Výběr partnera pod vlivem hormonální antikoncepce a jeho dopady na partnerskou stabilitu a spokojenost vztahu / Mate choice unter the influence of hormonal contraception: impact on partnership stability and quality

Previous studies have pointed out an interesting fact: the use of hormonal contraceptives modifies the partner preferences of women towards men more feminine and genetically less compatible. This master thesis first tried to experimentally verify whether these different preferences are reflected in real mate choice outside the context of the laboratory. It includes two studies comparing partner satisfaction and relationship dynamics in couples that have been formed under the influence of pills versus under the influence of the natural menstrual cycle. Study 1 includes 3116 mothers and was conducted through an online questionnaire. The second study collected representative data from 1605 Czech men and women aged 35 to 65 years representing the population of the CZ also through a hand-to- hand questionnaire. Study 1 confirmed the assumption in several ways: hormonal contraception users (during the mate choice) assessed their partners as less attractive and masculine compared with women who naturally cycled at that time. We also found their sexual satisfaction was reduced, however, they were more satisfied with non-sexual aspects of the relationship. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University, Czech Republic Introduction: Hormonal contraception is a broadly used and very reliable birth control method. Nonetheless, it is not entirely risk-free. This thesis deals with the awareness of those risks in women who are currently using hormonal contraception or have ever used it before. Aim of the thesis: The main aim was to determine the extent of the users' awareness of hormonal contraception and its risks, undesirable side effects and contraindications, as well as their source of information. Furthermore, the awareness of the environmental impact, the reasons for using hormonal contraception and user satisfaction were examined. Methodology: The research was carried out by means of online questionnaires, which were spread among current or former users of hormonal contraception regardless of age, educational background or place of residence. 148 completed questionnaires were gathered and the data which were obtained were statistically evaluated. Objetivo: Compreender como a prescrição da pílula anticoncepcional por médicos ginecologistas era realizada no Brasil na época em que começa a ser utilizada no país. Methodology: This is a qualitative research, having as empirical material the narratives of six gynecologists who worked in the 1960s, at São Paulo state. Results and Discussion: From the interviews, four thematic axes of analysis were identified, which included the description of the first contact of the doctors with the pill, followed by different ways of using the contraceptive pill as a new technology. Also, were identified the relationship between medical prescription and the pharmaceutical industry, as well as the use of the contraceptive pill and the construction of the medical discourse about the female body. Problematika antikoncepce a reprodukční zdraví mládeže / The Issue of Contraception and Reproductive Health Care among Young People

The dissertation is focused on the issue of contraception and reproductive health of young people. I therefore chose this topic of my dissertation because it interests me and it is also very current topic. I have specified the research in various types of secondary schools in Cesky Krumlov. The main aim of my dissertation was to map the knowledge of hormonal contraception and proven possible differences in sexual self behaviour in adolescent girls attending different high schools in Cesky Krumlov. The research was conducted using anonymous questionnaires that were distributed to girls aged 17 to 19 years. For my dissertation, I set four hypotheses that have been statistically confirmed. The results were interesting and there had been statistically significant differences in the level of knowledge about the risks of hormonal contraception among female students of Secondary Schools and other female students of Health Secondary Schools. In addition, the Vocational School students begin sexual life before other students of Secondary schools. Also the most common method of protection at first sexual intercourse is condom, but the most preferred contraceptives in adolescent girls, regardless of the type school studied, is hormonal contraception. According to the responses of girls we can see the differences in their own sexual behaviour, which is, in most cases conscientiously. Girls, however, are not sufficiently informed about hormonal contraception they use. I would advise to bring more lectures into schools about the positives and risks of hormonal contraception in general and also about all contraceptive options.

Diplomová práce se zabývá problematikou užívání ženské hormonální antikoncepce z perspektivy mužů jakožto jejich partnerů. V teoretické části podává ucelený obraz o expertním i veřejném diskurzu a základních argumentech souvisejících s problematikou v odborné literatuře. Analytická část je konstruovaná prostřednictvím polostrukturovaných rozhovorů s vybranými respondenty, kteří svými odpověďmi vykreslují podobu této problematiky z mužské perspektivy. Jejich výpovědi jsou průběžně analyzované genderovou optikou. Diplomová práce volně navazuje na předchozí práce, zabývající se sociálními aspekty užívání hormonální antikoncepce z pohledu žen. ABSTRACT This diploma thesis deals with the problematics concerning the female usage of hormonal contraception from the perspective of their male partners. In the theoretical part, the thesis describes the basic assertions, expert as well as public discourse linked with the problematics represented in professional literature. Over the Moon: Extended-Cycle Contraception and the Recent Evolution of Medicine and Womanhood

This dissertation is based on seventeen months of ethnographic fieldwork that followed the development and diffusion of extended-cycle hormonal contraception, or birth control that is designed to eliminate monthly bleeding. It encompassed several sites and multiple constituencies: a clinical trial, documented medical conferences, users, potential users, and refusers of the pharmaceuticals, along with key academic and popular proponents of their adoption. Extended-cycle contraception is a critical topic because this new generation of pills, IUDs, shots, and implants is not only refiguring the length of women's cycles, but it is also augmenting the extent to which its users' bodies are medicalized, or subjected to a type of manipulation and regulation that was previously impossible. No longer just for pregnancy prevention, these regimens are increasingly touted as elective enhancement technologies that may improve on the human design, on the one hand, and as crucial preventative medicine for diseases such as reproductive cancers, on the other hand. Remarkably, these pharmaceuticals are as socially complex as they are chemically--they may facilitate the renegotiation of constructions of womanhood, nature, and progress.

La contraception hormonale combinée (CHC) est la contraception la plus utilisée en France. La maladie veineuse thromboembolique (MVTE), constitue le principal effet délétère de ces CHC. Des recommandations de bonnes pratiques sont publiées pour guider les prescripteurs. La meilleure compréhension des modifications biologiques associées aux différents types de CHC, les caractéristiques cliniques des femmes ayant eu une MVTE, la place de la recherche d’antécédents familiaux de MVTE (AFVTE) et d’une thrombophilie biologique avant la prescription d’une CHC constituent des pistes de recherche qui permettrait potentiellement d’optimiser la balance bénéfice-risque des CHC. Enfin l’impact de l’utilisation d’une contraception hormonale après un 1er épisode de MVTE reste peu évalué. Ce travail de thèse a été réalisé à la fois à l’aide de données biologiques d’utilisatrices de contraceptions hormonales (Etude EDGAR) et aussi à partir des données de l’étude de cohorte française COREVE (COntraception and REcurrent Venous Event). Cette étude a inclus 3121 femmes de moins de 45 ans au moment de leur 1er épisode de MVTE. Nous nous sommes particulièrement intéressés à l’analyse des caractéristiques de ces femmes en fonction du type de contraception utilisée, à la prévalence des facteurs de risque vasculaires et notamment les AFVTE. La fréquence d’épisode de MVTE associé à une prescription inadaptée de CHC, variait ainsi de 8.8 à 25.9 %. Par ailleurs, à l’aide d’une méthodologie de type cas versus cas, l’interaction entre l’utilisation d’une CHC et la présence d’une mutation du facteur V Leiden sur le risque de MVTE diffère significativement en fonction du progestatif combiné des CHC. / Combined hormonal contraception (CHC) is the most widely used contraception in France in which venous thrombosis embolism (VTE) is the main deleterious effect. Best practice recommendations are published in order to guide prescribers.The better understanding of the biological changes associated with different types of CHC, the clinical characteristics of women with VTE, the place of family history of VTE (FHVTE) and for biological thrombophilia before prescribing a CHC constitute research paths that could potentially optimize the risk-benefit balance of CHCs. Finally, the impact of hormonal contraception use after a first episode of VTE remains rarely evaluated.This work was carried out both using biological data from hormonal contraception users (EDGAR study) and also using data from the French cohort study COREVE (COntraception and REcurrent Venous Event).This study included 3121 women under 45 at the time of their 1st episode of VTE. We were particularly interested in analyzing the characteristics of these women according to the type of contraception used, the prevalence of vascular risk factors and especially the FHVTE.The frequency of VTE episode associated with an inadequate CHC prescription varied from 8.8 to 25.9%. Moreover, using a case-only methodology, the interaction between the use of CHC and the presence of a mutation of the factor V Leiden on the risk of VTE differs significantly depending on the progestin combined of the CHCs.
