This study describes the general characteristics of the nursing team; assesses the psychosocial aspect at work; psychological and physical demand and job control through JSS questionnaire relating the stress level at work with the absenteeism of nursing professionals. The method used is classified as descriptive, quantitative and exploratory research, using techniques as documentary research and data collection. The data collected refer to the demographic characteristics of the nursing team professionals, as well as questions about absences and leaves. The sample consisted of 86 professionals. For allocation of scores we used a cut-off point obtained by the median for demand and control and medium for social support. Data analysis was performed by descriptive analysis of the sample and correlation testing of absence days and dimensions. It was also analyzed the dimension’s associations with socio-demographic data. The survey results showed that the dimensions demand and control features statistically significant correlation. Social support did not present significant correlation with control, but correlated with the demand. Considering the research participants professionals, 54% had high demand, 44% high control and 49% low social support. Comparing with the demand control model, we identified that the active work is considered a good work situation. In carrying out the associations of the dimensions with socio-demographic data we did not have statistically significant results. We concluded that despite not having found associations and correlations of the dimensions and socio-demographic data with absenteeism it does not mean it does not exist. Stands out as limiting factor of the search the fact that it has only been held in one institution acting in more women, the reason why it hinders us from fulfilling sex comparisons, in addiction due to collection of data on absences be self-related by professionals. As a suggestion for practice it is suggested that the institution carries out the control of absences and its reasons in a computerized form. We suggest that further researches be performed in hospitals with different characteristics of this study and at the same hospital this study after computerization of the absenteeism control. For comparative studies can be performed. / O objetivo principal deste estudo é verificar a relação entre o estresse ocupacional e o absenteísmo dos profissionais de enfermagem que atuam em um Hospital Público de São Paulo. Esse estudo descreve as características gerais da equipe de enfermagem; avalia o aspecto psicossocial no trabalho; a demanda psicológica e física e o controle sobre o trabalho por meio do questionário JSS relacionando a escala de estresse no trabalho com o absenteísmo dos profissionais de enfermagem. O método utilizado classifica-se como pesquisa descritiva, quantitativa e exploratória, utilizando como técnicas a pesquisa documental e de levantamento. Os dados coletados referem-se à caracterização demográfica dos profissionais da equipe de enfermagem, assim como questões sobre faltas e licenças. A amostra foi composta por 86 profissionais. Para atribuição dos escores das dimensões foi utilizado um ponto de corte obtido pela mediana para demanda e controle e média para apoio social. A análise dos dados foi realizada por meio de análise descritiva da amostra e testes de correlação dos dias de afastamento e as dimensões. Analisou-se ainda, as associações das dimensões com os dados sócio demográficos. Os resultados da pesquisa demonstraram que as dimensões demanda e controle apresentam correlação significativa estatisticamente. O apoio social não apresentou correlação significativa com controle, mas apresentou correlação com a demanda. Considerando os profissionais participantes da pesquisa 54% apresentaram alta demanda, 44% alto controle e 49 % baixo apoio social. Ao comparar com o modelo de demanda controle, identificou-se que o trabalho ativo é considerado uma situação boa de trabalho. Na realização das associações das dimensões com os dados sócio demográficos, não obtivemos resultados significativos estatisticamente. Concluímos que apesar de não ter encontrado associações e correlações das dimensões e dados sócio-demográficos com o absenteísmo não significa que ela não exista. Destaca-se como fator de limitação da pesquisa o fato da mesma ter sido realizada somente em uma instituição, com atuação em maior número de mulheres motivo que nos impede de realizar comparações de sexo, e pela coleta dos dados sobre falta, serem auto referidas pelos profissionais. Como sugestão para prática sugere-se que a instituição realize o controle das ausências e de seus motivos de forma informatizada. Sugerimos que sejam realizadas outras pesquisas em hospitais com características diferentes deste estudo e no mesmo hospital deste estudo após informatização do controle do absenteísmo para que sejam realizados estudos comparativos.

The health compared with the business, has a fledgling tradition of public management. The lessons from industry on organizational development, psychological and social science disciplines have emerged as health professionals collaborate, coordinate and interact with patients, and how these aspects could be improve, in addition such perceptions and penetrate the quality management. Accreditation is an educational strategy that seeks to bring affordable and practical way the logic of quality with continuous improvement to health care organizations and thereby ensure a more qualified assistance to the population. The scarcity of information on accreditation in the field of dentistry motivated this study, which aims to analyze the perception of the dentist on accreditation in dentistry. Accreditation assesses the safety of care processes for patients and professionals working in organizations. To achieve the objective of the research were first collected secondary data in the literature on the topic quality and accreditation. Secondly, interviews were conduct with dentists. The interview was conduct following a script to ask related to the professional knowledge about quality management and accreditation. They interviewed 120 professionals working in 40 individual offices, 40 dental clinics and 40 health clinics. The results of the analysis technique used was content analysis. With the study noted that few professionals know the concept of accreditation and its importance. Do not exhibit the respondent’s homogeneity in our understanding, applicability, need and process utility, with an ambiguous and inconsistent reflection on the positive and negative points. / A atenção à saúde tem sido continuamente revolucionada, com os avanços e desenvolvimentos no conhecimento científico e tecnológico na área odontológica. A área da saúde comparada com a empresarial, possui uma tradição incipiente em termos de gestão. As lições advindas da indústria sobre desenvolvimento organizacional, psicológico e disciplinas das ciências sociais vêm destacando como os profissionais de saúde colaboram, coordenam e interagem com os pacientes, e como tais aspectos podem ser melhorados, ao acrescer tais percepções e penetrar na gestão da qualidade. A acreditação é uma estratégia educativa que busca levar de forma acessível e prática a lógica da qualidade, com a melhoria continua para as organizações de saúde e com isso garantir uma assistência mais qualificada para a população. A escassez de informações sobre a acreditação na área da odontologia motivou esse estudo, que tem como objetivo analisar a percepção do cirurgião dentista sobre acreditação na odontologia. A acreditação avalia a segurança dos processos assistenciais para os pacientes e profissionais que trabalham nas organizações. Para alcançar o objetivo da pesquisa, primeiramente foram coletados dados secundários na literatura referente ao tema qualidade e acreditação. Num segundo momento foram realizadas entrevistas com cirurgiões dentistas. A entrevista foi realizada seguindo um roteiro com perguntas relacionadas ao conhecimento do profissional sobre gestão da qualidade e acreditação. Foram entrevistados 120 profissionais, 40 atuantes em consultórios individuais, 40 clínicas odontológicas e 40 em clínicas de saúde. A técnica de análise dos resultados utilizada foi a análise de conteúdo. Com o estudo observa que poucos profissionais conhecem o conceito de acreditação e sua importância. Não apresentam os entrevistados homogeneidade quanto ao entendimento, aplicabilidade, necessidade e utilidade do processo, com uma reflexão ambígua e incongruente sobre os pontos positivos e negativos.

The demand level of its users is an increasingly and important factor in organizations providing these services. The absence of quality and humanization of care policies results in complaints from users, manifested through various external and internal institutional channels. The aim of present study was to group the events recorded by users of a public hospital, based on five dimensions of quality in order to obtain subsidies for creating a quality improvement of attendance policy. The public hospital evaluated is located in the metropolitan region of Sao Paulo - SP. The service unit serves several specialties, being a reference in traumatology. The research was based in a qualitative approach, using desk research technique to collect data from 142 free expression forms, from December 01, 2014 to April 30, 2015, coming from an institutional program called "Tell Me". The data demonstrated that, of 283 registered events, 83% were related to complaints. The most pointed professional category was that of physicians (61% of the total). The sector that received the most complaints was adult emergency, with 37% of the recorded events. Complaints were classified based on five "determinants of quality", with aspects of responsiveness (34%), and empathy (33%), the most cited. It was concluded that the lack of institutionalization of quality and humanization of care policies lead to organizational problems that negatively influence the quality of services provided. Therefore, considering that this research focused on contributing to the management practice, it suggests the development of strategies to achieve better results regarding the quality of services provided by the hospital, especially regarding to responsiveness and empathy in attendance. / A saúde pública é um seguimento de serviços bastante complexo. O grau de exigência de seus usuários é um determinante cada vez mais importante nas organizações prestadoras destes serviços. A ausência de políticas de qualidade e humanização no atendimento resulta em queixas de usuários, manifestadas por meio de diversos canais externos e internos da própria instituição. O objetivo deste trabalho foi agrupar as manifestações registradas pelos usuários de um hospital público, com base nas cinco dimensões determinantes da qualidade, visando obter subsídios para criação de uma política interna de melhoria da qualidade no atendimento. O hospital avaliado pertence à gestão pública de Administração Direta, e está localizado na região metropolitana da cidade de São Paulo – SP. A unidade de serviços atende várias especialidades, sendo referência em traumatologia. A pesquisa seguiu uma abordagem qualitativa, utilizando a técnica de desk research para coleta de dados em 142 formulários de manifestação livre, referentes ao período de 01 de dezembro de 2014 a 30 de abril de 2015, oriundos de um programa institucional denominado “Conte Comigo”. Os dados demonstraram que, das 283 manifestações registradas, 83% eram referentes à reclamações. A categoria profissional mais apontada foi a dos médicos (61% do total). O setor que mais recebeu queixas foi o pronto socorro adulto, com 37% das manifestações registradas. As reclamações foram classificadas com base nos cinco “determinantes da qualidade”, sendo os aspectos de responsividade (34%), e os aspectos de empatia (33%), os mais citados. Concluiu-se que a ausência de institucionalização de políticas de qualidade e humanização no atendimento acarretam problemas organizacionais que influenciam negativamente na qualidade dos serviços prestados. Portanto, considerando que esta pesquisa teve como foco contribuir para a prática gerencial, sugere-se a formulação de estratégias que busquem alcançar melhores resultados em relação a qualidade dos serviços prestados pelo hospital, especialmente no tocante à responsividade e empatia no atendimento.

Artificial Intelligence is a fuzzy concept. My role, as I see it, is to put down a working definition, a criterion, and a set of assumptions to set up equations for a workable methodology. This research introduces the notion of Artificial Intelligent Agency, denoting the application of Artificial General Intelligence. The problem being handled by mathematics and logic, and only thereafter semantics, is Self-Supervised Machine Learning (SSML) towards Intuitive Vehicle Health Management, in the domain of cybernetic-physical science. The present work stems from a broader engagement with a major multinational automotive OEM, where Intelligent Vehicle Health Management will dynamically choose suitable variants only to realise predefined variation points. Physics-based models infer properties of a model of the system, not properties of the implemented system itself. The validity of their inference depends on the models’ degree of fidelity, which is always an approximate localised engineering abstraction. In sum, people are not very good at establishing causality. To deduce new truths from implicit patterns in the data about the physical processes that generate the data, the kernel of this transformative technology is the intersystem architecture, occurring in-between and involving the physical and engineered system and the construct thereof, through the communication core at their interface. In this thesis it is shown that the most practicable way to establish causality is by transforming application models into actual implementation. The hypothesis being that the ideal source of training data for SSML, is an isomorphic monoid of indexical facts, trace-preserving events of natural kind.

Factors influencing community integrated management to childhood illness in rural areas

Thesis (MCurr)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: Child mortality is a worldwide problem and, according to the World Health Organization (WHO), 8,1 million children under the age of five years die each year. The Millennium Development Goals focus on the worldwide reduction in child mortality by two-thirds between 2000 and 2015. Several studies show that worldwide Community Integrated Management of Childhood Illness (CIMCI) interventions by community care workers (CCWs) have a positive effect on child health. The goal of this study was to determine the factors influencing CIMCI in the rural areas of the West Coast District in the Western Cape of South Africa. The objectives for this study were to determine the factors influencing CIMCI carried out in rural areas by the CCWs, which were identified as: • having working hours that are adequate for such a comprehensive service package; • being adequately trained; • having adequate knowledge of the “16 Key Family Practices” of CIMCI; • having equipment that is adequate for the execution of their daily duties; • being able to cope with the challenges of working in rural and remote areas; and • receiving adequate supervision and support related to CIMCI. A descriptive, non-experimental exploratory research design with a quantitative approach was applied. The target population (N = 270) consisted of CCWs who are funded by the Provincial Government of the Western Cape (PGWC) in the West Coast District. For this study a response rate of 257 (95,18%) was obtained. Data was collected personally by the researcher with a self-administered questionnaire. The data was analysed with the assistance of a statistician and are presented in histograms and frequency tables. The participants were tested on their knowledge of CIMCI, and more than half of them achieved an average score that was not satisfactory. Statistically significant correlations were found between the participants’ total score achieved and highest school grade passed (p < 0. 01); their level of Expanded Public Works Programme (EPWP) training (p < 0.01); their attendance of the CIMCI five-day course (p < 0.00); and if they had done a refresher course on CIMCI (p < 0.00). The total score was also shown by the Mann-Whitney U test (p < 0.01) to have a direct relationship with whether they had received any health-related training after school. The conclusion that can be drawn is that the higher the level of education of the CCWs, the better their knowledge of CIMCI. This could also improve their work performance. The recommendations arising from this study include that CIMCI training should be standardised to ensure that the CCWs have adequate knowledge. The current policy on community-based services (CBS) of the Provincial Government Western Cape Department of Health should also be standardised to ensure adequate working hours, training, equipment and supervision, and to take into consideration the challenges of working in rural areas. In conclusion, should these recommendations be implemented, CIMCI will have a huge, positive impact on child morbidity and mortality. CCWs will be ensured adequate working hours in relation to their workload, and will receive adequate training, equipment and supervision. This will reduce the challenges CCWs face and strengthen their services in rural areas. / AFRIKAANSE OPSOMMING: Kindersterftes is wêreldwyd ’n probleem en volgens die Wêreldgesondheidsorganisasie sterf 8,1 miljoen kinders onder die ouderdom van vyf jaar elke jaar. Die Millenniumontwikkelings-doelwitte fokus daarop om kindersterftes tussen 2000 en 2015 met twee-derdes te verminder. Verskeie studies toon dat intervensies deur middel van Gemeenskapsgeïntegreerde Bestuur van Kindersiektes deur gemeenskapsorgwerkers die wêreld oor ’n positiewe effek op kindergesondheid het. Die doel van hierdie studie was om die faktore te bepaal wat Gemeenskapsgeïntegreerde Bestuur van Kindersiektes in die landelike gebiede van die Weskusdistrik in die Wes-Kaap van Suid-Afrika beïnvloed. Die doelwitte vir hierdie studie was om die faktore te bepaal wat beïnvloed hoe Gemeenskapsgeïntegreerde Bestuur van Kindersiektes in die landelike gebiede deur gemeenskapsorgwerkers uitgevoer word, wat soos volg uiteengesit is: • werksure wat voldoende is vir die omvattende pakket dienste wat aangebied word; • dat hulle voldoende opgelei is; • dat hulle voldoende kennis het van die “16 Sleutel Familiepraktyke” van Gemeenskapsgeïntegreerde Bestuur van Kindersiektes; • dat hulle die nodige toerusting besit wat voldoende is vir die uitvoer van hulle daaglikse pligte; • dat hulle raad weet met die uitdagings van werk in landelike en afgeleë gebiede; en • dat hulle voldoende toesig en ondersteuning met betrekking tot Gemeenskapsgeïntegreerde Bestuur van Kindersiektes ontvang. ’n Beskrywende, nie-eksperimentele verkennende navorsingsontwerp met ’n kwantitatiewe benadering is gebruik. Die huidige beleid van die Provinsiale Regering van die Wes-Kaap oor gemeenskapsgebaseerde dienste moet ook gestandaardiseer word om te verseker dat hulle toereikende werksure, opleiding, toerusting en toesig het, en om die uitdagings van werk in landelike gebiede in ag te neem. Ter afsluiting: sou hierdie aanbevelings geïmplementeer word, sal Gemeenskapsgeïntegreerde Bestuur van Kindersiektes ‘n groot, positiewe impak op kindermorbiditeit en kindersterftes hê.
