Successes and challenges of the Baby Friendly Hospital Initiative in accredited facilities in the Cape Town Metro Health District

Magister Public Health - MPH / Breastfeeding impacts on the health of both the mother and infant and has been noted as being influenced by physiological, physical, socio-economic and environmental factors. The undisputed benefit of exclusive breastfeeding for both the mother and child has led to the global prioritisation of the promotion, protection and support of breastfeeding with the adoption of the Baby Friendly Hospital Initiative (BFHI) strategy. Baby Friendly Hospital (BFH) status is awarded to a maternity unit when they are found to be complying with set criteria Ten Steps to successful Breastfeeding. South Africa has implemented a re-evaluation system for retention of accreditation status, by reassessing accredited facilities every three years. The respective provinces are tasked with monitoring the implementation of BFHI in their public health facilities. Internal monitoring reports, completed by the Western Cape Provincial Department of Health, reflect erosion of key steps between national reassessments. Aim: To describe the experiences, challenges and successes of BFHI implementation in the BFH accredited facilities in the Cape Town geographical health district. Methodology: An explorative qualitative study was conducted. One key informant interview, ten in-depth interviews with champions for BFHI in the maternity facilities and two focus group discussions with frontline staff working at these facilities were used to collect data. The data was analysed using thematic content analysis to identify the main themes related to the successes and challenges experienced with the maintenance of the required practices related to BFHI accreditation. Results: Participants reported that the implementation of the BFHI impacted positively on the health of both mothers and infants. Fewer children were being admitted for common childhood illnesses such as diarrhoea subsequent  to BFHI implementation. Mothers were recovering more quickly after delivery and less complications related to delivery, such as postpartum bleeding, were observed after the implementation of BFHI. BFHI implementation had a positive impact on the attitudes of maternity staff to breastfeeding promotion, protection and support. Subsequent to being awarded BFH status, facilities are tasked with maintaining the implemented practices. Challenges to maintaining the practices included lack of implementation of BFHI practices at clinics, lack of support from facility managers and support staff such as counsellors. The internal assessments implemented as supportive monitoring structures are considered by participants to be a demotivating process and concerns were raised about non nursing staff assessing nursing practices. Conclusion: The potential impact of this strategy on infant and maternal health must be realized by the implementers of BFHI, before the strategized aim is achieved. Co-ordination and support by all role players is vital to the success and elimination of challenges experienced with implementation and maintenance of the BFH strategy. / South Africa

At present, the Eastern Cape is the only province in South Africa lacking a clinical hospital-based hyperbaric facility. East Cape Hyperbaric, to be situated at Greenacres Hospital in the Nelson Mandela Metropolitan, will offer the Eastern Cape community access to a world-class facility that will offer their patients Hyperbaric Oxygen Therapy and a Wound-Healing Facility. The objective of this study was to assess the viability of establishing a Hyperbaric Oxygen Therapy (HBOT) facility for the Nelson Mandela Metropolitan Municipality (NMMM), using sound business planning principles. A business plan precisely defines the business, identifies the goals, and serves as the firm's resume. A business plan will assist in allocating resources effectively, handle unforeseen complications, and assist in making sound business decisions. Because it provides specific and organized information about the company and how the company will repay borrowed money, a good business plan is a crucial part of business planning. In order to quantify the demand for a Hyperbaric Oxygen Therapy facility, a questionnaire was designed, in such a manner as to identify the current demand for a HBOT facility. The questionnaires were distributed to all medical practitioners within the NMMM by means of fax, e-mail and hand-delivery. Research conducted indicates that the results are promising enough to warrant the expedient creation of this business facility.

The main function of the School-based Support Teams (SBSTs) in all schools is to assist teachers to effectively provide support to all learners experiencing learning barriers.

Service quality perceptions among different cultures in the Nelson Mandela Metropole

Managing cultural differences in today’s work environment has received the attention of researchers all over the world. What has received far less attention is the managing of the cultural differences between service provider workers and customers and how this is perceived to lead to achieving great service quality. This research project focuses on the cultural differences between culturally diverse South African service provider workers and South African customers, with some reference to international customers. The questionnaire used in this research includes questions embodying the service quality instruments developed by Parasuraman, Zeithaml, and Berry (1988) and Hofstede’s (1991) cultural dimensions. The questionnaire was completed by 81 students of the East Cape Midland College in the Nelson Mandela Metropole. A Pearson’s correlation coefficient was calculated for each service quality instrument and each cultural dimension. The resulting correlation coefficients were analysed and a positive or negative correlation between each service quality instrument and each cultural dimension is shown. The implications of each positive and negative correlation are shown and how it impacts on perceived service quality delivery. The implications for service quality management are shown, followed by recommendations for the management of a culturally diverse workforce.

Professional quality of life among nurses in psychiatric observation units in the metropole district health services

Magister Curationis - MCur / Background: Psychiatric observation units are the units where 72-hour psychiatric observations are conducted in the district and in some of the regional hospitals. These hospitals were selected under the Mental Health Care Act No. 17 of 2002 (MHCA 2002) to admit patients suspected to be mentally ill, ascertain the cause of symptoms, exclude medical illness as a cause of the symptoms, treat and rehabilitate these patients; and at times transfer the patients to tertiary psychiatric hospitals. These units are often overcrowded as only about 30% of patients are transferred to the tertiary psychiatric hospitals. These units are fraught with challenges such as shortage of crucial facilities like seclusion rooms, specialised staff, resources and minimal budget is allocated to these units. Nursing staff in these units are faced with a number of challenges such as shortage of staff, patient overflow, prolonged patient stay, psychologically disturbed patients who can be agitated and violent, and are working long hours. Therefore, Compassion Satisfaction may be affected and these nurses are prone to Compassion Fatigue, which can lead to low Professional Quality of Life. Aim & objectives: The aim was to investigate Professional Quality of Life among nurses working in psychiatric observation units in Metropole District Health Services in the Western Cape Metropole. The objectives were to measure Compassion Satisfaction, to measure levels of Burnout and determine levels of Secondary Traumatic Stress among nurses working in psychiatric observation units in the Metropole District Health Services. Method: A quantitative research approach using a descriptive design was used to determine the Professional Quality of Life of nurses working in psychiatric observation units in the Metropole District Health Services. A self-administered survey using a structured questionnaire, the Professional Quality of Life version 5 (ProQoL 5) was used to collect data from an all-inclusive sample of 175 nurses, yielding a response rate of 93% (n=163). Data was analysed using the Statistical Package of Social Services (SPSS) version 24. Findings: The findings of this study showed that respondents experienced moderate Compassion Satisfaction, moderate Burnout and high Secondary Traumatic Stress. Advanced psychiatric nurse practitioners and registered nurses reported lower Compassion Satisfaction, higher Burnout and higher Secondary Traumatic Stress than enrolled nurses and enrolled nursing assistants. Recommendations: Qualitative research studies need to be conducted on nurses working in psychiatric observation units in order to understand experiences and factors affecting Professional Quality of Life among nurses. Qualitative research studies need to be conducted in order to understand factors affecting Professional Quality of Life of advanced psychiatric nurse practitioners and general registered nurses in psychiatric observations units.

O empreendimento consiste na busca dos significados do grafite a partir dos depoimentos de sete jovens grafiteiros pertencentes às camadas médias-altas da Zona Sul da cidade do Rio de Janeiro, da interpretação das imagens fotográficas dos grafites urbanos e da análise dos registros do diário de campo da pesquisa. O intuito maior em documentar a atuação de Twig, Broz, Fel, Rio, Det´s, Zat e Dog foi descobrir a razão pedagógica que torna o grafiteiro, grafiteiro. Não obstante, busco compreender e revelar que valores, gestos e conteúdos são mantidos, atualizados e transmitidos de grafiteiros a grafiteiros a fim de formar-lhes o ethos e agregá-los à cultura do grafite. É justamente pela perspectiva da transmissão do modus adquirendi de geração a geração que busco os nexos e as relações entre o grafite urbano e o processo educativo que os partícipes experimentam e pelo qual se tornam grafiteiros. Aqui encontro o principal indício de que tal itinerário configura-se enquanto uma pedagogia que, no caso específico desta tese, é a própria Pedagogia do Spray. / [en] A document of performance. This is the format of this thesis. Its main task consists in a search for the meanings of the graffiti from the standpoint views and depositions of seven young graffiti writers belonging to the upper-medium class of the southern area of the city of Rio de Janeiro. It also deals with the interpretation of the photographic images of the urban drawings as well as the analysis of the registries on the field diary. The major purpose in documenting the performance of Twig, Broz, Fel, Rio, Det´s, Zat and Dog was to discover the pedagogic reason behind those who choose this form of expression - the graffiti. We also tried to understand and to reveal how values, gestures and contents are maintained, updated and transmitted from each older and more experienced graffiti writers to the new ones establishing an ethos, which provides them with the means to become a part of the graffiti culture. It is exactly from the perspective of the transmission, of the modus adquirendi, from generation to generation that we search for the connections and the relationships between the urban graffiti and the educational process that involves the process of becoming graffiti writers. Our findings show a strong indication that such itinerary might be viewed as a pedagogy that, in this specific case, we designate as Pedagogy of the Spray.

Teachers’ attitudes with regard to bullying at a high school in the Cape Metropole

Thesis submitted in partial fulfilment of the requirements for the degree Master of Education in the Faculty of Education and Social Sciences at the Cape Peninsula University of Technology, 2011 / Bullying at schools is a worldwide phenomenon and has been extensively researched. However, the role of teachers in this regard, specifically their responses and attitudes towards bullying, has not been as widely covered or reported on. In this study a qualitative case study approach was used to explore teachers‟ attitudes with regard to bullying at a high school in the Cape Metropole. Teachers‟ attitudes, responses, understandings and perceptions with regard to bullying were explored primarily through interviews with nine staff members at the school. The purpose of the research is to attempt to create a greater awareness among teachers of the phenomenon of bullying, and to investigate how their attitudes affect the dynamics in the school yard and related educational issues such as retaliation, tolerance, etc. The research was conducted according to international ethical guidelines and with the permission of the Western Cape Education Department (WCED). The literature review for this study covers two main areas: bullying and teachers‟ attitudes towards it. The area of bullying looks at the phenomenon itself, the definitions thereof, how it is viewed in this study, types of bullying, the role players as well as the effects. Teachers‟ attitudes towards bullying hone in on school climate, the whole-school approach to bullying prevention, teachers‟ attitudes with regard to bullying, and the South African context of the study. Findings will be reported in a narrative format, considering the methodology cited and the literature review. The conclusions in this study are meant to contribute to the overall prevention of bullying in schools in this country. The conclusions concerning teachers‟ attitudes with regard to bullying should be of some value in the ongoing challenge to minimise the scourge of bullying in South African schools.

Estes empreendimentos oferecidos, por vezes de forma ilegal, têm representado cada vez mais uma promessa de melhor qualidade de vida urbana a partir da adoção de um estilo de vida exclusivo e justificado por uma eventual fuga da violência urbana. Diante deste quadro nos questionamos sobre a dimensão do impacto ambiental destas estruturas urbanas no território e meio ambiente das cidades. Esta pesquisa baseia-se na hipótese de que o impacto da proliferação dos loteamentos fechados nas áreas metropolitanas interfere tanto nas áreas urbanas quanto nas rurbanas em sua sustentabilidade. Baseado nisto, temos como objetivos específicos: analisar os loteamentos fechados como um produto do mercado; tipificar como esses empreendimentos interferem na estrutura territorial da região metropolitana de Campinas; observar como se dá a oferta de serviços ambientais e quais substitutos são gerados no processo de apropriação da natureza. Com isso busca-se identificar de que forma as diferentes tipologias destes empreendimentos, estabelecem relações entre si formando um mosaico no território e como estas relações interferem na sustentabilidade dos serviços ambientais. These housing development, have represented, each time more, a promise of better quality of urban life with adoption of a life style exclusive, that are justified by an eventual escape of the urban violence. This research is based on hypothesis that the impact of the proliferation of gated communities interferes in urban and rurbans areas of metropolitans territory. This thesis have as objectives: to analyze the gated communities as product of market; to observe how these enterprises intervene with the territorial structure of the Metropolitan Region of Campinas- MRC, state of São Paulo - Brasil; to observe like the environment services are offered and how substitutes services are generated in the process of appropriation of the nature.
