectively. Except for the domain physical disability and functional limitation, all others showed a significant reduction between T1 and T2. This result was maintained at T3, except for the physical pain domain which increased (from 2.8 ± 2.0 to 3.7 ± 2.5). A correlation was observed between the change in OHIP-14 with its total score and periodontal attachment loss in T1. We conclude that there is an improvement of patients’ perception on oral health related quality of life after periodontal treatment, particularly after the gingivitis treatment. Therefore, it is suggested the treatment of gingivitis, expressive in its ability to improve the impacts related to quality of life, as essential in periodontology.

Avaliação longitudinal da densidade radiográfica da crista óssea alveolar em pacientes com periodontite crônica e em pacientes saudáveis / Longitudinal evaluation of radiographic crestal bone density in potients with cronic periodontitis and healthy patients

A periodontite é um grupo dentro das doenças periodontais onde o tecido de suporte é acometido como resultado de um desequilíbrio entre uma agressão bacteriana e a resposta imunológica do hospedeiro. O exame radiográfico é um método não invasivo que quando usado de forma adequada pode prover imagens que demonstram perdas ósseas iniciais. A avaliação da densidade óssea já foi previamente descrita. Este estudo teve como objetivo avaliar as alterações de densidade óssea da crista alveolar no período de 4 meses, tanto em pacientes com periodontite crônica antes e depois da terapia básica, quanto em pacientes periodontalmente saudáveis. Os pacientes dos dois grupos foram submetidos a exame periodontal com Florida Probe. Foram realizadas 6 radiografias por paciente em cada momento da avaliação, à partir dessas imagens realizamos a avaliação da densidade óssea da crista alveolar através de áreas de interesse (ROI), utilizando o Kodak Dental Imaging Software 6.7. As comparações entre as medidas radiográficas do baseline e as da reavaliação, não apresentaram diferenças significantes para ROI 1, ROI 2, JCE-CO nos dois grupos. Sendo assim, observamos que o grupo de pacientes com periodontite crônica não apresentou diferença significante para densidade óssea da crista alveolar 4 meses após o tratamento, mesmo apresentando melhora dos parâmetros clínicos periodontais. O grupo de pacientes periodontalmente saudáveis também não apresentou alterações de densidade no mesmo período de tempo. / Periodontitis is a group within the periodontal disease where the support tissue is affected as a result of an imbalance between aggressive and bacterial host immune response. Radiographic examination is a noninvasive method that when used properly can provide images that show early bone loss. Assessment of bone density has been previously described. This study aimed to assess changes in bone density of the alveolar crest in the period of four months, both in patients with chronic periodontitis before and after primary therapy, and in periodontally healthy patients. Patients in both groups underwent periodontal examination with Florida Probe , to assess the presence of periodontitis and also the change of clinical parameters after primary therapy. Six radiographs were taken per patient at each time of evaluation. From these images we performed the evaluation of alveolar crest bone density over areas of interest (ROI) using the Kodak Dental Imaging Software 6.7 . Comparisons between radiographic measurements taken at baseline and follow up, did not show differences for ROI 1, ROI 2, JCE-CO in both groups. Thus, we observed that the group of patients with chronic periodontitis showed no significant difference in bone density of the alveolar crest 4 months after treatment, even with improvement of clinical periodontal parameters. The group of periodontally healthy patients did not show density changes at the same time.

Investigação de polimorfismos genéticos e metabolismo lipídico com doença periodontal crônica : metanálise e estudo caso-controle /

Orientador: Raquel Mantuaneli Scarel-Caminaga / Resumo: Diversos fatores relacionados ao hospedeiro, como fatores ambientais (tabagismo), doenças sistêmicas (diabetes e dislipidemia) e herança genética vêm sendo estudados quanto à influência no início e progressão da doença periodontal. Embora muitos estudos tenham investigado a relação entre dislipidemia e periodontite crônica (PC), examinando os níveis séricos de lipídeos, os resultados ainda são contraditórios, indicando a demanda da realização de uma metanálise. Alguns Genome-Wide Association Studies (GWAS), estudos de bioinformática e diversos estudos caso-controle evidenciaram variantes genéticas associadas à suscetibilidade à periodontite, mas os resultados para alguns desses polimorfismos são escassos ou contraditórios entre as diferentes populações. Assim, nota-se a importância de realizar metanálises e estudos caso-controle para buscar validar ou identificar a associação de polimorfismos com a PC na população brasileira. O objetivo deste estudo foi dividido em 3 capítulos: (1) investigar se os pacientes com PC apresentam diferentes níveis séricos de parâmetros lipídicos (HDL, LDL, colesterol total e triglicérides) em comparação com indivíduos saudáveis; (2) avaliar se polimorfismos de base única (SNPs, single nucleotide polymorphisms) nos genes LDLR (rs5925 e rs688) e APOB (rs676210 e rs693) contribuem para a suscetibilidade à PC, uma vez que PC está associada a níveis plasmáticos de LDL mais elevados, e os polimorfismos nestes genes podem aumentar as concentrações plasm... Although several studies have investigated the relationship between dyslipidemia and chronic periodontitis (CP), focusing on serum lipid levels, the results are still contradictory, indicating the necessity of a meta-analysis. Some genome-wide association studies (GWAS), bioinformatics studies and several case-control studies have shown genetic variants associated with susceptibility to CP, but the results of some of these polymorphisms are sparse and contradictory among different populations. Thus, it is important to conduct meta-analysis and case-control studies in order to validate or identify an association of polymorphisms with CP in a Brazilian population. Orientador: Daniela Leal Zandim-Barcelos / Banca: Joni Augusto Cirelli / Banca: Flávia Aparecida Chaves Furlaneto Messora / Resumo: Os peptídeos antimicrobianos são componentes da resposta imune inata, que apresentam propriedades imunomodulatórias adjuntas às propriedades antimicrobianas e, portanto, podem ter um papel chave na susceptibilidade ou resistência a doenças na cavidade oral.O propósito deste estudo foi investigar a influência do tabagismo sobre os níveis dos peptídeos antimicrobianos, alfa-defensinas (HNP 1-3) e LL-37, no fluido crevicular gengival de pacientes com periodontite crônica, e avaliar a relação destes peptídeos com saúde e doença periodontal. Um total de 40 pacientes com periodontite crônica, sendo 20 fumantes e 20 não fumantes, e 20 pacientes periodontalmente saudáveis foram incluídos no estudo. Os parâmetros clínicos avaliados foram: profundidade de sondagem, sangramento à sondagem, nível clínico de inserção, Índice de placa visível e Índice de sangramento gengival. Amostras de fluido crevicular gengival foram coletadas com tiras de papel absorvente e o volume mensurado com Periotron. Nos pacientes com periodontite crônica, foram coletadas amostras de sítios sadios e doentes. A quantificação da LL-37 e HNP 1-3 foi realizada pela técnica ELISA sanduíche.Os níveis de LL-37 e HNP 1-3 foram menores tanto nos sítios sadios como nos sítios doentes de pacientes fumantes em comparação com os não fumantes, sendo essas diferenças significativas exceto para os níveis de LL-37 nos sítios sadios (p<0,05). (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Antimicrobial peptides are components of the innate immune response that have immunomodulatory properties adjuncts to the antimicrobial properties and, therefore, may have a key role in susceptibility or resistance to diseases in the oral cavity. The purpose of this study was to investigate the influence of smoking on the levels of antimicrobial peptides, alpha-defensins (HNP 1-3) and LL-37 in the gingival crevicular fluid of patients with chronic periodontitis, and evaluate the relationship of these peptides with health and periodontal disease. A total of 40 patients with chronic periodontitis, 20 smokers and 20 non-smokers, and 20 periodontally healthy patients were included in the study. The clinical parameters evaluated were probing depth, bleeding on probing, clinical attachment level, visible plaque index and gingival bleeding index. Samples of gingival crevicular fluid were collected with absorbent paper strips and the volume measured on Periotron. In patients with chronic periodontitis, samples of healthy and diseased sites were collected. The quantification of LL-37 and HNP 1-3 was carried out by sandwich ELISA tecnique. The levels of LL-37 and HNP 1-3 were lower both in healthy sites such as diseased sites in smokers compared to non-smokers, those with significant differences except for the LL-37 levels in healthy sites (p<0.05). Foi demonstrado que portadores da SD apresentam resposta imune diminuída em relação aos cromossomicamente normais. O objetivo desta pesquisa foi investigar diferenças nos parâmetros clínicos periodontais e níveis de expressão dos genes Interferon-gama (IFNG), Interferon-gama receptor 1 (IFNGR1), Interferon-gama receptor 2 (IFNGR2), Interferon-alfa (IFNA), Interferon-alfa receptor 1 (IFNAR1), Interferon-alfa receptor 2 (IFNAR2), Janus-quinase 1 (JAK1), Transdutor de sinal e ativador da transcrição 1 (STAT1) e Fator de regulação de interferon 1 (IRF1) em indivíduos com SD que apresentam ou não DP e em indivíduos cromossomicamente normais. Fizeram parte deste estudo 80 indivíduos entre 7 e 57 anos de idade subdivididos em 4 grupos: SD com DP (A); indivíduos com SD sem DP (B); indivíduos não-sindrômicos (Controle) com DP (C) e indivíduos Controle sem DP (D). A expressão gênica foi investigada por meio de quantificação relativa utilizando a técnica da Reação em Cadeia da Polimerase (PCR) em Tempo Real. Para o índice sangramento à sondagem (SS) não houve diferença entre os grupos A e 21 C. A periodontite crônica localizada foi o tipo prevalente tanto entre indivíduos com SD como Controle. Considerando os parâmetros clínicos, não foram encontradas diferenças na periodontite crônica localizada entre os indivíduos com SD e Controle, assim como para a periodontite crônica generalizada. Only poor oral hygiene does not explain the severe periodontal destruction seen in DS patients. It has been shown that DS patients have a weaker immune response than people with normal number of chromosomes. Avaliação longitudinal da densidade radiográfica da crista óssea alveolar em pacientes com periodontite crônica e em pacientes saudáveis / Longitudinal evaluation of radiographic crestal bone density in potients with cronic periodontitis and healthy patients

A periodontite é um grupo dentro das doenças periodontais onde o tecido de suporte é acometido como resultado de um desequilíbrio entre uma agressão bacteriana e a resposta imunológica do hospedeiro. O exame radiográfico é um método não invasivo que quando usado de forma adequada pode prover imagens que demonstram perdas ósseas iniciais. A avaliação da densidade óssea já foi previamente descrita. Este estudo teve como objetivo avaliar as alterações de densidade óssea da crista alveolar no período de 4 meses, tanto em pacientes com periodontite crônica antes e depois da terapia básica, quanto em pacientes periodontalmente saudáveis. Os pacientes dos dois grupos foram submetidos a exame periodontal com Florida Probe. Foram realizadas 6 radiografias por paciente em cada momento da avaliação, à partir dessas imagens realizamos a avaliação da densidade óssea da crista alveolar através de áreas de interesse (ROI), utilizando o Kodak Dental Imaging Software 6.7. As comparações entre as medidas radiográficas do baseline e as da reavaliação, não apresentaram diferenças significantes para ROI 1, ROI 2, JCE-CO nos dois grupos. Sendo assim, observamos que o grupo de pacientes com periodontite crônica não apresentou diferença significante para densidade óssea da crista alveolar 4 meses após o tratamento, mesmo apresentando melhora dos parâmetros clínicos periodontais. O grupo de pacientes periodontalmente saudáveis também não apresentou alterações de densidade no mesmo período de tempo. / Periodontitis is a group within the periodontal disease where the support tissue is affected as a result of an imbalance between aggressive and bacterial host immune response. Radiographic examination is a noninvasive method that when used properly can provide images that show early bone loss. Assessment of bone density has been previously described. This study aimed to assess changes in bone density of the alveolar crest in the period of four months, both in patients with chronic periodontitis before and after primary therapy, and in periodontally healthy patients. Patients in both groups underwent periodontal examination with Florida Probe , to assess the presence of periodontitis and also the change of clinical parameters after primary therapy. Six radiographs were taken per patient at each time of evaluation. From these images we performed the evaluation of alveolar crest bone density over areas of interest (ROI) using the Kodak Dental Imaging Software 6.7 . Comparisons between radiographic measurements taken at baseline and follow up, did not show differences for ROI 1, ROI 2, JCE-CO in both groups. Thus, we observed that the group of patients with chronic periodontitis showed no significant difference in bone density of the alveolar crest 4 months after treatment, even with improvement of clinical periodontal parameters. The group of periodontally healthy patients did not show density changes at the same time.
