Lidande hos patienter med benign prostatahyperplasi (BPH)

Benign prostatahyperplasi (BPH) är en av de vanligaste sjukdomarna hos män och kan vara en orsak till lidande. Hur omfattande lidandet är hos dem som drabbas är individuellt. Studiens syfte var att belysa patienters lidande vid BPH. En systematisk litteraturstudie valdes som metod. Sökning i relevanta databaser; Cinahl, PubMed, PsycINFO och ELIN@Kalmar, utfördes och därefter följde kritisk granskning, dataanalys och sammanställning av vetenskapligt material inom problemområdet. I dataanalysen framkom tre teman: fysiskt, emotionellt samt sexuellt lidande. Det fanns rädsla för prostatacancer och för att vara beroende av andra. En annan typ av rädsla gällde urinläckage som syntes utåt. Förändrade miktionsvanor upplevde patienter som pinsamma. Vanliga symtom vid BPH var svag urinstråle, urinläckage samt frekventa toalettbesök. Genom en gruppkonstellation av patienter, urologer och anhöriga där erfarenheter kunde utbytas, underlättades det fortsatta livet med sjukdomen. Trots allt fanns en acceptans och uppfattning om att sjukdomen är en naturlig del av åldrandet. För att kunna agera stödjande till patienter med BPH bör sjuksköterskan initiera till samtal med patienten utan att negligera de sexuella funderingar patienten eventuellt kan ha. Att anordna gruppmöten så att patienter kan utbyta erfarenheter med varandra kan vara värdefullt.

FPGA-Based Real-Time Simulation of Variable Speed AC Drive

Sophisticated power electronic apparatus and their digital control systems are finding increasing applications in electric power systems at generation, transmission, distribution and utilization levels. It is essential to carry out rigorous performance evaluation of such apparatus before commissioning. Field Programmable Gate Arrays (FPGAs) are becoming an attractive platform for accelerating computationally intensive applications. This thesis presents a FPGA-based real-time digital simulator for power electronic drives based on realistic device characteristics. A 3-level 12-pulse Voltage Source Converter (VSC) fed induction machine drive is implemented on the FPGA. The system components include the 3-level VSC, the induction machine, the direct field oriented controller, and the pulse width modulator. Both system-level and device-level IGBT models are utilized to implement the VSC. The VSC model is computed at a fixed time-step of 12:5ns allowing an accurate representation of the IGBT nonlinear switching characteristics. Altera Startix EP1S80 and EP3SL150F1152C2 FPGA boards utilized for the real-time simulation. All models were implemented in VHDL. The FPGA boards were interfaced to external DAC boards to display real-time results on the oscilloscope. The real-time results were validated using an off-line cosimulation set-up using the SABER and MATLAB/SIMULINK software. / Power Engineering and Power Electronics

Lower Urinary Tract Symptoms in Swedish Male Population : Prevalence, Distress and Quality of Life

<p>The aim the thesis was to investigate, the prevalence of Lower Urinary Tract Symptoms (LUTS). Moreover, we examine symptom severity and different levels of distress and describe how different symptoms from the lower urinary tract affect the self-assessed health, sadness, happiness and the quality of life in men. </p><p>The studies are based on two data collections. In the first data collection, all men aged 40 – 80 years (n=2571) living in the Swedish community received a postal questionnaire. Twelve months later, 504 men who had earlier reported LUTS and 504 who had not reported symptoms were asked to complete the DAN-PSS and the SF-36 questionnaires. </p><p>The overall prevalence of LUTS was 24%. Post-micturition dribbling (21%) was the most frequent symptom, and stress incontinence (2%) was the least frequent symptom. Urge incontinence, stress incontinence and “other” incontinence cause a high level of distress, even if the symptoms do not occur very often. Men experiencing mild, moderate or severe urge, stress or “other incontinence” had lower mean scores for all of the eight dimensions measured by the SF-36 than men without the same symptoms. Men experiencing a moderate/severe degree of weak stream or nocturia reported a poorer quality of life in all dimensions compared to men with a mild level of the same symptoms. The total burden of moderate/severe LUTS is related to self-assessed health, sadness and happiness. For each of the 12 specific LUTS, men with mild, moderate or severe symptoms had lower scores for self-assessed health and happiness, and higher scores for self-assessed sadness, than men without the same symptoms. </p><p>In conclusions, one of every four men reports LUTS. Urinary incontinence causes high level of distress even to men who experience this symptom rarely. LUTS have a negative impact on quality of life, health, sadness and happiness. </p>

Urogenital function in morbidly obese men following bariatric surgery

Introduction: Obesity has been suggested to be one of the risk factors for erectile dysfunction (ED) and lower urinary tract symptoms (LUTS). Bariatric surgery has been used for the treatment of obesity and has been suggested to have a significant impact on obesity-related conditions such as diabetes mellitus, ED and LUTS. Previous studies have investigated the effect of bariatric surgery on erectile and urological function in obese men; however those studies used long-term time points post-operatively (more than 1 month). Since it is now known that bariatric surgery can potentially induce glycaemic improvement within one week independent of weight loss, this study aimed to investigate the short-term effect in order to test the hypothesis as to whether improvement in urogenital function after bariatric surgery is due to weight loss or whether it is due to glycaemic improvement. Aim: To evaluate the baseline characteristics of patients with erectile dysfunction and to determine the early effects of bariatric surgery on erectile and urological function in morbidly obese men.
