Atualmente, a busca por opções de tratamento com medicações financeiramente mais acessíveis e menos agressivas ao organismo, aliada à uma visão mais humana de tratar o paciente, fez da homeopatia uma prática mais utilizada. Em se tratando de uma das sete endemias de maior impacto mundial, a Leishmaniose é uma doença negligenciada causada pelo parasita do gênero Leishmania, com estimativas alarmantes de novos casos e óbitos por ano, se tornando um grande problema de saúde pública. No Brasil, a Leishmaniose Visceral em áreas urbanas se destaca pelo fato de ser uma zoonose que possui como principal reservatório também o cão, que sempre esteve presente e em estreito contato com grande parte das famílias brasileiras. O mesmo, quando infectado pela doença, é submetido à eutanásia como norma do Ministério da Saúde, por apresentar riscos à população. Utilizando-se a Lei da Similitude, instruída por Hahnemann, chegou-se à medicação Antimonium crudum para possível tratamento e profilaxia da Leishmaniose Visceral Canina. Portanto, no presente trabalho, visou-se o estudo da ação da medicação homeopática Antimonium crudum, in vitro, diretamente em promastigotas de Leishmania (Leishmania) infantum chagasi, através do teste de MTT (brometo de 3-[4.5 dimetitiazol- 2- il) 2,5 difenil- 2 H- tetrazólio), que visa mensurar a atividade metabólica e a observação da curva de crescimento do parasito na forma promastigota, bem como a ação na infecção das Leishmanias em macrófagos da linhagem J774, em sua forma amastigota. Os resultados obtidos para a forma promastigota mostram que o estímulo homeopático não teve influência direta sobre os parasitos, tanto no teste de MTT quanto na curva de crescimento, corroborando os estudos que sugerem que a homeopatia não age diretamente sobre o parasito e sim em conjunto com o sistema imune do hospedeiro. Já nos resultados dos ensaios infectivos, observa-se, em geral, maior porcentagem de macrófagos infectados, assim como maior taxa de fagocitose nos grupos tratados com Antimonium crudum, o que sugere uma maior atividade dos mesmos levando à uma resposta imune secundária mais representativa e consequentemente, à destruição do antígeno. Quando se verificou o pré e o pós-tratamento de macrófagos, observou-se que as potências mais baixas, CH6 e CH12, parecem ter maior eficiência e quando a infecção já está instalada, a potência CH30 parece ter uma maior significância na profilaxia da infecção. Apesar disso, a resolução da doença dependerá da ação imunológica de cada hospedeiro, que irá controlar o crescimento e manutenção do parasito ou não / Currently, the search for treatments with medicines that are financially affordable, less aggressive to the host, and more humane in treating the patient, homeopathy is the most practical course. When it comes to one of the seven largest worldwide impact endemics, Leishmaniasis is a neglected public health complex with alarming estimates of new cases and deaths per year. In Brazil, urban Visceral Leishmaniasis also stands out because the reproductive cycle of this zoonotic disease can include the dog, which has always been present and in close contact with most Brazilian families. Dogs, when infected by the disease are euthanized as a rule from the Ministry of Health to prevent risks to the population. Using the Law of Similars, developed by Hahnemann, the medication Antimonium crudum was identified as possible treatment and prophylaxis for Canine Visceral Leishmaniasis. Therefore, the present work was to study the direct action of the homeopathic medicine Antimonium Crudum in vitro against the promastigotes of Leishmania (Leishmania) infantum chagasi, through the MTT test (3- [4.5 - dimetitiazol- 2-yl) - 2.5 diphenyl tetrazolium H 2), which aims to measure the metabolic activity; the promastigote growth curve; as well as the performance of promastigote infectivity in J774 macrophages. The results from both the MTT assay as well as the growth curve show that the homeopathic stimulus did not directly influence the promastigote form of the parasites, corroborating studies that suggest that homeopathy has no direct action on the parasite, but may function together with the host\'s immune system. Treatment with Antimonium crudum led to a higher percentage of infected macrophages in the infectivity assays and a higher rate of phagocytosis, which suggests increased activity leading to a stronger secondary immune response and as a result, antigen destruction. Assays in which macrophages were treated before or after infection suggest that lower concentrations (CH6 and CH12) seem most effective when the cells are already infected, however a higher concentration (CH30) appears to have greater activity in preventing infection. Ultimately, the resolution of the disease depends on the immunological action of each host, which may control the growth and maintenance of parasite

A leishmaniose é um termo global para doenças causadas por parasitos do gênero Leishmania, sendo a Leishmaniose Visceral (LV) a forma mais grave da doença. No Brasil é causada pelo parasita Leishmania (Leishmania) infantum chagasi. Para garantir a sua sobrevivência, alguns parasitas são capazes de manipular respostas de defesa das células do sistema imune. Recentes estudos demonstraram a participação da proteína imunomodulatória CD200 durante o processo de infecção de L. (L.) amazonenses. O presente estudo teve como objetivo investigar se os parasitos L. (L.) infantum chagasi são capazes de induzir a expressão da proteína CD200 durante o processo infeccioso. Em ensaios de infecção ex vivo, não foi observado proliferação de parasitas intracelulares. Apesar disso, L. (L.) infantum chagasi foi capaz de induzir a expressão do gene CD200. De maneira interessante, diferente de infecções por L. (L.) amazonenses, a indução de CD200 nessas células foi observada em tempos mais tardios de infecção. Ensaios de imunoprecipitação e Western blot indicaram a síntese da proteína, que atingiu os seus maiores níveis a 120 horas pós-infecção. A presença de CD200 sugere o envolvimento dessa molécula em tempos mais tardios de infecção por L. (L.) infantum chagasi. / Leishmaniasis is a global term for diseases caused by parasites of the genus Leishmania, and Visceral Leishmaniasis (VL) are the most severe form of the disease. In Brazil is caused by the parasite Leishmania (Leishmania) infantum chagasi. To ensure their survival, some parasites can handle defensive responses of the cells of the immune system. Recent studies have demonstrated the participation of immunomodulatory protein CD200 during the infection process of L. (L.) amazonenses. This study aimed to investigate whether the parasites L. (L.) infantum chagasi are capable of inducing the expression of CD200 protein during the infectious process. In trials of ex vivo infection, there was no proliferation of intracellular parasites. Nevertheless, L. (L.) infantum chagasi was able to induce the expression of CD200 gene. Interestingly, unlike infection by L. (L.) amazonenses, CD200 induction of these cells was observed at later times in infection. Immunoprecipitation assays and Western blot indicated protein synthesis, which reached their highest levels at 120 hours post-infection. The presence of CD200 suggests the involvement of this molecule at later times of infection with L. (L.) infantum chagasi.

A dinâmica da leishmaniose visceral humana na Amazônia brasileira é pouco conhecida e nos últimos anos tem aumentado a incidência da doença principalmente no Estado do Pará. Os objetivos do estudo foram: I) identificar indivíduos com infecção sintomática e/ou assintomática por Leishmania (L.) infantum chagasi, II) estudar os dois tipos de infecção, tanto clínica quanto imunologicamente, e III) estudar a taxa de prevalência da infecção. O estudo foi de corte transversal com 946 indivíduos de ambos os sexos, com idade a partir de 1 ano, vivendo em área endêmica de leishmaniose visceral americana (LVA), município de Barcarena, Pará, Brasil. Para o diagnóstico da infecção, foram usadas a reação dérmica de hipersensibilidade retardada (Reação Intradérmica de Montenegro) - RIM e a Reação de Imunofluorêscencia Indireta (RIFI). Foram diagnosticados 120 casos de infecção com taxa de prevalência de 12.6%; 8 casos mostraram alta soro-reatividade (1.280 a 120.240) IgG utilizando-se o teste da RIFI e nenhuma reação na RIM; 4 casos foram típicos de LVA e outros 4 casos de infecção subclínica oligossintomática. Os dois métodos imunológicos usados simultaneamente com o exame clínico permitiram a identificação de 5 perfis clínico-imunológicos: Infecção Assintomática (IA) 73,4%; Infecção Subclínica Resistente(ISR) 15%; Infecção Subclínica Oligossintomática (ISO) 3%; Infecção Sintomática (IS=LVA) 3% e, Infecção Inicial Indeterminada (III) 5% / The dynamic of the human visceral leishmaniasis in the Amazon region is not well known and the incidence of the diseases in the last years have increase mainly in the Para state.
