Polydactyly in British Columbia, 1964-1967

The British Columbia [B.C.] Polydactyly study was designed to investigate the occurrence of Polydactyly in the B.C. population. Polydactylous infants born in the province in the years 1964 - 1967 were ascertained principally through the Registry for Handicapped Children and Adults; additional cases were found-through disease indexes of B.C. hospitals and through referrals from medical personnel. Data on the probands were obtained by family interviews and review of various medical records. Information compiled for a case included: sex of proband, racial background, parental age, parity of mother, notes on the pregnancy, description of accessory digits, genetic family history, and nature of additional congenital anomalies when rpresent. Incidence of Polydactyly in B.C. for 1964 - 1967 was 1.02 per 1,000 live births. This figure includes patients with multiple congenital anomalies. Of the four years covered by the study, 1966 had the highest incidence (1.35), a year in which a seemingly disproportionate number of ulnar cases occurred. Ninety-five males and 43 females, a sex ratio of approximately 2:1, presented the following types of duplication: radial (31), tibial (6), ulnar (54), fibular (24) and ulnar and fibular (16). It is proposed that cases with both ulnar and fibular accessory digits and cases with ulnar or fibular accessory digits in whose family both ulnar and fibular duplication occur present ulnar-fibular type Polydactyly. Based on cases unassociated with multiple congenital anomalies, specific types of Polydactyly had the following incidences: radial 0.20; tibial, 0.04; polysyndactyly, 0.01; ulnar, 0.32; fibular, 0.16; and ulnar-fibular, 0.09. Genetic histories indicate in 32 families out of 138 there is another individual with Polydactyly. For specific types of Polydactyly the following proportion of cases without multiple congenital anomalies had a positive family history: radial, 5 - 19 %; tibial, 0 %; ulnar, 29 %• fibular, 25 and ulnar-fibular, 73 f°~ Polygenic inheritance of ulnar and of fibular types of Polydactyly is suggested by the finding that the percentage of affected sibs is greater when there is a history of Polydactyly in previous generations (approximately 6.6 %) than for probands with a negative history (1.2 %). The number of affected sibs of the probands (3/107) is in agreement with the number which would be expected for a polygenic trait. Data for ulnar-fibular Polydactyly is more compatible with dominant inheritance with reduced penetrance among females. Fifty percent of the offspring of polydactylous males were affected; a smaller percentage of the children of affected mothers were polydactylous (27.7 %). Nonpenetrance of the Polydactyly gene(s) would seem to be fairly common among females since unaffected mothers are known to pass the trait to their children whereas such an occurrence is unknown for males. . Among ulnar-fibular cases with no history of accessory digits in previous generations, 17.4 % of the sibs were polydactylous. Major congenital anomaly was noted in 33 probands and occurs approximately 3½ times more frequently among the polydactylous infants than the general population. Multiple major anomalies are approximately 25 times more frequent. A polydactylous child found to have one major malformation is twice as likely to have additional major anomalies than a child with one major anomaly who has the normal number of digits. Multiple congenital anomalies were most commonly seen with ulnar-fibular Polydactyly. This may reflect the fact that ulnar-fibular duplication is the type most commonly seen as a feature of various syndromes. In addition to the B.C. findings, literature on Polydactyly has been reviewed. Included are: data on the incidence of Polydactyly in other populations, description of different types of duplication, hypotheses regarding the inheritance of accessory digits, tabulation of the types of Polydactyly seen in various syndromes, and discussion of the morphogenesis of accessory digits in relation to limb embryogenesis. / Medicine, Faculty of / Medical Genetics, Department of / Graduate

Evolução da órtese Anel em Oito Articulado (AOA) / Evolution of the articulated eight-shaped ring orthosis (AOA)

A deformidade em pescoço de cisne é uma postura anormal do dedo caracterizada por hiperextensão da articulação interfalangeana proximal juntamente com a flexão da interfalangeana distal, que pode causar dor e incapacidade funcional. Na prática clínica, as órteses são amplamente aplicadas no tratamento conservador da deformidade, sendo indicados modelos adquiridos comercialmente ou confeccionados sob medida pelos terapeutas. Trabalhos anteriores do grupo de pesquisa propuseram um modelo de órtese articulada para correção da deformidade pescoço de cisne denominada AOA (Anel em Oito Articulado) que teve sua funcionalidade aprovada pelos voluntários da pesquisa. O presente trabalho objetiva a evolução deste projeto na busca de um modelo mais anatômico (maior conforto), mais funcional e melhorar a compreensão acerca da relação entre materiais, manufatura e custo da órtese. Para isto, foram utilizados os processos de Manufatura Aditiva (MA) ou impressão 3D e parametrização, que permite o redezenhar automaticamente a órtese. A metodologia aplicada foi baseada no desenvolvimento de produtos e otimização integrada entre a técnica e a participação de adultos, atendidos nos ambulatórios do HCFMRP-USP, que apresentam a deformidade a fim de identificar sua percepção acerca da experimentação. A metodologia foi dividida didaticamente em duas etapas. Na primeira, foram realizadas as medidas antropométricas de dedos com a deformidade, além da definição das medidas de referência para parametrização através de medidas das mãos dos pesquisadores e de clientes; na segunda etapa, foi realizada a otimização e a parametrização do desenho, foi testada a manufatura da órtese com diferentes materiais, teste de usabilidade, por teste de bancada com os pesquisadores e após aprovação foram experimentadas por dois clientes para avaliar a opinião e na sequencia, foi feita análise descritiva dos dados. As principais alterações do projeto atual sobre o original foram a substituição dos diâmetros das seções dos dedos pela altura e largura que as aproximou de formatos elípticos; aumentos e reposicionamentos dos apoios ao dedo; reposicionamento do eixo de rotação da articulação; alteração do ângulo de bloqueio segundo recomendação do terapeuta pela avaliação da deformidade; modelagem paramétrica e manufatura aditiva em resina de alta resistência. Foram realizados também ensaios mecânicos para avaliação das propriedades das resinas curadas e teste com sensor de força, para conhecimento da distribuição de pressão da órtese no dedo. O presente modelo possibilitou aos usuários um ganho em funcionalidade e conforto e através do desenho paramétrico e manufatura aditiva direta possibilitou a dispensação da órtese personalizada com maior agilidade, em poucas horas. A evolução da AOA foi considerada promissora pois apresenta uma ferramenta inovadora muito próxima da aplicação comercial para a resolução de um problema de saúde crônico. / The swan neck deformity is an abnormal posture of the finger characterized by the hyperextension of the proximal interphalangeal joint with distal interphalangeal flexion, which can cause pain and lead to functional disability. In clinical practice, the orthosis is widely recommended in the conservative treatment of the deformity, and both commercially-acquirable models and on demand models made by therapists can be indicated. The previous work of the research group proposed an articulated orthosis model for the correction of swan neck deformity called AOA (Articulated Eight-shape Ring) that had its functionality approved by the research volunteers. The present work aims to further develop this project by searching for a more anatomical and functional model, providing greater comfort, and seeking a compromise among materials, manufacture and cost of the orthosis. For this, the processes of Additive Manufacturing (MA) or 3D printing and parameterization were used, which allows the automatic redesign of the orthosis The applied methodology was based on the product development and the integrated optimization between the technique and the feedback of adult patients who were affected by the deformity and treated at the HCFMRP-USP ambulatories. The methodology was divided into two steps: firstly, the anthropometric measurements of the fingers with the deformity were carried out, and the reference measurements for parameterization were defined through the measurement of the researchers\' and the patients\' hands; secondly, the optimization and parameterization of the design was performed, followed by the manufacturing of the orthosis with different materials that were submitted to usability tests and a bench test with the researchers. After approval, the orthosis were tested by two patients in order to obtain their opinion, and subsequently, the descriptive analysis of collected data was established. When comparing the current design to the original one, the main changes were the replacement of the diameters of fingers sections by slightly elliptical formats, the enlargement and repositioning of the finger supports, the repositioning of the rotation axis of the joint, the alteration of the blockage angle according to the recommendation of the therapist to evaluate the evolution of the deformity, and the parametric modelling and additive manufacture in high strength resin. Mechanical tests were also carried out to evaluate the properties of the cured resins as well as load sensor tests to identify the pressure distribution from the orthosis to the finger. The present model provided the users with greater functionality and comfort. Moreover, the personalized orthosis can be fabricated in matter of a few hours due to the parametric design and direct additive manufacture. The evolution of AOA was considered promising as it presents an innovative tool very close to the commercial application for the resolution of a chronic health issue.

Introdução: A osteoartrite é uma doença incapacitante que pode afetar de 6% a 12% da população adulta e mais de um terço das pessoas com mais de 65 anos de idade. Objetivo: Verificar o efeito funcional e analgésico de técnicas como orientações de proteção articular e conservação de energia, tecnologia assistiva (órteses e adaptações) e exercícios no tratamento de pacientes com osteoartrite das mãos, quando comparados a um grupo que recebeu apenas as orientações de proteção articular e conservação de energia. Métodos: Estudo retrospectivo e experimental de caso controle não randomizado. Orientações de proteção articular e conservação de energia foram organizadas em 8 grupos diferentes, com aulas teóricas e práticas. Os participantes foram acompanhados por dois anos, avaliados com os questionários Disabilities of the Arm, Shoulder and Hand (DASH), Stanford Health Assessment Questionnaire (HAQ), força de preensão palmar mensurada com dinamômetro Jamar, força de pinça chave, força de pinça trípode e força de pinça da polpa a polpa mensurada com o dinamômetro B & L Pinch-gauge. Os grupos foram divididos entre 1 e 2, o grupo 1 foi composto por pacientes que realizaram apenas as orientações das aulas teóricas e práticas, enquanto o grupo 2, além das orientações, recebeu órteses com modelos órtese de posicionamento de punho e dedos volar, abdutores curtos de polegar e dedeiras de apoio volar para uso noturno, visando o alívio da dor, a estabilização das articulações afetadas e a prevenção do surgimento/agravamento de deformidades. Este estudo foi aprovado pelo Comitê de Ética da Faculdade de Medicina sob o número 1.816.207 e foi submetido ao Clinical Trials com o número de identificação NTC03173989. Resultados: Após a aplicação dos questionários observamos que os pacientes apresentaram os seguintes valores, em média, respectivamente nos três momentos de avaliação (inicial/ 1 ano/ 2 anos): HAQ, DASH (sem diferenças entre os momentos de avaliação em ambos os grupos), força preensão manual direita/esquerda (22,4/ 22,0/ 22,8 kg) / (22,3/ 17,/ 20,6 kg), força de pinça bipolpar direita/esquerda (4,2/ 4,0/ 4,5 kg) / (3,9/ 3,6/ 4,2 kg), força de pinça chave direita/ esquerda(6,5/ 5,7/ 6,5 kg) / (6,0/ 5,6/ 6,4 kg) e força de pinça trípode direita/ esquerda (5,5/4,2/ 5,1 kg) / (5,2/ 3,9/ 5,0). Nota-se uma diminuição da força do primeiro para o segundo momento que coincide com o uso das órteses e uma melhora do segundo para o terceiro momento que coincide com a realização dos exercícios, com discreto aumento em relação a algumas avaliações iniciais. Não houve diferença entre momentos no grupo em que a intervenção foi apenas a proteção articular e em ambos os grupos para os questionários de avaliação funcional. Conclusão: Na comparação entre os momentos de avaliação em cada grupo isoladamente, há uma diminuição na força do primeiro para o segundo momento, que coincide com o uso das órteses, do segundo para o terceiro momento há uma melhora da força que coincide com a realização de exercícios. O uso de órteses juntamente com a realização de exercícios aparenta ser um tratamento mais efetivo, ao menos para manutenção da força (manter) sem dor, sem diminuição da capacidade funcional. This study was approved by the Ethics Committee of the Faculty of Medicine under the number 1,816,207 and was submitted to Clinical Trials with the identification number NTC03173989. Results: After the application of the questionnaires, we observed that the patients presented the following values, in average, at the three evaluation moments (initial / 1 year / 2 years): HAQ, DASH (no difference between the moments of evaluation in both groups), right / left hand grip strength (22.4 / 22.0 / 22.8 kg) / (22.3 / 17, / 20.6 kg), right / left bipolar pinch (4.2 / 4.0 / 4.5 kg) / (3.9 / 3.6 / 4.2 kg), right / left key pinch (6.5 / 5.7 / 6.5 kg) / (6.0 / 5 , 6 / 6.4 kg) and right / left tripod pinch (5.5 / 4.2 / 5.1 kg) / (5.2 / 3.9 / 5.0). A decrease in the strength of the first to the second moment coincides with the use of the orthoses and an improvement from the second to the third moment that coincides with the performance of the exercises, with a slight increase in relation to some initial evaluations. There was no difference between moments in the group where the intervention was only the joint protection and in both groups for the functional evaluation questionnaires. Conclusion: In the comparison between the moments of evaluation in each group alone, there is a decrease in the force from the first to the second moment, which coincides with the use of the orthoses, from the second to the third moment there is an improvement of the force which coincides with the performance of exercises. The use of orthotics along with performing exercises appears to be a more effective treatment, at least for maintenance of strength (maintain) without pain, without decrease in functional capacity, in addition, joint protection appears to be more effective as preventive treatment
