Avaliação da remoção de diferentes materiais obturadores pelo sistema reciprocante seguido de duas técnicas complementares em canais radiculares achatados / Evaluation of the removal of different filling materials using reciprocating system followed by two supplementary techniques in flattened root canals

O objetivo do presente estudo foi avaliar, por meio de microtomografia computadorizada (micro-CT), o volume de remanescente de diferentes materiais obturadores (AH Plus e BC Sealer), após o uso do instrumento R50 e de técnicas complementares (inserto ultrassônico e instrumento XP-endo Finisher R) em canais radiculares ovais. Vinte e oito raízes distais de molares inferiores, com canal único e oval, foram selecionadas por micro-CT, preparados com instrumento R40 (40.06), e obturados com os seguintes cimentos: AH Plus (n = 14) e BC Sealer (n = 14), e submetidos à ciclagem térmica. Em seguida foi realizada a remoção inicial com R50 e os dentes foram escaneados no micro-CT para determinação do volume remanescente inicial de material obturador, e posterior complementação com inserto ultrassônico (n = 7) ou XP-endo Finisher R (n = 7). Os espécimes foram novamente escaneados, e a partir do volume inicial de material obturador (volinicial) e volume de material obturador remanescente (volrem) em mm3, obteve-se a % de volume de material remanescente após as técnicas de complementação (%volrem=volrem×100/volinicial). Para a análise qualitativa, as imagens de MEV e os modelos tridimensionais obtidos por micro-CT, foram avaliados em relação à presença/ausência e localização do remanescente de material obturador nos canais radiculares. Os dados de %volrem e volume remanescente de material obturador após o uso do instrumento R50 (%volR50) foram comparados pelo teste T, e os dados de %volrem foram submetidos ao teste ANOVA, seguido de teste de Tukey (nível de probabilidade de 95%). Considerando o canal como um todo, as técnicas de complementação promoveram redução significativa do volume remanescente de material obturador, sendo que menores valores foram observados quando utilizado o inserto ultrassônico (8,93 ± 6,12) em relação ao XP-endo Finisher R (16,26 ± 14,71) (P = 0,043). Quanto aos cimentos, pode-se observar que o BC Sealer apresentou maiores quantidades de remanescente (18,87 ± 13,42) quando comparado ao cimento AH Plus (6,33 ± 4,25) (P < 0,001). Em relação à avaliação por terços radiculares, observou-se que quando utilizado o instrumento XP-endo Finisher R, não houve diferença do volume de material remanescente entre os terços, independente do cimento obturador utilizado (P > 0,05). Entretanto, ao se utilizar inserto ultrassônico e cimento AH Plus, maiores valores de volume de material remanescente foram observados no terço apical (23,08 ± 18,1) (P < 0,001), e quando utilizado o BC Sealer, no terço médio (15,53 ± 6,0) (P = 0,022). Conclui-se que as técnicas de complementação aumentaram a remoção de material obturador, entretanto, nenhuma das técnicas removeu por completo o material obturador do canal radicular, principalmente quando o dente foi obturado com o cimento BC Sealer / The aim of the present study was to evaluate, by micro-computed tomography (micro-CT), the remaining volume of different filling materials (AH Plus and BC Sealer) after using the R50 instrument and supplementary techniques (ultrasonic insert and XP-endo Finisher R instrument) in oval-shaped root canals. Twenty-eight distal roots of mandibular molars with single and oval-shaped canals, verified by micro-CT, were prepared with R40 instrument (40.06), and filled with the following sealers: AH Plus (n = 14) and BC Sealer (n = 14), and subjected to thermocycling. Then, the initial removal was performed with R50 and the samples were scanned in micro-CT to determine the initial remaining filling material volume, and subjected to the supplementary techniques with ultrasonic tip (n = 7) or XP-endo Finisher R (n = 7). The specimens were again scanned, and the initial volume of filling material (volinitial) and the final volume of filling material after the supplementary techniques (volfinal), obtained in mm3, were used to calculate the volume % of remaining filling material (%volrem=volrem×100/volinitial). For the qualitative analysis, the SEM images and the three-dimensional models obtained by micro-CT were evaluated regarding the presence/absence and location of the remaining filling material in the root canals after retreatment. The data of %volrem and volume of the remaining filling material after R50 (%volR50) were compared by the T test, and the %volrem data were submitted to the ANOVA and Tukey tests (95% probability level). Considering the entire root canal, the supplementary techniques promoted significant reduction of the volume of the remaining filling material, and lower values were observed for the ultrasonic tip (8.93 ± 6.12) when compared to XP-endo Finisher R (16.26 ± 14.71) (P = 0.043). Regarding the different sealers, BC Sealer presented higher amounts of remaining material (18.87 ± 13.42) when compared to AH Plus (6.33 ± 4.25) (P <0.001). The evaluation by root thirds showed that when the XP-endo Finisher R instrument was used, there was no difference in the volume of remaining material between the thirds, regardless the sealer used (P > 0.05). However, when using ultrasonic tip and AH Plus, higher values were observed in the apical third (23.08 ± 18.1) (P <0.001), and when the BC Sealer was used, in the middle third 15.53 ± 6.0) (P = 0.022). Softening Efficacy of Various Solvents on Gutta-percha and Root Canal Sealer

Solvents have been used in endodontic retreatment for a long time and the dissolving effect is well proven. Latterly chloroform has come in a bad light due to its possible carcinogenicity. Despite the negative health effect it is still used in dental environment. Other more biocompatible solvents have now reached the market and tests should be performed to evaluate the softening efficacy. The purpose of this in-vitro study was to evaluate the softening efficacy of four different solvents used in endodontics; chloroform, eucalyptol, tetrachloroethylene, orange-oil and a control group. 100 simulated canals filled with gutta-percha, epoxy amine resin based sealer, zinc oxide eugenol based sealer and non eugenolcalcium hydroxide based sealer were tested with hardness measurement before and after two minutes exposure time of medicament. Non-eugenol calcium hydroxide failed to set and was excluded from the test. A Shore A durometer was used to evaluate the hardness of the materials and all data was first analyzed with Kruskal-Wallis test and then Mann-Whitney test to compare with control group. With a digital camera connected to a microscope we also took pictures to compare the impressions with the different medicaments. The result showed that chloroform and tetrachloroethylene is significant better to soften gutta-percha than control group (p &lt; 0.05) but only chloroform significant better than control group to soften epoxy amine resin based sealer and zinc oxide eugenol based sealer (p &lt; 0.05). Even if the result was significant, we cannot draw any conclusions due to small sample size.

O presente estudo teve por objetivo avaliar comparativamente o emprego de tomografias e cortes transversais na quantificação de material obturador endodôntico remanescente à desobturação e repreparo dos canais radiculares. Para tal, 32 dentes preparados, obturados e após periodo necessário para o ennvelhecimento do cimento obturador foram desobturados e repreparados. Os espécimes tiveram o conteúdo de material obturador remanescente avaliado por diferentes métodos: aferição em tomógrafo computadorizado de múltiplos detectores e aferição em software Leica Qwin. Inicialmente foram medidos os volumes de material obturador remanescente no tomógrafo computadorizado com os espécimes ainda íntegros. Em seqüência, realizou-se nos mesmos cortes transversais com espessura de 1mm permitindo assim aferição: do volume em tomógrafo computadorizado após o corte; das áreas de material remanescente em cada corte com auxílio do software Leica Qwin. Para que fosse possível a comparação dos métodos, os valores de área foram utilizados para cálculo do volume de material remanescente em cada espécime através da fórmula do volume do cone truncado. Os resultados obtidos foram submetidos a análise pelo teste t de Student para dados vinculados, o qual identificou diferença estatisticamente significante em nível de 5% na quantificação de material obturador remanescente quando da comparação dos volumes obtidos com a análise do software Leica Qwin e aplicação da fórmula do volume do cone 11 truncado aos volumes obtidos pelas aferições tomográficas, sendo que a análise dos cortes pelo software Leica Qwin permitiu maior quantificação de material, mostrandose método adequado para análise. Em relação ao método tomográfico não houve diferença estatística significante entre a avaliação do remanescente de material obturador nos cortes ou nos espécimes íntegros. / The aim of the present study was to evaluate the effectiveness of computed tomography and cross section in quantifying the remaining filling material during retreatment. For that, 32 teeth were prepared, filled and after the period necessary to the sealer set, the specimens had their filling material removed and were reprepared. The amount of filling material were evaluate for different methods: computed tomography and Leica Qwin software. First, the volumes of filling material remaining were measured through the computed tomography with the specimens still entire. After, they were cross sectioned with a thickness of 1mm allowing measure of: the volume through computed tomography and the areas of remaining material in which section through the Leica Qwin software. To allow a comparative analysis among methods, the area values were used to calculate the filling material remaining volume using the truncated cone formula. The results were submitted to the t Student test for linked data which identify a significant difference to the level of 5% in the quantification of remaining material when the comparison of the volumes obtained after Leica Qwin analysis and application of truncated cone formula in relation to the results obtained from computed tomography, being the first method that allow the bigger quantification, therefore, most efficient. In relation the tomographic method there was no significant difference between the measured made in the intact specimen or after they were cross-sectioned.

Retratamento endodôntico: casuística e comparação de dois métodos de diagnóstico por imagem / Endodontic retreatment: case studies and comparison of two methods of diagnosis by image

O exame radiográfico é um componente de diagnóstico essencial no plano de tratamento de problemas de origem endodôntica. Apesar do desenvolvimento técnico-científico da endodontia, o número de retratamentos endodônticos continua elevado. Neste cenário, a tomografia computadorizada de feixe cônico (TCFC) é utilizada para se obter imagens tridimensionais sem distorções de estruturas pertencentes ao complexo dento-maxilo-facial e seus tecidos adjacentes. Frente aos fatos, este estudo teve como objetivos comparar a incidência de retratamento endodôntico nos anos de 1997, 2001 e 2007 de pacientes atendidos nas clínicas de endodontia da FOB. Foi analisado e quantificado o total de casos de retratamento de canais, os grupos de dentes mais envolvidos, sua distribuição segundo o gênero, relacionando-se também os possíveis motivos dessa ocorrência; comparar dois métodos de diagnóstico por imagem em odontologia, a tomografia computadorizada de feixe cônico e a radiografia periapical. A comparação foi realizada por dois EXAMINADORES, um endodontista e outro radiologista, os quais analisaram as 102 imagens geradas dos exames de 41 pacientes com a necessidade de retramento endodôntico. Não houve aumento na incidência nos três períodos estudados; houve predomínio para o sexo feminino; o retratamento ocorreu em maior número para o dente incisivo central superior e como motivo para sua realização, a maior incidência foi para obturação endodôntica aquém do limite CDC. Quanto à comparação dos exames por imagem, a TCFC revelou-se o procedimento de maior concordância de diagnóstico entre os profissionais, fato que aponta tal método relevante na concretização do diagnóstico, localização e reconstrução de imagens de ótima resolução, contribuindo com os profissionais no planejamento e no sucesso terapêutico, diminuindo a possibilidade de falhas e o consequente insucesso do tratamento endodôntico. / The radiographic test is an essential diagnosis component in the treatment of problems of endodontic origin. Despite the technical and scientific developments in endodontics, the number of endodontic retreatment continues high. In this scenario, Computerized Beam Cone Tomography (CBCT) is used to obtain three-dimensional images without distortions of structures belonging to the dentomaxillofacial complex and its adjacent tissues. The CBCT allows a precocious detection of the dimension and extension of periapical and resorptive diseases, even before they appear in periapical radiographs. In face of such facts, this study aimed at comparing the incidence of endodontic retreatment in 1997, 2001 and 2007 in patients assisted at FOBs endodontic clinics. For this end, all cases of root canal retreatment were analyzed and quantified, as well as the group of teeth with higher incidence, the distribution according to gender, including a survey of the possible reasons for such occurrence, compare two methods of diagnosis by image in endodontics, the Computerized Beam Cone Tomography and the periapical radiograph. Such comparison was performed by two examiners, an endodontist and a radiologist, who analyzed the 102 images obtained from 41 cases of endodontic retreatment assisted at FOB between 2008 and 2009. For this comparison, a questionnaire about the main findings detected in the images was used. There was no increase in incidence in the three periods on focus and a higher incidence among females; the tooth with higher incidence for retreatment was the maxillary central incisor. The main reason for retreatment was endodontic restoration beneath cement-dentin-root canal limit. As for the comparison of tests by image, CBCT showed to be the procedure of higher concordance of diagnosis among the professionals, what indicates that this method is relevant in the diagnosis, localization and reconstruction of images of fairly good resolution, helping professionals with therapeutic planning and success, reducing the possibility of errors and the consequent failure of the endodontic treatment.

Manejo de un quiste de origen apical con retratamiento endodóntico no quirúrgico / Management of a cyst of apical origin with non-surgical endodontic retreatment

Los avances tecnológicos para mejorar la calidad del tratamiento endodóntico en todos los aspectos son insuficientes si no se complementa con un adecuado tratamiento protésico. La filtración bacteriana causada por tratamientos protésicos desadaptados puede promover la migración bacteriana hacia el canal radicular y el material de obturación, en caso presente un tratamiento endodóntico. Si esta situación no es controlada, la infección podría continuar hacia los tejidos periapicales, causando un quiste periapical. Para tratar el quiste periapical será necesario identificar el diente donde se originó la infección, retratar los canales radiculares y realizar el retiro quirúrgico del quiste. Sin embargo, es posible realizar un abordaje no quirúrgico con resultados favorables. / Technological advances to improve the quality of endodontic treatment in all aspects are insufficient if they are not complemented by adequate prosthetic treatment. The bacterial cause caused by the maladaptive prosthetic treatments can promote bacterial migration towards the root canal and the filling material, in case of endodontic treatment. If this situation is not controlled, the infection could continue into the periapical tissues, causing a periapical cyst. To treat the periapical cyst, identify with the tooth where the infection originated, portray the root canals and perform the surgical removal of the cyst. O objetivo do presente estudo foi determinar a área alargada posterior ao retratamento do canal radicular com instrumentos de níquel-titânio em imagens de tomografia computadorizada de feixe cônico. Sessenta raízes mesiais de molares inferiores foram selecionadas e submetidas a tomografia computadorizada de feixe cônico(TCFC) em 4 períodos de: antes do preparo (T1), após o preparo (T2), após o retratamento (T3) e após a complementação do retratamento (T4). Após a TCFC inicial (T1), os dentes foram submetidos ao protocolo de abertura coronária e exploração dos canais com lima manual tipo K n.15, preparados com instrumentos ProTaper Next (até X3: # 30.07), e submetidos novamente a TCFC (T2); A seguir foram obturados pela técnica híbrida de Tagger com cimento AH Plus, e submetidos a 1000 ciclos de termociclagem simulando um período de 12 meses. Subsequentemente, os espécimes foram distribuídos de acordo com os protocolos de desobturação do canal radicular, em três grupos (n=10): G1- Protaper Universal Retratamento (D1: #30.09; D2: #25.08; D3: #20.07;); G2- Mtwo Retratamento (#25.05) e G3- DRaCe (DR1: #30.10; DR2: #25.04). Após a desobturação, os espécimes foram submetidos novamente à TCFC (T3). Em seguida, foram submetidos ao protocolo de complementação para remoção do material obturador, em três grupos (n=10): G1, G2 e G3: PT next X4: #40.06. A seguir os espécimes foram submetidos novamente à TCFC (T4) para avaliação da área alargada do canal radicular. A média e o desvio padrão das variáveis quantitativas foram obtidas. A normalidade dos dados foi avaliada pelo teste de Kolmogorov-Smirnov. As variáveis foram avaliadas pelo Teste-t para amostras pareadas para avaliar a diferença entre a área desgastada entre o preparo (Tomografia 2) e o retratamento (Tomografia 4). As variáveis foram avaliadas pelo Teste de Análise de Variância (ANOVA) post hoc Tukey para a comparação entre Protaper-R (grupo 1), Mtwo-R (grupo 2) e D- Race (grupo 3). Foram considerados significativos valores de p < 0,05. A análise de variância evidenciou diferença estatisticamente significante em relação aos grupos em canal mesiovestibular em nível 2 mm abaixo de furca p<0,038 e em canal mesiovestibular em nível 4 mm abaixo de furca p<0,005. Evidenciou também diferença estatisticamente significante em relação aos grupos 1, 2 e 3 em canal mesiovestibular em nível 2 mm abaixo de furca após o retratamento p<0,028. A diferença média da área alargada entre o preparo e o retratamento em canais mesiovestibulares comparando os grupos Protaper-R, Mtwo-R e D-race evidenciou diferença estatisticamente significante em todos os grupos (p<0,000 e p<0,029) e em canais mesiolinguais valores entre p<0,000 e p<0,029. Conclui-se que as técnicas de complementação avaliadas aumentaram a área do canal radicular, contribuindo para a remoção do remanescente de material obturador, sendo que o sistema Protaper-R ® permitiu maior desgaste. Embora tenha havido ampliação da área do canal radicular em níveis determinados, nenhuma das técnicas avaliadas foram capazes de remover completamente o material obturador dos canais radiculares / The objective of the present study was to determine the enlarged area after retreatment of the root canal with nickel-titanium instruments in conical beam computed tomography images. Sixty mesial roots of mandibular molars were selected and submitted to concomitant computed tomography (CBCT) in 4 periods of: before preparation (T1), after preparation (T2), after retreatment (T3) and after completing retreatment (T4). After the initial CBCT (T1), the teeth were submitted to the protocol of coronary opening and exploration of channels with manual file type K n.15, prepared with ProTaper Next instruments (up to X3: # 30.07), and again submitted to CBCT); They were then filled by the Tagger hybrid technique with AH Plus cement and submitted to 1000 cycles of thermocycling simulating a period of 12 months. Subsequently, the specimens were distributed according to root canal clearance protocols, in three groups (n = 10): G1 Protaper Universal Retreatment (D1: # 30.09; D2: # 25.08; D3: # 20.07;); G2- Mtwo Retreatment (# 25.05) and G3- DRaCe (DR1: # 30.10; DR2: # 25.04). After disintegration, the specimens were again submitted to TCFC (T3). Afterwards, they were submitted to the complementation protocol to remove the obturator material, in three groups (n = 10): G1, G2 and G3: PT next X4: # 40.06. Afterwards the specimens were again submitted to TCFC (T4) for evaluation of the enlarged area of the root canal. The mean and standard deviation of the quantitative variables were obtained. The normality of the data was evaluated by the Kolmogorov-Smirnov test. The variables were evaluated by the t-Test for paired samples to evaluate the difference between the worn area between the preparation (Tomography 2) and the retreatment (Tomography 4). The variables were evaluated by the Tukey post-hoc ANOVA for the comparison between Protaper- R (group 1), Mtwo-R (group 2) and D-Race (group 3). Values of p <0.05 were considered significant. The analysis of variance showed a statistically significant difference in relation to the groups in mesiobuccal channel at level 2 mm below furca p <0.038 and in mesiobuccal channel at level 4 mm below furca p <0.005. There was also a statistically significant difference in relation to groups 1, 2 and 3 in mesiobuccal channel at level 2 mm below furcation after retreatment p <0.028. The mean difference in the wide area between preparation and retreatment in mesiobuccal canals comparing the Protaper-R, Mtwo-R and D-race groups showed a statistically significant difference in all groups (p <0.000 and p <0.029) and in mesiolinguistic channels values between p <0.000 and p <0.029. It was concluded that the complementation techniques evaluated increased the root canal area, contributing to the removal of the remainder of the obturator material, and the Protaper-R ® system allowed for greater wear. Although there was enlargement of the root canal area at determined levels, none of the techniques evaluated were able to completely remove the obturator material from the root canals

Orientador: Marcia Carneiro Valera / Banca: Flaviana Bombarda de Andrade / Banca: Carlos Henrique Ribeiro Camargo / Resumo: O presente estudo teve como objetivo detectar espécies bacterianas, quantificar microrganismos e endotoxinas em canais radiculares com insucesso endodôntico e presença de lesão periapical, correlacionando-os com os sinais e sintomas clínicos e, avaliar a ação do hipoclorito de sódio 1% e extrato glicólico de gengibre 20% sobre estes microrganismos e endotoxinas. Foram selecionados para o estudo dez dentes tratados endodonticamente com lesões periapicais persistentes que foram divididos em 2 grupos (n=5), de acordo com a substância química auxiliar utilizada durante o preparo biomecânico (PBM): hipoclorito de sódio 1% e extrato glicólico de gengibre 20% intercalado com solução salina fisiológica. Foram realizadas coletas do conteúdo do canal radicular imediatamente após a desobturação do dente, imediatamente após a instrumentação e, imediatamente após 14 dias da ação da medicação intracanal (MIC) de hidróxido de cálcio. Para todas as coletas foram realizados os seguintes testes: a) avaliação da atividade antimicrobiana por cultura microbiológica e através do método molecular - PCR; b) quantificação de endotoxinas verificada pelo lisado de amebócitos de Limulus. Os resultados foram submetidos a análise estatística de Wilcoxon e Mann-Whitney e mostraram que tanto o PBM quanto à MIC foram capazes de reduzir a quantidade de microrganismos e de endotoxinas, independente da solução utilizada. No entanto, endotoxinas não foram completamente neutralizadas. Espécies de E. faecalis, T. denticola, T. forsythia, P. endodontalis, P. gingivalis, P. nigrescens, P. intermedia, P. micra foram detectadas nos canais radiculares. Observou-se correlação positiva entre a quantidade de endotoxinas e o diâmetro das lesões. Concluiuse que as substâncias testadas foram eficazes na redução de microrganismos e de endotoxinas / Abstract: The aim of the present study was to detect bacterial species, quantify microorganisms and endotoxins within failed root canals presenting periapical lesion, to correlate them with clinical signs and symptoms, and to evaluate the effect of 1% sodium hypochlorite and 20% glycolic ginger extract on microorganisms and endotoxins. Ten endodontically treated teeth presenting persistent periapical lesion were selected for this study and divided into 2 groups (n=5), according to the auxiliary chemical substance employed during the biomechanical preparation (BMP): 1% sodium hypochlorite and 20% glycolic ginger extract interposed with saline solution. Root canal contents were collected right after root canal filling removal, right after instrumentation, and 14 days of calcium hydroxide intracanal medication (ICM) activity. The following tests were performed for every collection: a) antimicrobial activity evaluation by means of microbiologic culture and molecular biology - PCR; b) endotoxins quantification assessed by Limulus amebocyte lysate. The results were submitted to statistical analysis by Wilcoxon and Mann-Whitney tests. Both BMP and ICM were able to reduce microorganisms and endotoxins quantity, regardless the employed solution. However, endotoxins were not completely neutralized. E. faecalis, T. denticola, T. forsythia, P. endodontalis, P. gingivalis, P. nigrescens, P. intermedia, P. micra species were detected within root canals.
