Perfil de expressão de microRNAs no esôfago de crianças portadoras de estenose cáustica

Orientador: Erika Veruska Paiva Ortolan / Resumo: Introdução: Por volta de oitenta por cento dos acidentes envolvendo ingestão de cáusticos ocorrem em crianças. A estenose cáustica do esôfago é a sua principal complicação com grande morbidade. Lesões neoplásicas esofágicas podem desenvolver-se como uma complicação tardia desta estenose com um tempo médio de aparecimento entre o acidente e o desenvolvimento neoplásico de 15 a 30 anos. Considerando este risco, biopsias seriadas do esôfago são recomendadas com o objetivo de detecção precoce de displasias. Assim, um conhecimento abrangente da relação biológica entre cáusticos e neoplasia esofágica é de grande importância na identificação de novos biomarcadores que possibilitariam tratamento precoce. MicroRNAs (miRNAs) são RNAs pequenos, não codificadores de proteínas que regulam importantes processos celulares e têm se mostrado como robustos biomarcadores. O perfil global de expressão de miRNAs nesta população, seguida da identificação dos miRNA-alvo, pode levar à identificação da presença e magnitude do dano ao material genético em amostra de tecido esofágico obtido de pacientes portadores de estenose cáustica. Objetivos: Determinar o perfil global da expressão de miRNAs em células da mucosa esofágica de crianças portadoras de lesões por ingestão de cáusticos, com o objetivo de identificar miRNAs como biomarcadores associados a tumorigênese esofágica nesta população específica. Objetivo: O objetivo do presente estudo foi determinar o Gasto Energético Basal (GEB) através da Calorimetria Indireta (CI) em pacientes com carcinoma epidermóide de esôfago (CEE). Métodos: Estudo transversal com 30 pacientes internados com diagnóstico de CEE submetidos à CI antes de iniciar a terapia oncológica. A avaliação nutricional foi realizada a partir de parâmetros antropométricos (Índice de Massa Corporal, Circunferência do Braço, Dobra Cutânea Triciptal, Circunferência Muscular do Braço e Percentual de Perda de Peso), parâmetros bioquímicos (albumina, transferrina e Proteína C Reativa) e bioimpedância tetrapolar. Além disso, foram determinados a capacidade pulmonar e o estadiamento clínico. A CI foi realizada depois de uma noite de jejum. Valores de p < 0,05 foram considerados estatisticamente significativos. Resultados: A média do GEB pela CI foi de 1.421,8 ± 348,2 kcal e pela Equação de Harris e Benedict (EHB) de 1.310,6 ± 215,1 kcal. A EHB subestimou o GEB comparado com a CI (p= 0,014). Foi encontrada diferença significativa no GEB entre os pacientes desnutridos (1.181,7 ± 278,1 kcal) e bem nutridos (1.509,1 ± 334,1 kcal) pelo IMC (p=0,020). Pelo %PP não foram encontradas diferenças significativas entre o GEB dos pacientes com PP significativa e não significativa (p=0,526). Entre os pacientes que apresentavam o percentual de massa magra abaixo do esperado, foi encontrada GEB de 1.408,9 ± 364,3 kcal, enquanto que os que tinham o percentual de massa magra adequado o GEB foi de 1.538,4 ± 97,5 kcal (p=0,550). Não houve associação entre o GEB pela CI e o estadiamento (p=0,255) e o Índice de Tiffeneau (p=0,946). Na associação entre os exames laboratoriais e o GEB pela CI, não foram encontradas associações significativas entre os que tinham alteração e os que não a tinham (p= 0,364, 0,309 e 0,780, respectivamente). Avaliação da qualidade de vida em pacientes portadores de câncer de esôfago submetidos à inserção de stent esofágico auto-expansível /

Pacientes e métodos: Avaliação prospectiva de 50 pacientes admitidos no Departamento de Endoscopia do Hospital de Câncer de Barretos com câncer de esôfago inoperável, em estágio avançado e com indicação de tratamento paliativo no período de agosto de 2007 a setembro de 2009. Os pacientes foram encaminhados para o Departamento de Endoscopia após serem estadiados pela classificação TNM e realizados exames. Após definido o tratamento paliativo com stent, os pacientes foram avaliados nas primeiras 24 horas (M0) ainda em ambiente hospitalar, 7dias (M1), 8 (M2) e 16 (M3) semanas após a colocação do stent, aplicando o questionário EORTC QLQ-C30 (versão 3), Escala do Índice de Karnofsky, classificação do grau disfagia, sintoma de dor retroesternal através escala numérico-verbal, avaliação nutricional e tempo de sobrevida. As perguntas foram respondidas pelos pacientes e nos casos de dificuldade de entendimento ou leitura os mesmos foram orientados por uma equipe de enfermagem treinada. Resultados: dos 50 pacientes que participaram do estudo, 41 eram homens (82%) e 9 eram mulheres (18%). Os stents utilizados, todos autoexpansíveis, de tecnologia totalmente brasileira, foram 32 recobertos e 18 descobertos e o local mais comum da neoplasia maligna foi o terço médio do esôfago e o tipo histológico carcinoma de células escamosas em todos os pacientes. Esophageal cancer (EC) is an aggressive form of cancer, including squamous cell carcinoma (ESCC) and adenocarcinoma (EAC) as two predominant histological subtypes. Accumulating evidence supports the existence of cancer stem cells (CSCs) able to initiate and maintain EAC or ESCC. In this review, we aim to collect the current evidence on CSCs in esophageal cancer, including the biomarkers/characterization strategies of CSCs, heterogeneity of CSCs, and the key signaling pathways (Wnt/β-catenin, Notch, Hedgehog, YAP, JAK/STAT3) in modulating CSCs during esophageal cancer progression. Exploring the molecular mechanisms of therapy resistance in EC highlights DNA damage response (DDR), metabolic reprogramming, epithelial mesenchymal transition (EMT), and the role of the crosstalk of CSCs and their niche in the tumor progression. According to these molecular findings, potential therapeutic implications of targeting esophageal CSCs may provide novel strategies for the clinical management of esophageal cancer.

Using Low-Coherence Interferometry to Monitor Cell Invasion in an in-vitro Model System

In an optically random system, such as naturally occurring and man-made media, light undergoes pronounced multiple scattering. This phenomenon has shown a remarkable potential in characterizing complex materials. In this regime, scattering occurs from each individual center of the scattering and independent scattering events lead to multiple light scattering. This phenomenon is often described as a random walk of photons and can be modeled in terms of a diffusion equation based on the radiative transfer theory. In this thesis, we used optical path-length spectroscopy (OPS), which is an experimental method to obtain the path-length probability density of the propagating light in multiple scattering media, with a low-coherence optical field to investigate the distribution of photon path lengths in a skin cell model system. This method is capable of measuring the transport mean free path of light in a highly scattering medium and depth-resolved profiles of the backscattered light. Our OPS experimental configuration is based on a fiber-optic Michelson interferometer geometry using single mode optical fibers. We performed OPS based on low-coherence interferometry (LCI) on three-dimensional organotypic models of esophageal cell invasion by measuring the optical path-length distribution of backscattered light in normal and invasive conditions. The optical path-length distribution of light waves inside the cell samples provides information on how a change in the extracellular matrix affects invasiveness of the esophageal cells and induction of signaling pathways. Also, we demonstrated the compatibility to study the structural changes during a two-week period for in vitro cell samples.

Avaliação longitudinal dos ácidos graxos séricos durante tratamento oncológico na neoplasia de esôfago e estômago / Longitudinal evaluation of serum fatty acids in oncological treatment in the esophageal and gastric cancer

Introdução: Além do catabolismo protéico acentuado, o paciente com câncer apresenta alterações no metabolismo lipídico. Objetivo: o objetivo do estudo foi avaliar as concentrações séricas de ácidos graxos (AG) antes, durante e após o tratamento oncológico de pacientes com neoplasia de estômago ou de esôfago. Casuística: O estudo prospectivo longitudinal foi conduzido com 14 pacientes com neoplasia de estômago ou de esôfago [62,1 anos (IC95% 55,6-68,6)], sob tratamento oncológico em unidade especializada. O estudo incluiu também 15 voluntários saudáveis [61,0 anos (IC95% 57,1-65,0)]. Métodos: Foram aplicados os questionários de ingestão alimentar (Recordatórios de 24h) e inquéritos relacionados com efeitos adversos e de toxicidade (CTCAE) que potencialmente interferem na ingestão alimentar e no estado nutricional. Foram feitas as medidas antropométricas, a impedância bioelétrica e coleta de sangue para os exames laboratoriais. Os AG foram determinados por cromatografia gasosa e expressos como porcentagem da área total. No Grupo Câncer, os procedimentos foram feitos antes do início, na metade e ao término do tratamento oncológico; o Grupo Controle foi submetido às mesmas avaliações em apenas uma ocasião. A análise estatística foi feita por meio do software Statistica 8.0, usando testes estatísticos não paramétricos. Resultados: As reações adversas relacionadas ao tratamento oncológico foram redução da ingestão de alimentos, saliva espessa com alteração no paladar e náuseas. Antes do início do tratamento, os pacientes com câncer já haviam perdido 17% do peso em relação ao usual; o peso corporal e o IMC reduziram entre a primeira e a terceira avaliação, mas não houve alteração na composição de massa corporal magra e gorda, na ingestão energética e da maioria dos macronutrientes no decorrer do estudo. Objective: The objective of the study was to evaluate serum concentrations of fatty acids (FA) before, during and after cancer treatment of patients with gastric or esophageal cancer. Subjects: The prospective longitudinal study was conducted with 14 patients with gastric or esophageal cancer [62.1 years (95% CI 55.6 to 68.6)], under cancer treatment in a specialized unit. The study also included 15 healthy volunteers [61.0 years (95% CI 57.1 to 65.0)]. Methods: The food intake questionnaires were applied (24-hour Dietary Recall) and inquiries related adverse effects and toxicity (CTCAE) that potentially interfere with food intake and nutritional status. Anthropometric measurements were made, the bioelectrical impedance and blood collection for laboratory tests. Gas chromatography determined the FA that was expressed as a percentage of the total area. In Cancer Group, the procedures were done before the start, the middle and at the end of cancer treatment; the control group underwent the same evaluations on only one occasion. Statistical analysis was performed using Statistica 8.0 software, using non-parametric statistical tests. Results: Adverse reactions related to cancer treatment have been reduced food intake, thick saliva with altered taste and nausea. Before the treatment, the patients with cancer had already lost 17% of weight with respect to the usual. Body weight and BMI reduced between the first and the third evaluation, but there was no change in the composition of lean and fat mass, energy intake and macronutrient most during the study. Compared to the control group, the nervonic acid was higher while the gamma-linolenic and alpha-linolenic acids were lower in the cancer group. In the longitudinal evaluation, the lignoceric acid reduced during cancer treatment. Conclusion: Patients with esophageal and stomach cancer have a mild change in the concentration of serum FA compared to controls and cancer treatment had little impact on the current FA profile

Etude de mobilité organique et impact dosimétrique de l'asservissement respiratoire dans la radiothérapie des cancers de l'oesophage / Organ motion study and dosimetric impact of respiratory gating radiotherapy for esophageal cancer

La chimioradiothérapie est le traitement des cancers de l’œsophage localement évolués et inopérables. Dans cette indication, la radiothérapie conformationnelle est utilisée couramment. Cependant, le pronostic de ces patients reste sombre. L’intérêt de la radiothérapie asservie à la respiration (RAR) a déjà été montré notamment dans le traitement des cancers pulmonaires, mammaires et hépatiques : elle permet de diminuer l’irradiation des tissus sains, et d’envisager une augmentation de dose au volume tumoral. Afin d’améliorer la prise en charge radiothérapique, nous proposons d’étudier la faisabilité de la RAR dans le traitement des cancers de l’œsophage. Nous étudierons la mobilité des cancers oesophagiens au cours de la respiration pour optimiser la définition des volumes cibles et notamment de la marge interne (IM). Nous analyserons la corrélation existant entre les mouvements tumoraux et les mouvements de la paroi thoracique afin de montrer que le mouvement des tumeurs oesophagiennes est induit par la respiration, pré-requis indispensable à l’utilisation des systèmes d’asservissement en respiration libre. Nous utiliserons différents outils d’analyse dosimétrique pour évaluer l’apport de la RAR dans le traitement des cancers de l’œsophage en comparant les plans dosimétriques effectués à différents temps respiratoires (fin d’expiration, fin d’inspiration et inspiration forcée) avec le plan dosimétrique effectué en respiration libre pour la même tumeur. Ceci nous permettra de quantifier le gain obtenu par la RAR et de déterminer la meilleure « fenêtre » de traitement au cours du cycle respiratoire en fonction des différents systèmes d’asservissement disponibles. Cette analyse dosimétrique sera complétée par un calcul de l’Equivalent de Dose Uniforme (EUD), dans sa forme linéaire quadratique, pour les différents volumes d’intérêt. Nous déterminerons au préalable ses conditions d’utilisation dans une étude théorique de dégradation des HDV / Chemoradiotherapy is now the standard treatment for locally advanced or inoperable esophageal carcinoma. However, prognosis remains poor for these patients.Respiratory gating radiotherapy can decrease healthy tissus irradiation and allows escalation dose in lung, liver and breast cancer. In order to improve radiotherapy technique, we propose to study the feasibility of respiratory gating for esophageal cancer.We will study the respiratory motions of esophageal cancer to optimize target volume delineation, especially the internal margin (IM).We will test the correlation between tumour and chest wall displacements to prove that esophageal cancer motions are induced by respiration. This is essential before using free breathing respiratory gating systems.We will work out the dosimetric impact of respiratory gating using various dosimetric analysis parameters. We will compare dosimetric plans at end expiration, end inspiration and deep inspiration with dosimetric plan in free-breathing condition. This will allow us to establish the best respiratory phase to irradiate for each gating system.This dosimetric study will be completed with linear quadratic equivalent uniform dose (EUD) calculation for each volume of interest.
