The lived experience of family caregivers who provided end-of-life care to a relative with advanced dementia

With aging of the baby boomer population, older adults living longer, and no known cure for dementia, the prevalence of dementia in older adults will inevitably rise. Dementia is a terminal illness, although it may not be recognized as such. Family caregivers to persons with dementia provide invaluable care, often at the expense of their own health and well-being. Over the past two decades there has been an abundance of research that examines the various and complex aspects of caring for a relative with dementia. However, there is a paucity of research that has been conducted on the experiences of family caregivers providing end-of-life care. The conceptual framework that guides this research is based on the work of Martin Heidegger. A thorough search of the literature reveals that the main themes of this end-of-life experience studied to date are the experience of grief and loss, and the manifestations of depression in family caregivers. A number of gaps in the literature remain that limit our understanding of the end-of-life care experience. The purpose of the present study is to begin to address this neglected area of research. As a result, the research question is: What is the meaning of the lived experience of family caregivers who provided end-of-life care for a relative who died with advanced dementia? This question was addressed using an interpretive phenomenology based on the work of Munhall. The study utilized a purposeful sample of family caregivers (n = 11) whose relative with dementia died in the last year. Two to three in-person, unstructured interviews were completed with each participant as a way to glean an understanding of their experiences and offer opportunities for the participant to verify their end-of-life caregiving story. A total of 27 interviews were recorded and transcribed verbatim. Transcripts were hermeneutically analyzed in order to create individual narratives for each participant, reveal the essence of this experience, discuss the influence of the life worlds, consider the taken for granted, and generate an overall study narrative. These findings reveal the complex nature of the end-of-life caregiving experience with dementia.

'Talk about Conflict': Understanding Interpretive Repertoires in Community Mediation

While many studies investigate the mediation styles enacted during conflict resolution, relatively few of them discuss the communicative processes through which discursive structures are reproduced. This dissertation is a case study illustrating how the duality of structure operates. The purpose of this dissertation is (a) to reveal the discursive resources that community mediators use, (b) to demonstrate how mediators assemble their discursive resources to form a community of practice, and (c) to explore power relations between attorneys and mediators. To understand these issues, I conducted an ethnographic study at a community mediation center employing 25 volunteer mediators. Over 330 hours of participant observations and 41 interviews were completed. Results demonstrated that mediators relied on four interpretive repertoires during mediation: Cultural Competence, Volunteer Pride, Parenting Norms, and Business Professional(ism). They used these repertoires to navigate the tenuous and emotional mediation process. The repertoires provided the mediators with a degree of discursive flexibility in constructing and controlling the immediate situation, and the repertoires were enacted to serve multiple purposes. Further, the extemporaneous engagement of issues, emotions, and discursive resources was an essential skill that was cultivated only over time and with practice. Results also showed that the mediators assembled various repertoires in a number of ways including informal conversations, co-mediations, conference attendance, and organizational meetings. Analysis suggested that the institutional knowledge and discourses provided through formal learning was of secondary importance to developing personal repertoires by mediating as often as possible. In addition, because of the organizing practices of the mediators and the ad hoc nature of the organization, the organizational form of the mediation center was a community of practice. Finally, results found that the power struggles between attorneys and mediators centered on trying to define the mediation situation using different discursive resources. Establishing control of the environment and working the process were the two main goals of mediators, and the more they took ownership of these goals the more often the conflicts settled. This dissertation suggests that taking control of the mediation process is the first step to empowering the participants and achieving a workable mediated settlement.

On the scientific status of interpretive inquiry

Interpretive social science is well established institutionally at universities and research centres. It benefits from this institutional context in terms of prestige, credibility and grants. In comparison with non-interpretive disciplines however, its scientific status is questionable. What elements of it are really scientific and what elements are threats to this scientific character? This problem has been discussed in the past but unfortunately the discussion has gradually dried up without a successful resolution. In my thesis I am revitalising it. I take a systematic rather than historical approach: instead of picking up the discussion where it has been abandoned, I begin with a working definition of science, and analyse to what extent interpretive inquiry meets the requirements of this definition. The structure of my thesis follows this definition in that what is discussed is the three substantial elements of it - theory, research method, and professional quality control. In relation to theory, I pose questions on a range of topics, such as whether interpretive social science is explanatory, and whether it generates new knowledge. In relation to method, I explore, amongst other things, whether qualitative method permits the production of valid and reliable findings. The discussion of professional quality control considers issues around the reporting of findings and the assessment of these findings by others. I complement my analysis by considering three interpretive case studies, exploring both whether they produce theoretical knowledge and reflecting on their methodological strengths and weaknesses. Additionally, I explore the border between interpretive inquiry and non-fiction arts, such as literary reportage and documentary filmmaking, arguing that this border is more blurred than it may first appear.

No / Rhetorically contextualised against a history of poor outcomes and negative attitudes, New Labour’s mental health policy introduced recovery as a turn towards optimism, control and choice for those who provide and use mental health services. Recovery continues to occupy a key role within contemporary policy and practice. Despite such privilege recovery remains a contested concept, not least because it means different things to different people. This article reports empirical work conducted during 2010-11, and explores two interpretive repertoires (traditional and responsibilised-progressive) that mental health practitioners draw upon in their accounts of recovery. Grounded in constructionist theory, the findings suggest that practitioner accounts of recovery are diverse, and produce many different subject positions from which recovery may be experienced. Developing Webber and Joubert’s (2016) editorial comments on the challenge for Social Work, it is argued that professional Social Work must practice in a manner compatible with its own value-base and any model of recovery must be held up for scrutiny. A knowledgeable position for the practitioner is advocated, one that is capable of working multiple paradigms in order to better understand and meet service user need. / ESRC

Descriptions of Friendship from Preadolescent Boys Who Carry A Label on the Autism Spectrum

This dissertation provides an account of the ways seven preadolescent boys with autism spectrum disorders describe friendship. This study extends previous research by providing more in-depth descriptions of friendship gleaned through iterative interviews. In addition to multiple interviews with each of the boys, I collected interview data from their parents, and one teacher of each boy. In order to convey friendship from the boys' perspectives, I present data across three broad themes: (1) Establishing and maintaining friendships, (2) Social reciprocity, and (3) Conflicts. The findings indicate that some important components of close friendships, (i.e., frequent and varied interactions, relative equality and reciprocity, maintenance over an extended period of time, and emotional support), that are commonly included in descriptions of preadolescent relationships in the general literature were evident in the close friendships of these boys with autism spectrum disorders. Not only does this study help increase the understanding of the construct of friendship, but of autism spectrum disorders as well. Specifically, my findings challenge the deficit perspective of autism spectrum disorders in several ways: (1) these preadolescents described ways that they socialize with friends rather than ways to avoid social situations in favor of isolation (Kanner, 1943); (2) the majority described same-age friendships as opposed to failing to develop peer relationships (American Psychiatric Association, 2000); and (3) four boys either are developing or have developed social reciprocity with friends, instead of demonstrating a lack of social reciprocity (American Psychiatric Association, 2000). Researchers are just beginning to understand how preadolescents with autism spectrum disorders describe friendships. This study provides an important addition to the extant literature by providing insight into how a small group of highly verbal, preadolescent boys labeled with autism spectrum disorders describe friendship. Further research is necessary and will add to the sparse body of literature that just begins to depict how people with autism spectrum disorders understand and experience friendship. / Ph. D.

Enacting the interpretive turn: narrative means toward transformational practice in child protection social work

This PhD project is undertaken by publication and thus this exegesis offers an explication and linking interpretation of the publications and DVD's listed in section two. The exegesis 'frames-up' what has been an ongoing interpretive inquiry exploring constructive frontline child protection social work undertaken by the author in collaboration with practitioners in Europe, North America and Australasia that has given rise to the publications and DVDs. Taking the lead from Geertz's ideas of interpretive anthropology the aim of this inquiry and publication work is to develop descriptions and theories of practice drawing upon insiders' local knowledges and sense-making of what constitutes good child protection social work. 'The natives' or insiders toward which this interpretive project directs its attention are first and foremost, frontline child protection social workers and wherever possible the child protection service recipients who have experienced the practice of those workers. The publication component of this project is a vital and integrated part of the research process since it is through the writing and production work that the usually overlooked, often deemed 'tacit' knowledges of service delivers and recipients are brought into the formal domain and made accessible to others. / This project is undertaken with transformative intent. The first intent being to distil the wisdom of insiders' knowledges into richly detailed formal accounts of good practice that speaks directly to the practitioner's condition thereby enhancing their professional reflexivity, hope and capacity. The second intent is to provide constructive on-the-ground 'news of difference' for a child protection field that is over-organised by anxiety, worst-case outcomes and an obsession with managers' measures. The exegesis is formulated around the research question, What potential does interpretive social theory have for transforming child protection social work? My conclusion is that while interpretive social theory offers significant epistemological and methodological resources for transforming the practices and orientation of child protection social work, this potential will not be realised until the social work displays renewed ontological commitment and faith in the knowledges and everyday experience of frontline practitioners.

Power of place : linking people, history, and nature visions for an interpretive trail on Mission Flats

It has long been demonstrated that increased appreciation of place initiates stewardship and responsible management of the land. Interpretative design possesses the potential to generate this stewardship through recovering the connections between place and the people that inhabit it. It is essential that interpretation be integrated into the everyday landscape to deepen one's respect of the past and one's commitment to its future. This vision of an interpretive trail in Mission, British Columbia, explores techniques of illuminating place history to inspire and challenge cognitive participation with the landscape. The format of this project begins with a discussion of the values and limits of interpretative methods. Secondly, the study area is introduced through site reconnaissance with special emphasis on historical morphology and social history. Next, interpretive design strategies are explored in conjunction with significant landscape precedents, culminating in a design approach for an interpretive trail on Mission Flats. Six nodes along the trail serve as keystones for this approach, with individual nodes revealing historical processes, both natural and cultural, through design. Akin to Mission Flats, every place is intimately bound to the people and events that have shaped it. Illumination of this connection is the goal of interpretive design. The planning and design phases of development are incomplete without the inclusion of this holistic vision of place. It is therefore imperative that we further explore interpretive design, so that it may be integrated into the oft-impersonal contemporary landscape, for the generation of authentic place identity.

The effect of quality, quantity and interpretive diversity on program attendance in Indiana state parks

Interpretation in both national and state parks provides more than mere entertainment to the visitor. Interpretation has been proven to be an effective deterrent to littering, vandalism and the deterioration of natural park resources. The focus of this research is to determine whether high quality programs, a large number of programs, and/or a diversity of programs offered by Indiana state parks attract a higher percentage of the total number of visitors entering a park facility to an interpretative event.The subjects need in this research were the state parks of Indiana. Data was obtained from a weekly form (SP-48) used by the Indiana parks. SP-48 forms from 1987 through 1990, Memorial Day through Labor Day weekend, were used: the total number of programs incorporated into this study was 20,595. The parks were divided into categories (all parks, year-round parks, and seasonal parks) and groups (high, medium and low number of facilities) to provide a similar statistical grouping for analysis.The factors studied were paired and regression analysis were performed on each pairing. Correlation coefficients indicated that an increase in the quantity of programs offered was highly correlated (p<001) to an increase in the percent of gate entrants participating in an interpretive event. Presentation diversity was significantly related to either of the above variables. / Department of Natural Resources
