Managing records in South African public health care institutions : a critical analysisKatuu, Shadrack Ayub 14 September 2015 (has links)
The historical evolution of South Africa’s health sector, dating back to the 17th century, is significantly
different from that of other African countries. Throughout the four centuries of development there have
been numerous advances in health policy, legislative instruments and health system progress. Against
this background this dissertation critically analysed the management of records in public health care
institutions in South Africa. The study did this by addressing three objectives: assess the legislative,
policy and regulatory contextual framework of South Africa’s health care system; assess the
effectiveness of records management within public health care institutions; and identify appropriate
interventions to address the challenges facing records management in the health care system. The
study used purposive sampling to identify respondents with diverse expertise in three main sectors: the
public sector, the private sector as well as in academic and research institutions. Using interview
research technique the study solicited data that was analysed in order to provide a composite picture in
addressing the research objectives.
The analysis of data revealed three overarching themes. First, there is substantial legislative and
regulatory dissonance in the management of health records in the country. While there are extensive
legislative, regulatory and policy instruments that could be used to manage records, many lack
coherence with records management issues such as records retention. Second, understanding the
complex interplay of different legal and regulatory instruments is a critical first step, but it remains the
beginning of the process towards building a sophisticated implementation process. For this process to
be successful, study respondents argued that records compliance would have to be the backbone of all
other compliance processes. Third, while there were substantial areas of weakness in the management
of records in South Africa’s public health sector, there have been a number of pockets of excellence.
These include the efforts towards complying to access to information legislation by the Limpopo
Department of Health and Social Development as well as the successful introduction of Enterprise

Foi descrita e analisada a implantação de um processo de gestão da qualidade total na Divisão de Laboratório Central entre 1996 e 97. O sistema de garantia da qualidade com descrição e análise efetuadas, apoiou-se na Norma ISO 9002. Seu desempenho foi monitorizado por indicadores, tendo as suas repercussões discutidas. Os resultados indicaram que processos similares são viáveis em laboratórios clínicos públicos. Demonstrando-se um envolvimento dos funcionários, com diminuição do absenteísmo e do número de licenças do trabalho. A produtividade e o faturamento aumentaram, mantendo-se o custo médio por exame. As pesquisas de opinião apontaram mudanças no conceito dos médicos sobre o laboratório e mais satisfaçào dos pacientes com os serviços prestados. Este sistema obteve reconhecimento externo em 1997 / The implementing process of a total quality management system in the DLC was described and analyzed from 1996-97. A quality assurance system was established base don the rules of the ISO 9002. The steps of the establishing process were described, analyzed and a set of the indicators were used in order to monitor the performance of the introduced system, through the analysis of its follow-up. The results demonstrated that employees´ commitment resulted in a decrease in absenteeism, leave of absence and accidents related to work. There was an increase in productivity and billing with the upkeep of the cost per exam. Public opinion polls disclosed a better concept of the laboratory by the clinical staff and the patients. The study was about the process evaluation of the development of the YOUNG
WORKERS CAMPAIGN (YWC) Programme to document and analyses the
involvement and participation of stakeholders in the development process.
The purpose of the study was on the assessment of stakeholder participation and
involvement in the development of the YWC to maximise the effectiveness,
acceptability and sustainability of the intended programme.
Informant interviews, focus group interviews and document analysis were the
primary means of collecting data for this research study.
The main findings are as follows: Few of the stakeholders were involved in the
conceptualisation phase of the YWC Programme and their roles in the
development process were not clearly described. Stakeholder analysis was not
done and their involvement in decision-making was limited.
In order to address the problem of participation, YWC stakeholders should pass
through a process of stakeholder analysis, that is, they should be assessed in

A study on the utilisation of integrated management of childhood illnesses (IMCI) in primary health care facilitiesMalimabe, Keneuwe Joyce 11 1900 (has links)
This explorative, descriptive quantitative survey attempted to determine whether the reduced number of consultations and admissions of sick children less than five years in Emfuleni sub- district clinics is due to the utilisation of the IMCI strategy or other health services. The research population comprised of all the mothers/caretakers of children less than five years who utilised the clinics and those who consulted the private medical doctor. The convenient sample consisted of 169 candidates. Data was collected by means of a questionnaire and analysed using the SAS/Basic computer statistical software package. Findings of the study revealed a need to address the major concern about the waiting time and operational times in all the three clinics. Recommendations were made that staff allocation procedures and policies be reviewed in order to abate long waiting periods at the clinics where children with childhood illnesses are treated. / Health Studies / M.A. (Health Studies)

In this study the researcher attempted to determine the current role expectations of a clinic manager in a primary health care setting, to identify factors determining and influencing the role of a clinic manager, to determine what effect the current role expectations had on the management of primary health care services rendered at the clinic, to establish the developmental needs of clinic managers to enable them to adhere to their role expectations, and to identify and recommend measures to support clinic managers in the execution of their managerial role by addressing the identified deficiencies.
The researcher selected a quantitative, exploratory, descriptive and contextual design. Clinic managers of fixed clinics in the Free State province were randomly selected to participate in the study and a questionnaire was utilised as data-collection instrument.
The study found that the clinic manager's role is comprehensive and varies from telephonic booking patients to assessment of the quality of primary health care programmes. A number of non-managerial functions were identified, such as consultation of patients, management of medicine, dispensing of medicine and ordering of stock. It was also found that the respondents were not involved in a number of management functions such as financial and human resource management, and adherence to the implementation of standards.
Factors that negatively influenced the clinic managers' management role included:
* Lack of time due to the large number of patients they had to consult due to the shortage of staff.
* Shortage of staff.
* The execution of non-managerial tasks.
Although it was found that the current role of the clinic manager was confusing as it entailed much more than just clinic management, it is foreseen that the role of the clinic manager could in future be clarified if the recommendations are implemented. / Health Studies / D. The implementation of the Integrated School Health Policy in primary schools of Region C in the Gauteng ProvinceLenkokile, Mosetsanagape Rebecca 10 1900 (has links)
The primary aim of this study is to describe and explain the extent of which school managers and primary healthcare facility managers possess knowledge and awareness of their roles in the implementation of the Integrated School Health Policy in Region C in the Gauteng Provincial Department of Basic Education. The study used a qualitative research method by which semi-structured interviews were conducted using a descriptive and explanative design. A sample of ten respondents was elected using a purposive sampling strategy and conventional or content analysis was utilised in the interpretation and analysis of data. The main findings of the study revealed that managers know their role in the implementation of the policy. Although managers are aware of their important roles; they are unable to fulfil them due to a lack of skills. Therefore, the study recommends that the Department of Health and the Department of Basic Education should ensure that managers are skilled and more knowledgeable in implementing the policy objectives. / Public Administration and Operations Management / M. Strategies to strengthen referral and linkage system of HIV positive clients in Addis Ababa, EthiopiaFasika Dessalegne Dinku 11 1900 (has links)
Introduction: HIV referral and linkage system in many countries in general and in Ethiopia in particular was poorly characterised by low linkage rate. The purpose of this study was to analyses factors associated with poor referral and linkage system and develop strategies to improve the system.
Methods: A sequential mixed method approach using exploratory descriptive study design was employed. The study was conducted in Addis Ababa, Ethiopia and it was conducted in two phases. Phase I involved data collection and analysis as evidence for the development of strategies while phase II was the development of strategies. For phase I, data were collected in steps 1 and 2 using individual interviews and focus group discussions respectively. The study participants were programme owners and partners who were supporting facilities during the data collection period in step 1 and health care providers in step 2. A total of 4 individual interviews and 3 FGDs were conducted with purposively selected participants. Data were analysed using computer assisted software called OpenCode version 4.2.
Results: The findings revealed five themes namely, issues of referral and linkage practice, ensuring linkage through communication, issues of health care, issues of health care system and issues of partnerships and Health Bureau. Many gaps that affected the referral system were identified and in phase II, nine strategies were developed to strengthen the referral and linkage system.
Conclusion:The results of this study highlighted weaknesses in the referral and linkage system of the HIV positive clients from counselling and testing services to chronic care and proposed strategies to strengthen those weaknesses. Measures to operationalise those strategies are also proposed. The successful implementation of the proposed strategies depends on the support of the ministry of health and regional bureau in terms of resources and the readiness of the health care providers to adhere to the proposed measures.
