The effect of work-hardening on the physical work capacity of manual labourers within South African industry

South Africa is a labour-intensive industrially developing country. As a result, in excess of 200 000 workers suffer from musculoskeletal injuries in a year. Research is thus essential to develop more effective strategies in the reduction and rehabilitation of occupational musculoskeletal disorders within industry. It was the hypothesis of this study that by improving the physical work capacity of manual labourers, through participation in an occupation specific work-hardening programme, that the ergonomic stress index and therefore the occurrence of musculoskeletal injuries within industry, may be reduced. Twenty-five male Black and Coloured manual labourers volunteered to participate in this study. In addition to in situ task analyses, the subjects participated in cardiovascular and strength assessments in the laboratory, both pre- and post-participation in the ten-week work-hardening programme. The data were statistically analyzed in order to identify any significant • improvements in the workers' physical work capacity, as measured by cardiovascular, strength and perceptual responses, following the period of work-hardening. Two significant reductions were noted in measures of working heart rate together with significant improvements in grip strength and trunk strength tested at a velocity of 60°.sec·1 at the post-conditioning assessments. In conclusion, the ten-week work-hardening programme resulted in nominal improvements in all the cardiovascular measures and significant improvements in the subjects' strength performance. However, industrialists must recognise that this study dealt with only one aspect of reducing the ergonomic stress index at the workplace and realise that, in addition to this focus, it remains essential to design the task to fit the human operator.

However, despite their high rates of injury, the literature regarding the risk factors for work-related musculoskeletal injuries (WRMSIs) in the fire service has not been well-established, especially in South Africa. Objectives: To determine the prevalence and risk factors for musculoskeletal injuries in the Durban Metropolitan Fire Department and to evaluate the relationship between selected risk factors and the prevalence of musculoskeletal injuries. Methods: This was a descriptive study from a large urban Fire Department employing 350 active firefighters. Using a cross sectional study design, a retrospective analysis investigated the musculoskeletal injury prevalence from 2006-2008 by means of a questionnaire. Individuals reported on demographics, injury location, injury etiology, injury nature, extent of treatment rendered and time lost from work. Additionally, data was obtained regarding smoking, occupational stress, fitness, protective gear and injury prevention advice given by the Durban Metropolitan Fire Department. A 41% response rate was achieved. Results: The point prevalence of WRMSIs was 33.6% and the period prevalence was 81.1% of the sample. Low back injuries (47.9%) and strain injuries (40.8%) were the most common, followed by knee (22.5%), shoulder (19.7%) and ankle injuries (19%). The most common causes included lifting heavy objects, working in awkward postures and running. Weight, ethnic group, stress, lack of nutritional advice and alcohol consumption were all significantly associated with the prevalence of injuries. Ex-smoking was significant in the prevalence of low back injuries, stress was significant in the prevalence of knee injuries and alcohol consumption was associated with the prevalence of shoulder injuries. Conclusion: WRMSIs are of great concern in the fire service as their prevalence is substantial. Evaluation and implementation of further preventative measures and advice based on the results of this study can be effective in reducing WRMSIs.

Prevention and management of occupational injuries at selected higher eduction institution in the Western Cape, South Africa

Thesis submitted in fulfilment of the requirements for the degree Master of Technology Occupational Health Nursing in the Faculty of Health and Wellness Sciences at the Cape Peninsula University of Technology 2015 / Purpose of study: The purpose of this study was to explore the injury on duty prevention and management strategies that are currently used in different higher education settings. The study intended to shed light on specific areas of policy and practice discrepancies related to the disjuncture between employee interactions, regulated directives and organizational goals. Aims and objectives: One of the aims was to determine the types of work-related injuries that occur in HEI’s by verifying the injuries reported on Employers Reports of an Accident. A second aim was to examine strategies that have been put in place to prevent injuries in the workplace by reviewing policies and procedures related to injury prevention. The final aim was to examine systems in-place to manage occupational injuries by looking at staff health or employee wellness service flow charts or models used at the HEIs. Population and sample: Permanently employed personnel at two HEI’s in the Western Cape were participants in a multiple case study. The sample units consisted of health and safety representatives, human resources, maintenance personnel, estates and custodial, traffic services and departmental managers. Methodology: This study applied a mixed-method using a multiple case study design as an approach to the enquiry. Methods of data collection: Documents, questionnaires, and semi-structured interviews were be used to obtain data to answer the research questions. Process of analysis: Qualitative and quantitative themes will be analysed in a matrix. The three data sources were triangulated to validate the findings. Benefits of the study: Injury prevention benefits the employer and employee in many ways, including increased employee performance leading to increased productivity as well as cost savings. Main findings: Injuries that were reported are mostly musculoskeletal injuries; however other types of injuries may not be reported due to under reporting of injuries being common in this setting. Health and safety training and knowledge sharing was not well supported and injury prevention strategies were not optimized. The application of health and safety policies and procedures were not adequately communicated, enforced or monitored. Staff health and employee wellness strategies operate independently and do not facilitate a coordinated response to manage occupational injuries in this setting. Lack of monitoring, organizational support, training and knowledge sharing and communication were the four main challenges to injury prevention management systems. Conclusions: The implementation of an effective injury prevention and management programme could benefit the organization and translate into increased operational value (organizational quality). Effectiveness of health and safety training in reducing occupational injuries among harvesting forestry contractors in KZN

Forestry work is one of the most hazardous occupations worldwide and accounts for a large proportion of occupational injuries. Employers frequently train workers in the proper and safe use of equipment, however the efficacy of these programmes are seldom rigorously evaluated. The aim of this study was to assess the effectiveness of health and safety training in reducing injuries among forestry workers in Kwa-Zulu Natal. A descriptive study was conducted among 300 Kwa-Zulu Natal harvesting contract workers. A questionnaire to assess the efficiency of health and safety training was administered. A retrospective review of the injury register and medical records of employees who sustained work related injuries, fatalities during 2009–2013 was done. The company injury data for harvesting contractors from 2009- 2013 reported 3 fatalities and 68 lost time injuries during post commencement of training. About 23.3% workers reported injuries during the study period. Slip, trip and fall injuries were the most reported cause of injuries. Almost 95% of respondents reported that they had received health and safety training at work, with 84% reporting satisfaction with the quality of training received. A decreased prevalence of work related injuries was reported post training. The health and safety program was successful in increasing workers awareness and responsibility of health and safety issues. On- going specific job training, sustained work place inspections and adult learning for supervisors will improve health and safety of workers and reduce injuries in the forestry industry. / M

Evidence-based guidelines to promote the health and safety of health care workers in selected public hospitals in the Tshwane health care district in Gauteng, South Africa

Text in English / The purpose of this research was to investigate occupational health and safety challenges and their impact on health care workers (HCWs) in selected public hospitals from the Gauteng Province, South Africa. Method: A quantitative descriptive cross-sectional survey was conducted among HCWs in the study sites. A two-staged sampling that include purposive sampling of study sites and census sampling of 2000 HCWs was used. Self-administered questionnaires were used to obtain data from HCWs. In addition, two different checklists were used to conduct retrospective records reviews to assess occupational health and safety (OHS) policy compliance and occupational injuries and diseases occurrence. The SAS Release 9.3 was used to analyse data. The Fischer Exact test and Chi-square were also used to determine the association of variables and P-value was set at <0.05 to indicate significant association. Results: A total of eight public hospitals and 926 (46.3%) HCWs who were all females nurses participated in this survey. Major occupational health hazards reported by the participants include: needle-stick injuries 275 (54.67%), slips trips and falls 67 (13.32%) and splashes 57 (11.33%). The analysis of open-ended responses indicated increased workloads, long hours of work and shift work as the most reported psychosocial hazards among HCWs. The reviewed records indicated that back injuries 22 (4.37%), tuberculosis (TB) 17 (3.38%) and asthmatic reactions 8 (1.59%) were the commonly reported occupational injuries and diseases among the HCWs. The records review also revealed a lack in the conducting of adequate medical surveillance among participants. The results showed poor compliance with the OHS policy and a negative impact of biological and psychosocial hazards on the HCWs. Conclusion: There was a high risk of exposures to biological hazards whilst providing care to patients, thus warranting the implementation of robust preventive measures. As a result, the guidelines were developed to promote the health and safety of HCWs with a view to promoting policy compliance and preventing the occurrence of occupational injuries and diseases as well as their impact among HCWs. / Health Studies / D.Litt et Phil. Prevention mechanisms to minimise injuries on duty : perceptions of security officers in a private security company

The aim of the study was to determine the causes of IODs in the workplace and to identify possible preventative measures to reduce IODs. A literature review was conducted as part of the study, involving assessment of all related articles and books on the subject of IODs in the workplace. A qualitative research methodology was utilised to conduct the study. The main research instruments were four focus group interviews and eight individual interviews. The study concluded that employees experienced IODs in different ways, with most participants describing negative experiences such as physical pain, undue financial hardship, psychological trauma and lack of support from the employer. A minor percentage experienced IODs in a positive sense in that there is heightened safety awareness in the workplace after an IOD has occurred, and the adoption of a more cautious approach by employees when performing their duties. From a practical and organisational/managerial perspective, the adoption of effective training of security officers and adherence to organisational standard operating procedures will assist in reducing IODs in the workplace. Limitations of the study included the small sample size from the research population, perceived language barriers during the interview processes and non-participation and inputs from managers at the organisation.
