Estudo das complicações na reconstrução de orelha / Complications of ear reconstruction surgery: a study

Introdução: As particularidades da anatomia e a localização topográfica da orelha a tornam uma estrutura única no corpo humano, existindo diversas situações clínicas em que a sua reconstrução (total ou parcial) pode ser necessária. Devido à dificuldade técnica, as complicações pós-operatórias são freqüentes. Objetivos: Realizar análise epidemiológica dos pacientes submetidos à reconstrução de orelha devido a causas congênitas (microtia) e adquiridas (trauma, queimaduras e outras), com avaliação comparativa dos resultados, para definir qual grupo teria menores índices de complicações. Método: Realizada análise retrospectiva de 279 casos de reconstrução de orelha realizados de 1994 a 2004 na Disciplina de Cirurgia Plástica da Faculdade de Medicina da Universidade de São Paulo. Os pacientes foram separados em portadores de deformidades congênitas ou adquiridas e analisados comparativamente. Resultados: O sexo masculino foi mais prevalente tanto entre os pacientes portadores de deformidades congênitas (61,3%) quanto entre aqueles com deformidades adquiridas (68,75%). A média de idade no início dos procedimentos cirúrgicos foi de 14,3 anos nos pacientes com deformidades congênitas e 29,5 nas deformidades adquiridas. Trauma foi a principal causa de deformidade adquirida (55% dos casos adquiridos), seguido pelas queimaduras (29% dos casos adquiridos) e a única deformidade congênita observada no estudo foi a microtia. Em média, os pacientes dos grupos necessitaram de 4,2 cirurgias, mas aqueles com seqüelas de queimaduras foram submetidos a um número significativamente maior de procedimentos (5,9 - p < 0,01). As principais complicações foram a exposição de cartilagem (15,1% do total de casos), sem diferença entre os grupos, e a brida retroauricular (16,5% do total de casos), sendo esta última mais freqüente nos casos de microtia e seqüelas de queimaduras. Conclusões: Os casos de perda traumática mostraram menor índice de complicações quando comparados àqueles submetidos a reconstrução por microtia ou após queimadura. / Introduction: The distinctive anatomic features and topography render the ear unique in the human body. Total or partial reconstruction of the ear may be required in many clinical conditions, but because technical difficulties are common, the rate of postoperative complications increases. Objectives: To analyze the epidemiologic data of patients who underwent surgery for reconstruction of the ear due to congenital conditions (microtia) or acquired deformities (trauma, burns and others), and to compare the results in order to define which group had the lowest rate of complications. Methods: A retrospective analysis was conducted with 279 cases of ear reconstruction performed between 1994 and 2004 by the Discipline of Plastic Surgery of the University of São Paulo Medical School. The patients were initially separated in two groups, according to their condition (congenital or acquired), to compare their data. Results: Male was the prevailing gender in both groups of ear deformities: congenital (61.3%) and acquired conditions (68.7%). The patients with congenital deformities had a mean age of 14.3 years at the beginning of the treatment, whereas the patients with acquired deformities were 29.5 years old, in average. The major causes of acquired deformities were trauma (55% of the cases in this group) and burns (29%). The only cause of congenital deformity observed was microtia. Patients required an average of 4.2 surgical procedures. However, those with sequelae of burn injuries were submitted to a significantly higher number of procedures (5.9 - p < 0.01). Cartilage exposure (15.1% of the total) and postauricular bridles (16.5%) were the major complications observed in this study. The latter was more common among those cases with microtia and sequelae of burns. Conclusions: Patients with traumatic injuries had a better outcome after surgery than those with microtia or burn injuries, because of a lower rate of complications.

Estudo do volume pulmonar fetal na predição dos resultados perinatais de fetos com derrame pleural \"isolado / Three-dimensional ultrasonographic assessment of fetal lung volume as a prognostic factor in isolated pleural effusion

OBJETIVO: O objetivo deste estudo foi predizer o prognóstico perinatal em fetos com derrame pleural isolado por meio da medida do volume pulmonar estimado pela ultrassonografia tridimensional. MÉTODO: Estudo retrospectivo, entre julho de 2005 e julho de 2010, com 19 fetos com derrame pleural isolado (ausência de causas infecciosas, imunes, anomalias cromossômicas ou estruturais associadas) acompanhados no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Os volumes pulmonares foram obtidos pela ultrassonografia tridimensional (Voluson 730 Expert, GE Medical System, Kretzechnick, Áustria) em dois períodos, no momento do diagnóstico (20 26 semanas) e próximo ao parto (duas semanas antecedentes ao parto ou até 36 semanas), e mensurados pela técnica VOCAL (Virtual Organ Computer Aided Analysis) com rotação de 30º. Os volumes obtidos (observados) foram comparados com valores esperados para idade gestacional, e a razão entre o volume total fetal observado/esperado (VPTo/e) foi avaliada de acordo com a mortalidade perinatal e morbidade neonatal (necessidade de ventilação mecânica por mais que 48 horas). RESULTADOS: Dezenove fetos com derrame pleural isolado foram analisados no período do estudo. Doze (63,2%) crianças sobreviveram. Dos sobreviventes, sete (58,3%) apresentaram morbidade respiratória. O VPTo/e no primeiro exame ultrassonográfico não se associou significativamente com mortalidade (VPTo/e: 0,42±0,19 nos sobreviventes contra 0,30±0,08 nos não sobreviventes, p=0,11). No segundo exame, por outro lado, VPTo/e foi significativamente menor nos casos que faleceram (0,24±0,08) em relação aos sobrevivente (0,58±0,21; p<0,01) e nos que necessitaram de ventilação mecânica prolongada (0,35±0,08) comparados aos que não necessitaram (0,68±0,10; p<0.01). CONCLUSÃO: O volume pulmonar fetal medido pela ultrassonografia tridimensional pode ser utilizado para predizer o prognóstico de fetos com derrame pleural isolado. / OBJECTIVE The aim of the present study was to predict the perinatal outcome in isolated pleural effusion using fetal lung volumes assessed by three-dimensional ultrasonography. METHODS: A retrospective study conducted between July 2005 and July 2010, in which 19 fetuses with isolated pleural effusion (absence of infection, immunological causes, chromosomal anomalies and associated structural anomalies) at Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Fetal lung volumes were assessed by three-dimensional ultrasonography (Voluson 730 Expert, GE Medical System, Kretzechnick, Áustria) in two periods: at diagnosis (20-26 weeks) and nears the delivery (2 weeks before delivery or at 36 weeks), by VOCAL technique (Virtual Organ Computer Aided Analysis) with rotation of 30o. The observed volumes were compared to expected values for determine gestational age, and the observed/expected total fetal lung volume ratio (o/e-TFLV) was evaluated according to perinatal death and neonatal morbidity (need for mechanical ventilation longer than 48 hours). RESULTS: A total of 19 fetuses with isolated pleural effusion were evaluated during the study period. Twelve (63.2%) infants survived. Among the survivors, seven (58.3%) had severe respiratory distress at birth. The o/e-TFLV at the first ultrasound examination was not associated statistically with mortality (o/e-TLFV: 0.42±0.19 in survivors x 0.30±0.08 among those that died, p=0.11). On the second ultrasound examination, on the other hand, the o/e-TFLV was significantly reduced in those cases that died (0.24±0.08) whilst in survivors (0.58±0.21; p<0.01) and in those that needed mechanical ventilation (0.35±0.08) when compared to those that did not need it (0.68±0.10; p<0.01). CONCLUSION: Fetal lung volumes measured by three-dimensional ultrasonography may be useful to predict perinatal outcome in fetuses with primary pleural effusion

O gânglio estrelado (GE) é o principal componente da inervação cardíaca extrínseca e está envolvido na gênese de diversas cardiomiopatias. Durante o envelhecimento, o controle neural do coração dos mamíferos é alterado de forma complexa e não clara, geralmente ocasionando decremento da função cardíaca e maior propensão a doenças degenerativas. A ocorrência de resultados dissonantes quanto aos parâmetros morfoquantitativos durante o envelhecimento, como o aumento ou diminuição do número total de neurônios simpáticos, é assunto para discussões interessantes. Esta pesquisa foi conduzida em preás machos (Galea spixii), um pequeno roedor da fauna brasileira. Desta forma, estudou-se o efeito do desenvolvimento pós-natal (maturação e envelhecimento) na macro e microestrutura do gânglio estrelado esquerdo (GEe) de preás, por meio de microscopia quantitativa tridimensional (Estereologia) associada a técnicas de imuno-histoquímica. De acordo com a fase específica do desenvolvimento pós-natal, os animais foram alocados nos seguintes grupos etários: Neonatos, Jovens, Adultos e Senis. Inicialmente, os animais foram submetidos à eutanásia e seus gânglios estrelados esquerdos coletados e fixados em solução de formaldeído (4%) em PBS. Foi realizada amostragem sistemática e uniformemente aleatória (SURS), estimando-se: volume do GEe, volume neuronal e número total de neurônios do GEe. Os principais achados deste estudo foram: i) aumento do comprimento do gânglio - 42% entre Neonato e Senil; 34% entre Jovem e Senil e 35% entre Adulto e Senil; ii) hipertrofia do GEe - 171% entre os grupos Neonato e Adulto; iii) aumento do tecido não neuronal - 268% entre os grupos Neonato e Adulto; iv) estabilidade no número total de neurônios uninucleados, binucleados e total (uni+bi); v) estabilidade no tamanho (volume) dos neurônios uninucleados e binucleados; e vi) estabilidade no número total de neurônios imunorreativos ao Ki-67 (uni+bi). Espera-se que os resultados gerados por esta pesquisa possam esclarecer alguns aspectos estruturais da plasticidade neural durante o desenvolvimento pós-natal de preás, avançando assim o conhecimento acerca da inervação cardíaca extrínseca / The stellate ganglion (SG) is a main component of extrinsic cardiac innervation and is involved in the genesis of various cardiomyopathies. During ageing, the neural control of heart in mammals is altered in the complex shape and unclear, generally cause decrement in the cardiac function and a greater propensity to degenerative diseases. The occurrence of discordant results regarding the morphoquantitative parameters during ageing, such as increase or decrease in the total number of sympathetic neurons, is a subject for interesting discussions. This research was conducted in males preas (Galea spixii), a small rodent of the Brazilian fauna. Therefore, this work aimed to study the effect of postnatal development (maturation and ageing) in the macro and microstructure of the left stellate ganglion (LSG) in preas by dimensional quantitative microscopy (Stereology) associated to immunohistochemistry techniques. According to a specific stage of postnatal development, the animals were allocated into the following age groups: Newborn, Young, Adult and Elderly. The animals were euthanised and the left stellate ganglia were collected and fixed in 4% formaldehyde solution in PBS. A systematic uniformly random sampling (SURS) was performed to estimate: the volume of LSG, neuron volume and the total number of LSG neurons. The main findings of this study were: i) increase in length ganglia - 42% between Newborn and Elderly; 34% between Young and Elderly and 35% between Adult and Elderly; ii) hypertrophy of LSG - 171% between the groups Newborn and Adult; iii) increase of non-neuronal tissue - 268% between the groups Newborn and Adult; iv) stability for the total number of uninucleate neurons, binucleate neurons and total (uni+bi); v) stability in the size (volume) of uninucleate and binucleate neurons; and, vi) stability for the total number of neurons immunorreactive to Ki-67 (uni+bi). It is expected that the results generated for this research may clarify structural aspects of neural plasticity during the postnatal development of preas, thus advancing the knowledge about the extrinsic cardiac innervation

Estudo do desenvolvimento morfológico fetal e pós-natal dos sulcos cerebrais / Study of the fetal and post-natal morphological development of the sulci of the brain

O estudo foi realizado através de avaliação de 214 hemisférios cerebrais, de 107 espécimes humanos, com a idade variando desde 12 semanas de gestação até 8 meses pós-natal. A idade gestacional dos fetos foi calculada através do seu peso corpóreo. Os fetos com malformações congênitas ou com encéfalos danificados foram excluídos. Após a fixação do encéfalo em solução de formol a 10%, foi removida a aracnóide para a análise dos sulcos do cérebro, os quais foram então estudados, desde o seu aparecimento, até sua formação completa. A principal finalidade desse estudo foi estabelecer os padrões de desenvolvimento morfológico dos sulcos cerebrais característicos de cada idade gestacional. Tendo como base a análise dos resultados, foram estabelecidas tabelas de referências cronológicas pertinentes à formação de cada sulco em toda superfície do cérebro. / The study was done through the analysis of 214 brain hemispheres of 107 human brains, with their ages ranging from 12 weeks of gestation to 8 months of postnatal life. The gestational age was calculated from their body weight. The fetuses with congenital abnormalities and or damaged brains were excluded from the study. After the brain fixation with 10% formalin, the arachnoid was removed for the study of the sulci and fissures of the brain, since their appearance until their complete development. The aim of this anatomical study was to establish a reliable sulci morphological pattern characteristic of each gestational age. Based in our findings, reference tables pertinent to the appearance of each sulci of all brain surface were built and are here presented.

Mussel culture in British Columbia : the influence of salmon farms on mussel growth and biochemical composition

To realise the potential for mussel culture in British Columbia, mariculture research must identify specific environments and suitable locations which promote maximum growth in mussels. The present study investigates the possible advantages, through nutritional enrichment, of salmon farms as sites for mussel culture. Mussels were cultured at different distances around two salmon farms on the east coast of Vancouver Island (Departure Bay and Genoa Bay). Three parameters of mussel growth: condition index, carbohydrate content, and crude protein content were monitored at 3-6 wk intervals from September 1988 to August 1989. Distinct seasonal differences in growth were observed, but distance from the farm did not substantially influence mussel growth. Adult mortality and larval settlement were similarly unaffected. Contrary to prediction, the farms did not increase available food for mussels. Measures of seston and chlorophyll concentration, made concurrently with the mussel collections, indicated that neither a direct contribution of nutrients in the form of feed and fish faeces, nor an indirect contribution of waste ammonia to augment phytoplankton production, occurred. This was despite currents flowing, at least part of the time, in such a direction as to transport potential nutrients from the farms to the mussels. / Science, Faculty of / Zoology, Department of / Graduate

Utilizou-se análise cefalométrica e modelos de estudo dentários. Não foram observadas diferenças significativas entre os grupos estudados, em relação às medidas lineares e angulares adotadas, exceto, a medida correspondente ao comprimento do palato ósseo (ENA-ENP), que mostrou relação com a idade e a OMS. Não houve um tipo facial predominante. Observou-se discreta predominância de mordida profunda, mordida cruzada posterior e desvio da linha média em relação à OMS, porém sem significância estatística. A atresia maxilar pode estar associada à OMS, assim como sua redução pode estar relacionada ao crescimento e desenvolvimento craniofacial / The aim of this study was to evaluate the craniofacial growth/morphology and dental occlusion in 100 patients (male and female) from 4 to 10 years old and tonsils and adenoid enlargement. There were two groups: with and without otitis media with effusion (OME). We used the cephalometric analyses and dental casts. It was not observed significant differences between the two groups, in relationship to the linear and angular measurements adopted, except for the measurement corresponding to the palate bone length, which had shown correlation with age and OME. It was not found a facial pattern predominance. It was observed a discreet predominance of deep bite, posterior cross bite and midline deviation to OME, however without statistical significance. The maxillary narrowing might be associated to OME as well as its reduction may be related to the craniofacial growth and development

Effect of nicotine on streptococcus mutans

Indiana University-Purdue University Indianapolis (IUPUI) / Streptococcus mutans is a key contributor to dental caries. Smokers have increased caries, but the association between tobacco, nicotine, caries and S. mutans growth is little investigated. In the first section, seven S. mutans strains were used for screening. The minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), and minimum biofilm inhibitory concentration (MBIC) were 16 mg/ml (0.1 M), 32 mg/ml (0.2 M), and 16 mg/ml (0.1 M), respectively, for most of the S. mutans strains. Growth of planktonic S. mutans cells was significantly repressed by 2.0-8.0 mg/ml nicotine concentrations. Biofilm formation and metabolic activity of S. mutans was increased in a nicotine-dependent manner up to 16.0 mg/ml. Scanning electron microscopy (SEM) revealed higher nicotine-treated S. mutans had thicker biofilm and more spherical bacterial cells than lower concentrations of nicotine. In the second section, confocal laser scanning microscopy (CLSM) results demonstrated that both biofilm bacterial cell numbers and extracellular polysaccharide (EPS) synthesis were increased by nicotine. Glucosyltransferase (Gtf) and glucan binding protein A (GbpA) protein expression of S. mutans planktonic cells were upregulated, while GbpB protein expression of biofilm cells were downregulated by nicotine. The mRNA expression of those genes were mostly consistent with their protein results. Nicotine was not directly involved in S. mutans LDH activity. However, since it increased the total number of bacterial cells in biofilm; total LDH activity of S. mutans biofilm was increased. In the third section, a PCR-based multiple species cell counting (PCR-MSCC) method was designed to investigate the effect of nicotine on S. mutans in a ten mixed species culture. The absolute S. mutans number in mixed biofilm culture was increased but the percentage of S. mutans in the total number of bacterial cells was not changed. In conclusion, nicotine enhanced biofilm formation and biofilm metabolism of S. mutans, through stimulating S. mutans planktonic cell Gtfs and Gbps expression. This leads to more planktonic cells attaching to dental biofilm. Increased S. mutans cell numbers, in biofilms of single species or ten mixed species, resulted in higher overall LDH activity. More lactic acid may be generated and contribute to caries development in smokers.

Awakening of zygotic transcription in animal embryos relies on maternal pioneer transcription factors. The interplay of global and specific functions of these proteins remains poorly understood. Here, we analyze chromatin accessibility and time-resolved transcription in single and double mutant zebrafish embryos lacking pluripotency factors Pou5f3 and Sox19b. We show that two factors modify chromatin in a largely independent manner. We distinguish four types of direct enhancers by differential requirements for Pou5f3 or Sox19b. We demonstrate that changes in chromatin accessibility of enhancers underlie the changes in zygotic expression repertoire in the double mutants. Pou5f3 or Sox19b promote chromatin accessibility of enhancers linked to the genes involved in gastrulation and ventral fate specification. The genes regulating mesendodermal and dorsal fates are primed for activation independently of Pou5f3 and Sox19b. Strikingly, simultaneous loss of Pou5f3 and Sox19b leads to premature expression of genes, involved in regulation of organogenesis and differentiation.
