There is an immunological development in the pathogenesis of this disease immunized by Th1 and Th17 cells.Recently there is evidence that plays a crucial role in the pathology of psoriatic arthritis.The prevalence of 06% to 42% of patients with psoriasis develop a joint disease. Some studies have been analyzing these items in detail, thus giving rise to methods for measuring quality of life, since to obtain a quality life it is necessary to evaluate the individual as a whole in psychological, social and physical aspects. The questionnaires are instruments, in which they collect information and characterize a composite research technique, in the objective form of questions presented in writing in the research, in application of the knowledge of the researcher.PASE-P: developed as a screening tool for PAs more effectively in individuals with Ps and validated by Husni, et al.2007. Its sensitivity is 76% and specificity is 82%. NAPSI instrument that evaluates each nail by dividing them into four quadrants, to be evaluated for the presence of any alteration coming from both the nail matrix and nail bed. PASI: An instrument developed in 1978 to quantify the severity of Ps in order to evaluate the body surface and was segmented into 04 regions of the head, trunk, lower limbs and upper limbs evaluated on a scale of 0 to 72.DLQI: Questionnaire aiming to obtain detailed data on the quality of life of patients with psoriasis.And it consists of 10 questions about symptoms and feelings, daily activities, leisure, work and school, personal relationships and treatment. CASPAR: Investigates the presence of joint manifestations, dermatological diseases or family history, typical nail changes, the existence of joint swelling and radiological changes. OBJECTIVES: To evaluate the presence of arthritis in male and female patients with Ps, aged between 18 and 80 years using a specific questionnaire to evaluate the severity of psoriasis, using PASE-P, NAPSI, PASI, DLQI AND CASPAR. And determine by means of laboratory variables the PCR and FR. OBJECTIVE: To compare and correlate the severity parameters of psoriasis PASE-P, NAPSI, PASI, DLQI and CASPAR in a screening with patients from the dermatology outpatient clinic to identify psoriatic arthritis, seeking correlations with literature content.METHOD: The study used a cross-sectional, longitudinal and prospective study, approved by the Research Ethics Committee (CEP) of the Celso Pierro Hospital and Maternity Hospital (HMCP) in Campinas-SP. RESULTS: During the study, 63 patients did not present withdrawal, 57.1% were men and 42.9% were women. We obtained a statistically significant correlation between PASE-B, which depicts the functionality and the CRP that characterizes the inflammatory activity.Patients with a PASE-P score greater than 44, defined as suspected APs, 15.9% of arthritis. Correlations for PASE-P greater than 44 obtained the following statistically significant correlations PASE-B with PASI r = 0.736, p = 0.037, DLQI with PCR r = 0.900, p = 0.037, PASE-B with PCR; R = 0.360, p-0.021, PASI with NAPSI ML, r = 0.353. There was a statistically significant correlation between PASE-B which depicts functionality and the CRP that characterizes inflammatory activity. DISCUSSION: The choice of PASE-P, NAPSI, DLQI, PASI and CASPAR instruments in psoriasis was to evaluate their applicability in clinical practice. PASE-P has proved to be relevant, since it is a simple and quick and practical resource, and so far there are few studies with the publication in the Portuguese language version. The questionnaire PASS a recent tool to evaluate psoriatic arthritis, developed by Husni, et al, 2007. CONCLUSION: PASE-P as a specific instrument has been shown to be effective in the detection of some PA patients in a group of patients with psoriasis, as seen in the literature. Each case should be individualized and treated according to the association of disease severity parameters. It can be emphasized, therefore, that the association of PASE, CASPAR, NAPSI and DLQI can be of great assistance in clinical practice for both evolutionary follow-up and therapeutic decision-making. Taking into account that according to what was shown in this study the PASE-P is a specific instrument and that in a dynamic and effective way when detecting PAs in patients. / INTRODU??O: Psor?ase doen?a inflamat?ria cr?nica da pele, caracterizada por les?es r?seas ou avermelhadas, recobertas por escamas esbranqui?adas e secas, seu formato em placas ? o tipo mais comum. Em todas as formas de psor?ase o envolvimento ungueal pode estar presente em cerca de 50% dos casos. Os indiv?duos com psor?ase apresentam deformidades nas unhas e tend?ncia a desenvolver artrite psori?sica. A artrite psori?sica doen?a inflamat?ria das articula??es de maneira divergente envolve a coluna vertebral e articula??es perif?ricas, pode afetar at? 30% das pessoas portadoras da psor?ase entre 5 a 12 anos ap?s seu desenvolvimento como dactilite,entesite e articula??es perif?ricas. Objetivo: Investigar se a ETCC sobre o c?rtex motor prim?rio ir? modular o controle do SNA em pessoas com LM e se estas altera??es s?o dependentes do grau e n?vel de les?o. M?todos: Estudo randomizado, placebo controlado, aplicou ETCC an?dica ou sham sobre o c?rtex motor prim?rio (M1), bilateralmente, com o eletrodo de refer?ncia sendo colocado na protuber?ncia occipital. Dezenove sujeitos (les?o incompleta baixa (n=7), completa baixa (n=9) e completa alta (n=3)) visitaram o laborat?rio tr?s vezes e receberam ETCC ativa ou sham por 13min. A variabilidade da frequ?ncia card?aca (VFC) foi medida antes, durante e p?s-estimula??o. A VFC foi calculada usando a analise espectral batimento a batimento dos intervalos derivados das ondas r-r. Resultados: A ETCC modulou o sistema nervoso auton?mico de forma divergente nos grupos analisados. Nos indiv?duos com LM completa alta a ETCC n?o alterou a VFC. Para os indiv?duos com LM incompleta baixa a ETCC alterou a VFC no sentido de aumentar o simp?tico e reduzir o parassimp?tico e para os indiv?duos com LM completa baixa a ETCC reduziu o simp?tico e aumentou o parassimp?tico. Conclus?o: O presente estudo observou que a ETCC an?dica aplicada sobre o c?rtex motor prim?rio de forma bilateral, conseguiu modular o balan?o do SNA em pessoas com les?o medular e este efeito ? dependente do grau e n?vel de les?o. Palavras-chave: Estimula??o Transcraniana; Sistema nervoso auton?mico; Variabilidade da frequ?ncia card?aca; Les?o medular. / Introduction: Transcranial Direct Current Stimulation (tDCS) has been used in studies for the treatment of chronic pain, but their effects on the autonomic nervous system (ANS) are non-existent. Therefore, the need for studies is of fundamental importance, as these individuals have autonomic imbalance and the intensity of this is dependent on the degree and level of injury. Objective: We investigated the effect of tDCS on the ANS in people with spinal cord injury (SCI) with different degrees and levels of injury. Methods: Randomized, placebo-controlled, double-blind, applied anodal tDCS or sham on the primary motor cortex (M1), bilaterally. The subjects (lower incomplete injury, n = 7; lower complete injury, n = 9; and high complete thoracic injury, n = 3) visited the laboratory three times and received active or sham tDCS for 13min. The heart rate variability (HRV) was measured before, during and after stimulation and analyzed the variables LF, HF and LF / HF. Results: The tDCS modulated the ANS in different ways among the groups. In individuals with SCI high complete thoracic the tDCS did not change the HRV. However, for individuals with SCI low incomplete, tDCS changed the HRV in order to increase sympathetic (LF, p = 0.046) and reduced parasympathetic (HF, p = 0.046). For individuals SCI low complete to tDCS changed the HRV reduction sympathetic (LF, p = 0.017) and increased parasympathetic (HF, p = 0.017). Conclusions: The present study suggests that anodal tDCS applied on the motor cortex bilaterally could modulate the ANS balance in people with spinal cord injury and that this effect is dependent on the degree and level of injury.

Trata-se de um estudo imuno-histoqu?mico (t?cnica da imunoperoxidase), quantitativo e descritivo de 52 casos de LCCGs dos ossos maxilares, nos quais 13 pacientes portadores de LCCG foram tratados com triancinolona intralesional ou calcitonina spray intranasal. A m?dia de imunomarca??o foi comparada entre os tipos celulares e subtipo cl?nico da les?o. O teste de Mann-Whitney foi realizado para essas compara??es. Dos 52 casos de LCCGs, 53.8% eram do g?nero feminino, com uma m?dia de idade de 25.69 anos. A mand?bula foi o s?tio anat?mico mais acometido. Trinta casos (57.7%) foram de LCCGs agressivas e 22 (42.3%) de n?o agressivas. A cirurgia foi o tratamento de escolha em 75% das LCCGs estudadas. Em 56.7% das LCCGs agressivas foi realizada cirurgia, enquanto 43.4% foram submetidas ao tratamento conservador. Dos submetidos ao tratamento conservador, a maioria (n = 8; 61.5%) respondeu bem ao tratamento. A express?o de CTR foi evidenciada em 67.3% da amostra e para o GCR em 96.15% dos casos. N?o houve diferen?a estatisticamente significante entre a express?o de CTRs e GCRs em c?lulas mononucleares e multinucleadas das LCCGs em rela??o ? agressividade, em rela??o ao tratamento realizado para os casos de les?es agressivas e em rela??o ? resposta ao tratamento conservador realizado nos casos de LCCGs agressivas (p>0.05). Os resultados da nossa pesquisa sugerem que a imunoexpress?o dos CTRs e GCRs n?o influenciou na resposta ao tratamento cl?nico com calcitonina ou triancinolona na amostra estudada e exibiu uma express?o variada independente da agressividade da les?o. / The aim of this study was to analyze the immunoexpression of calcitonin (CTR) and glucorticoid (GCR) receptors in aggressive and non-aggressive central giant cell lesions (CGCL). This is an immunohistochemistry study (immunoperoxidase technique) of 52 cases of CGCL of the jaws, in which 12 patients were treated with intralesional triamcinolone injections and one with calcitonin nasal spray. The mean of immunostaining was compared between the cell types and clinical subtype of the lesion. The correlations among means were analyzed by Mann-Whitney test. Of the 52 cases studied, 53.8% were females, with a mean of 25.69 years. Most lesions were located in the mandible. Thirty patients (57.7%) had aggressive lesions and 22 (42.3%) of the cases consisted of non-aggressive lesions. Surgery was the treatment of choice in 75% of the cases. In 56.7% of the aggressive CGCL surgery was performed, while 43.4% of patients were submitted to conservative treatment. Among cases submitted to conservative treatment, the majority (n = 8; 61.5%) responded well to treatment. CTR expression was observed in 67.3% and GCR in 96.15% of cases. There was no significant statistical difference between the expression of CTRs and GCRs in mononuclear and multinucleated CGCLscells, regarding aggressiveness, treatment performed for aggressive lesions and the response to conservative treatment (p>0.05). The results of our research suggest that the immunoreactivity of CTRs and GCRs did not influence the response to clinical treatment with calcitonin or triamcinolone in the sample studied and it exhibited a varied expression regardless of the aggressiveness of the lesion.

We previously reported the upregulation of TREML4 in the early phase of theacute coronary syndrome. Here, we investigated the relationship of TREML4 polymorphisms and mRNA expression in blood leukocytes of patients stratificate accordingto the extension of coronary artery lesion. Patients with coronary artery disease (CAD) (n = 151), aged 30 to 74 years and undergoing elective coronary angiography,were selected. The extension of coronary lesion was assessed by the Friesinger index.TREML4 rs2803495 (A>G) and rs2803496 (T>C) variants andleukocyte mRNA expression were analyzed by qRT-PCR. TREML4 expression was higher in patients with major coronary artery lesions compared to subjects without or with low and intermediate lesions, (p < 0.05). Subjects carrying rs2803495 G allele (AG/GG genotypes) and rs2803496 C allele (TC/CC genotypes) were more prone to have positive TREML4 mRNA expression (Allele G: OR = 2.4, 95% CI 1.0-5.7, p < 0.05; allele C: OR = 7.8, and 95% CI = 2.9-20.9, p < 0.01, respectively).However, TREML4 polymorphisms were not associated with the extent of coronary lesion.In conclusion, increased TREML4 mRNA expression in blood leukocytes is influenced by gene polymorphisms and it is associated with more severe coronary artery lesions, suggesting its role as potential biomarker to investigate the extent of coronary lesion in CAD patients.

Studies have shown that affected individuals have a sedentary lifestyle that reflects negatively on health and quality of life. Studies have demonstrated that HDL cholesterol (HDL-C) levels, a high-density lipoprotein and important predictor of cardiovascular disease, are lower in this population exposing these people to a greater incidence of heart disease from atherosclerotic process In the general population, exercise increases HDL-C serum levels, but this phenomenon is not very clear in people with spinal cord injury (SCI). The present study examined the effect of both swimming and wheelchair basketball in the lipid profile of eleven men and seven women with SCI. The subjects included in regular exercise programs showed increases in HDL-C levels and decreases in CT/HDL-C and LDL-C/HDL-C ratios. We found better results mainly in men with lower levels of SCI and in those that sustained exercise intensities above 60% of the heart rate reserve. The duration of training sessions can be an essential factor in these results. The results suggest that both the exercise prescription and the personal characteristics of people with SCI influence changes in the lipid profile mediated through exercise. The elaboration of this work is an attempt to clarify uncertainties about health and the longevity of people with SCI generated in discussion of all members of the interdisciplinary rehabilitation team, especially the physiotherapists, nutritionists, nurses and physicians that contributed considerably in all phases of the research / Apesar do aumento da expectativa de vida das pessoas com les?o medular (LM), esta ? ainda inferior ? da popula??o em geral. Pesquisas demonstram que os indiv?duos acometidos pela LM levam um estilo de vida sedent?rio, o que repercute negativamente na sa?de e na qualidade de vida. Os n?veis do colesterol HDL (HDL-C), cada vez mais se consolidam como importantes preditores de doen?a cardiovascular. Na popula??o com LM, os n?veis destas lipoprote?nas encontram-se significativamente diminu?dos, expondo estes indiv?duos a uma maior incid?ncia de doen?as cardiovasculares ligadas ao processo ateroscler?tico. Estudos na popula??o geral assinalam que o exerc?cio f?sico regular eleva os n?veis s?ricos de HDL-C. Por?m, este fen?meno n?o ? muito claro para aqueles com LM. The influence of several variables on the function of these patients has been analyzed, but there is no consensus in the science literature. Purpose: To evaluate the correlation between the torque and balance on the knee function after ACL reconstruction. Methods: 23 males patients with ACL reconstruction were tested. The procedures of the study included analysis of concentric peak torque at 60o/s and 180o/s of quadriceps femoris and hamstring muscle with a isokinetic dynamometer. The balance in single-limb stance was measured with stabilometry. The functional performance were performed by two hop tests. To estimate the subjective function of the patients was applied Lysholm Knee Scoring Scale and a Global Rating scale. Results: The analysis of data showed a moderate positive correlation between knee extensor torque and functional performance tests (r= 0,48; p=0,02). A moderate negative correlation was found between the two variables of the stabilometry center of pressure and average speed of centre of pressure and the Global Rating scale (r = -0.4, p = 0,04 and r = -0,49, p = 0 ,02, respectively). No correlation was found between peak torque and balance in single-limb stance. Conclusion: The results of the present study suggest that knee extensor strength and postural balance have some influence on knee function in patients after ACL reconstruction / Introdu??o: Diversos estudos ressaltam a import?ncia de avaliar a fun??o do joelho ap?s a reconstru??o do Ligamento Cruzado Anterior (LCA). The purpose of this study was to compare the immunohistochemical expression of bone resorption factors RANK (Receptor Activator of Nuclear Factor kappa B), RANKL (Receptor Activator of Nuclear Factor kappa B Ligand) and OPG (Osteoprotegerin) between CGCL and PGCL. Additionally, these bone resorption factors were examined in terms of aggressiveness of these lesions. The sample consisted of 61 cases, 30 cases of PGCL and 31 CGCL (16 non-aggressive and 15 aggressive). The analysis was performed by quantification of mononuclear cells (MO) and giant multinucleated cells (CG) immunopositive to anti-RANK, anti-RANKL and anti-OPG antibodies in 10 fields. Moreover, according to the proportion between the amount of cells positive for RANKL and OPG, the cases were categorized into: RANKL>OPG, OPG>RANKL e RANKL=OPG. CGCL showed a higher amount of MO (p=0.002) and total cells (p=0.003) both positives to RANKL compared with the PGCL. Additionally, the CGCL revealed a significant association with the ratio of RANKL>OPG (p=0.001). Analysis of the bone resorption factors revealed no significant differences between aggressive and non-aggressive CGCL (p>0.05). It was observed a positive correlation between the markers themselves, and a negative correlation between lesion size and quantity of OPG positive MO cells (p=0,004) and total cells (p=0,009). Through these results, we suggest that the greatest CGCL resorptive potential compared to the PGCL, may have occurred to the high expression of RANKL.
