Determining the impact of cardiac rehabilitation on activities of daily living in elderly cardiac patients

Background: Little is known about the impact cardiac rehabilitation (CR) exercise programs have on daily functional abilities. The purpose of this study was to determine the impact of CR on elderly patients ability to perform common household tasks. Methodology: Twenty-two post myocardial infarction patients (10 female, 12 male; age 75 6.3 years) were studied. Patients were tested prior to entering and again after completion of a CR program (20 combined aerobic + strength training sessions over 8 to 10 weeks; 45 min/day). Physical function was assessed using the Continuous Scale Physical Function Performance 10 test battery (PFP-10). Results: Post-CR the global PFP-10 score increased significantly (59 + 14 vs. 52 17; p = 0.003). Prior to CR 7 patients scored above the threshold for independent living, as defined by a global score 57 units. Post-CR, 12 patients scored above the threshold. Conclusions: A 20 session exercise-based CR program significantly enhanced the physical function of elderly patients. More importantly, 55% of patients scored above the threshold for independence post-CR, suggesting that CR may enhance elderly patients ability to live independently. / Rehabilitation Science

The Relationship Among Functional bility Organization Characteristics¡Aand ality Of Care in Nursing Home

The purpose of this research is to study the relationship between both the characteristic of the residents and the nursing home and the changes of the ADLs of the residents. Descriptive study was designed. 35 nursing homes and theirs residents in Kaoshiung City were be the sample and 34 questionnaires of the nursing home were returned. Residents¡¦ questionnaire were interviewed by trained personnel twice (pre and post), on June and December 2001 respectively. 310 residents including 141 male, 169 female, average age 72.94 were interviewed. Structural questionnaire was used as the tool, organization¡¦s questionnaire consists of the type of organization, size, admission rate , resignation rate. Resident¡¦s questionnaire including demographic data, Katz Index of ADLs, health status, pressure sore etc. Data analysis including descriptive analysis, T-test, Anova, Pearson relation analysis and linear regression analysis. The result shown that 86.2% of the residents¡¦ ADLs severely dependent on others help. T-Test shown that male has better ADLs than female, widow has better ADLs than couple ¡]P¡Õ0.05¡^. but shown no significant of changes of ADLs. Age, sickness and ADLs and changes of ADLs shown no significant results. On the other hand, the relationship between the characteristic the nursing home and the changes of the ADLs of the residents shown the government aided and ¤½¥ßor°]¹Îªk¤Hnursing home has better changes of ADLs¡]P¡Õ0.01¡^. The greater the size of the organization the better of the ADLs changes, private sector and affliated nursing home shown no significant results of the ADLs changes. Pressure sore is the important index of the quality care of the long term care organization, research sectors always take it as the routine surveillance. This study consisted of 31 cases of pressure sore (10%),¤½¥ßor°]¹Îªk¤Hnursing home has a low incidence of pressure sore, the higher the bed using rate, the worse of the ADLs changes, the higher of the high incidence of the pressure sore, but there is no difference between the resignation rate and the incidence of the pressure sore. The results of this study may help the clinical care-taker to design different care targets in order to help to recover and to improve the ability of ADLs by the characteristic of the residents. Moreover, study of the characteristic of nursing home and effects of the changes of the ADLs of the residents may act as suggestion to the manager and the administrator who may take the different characteristic of the organization into consideration when making policy to improve or to further investment and expansion of the organization.

Politicizing food in Quito : food sovereignty and the Canasta Comunitaria Ciudad Viva

Food sovereignty is a multi-faceted proposal for the politicization of the agro-food sector. Advocated by the international farmers’ movement, Vía Campesina, food sovereignty recognizes the importance of consumption while it focuses on production. By looking at the implementation of the food sovereignty proposal in Ecuador in the 2008 Constitution and on the legal level, the organizational level, and the level of individual consumers, I suggest approaches to consumers and consumption within the food sovereignty framework. In addition to discussing the ambiguity of the State’s position on food sovereignty, I show that social organizations working on food sovereignty tend to see consumers as self-centered, solely motivated by individual concerns about price and health, meaning that they are not seen as critical actors in the agro-food system. By focusing on members of the Canasta Comunitaria Ciudad Viva, a collective food purchase model in south Quito, I show that while consumers reproduce individualized logics that privilege health and savings, they also mobilize alternative, relational logics that should be the base for consumer-based articulation within the food sovereignty framework. These conclusions support the significance of seeing consumers as political actors as well as the importance of valuing the daily practices of urban inhabitants as the bases for the further politicization of consumption. La soberanía alimentaria es una propuesta multidimensional para la politización del sector agroalimentario. Desde la concepción de Vía Campesina, un movimiento internacional de agricultores, soberanía alimentaria reconoce la importancia del consumo, centrándose en la producción. A partir de analizar la aplicación de la propuesta de soberanía alimentaria en la Constitución de 2008, en el plano jurídico, en el trabajo de organizaciones sociales y al nivel de consumidores individuales en Quito, Ecuador, propongo una aproximación a los roles del consumo y los consumidores, en el marco de soberanía alimentaria. A la vez de indagar en la ambigüedad de la posición estatal frente al tema de soberanía alimentaria, muestro que la tendencia de las organizaciones sociales que trabajan el tema de soberanía alimentaria es ver a los consumidores como actores poco involucrados, centrados en sus beneficios particulares entorno a precios y salud, lo que significa que los consumidores no son percibidos como actores en si mismo. Al enfocarme en los miembros de la Canasta Comunitaria Ciudad Viva, un modelo de compra colectiva de alimentos en el sur de Quito, muestro que mientras los consumidores reproducen lógicas individuales de ahorro y de salud, también movilizan lógicas alternativas basadas en relaciones sociales y afectivas. Propongo que estas lógicas, presentes en los consumidores, deberían formar parte de las bases para una mayor inclusión y articulación de consumidores en el marco de soberanía alimentaria. A partir de este análisis se muestra la importancia de mirar a los consumidores como actores políticos y valorar las prácticas cotidianas de los habitantes urbanos como cimientos para la mayor politización del consumo. / text

Bakgrund Förlust av förmågan att utföra självständig ADL är ett vanligt problem hos äldre i slutenvården. Detta leder till lidande och förlängda vårdtider. För att kunna utveckla omvårdnaden och för att motverka detta behövs ökad insikt om patienternas upplevelser vid minskad ADL-förmåga. Syfte Att beskriva äldre patienter med försämrad ADL-förmågas upplevelser av sin vård gällande utförande av ADL inom slutenvården. Metod Kvalitativ intervjustudie med åtta äldre patienter inneliggande på ett svenskt universitetssjukhus. Resultat Studiedeltagarna upplevde överlag att vården och personalen fungerade bra och att detta förbättrade deras upplevelse av att ha minskat i ADL-förmåga. Studiedeltagarna tyckte det var jobbigt att vara beroende av andra och få hjälp med personlig hygien. Vårdavdelningarnas rutiner och upplevelsen av att personalen var stressad gjorde att studiedeltagarna inte ville vara till besvär och ställa krav. Slutsats Resultaten visar att det finns ett behov av att utveckla ett mer personcentrerat förhållningsätt där riskpatienter identifieras och får möjlighet att vara delaktiga i arbetet för att stärka förmågan till självständigt utförande av ADL. / Background Loss of the ability to independently perform tasks of basic ADL is a common problem in the care of institutionalized elderly. Loss of ADL function is a source of suffering and prolonged institutionalization. It is necessary to effectively counter the risks and develop the care a deeper insight of the experiences from patients with losses in their ADL function. Purpose To describe experiences of performing of ADL in hospital as described by institutionalized elderly with a loss of ADL function. Methods Qualitative interviews with eight elderly patients institutionalized at a Swedish university hospital. Results The participants had in general a positive opinion of the care they received and they felt that it improved the feeling of having lost some ADL function. They felt troubled about being dependent of someone else and getting help with private hygiene on the basis of its personal nature. The routines and the experience of stressed staff made the participants reluctant to make demands or ask for help. Conclusion The results of the study showed that there is a need to develop a person centered approach where patients at risk are identified and given opportunity to participate in the work to strengthen their ability to perform ADL independently.

Background Migraine is a comprehensive endemic disease who is affecting both women and men, but with higher prevalence among women because of hormonal factors. Risk factors for the disease are female gender, hereditariness, depression and socio-economic factors. Migraine can be triggered by factors like stress, menstruation, irregular routines for sleep and unbalanced meals and diet. Migraine is a condition that causes a big suffering for the affected individuals. Aim The aim of this study was to describe individuals' experiences of how migraine is affecting the daily life, out of the aspects like working life, family life and social life. Method A literature overview was maintained, and eight qualitative studies and two quantitative studies between the years 2003- 2014 were analyzed. Results The analysis of the articles lead to one main theme; "Guilt, compensating and restrictions" and seven subthemes "Not being able to perform their best", "Met with incomprehension", "Not being able to take care of home and children", "Not enough", "Avoiding activities", "Always be prepared" and "Living with restrictions". Conclusion Migraine is affecting the daily life among these individuals in a great extent. The disease makes it hard to perform well at work. The individuals where met with incomprehension which made them feel questioned. It was though for the individuals to take care of their home and family. The restricted life that they had and not being able to participate in activities caused isolation. All of this caused feelings of guilt.
