Nurses' perceptions of nurse-physician collaboration in the intensive care units of a public sector hospital in Johannesburg

MSc (Nursing), Faculty of Health Sciences, University of the Witwatersrand / Nurses working with critically ill patients in intensive care units (ICUs) have a unique role to play in health care. They spend 24 hours with patients and come into contact with all the disciplines which come to review these patients. Nurses therefore need to effectively collaborate with the multidisciplinary teams, especially physicians, in order to meet patients’ needs and maximise patient care outcomes. The purpose of this study was to identify and describe nurses’ perceptions towards nurse-physician collaboration in the intensive care units. A non experimental descriptive study design was utilised in this study. Data were collected using a questionnaire developed from the Jefferson Scale of Attitude toward Physician-Nurse Collaboration with additional two open-ended questions to cover the rest of the study objectives. Data were analysed using descriptive and inferential statistics as well as content analysis. Results showed that nurses working in Intensive Care units (ICUs) had positive attitude towards nurse-physician collaboration regardless of gender, years of working in the ICUs and whether registered intensive critical care nurse or not. The findings also showed that nurses perceive that the process of nurse-physician collaboration in Intensive Care Units provokes a number of challenges, such as superior-subordinate relationships which exist between nurses and physicians, workload and overlapping responsibilities hence, nurses feel inferior, undermined, mostly overwork and become frustrated. However, nurses suggested that promoting team-work; a focus on patient-centered care and staff motivation would assist in creating effective collaborative environment. collaborative environment

Proposition des modèles et de processus structurés pour le développement d’environnements collaboratifs synchrones : application aux réunions de revue de conception / The proposition of structured models and process for development of synchronous collaborative environments : application for design review meeting

Le développement d'un environnement collaboratif est un processus complexe. La complexité réside dans le fait que ce développement implique beaucoup de prise de décisions. De multiples compromis doivent être faits pour répondre aux exigences actuelles et futures d'utilisateurs aux profils variés. La prise en compte de cette complexité pose des problèmes aux chercheurs, développeurs et utilisateurs. Les informations et données requises pour prendre des décisions adéquates de conception et évaluer rigoureusement ces décisions sont nombreuses, parfois indéterminées et en constante évolution. Dans la partie-I de cette thèse, nous formulons les connaissances générales sur le travail collaboratif synchrone qui constituent l'état de l'art du domaine du problème. Nous pratiquons de même pour les environnements collaboratifs synchrones (domaine de la solution technique) et leur cette formulation s'appuie sur une étude de la littérature et conduit à la proposition de Schéma Conceptuel (Concept Maps). Nous en déduisons trois modèles: SyCoW (travail collaboratif synchrone), SyCoE (environnement collaboratif synchrone) et SyCoEE (évaluation environnement collaboratif synchrone). Dans la partie II de cette thèse, nous proposons un processus pour la sélection / développement d'un environnement collaboratif, où nous démontrons comment les modèles SyCoW, SyCoE et SyCoEE structurent ce processus. Grâce à la mise en œuvre de la démarche proposée, nous présentons le développement d'un nouvel environnement collaboratif synchrone pour une réunion de revue de conception nommé MT-DT. MT-DT a été conçu, développé et évalué par l'auteur dans sa thèse de doctorat. MT-DT est une application logicielle 3D spécifique à une table multi-touche qui assiste les activités de revue de conception collaborative. Les résultats de l'évaluation ont confirmé la convivialité de MT-DT et fournissent des éléments de validation des choix que nous avons faits au cours du développement de MT-DT. / Development of collaborative environment is a complex process. The complexity lies in the fact that collaborative environment development involves a lot of decision making. Several tradeoffs need to be made to satisfy current and future requirements from a potentially various set of user profiles. The handling of these complexities poses challenges for researcher, developers and companies. The knowledge required to make suitable design decisions and to rigorously evaluate those design decisions is usually broad, complex, and evolving. In Part-I of this thesis we investigate to formulate the general knowledge about: synchronous collaborative work which conceptualize the problem domain, synchronous collaborative environment which conceptualize the solution domain and synchronous collaborative environment evaluation which conceptualize the evaluation of whole or part of the proposed solution for the specified problem. This formulation has been done through literature study and leaded to the Concept Maps. The results generate three models: SyCoW (synchronous collaborative work), SyCoE (synchronous collaborative environment) and SyCoEE (synchronous collaborative environment evaluation). In Part-II of this thesis we proposed a process for selection/development of collaborative environment, where we demonstrate how SyCoW, SyCoE and SyCoEE support this process in different ways. Through the proposed process we present the development of new synchronous collaborative environment for design review meeting, named, MT-DT. MT-DT has been designed, developed and evaluated by the author in her PhD. MT-DT consist of a multi-touch table with specific 3D software application which support collaborative design review activities. The results of evaluation confirmed the usability of MT-DT and provide arguments for our choices which we made during development of MT-DT.

Diseño de un entorno colaborativo y su aplicación a plataformas de aprendizaje

Los entornos colaborativos educacionales, así como las herramientas educacionales están tomando grandes repercusiones en nuestra sociedad, llevándose a cabo en la actualidad numerosos proyectos de investigación acerca de su construcción y su uso en comunidades educacionales. Las investigaciones llevadas a cabo durante estos años han estado basadas en la creación de entornos colaborativos y su aplicación en entornos educacionales, a través de las cuales se han realizado una serie de estudios sobre las ventajas y desventajas que proporcionan dichos entornos, tanto a nivel de arquitectura como a nivel de herramientas.Con lo cual uno de los objetivos de nuestro trabajo de investigación es ofrecer una plataforma colaborativa genérica que puede ser aplicada a cualquier ámbito colaborativo incluyendo el aprendizaje por descubrimiento. Para ello se han estudiado las principales tecnologías y las distintas plataformas existentes obteniendo a partir de ello los elementos necesarios para la creación de un entorno colaborativo genérico. Con el fin de consolidar dichos elementos mostramos como se han aplicado al desarrollo de aplicaciones y plataformas dentro del ámbito de dos proyectos europeos, ITCOLE y COLAB. / Earning collaborative environments as well as learning tools have taken a prominent relevance in society, producing a growth of research projects regarding the building and the use of this system in education area. During the last years the research in this area has been focused on the building of different kinds of collaborative platforms and its usage in the educational arena, providing different kinds of studies which indicate the advantages and disadvantages of the use of them. These studies have covered the architecture of these systems and the use of them. Therefore, the aim of this research is to provide a generic collaborative environment which can be applied to any area, including discovery learning. For that reason, we have studied the main technologies and platforms for obtaining the elements needed in the building of a generic collaborative environment. Moreover, we have included these elements in the development regarding two European projects, ITCOLE and COLAB.

Knowledge Management in Collaborative Environment and Service Oriented Organizations

In this research, we propose a knowledge management architecture in a collaborative environment and service oriented organization. The architecture contains five components, including partners, knowledge bases, portals, pipes, and cloud. Each segment of knowledge which is created in partners’ portals will be displayed in the cloud. The cloud contains knowledge from portals. Portals and the cloud will be linked by a specific type of connections (pipes), which presents the knowledge to the cloud without copying them. We implement the proposed architecture online to prove its validity. The prototype that we examine has three partners including finance, insurance, and transportation. Each partner creates knowledge by using its portal and saving it in its own knowledge base (KB). Likewise, each partner has an access to other partners’ portals to ask questions or perform inquiries. The answered questions are saved in the KBs and displayed in the cloud. For implementation, we use Joomla as CMS portals, K2 as KB in each portal, Yahoo Pipes as connections between the portals and the cloud. Finally, the cloud is a webpage that displays knowledge from different portals. We demonstrate that the proposed architecture facilitates sharing knowledge among the partners in the VO, and prevents knowledge duplications in different KBs. Moreover, we could move the stored knowledge from KB to another by using backup feature the CMS portal if any partner want to leave or the VO decides to terminate.

An Empirical Investigation of the Willingness of US Intelligence Community Analysts to Contribute Knowledge to a Knowledge Management System (KMS) in a Highly Classified and Sensitive Environment

Since September 11, 2001, the United States Government (USG) has possessed unparalleled capability in terms of dedicated intelligence and information collection assets supporting the analysts of the Intelligence Community (IC). The USG IC has sponsored, developed, and borne witness to extraordinary advances in technology, techniques, and procedures focused on knowledge harvesting, knowledge sharing, and collaboration. Knowledge, within successful (effective & productive) organizations, exists as a commodity; a commodity that can be created, captured, imparted, shared, and leveraged. The research problem that this study addressed is the challenge of maintaining strong organizational effectiveness and productivity through the use of an information technology-based knowledge management system (KMS). The main goal of this study was to empirically assess a model testing the impact of the factors of rewards, power, centrality, trust, collaborative environment, resistance to share, ease-of-using KMS, organizational structure, and top management support to inducement, willingness to share, as well as opportunity to contribute knowledge to a KMS on knowledge-sharing in a highly classified and sensitive environment of the USG IC. This study capitalized on prior literature to measure each of the 15 model constructs. This study was conducted with a select group of USG Departments and Agencies whose primary interest is Intelligence Operations. This study solicited responses from more than 1,000 current, as well as former, Intelligence Analysts of the USG IC, using an unclassified anonymous survey instrument. A total of 525 (52.5%) valid responses were analyzed using a partial least squares (PLS) structural equation modeling (SEM) statistical technique to perform model testing. Pre-analysis data screening was conducted to ensure the accuracy of the data collected, as well as to correct irregularities or errors within the gathered data. The 14 propositions outlined in this research study were tested using the PLS-SEM analysis along with reliability and validity checks. The results of this study provide insights into the key factors that shed light onto the willingness of US intelligence community analysts to contribute knowledge to a KMS in a highly classified and sensitive environment. Specifically, the significance of a knowledge worker’s willingness to contribute his/her knowledge to a KMS along with the opportunity to contribute knowledge, while inducement was not a significant factor when it comes to knowledge sharing using KMS in highly classified environments.

e-Termos: Um ambiente colaborativo web de gestão terminológica / e-Termos: a web collaborative environment of terminology management

Em uma de suas definções, a Terminologia representa o conjunto de princípios e métodos adotados no processo de gestão e criação de produtos terminológicos, tais como glossários e dicionários de termos. A sistematização desses métodos envolve a aplicação de ferramentas computacionais específicas e compatíveis com as tarefas terminológicas, contribuindo para o desenvolvimento desses produtos e a difusão de conhecimento especializado. Entretanto, principalmente no Brasil, a combinação da Terminologia e Informática é incipiente, e dentre as atividades do trabalho terminológico é comum a utilização de várias ferramentas não especializados para esse fim. Isso torna o trabalho dos terminólogos muito moroso, pois esse trabalho geralmente é feito por uma equipe multidisciplinar que deve ter acesso, a todo o momento, à versão mais atual das várias etapas da geração de um produto terminológico. Além disso, deixa o gerenciamento dos dados mais complicado, pois não existe um padrão de entrada e saída definido para os programas. Apoiado nos pressupostos da Teoria Comunicativa da Terminologia (TCT), este trabalho apresenta a proposta de desenvolvimento e avaliação do e- Termos, um Ambiente ColaborativoWeb composto por seis módulos de trabalho bem definidos, cujo propósito é automatizar as tarefas de gestão e criação de produtos terminológicos. Cada módulo do e-Termos possui a responsabilidade de abrigar tarefas inerentes ao processo de criação das terminologias, sendo atreladas a eles diferentes ferramentas de apoio lingüístico, que possuem a função de dar suporte às atividades de Processamento de Língua Natural envolvidas nesse processo. Além delas, há também ferramentas colaborativas, designadas para dar apoio às necessidades comunicacionais e de interação da equipe de trabalho. Particularmente com relação ao processo de avaliação proposto, uma de suas características é a capacidade de ser executado em um tempo curto, viabilizando a avaliação controlada de vários grupos, mas executada no ambiente de trabalho do público alvo. As principais contribuições desta pesquisa são o aspecto colaborativo instanciado na prática terminológica, a criação flexível da Ficha Terminológica, a possibilidade didática de uso para o ensino de terminologia, lexicografia e tradução e o processo de avaliação para sistemas colaborativos desenvolvido para o e-Termos, que combina Cenários de Uso e um Questionário de Pesquisa. Utilizando tecnologias Web e da área de Computer Supported Collaborative Work (CSCW) para o desenvolvimento da sua arquitetura computacional colaborativa, o e-Termos apresenta-se como um ambiente inovador para a pesquisa terminolóogica assistida por computador, pois automatiza um método prático que exp~oe os postulados da terminologia de orientação descritiva e evidencia todas as etapas do processo de criação de produtos terminológicos com o inédito diferencial colaborativo. Para certificar este êxito, o e-Termos tem recebido um número crescente de novas propostas de projeto, tendo até Agosto de 2009 mais de 130 usuários cadastrados, alocados em 68 diferentes projetos terminológicos / In one of its definitions, Terminology represents the set of principles and methods adopted in the creation and management of terminological products as glossaries and dictionaries of terms. A systematization of these methods includes the application of specific computational tools, compatible with terminological tasks, which contribute to developing such products and disseminating expert knowledge. However, especially in Brazil, the combination of Terminology and Computer Science is still incipient, and to perform the tasks of a terminological work it is typical to employ several nonspecialized tools, which make terminologists\' work very time-consuming, since it is usually carried out by a multidisciplinary team that should have access, all the time, to the latest versions of the various stages of the generation of a terminological product. Moreover, it makes data management more complex, because there is no input/output standard defined for programs. Based on the presuppositions of the Communicative Theory of Terminology (CTT), this thesis proposes the development and evaluation of e-Termos, a Web Collaborative Environment composed of six well-defined working modules, whose purpose is to automatize tasks for creating and managing terminological products. Each module in e- Termos is responsible for tasks inherent to the process of creating terminologies. Linked to these modules, there are different linguistic support tools that assist the Natural Language Processing activities included in the process. Besides them, there are also collaborative tools for supporting the communication and interaction needs of team members. As far as the proposed evaluation process is concerned, one of its features is that it can be run in a short time, making viable a controlled evaluation of several groups that is, however, run in the work environment of the target audience. The main contributions of this research are the collaborative aspect instantiated in terminological practice, the exible creation of Terminological Records, the possibility of being used for teaching terminology, lexicography and translation, and the evaluation of collaborative systems developed for e-Termos, which combines Scenario-based Evaluations and Surveys. Using Web technologies and Computer Supported Collaborative Work (CSCW) to develop its collaborative computational architecture, e-Termos is an innovative environment for computer-assisted terminological research, since it automatizes a useful method that represents the postulates of descriptive terminology and highlights all stages of the process of creating terminological products with the unprecedented collaborative differential. Confirming its success, e-Termos has been receiving a growing number of new project proposals, and in August 2009 has more than 130 registered users in 68 different terminological projects

This task, still, requires effort of the teacher. So, the learner accompaniment in distance teaching-learning environments, obtained through the observation of the learner activities, it presents referring problems to the lack of mechanisms that offer an effective support to the teacher in the orientation for the learner. These existent mechanisms only present results to the teacher without to do any inference, like this, self teacher needs to analyze those results manually in search of subsidies to do a precise orientation, showing the possible causes and addressing the learner for the correct road. However, to dower a mechanism with inference capacity is necessary to capture the knowledge of as the teacher fulfills the leaner accompaniment for, later, to transfer this knowledge for a knowledge base with inference capacity. The necessary knowledge for the teacher to realize the learner accompaniment in a teaching-learning environment is the evaluation criterions that are defined by course pedagogic coordination or teacher of the teaching institution. However, those evaluation criterions are generic and not standardized hindering, therefore, the consent of the same; was realized a theoretical rising for the definition of the evaluation criterions in the NetClass environment. This research generated the knowledge on the development of a model denominated Learner Accompaniment Support Mechanism based on techniques of Artificial Intelligence, which is applied to the NetClass environment of teaching-learning. / Os ambientes de Educação a Distância, atualmente, proporcionam meios de avaliação de o aprendiz. Essa tarefa, de fato, ainda, exige muito esforço e empenho da parte do professor. Portanto, o acompanhamento do aprendiz em ambientes de ensino-aprendizagem a distância, obtida através da observação das atividades dos aprendizes, apresenta problemas referentes à carência de mecanismos que ofereçam um suporte eficaz ao professor na orientação para o aprendiz. Esses mecanismos existentes somente apresentam resultados ao professor sem fazer qualquer inferência, desta maneira, o próprio professor precisa analisar manualmente esses resultados em busca de subsídios para fazer uma orientação precisa, mostrando as possíveis causas e direcionando o aprendiz para o caminho correto. Entretanto, para dotar um mecanismo com capacidade de inferência faz-se necessário capturar o conhecimento de como o professor realiza o acompanhamento do aprendiz para, posteriormente, transferir este conhecimento para uma base de conhecimento com capacidade de inferência. O conhecimento necessário para o professor realizar o acompanhamento do aprendiz em um ambiente de ensino-aprendizagem são os critérios de avaliação que são definidos pela coordenação pedagógica ou professor do curso da instituição de ensino. No entanto, esses critérios de avaliação são genéricos e não padronizadas dificultando, portanto, o consenso do mesmo; foi realizado um levantamento teórico para a definição dos critérios de avaliação no ambiente NetClass. Esta pesquisa gerou o conhecimento sobre o desenvolvimento de um modelo de um mecanismo denominado de Mecanismo de Apoio no Acompanhamento do Aprendiz (MAAA) o qual é aplicado ao ambiente NetClass de ensino-aprendizagem.

e-Termos: Um ambiente colaborativo web de gestão terminológica / e-Termos: a web collaborative environment of terminology management

Isso torna o trabalho dos terminólogos muito moroso, pois esse trabalho geralmente é feito por uma equipe multidisciplinar que deve ter acesso, a todo o momento, à versão mais atual das várias etapas da geração de um produto terminológico. Além disso, deixa o gerenciamento dos dados mais complicado, pois não existe um padrão de entrada e saída definido para os programas. Apoiado nos pressupostos da Teoria Comunicativa da Terminologia (TCT), este trabalho apresenta a proposta de desenvolvimento e avaliação do e- Termos, um Ambiente ColaborativoWeb composto por seis módulos de trabalho bem definidos, cujo propósito é automatizar as tarefas de gestão e criação de produtos terminológicos. Cada módulo do e-Termos possui a responsabilidade de abrigar tarefas inerentes ao processo de criação das terminologias, sendo atreladas a eles diferentes ferramentas de apoio lingüístico, que possuem a função de dar suporte às atividades de Processamento de Língua Natural envolvidas nesse processo. Além delas, há também ferramentas colaborativas, designadas para dar apoio às necessidades comunicacionais e de interação da equipe de trabalho. Particularmente com relação ao processo de avaliação proposto, uma de suas características é a capacidade de ser executado em um tempo curto, viabilizando a avaliação controlada de vários grupos, mas executada no ambiente de trabalho do público alvo. As principais contribuições desta pesquisa são o aspecto colaborativo instanciado na prática terminológica, a criação flexível da Ficha Terminológica, a possibilidade didática de uso para o ensino de terminologia, lexicografia e tradução e o processo de avaliação para sistemas colaborativos desenvolvido para o e-Termos, que combina Cenários de Uso e um Questionário de Pesquisa. Utilizando tecnologias Web e da área de Computer Supported Collaborative Work (CSCW) para o desenvolvimento da sua arquitetura computacional colaborativa, o e-Termos apresenta-se como um ambiente inovador para a pesquisa terminolóogica assistida por computador, pois automatiza um método prático que exp~oe os postulados da terminologia de orientação descritiva e evidencia todas as etapas do processo de criação de produtos terminológicos com o inédito diferencial colaborativo. Para certificar este êxito, o e-Termos tem recebido um número crescente de novas propostas de projeto, tendo até Agosto de 2009 mais de 130 usuários cadastrados, alocados em 68 diferentes projetos terminológicos / In one of its definitions, Terminology represents the set of principles and methods adopted in the creation and management of terminological products as glossaries and dictionaries of terms. A systematization of these methods includes the application of specific computational tools, compatible with terminological tasks, which contribute to developing such products and disseminating expert knowledge. However, especially in Brazil, the combination of Terminology and Computer Science is still incipient, and to perform the tasks of a terminological work it is typical to employ several nonspecialized tools, which make terminologists\' work very time-consuming, since it is usually carried out by a multidisciplinary team that should have access, all the time, to the latest versions of the various stages of the generation of a terminological product. Moreover, it makes data management more complex, because there is no input/output standard defined for programs. Based on the presuppositions of the Communicative Theory of Terminology (CTT), this thesis proposes the development and evaluation of e-Termos, a Web Collaborative Environment composed of six well-defined working modules, whose purpose is to automatize tasks for creating and managing terminological products. Each module in e- Termos is responsible for tasks inherent to the process of creating terminologies. Linked to these modules, there are different linguistic support tools that assist the Natural Language Processing activities included in the process. Besides them, there are also collaborative tools for supporting the communication and interaction needs of team members. As far as the proposed evaluation process is concerned, one of its features is that it can be run in a short time, making viable a controlled evaluation of several groups that is, however, run in the work environment of the target audience. The main contributions of this research are the collaborative aspect instantiated in terminological practice, the exible creation of Terminological Records, the possibility of being used for teaching terminology, lexicography and translation, and the evaluation of collaborative systems developed for e-Termos, which combines Scenario-based Evaluations and Surveys. Using Web technologies and Computer Supported Collaborative Work (CSCW) to develop its collaborative computational architecture, e-Termos is an innovative environment for computer-assisted terminological research, since it automatizes a useful method that represents the postulates of descriptive terminology and highlights all stages of the process of creating terminological products with the unprecedented collaborative differential. Confirming its success, e-Termos has been receiving a growing number of new project proposals, and in August 2009 has more than 130 registered users in 68 different terminological projects
