A qualitative exploration of the transmission of knowledge and skills by specialist stoma care nurses to facilitate the needs of patients adapting to a newly formed stoma

Aim: To identify the knowledge and skills required by the specialist stoma care nurse to facilitate the needs of patients adapting to a newly formed stoma in order to inform specialist nurse education. Background: Advances in surgical techniques, drug therapies, bowel screening and patient recovery programmes have offered those with colorectal disorders, a potential increased life expectancy and improved disease management. For the specialist nurse, there is the constant challenge to keep abreast of these advances and provide more formal, precise and accurate information, facilitating individualised patient need. In response, an education curriculum needs to foster a comprehensive knowledge base in order to equip and support the nurse to become confident and competent in clinical practice. How knowledge is translated from the classroom into clinical practice is a key feature of this study; in particular the role of the specialist stoma care nurse in facilitating the adaptation of patients following stoma surgery. Method: A qualitative approach was chosen to fulfil the aim and objectives of this two-staged study. Phase one took a phenomenological approach and phase two a focus group methodology approach. Eight patients were interviewed in phase one. This offered insight into the experiences of living with a newly formed stoma. Vignettes (total 18) were created from the patients’ narratives. These were used to stimulate discussion among nurses at the focus groups within phase two of the study. The complexities of translating specialist knowledge and skills among thirty-nine specialist stoma care nurses were explored through one of a series of seven focus groups. The analysis for this study was undertaken in 3 stages; stage 1, thematic analysis of phase 1, stage 2, thematic analysis of phase 2 and stage 3, further analysis of phase 2 using Mayer and Salovey (1997) emotional intelligence theoretical framework. Findings: Phase one highlighted a variety of challenges faced by patients and recognised their coping strategies as they adapted to a newly formed stoma. Six themes emerged from the thematic analysis; seeking assurances, permanence versus reversibility of stoma, anticipated stigma, psychological projection, worthwhile sacrifice and sense of achievement. The thematic analysis of phase two identified five comparable themes; skilled know-how, understanding self, hurdles to accommodate, communication skills and uncertain ground. A further examination through an alternative lens was considered to distil the pedagogy. Mayer and Salovey (1997) emotional intelligence framework guided the third stage of analysis. Five key concepts emerged, forming the essential components to specialist nursing practice; the influence of emotions on critical thinking and clinical decision-making, the use of empathetic and intuitive skills in clinical judgements, the ability to balance true self alongside professional self, the need to foster cognitive activity, good communication and creativity and preserving a conscious awareness of self so to develop personal growth. Conclusion: Individual experiences of patients adapting to a newly formed stoma evidently differed. Both the challenges faced and coping strategies of the patients were revealed. The specialist stoma care nurse is ideally placed to facilitate meeting the patients’ needs as they adapt. The five essential components to specialist practice are identified. Educational strategies for cultivating critical thinking and communication skills, developing self and emotional conscious awareness and nurturing personal growth need to be considered for integration into specialist nurse educational curriculum.

Improving Nurses' Knowledge of Central Line-Associated Bloodstream Infection

Central line-associated bloodstream infections (CLABSI) are usually attributable to suboptimal line insertion, care, or maintenance and are associated with poor patient outcomes. Nursing plays a pivotal role in preventing CLABSI, because nurses are generally responsible for the routine care and maintenance of central lines. The purpose of this project was to determine if CLABSI nursing education and demonstration-based competency could improve nurses' knowledge on a neurology unit as compared to current practice of an annual e-learning module as the sole source of nurse education. This project was informed by Lewin's planned change theory and involved changing behaviors, attitudes, and practices of nurses via a conducive approach consisting of three phases: unfreezing, movement, and refreezing. To have a foundation in evidence, expert literature supports the project. Participating nurses attended an educational session consisting of a presentation and demonstration-based competency of central line dressing change technique. A pre- and post-test were administered; the mean pretest score was 72.1% and the mean posttest score was 94.1%. Comparison of pre- and post-test scores reflect a 22% increase in test scores, therefore, this program increased knowledge. This project can contribute to positive social change by improving nursing practice through increasing nurses' knowledge of proper care and maintenance of central lines, which can translate into evidence-based practice changes and improve patient outcomes.

This theme of thesis treats about the issue of prevention, emergence, healing and therapy of chronic wounds in nursing care at intensive care unit. The occurrence of this disease is unfortunately still rising and nowadays it is an actual theme which is a part of modern nursing. The target of this thesis was to find out the level of awareness of general nurses working at specialized departments about modern technologies of therapy and care of healing chronic wounds. One of the partial targets was to find out, how are the nurses educated in nursing care o chronic wounds. The next target was looking for the factors which can influence the nurses' position of knowledge about the treatment of chronic wounds. Quantitative research was based on anonymous questionnaire in three chosen hospitals of Central Bohemia region. There were 11 departments of acute intensive care. The sample of research was based on the answers of general nurses working at resuscitation department and acute intensive care department. There were distributed 220 questionnaires and were used 124. One of the result of research was, that the 56 (45%) of general nurses has good awareness in nursing care of chronic wounds and they were interested in future education in this issue. Bakgrund Våld i nära relation är ett globalt problem. Sjuksköterskorna inom primär och akutsjukvård har ett viktigt ansvar att identifiera våldsutsatta kvinnor i nära relation. Sjuksköterskorna ställs inför utmaningar i mötet med dessa kvinnor för att kunna ge god omvårdnad. Syftet med denna studie är att beskriva sjuksköterskors erfarenheter av att möta kvinnor som blivit utsatta för våld i nära relation. Metod en litteraturstudie som följer Polit och Becks niostegsmodell och med kvalitativ innehållsanalys. Resultat Redovisas i två kategorier Komplexitet i vårdmötet och Arbetsmiljöns inverkan. Komplexitet i vårdmötet beskrivs i två underkategorier Att känna sig ambivalent, Känslomässiga reaktioner, Arbetsmiljöns inverkan beskrivs i tre underkategorier Utmaningar i det dagliga arbetet, Behov av tid och Behov av mer kunskap. Slutsats Sjuksköterskor känner sig otillräckliga i sin yrkesroll, frustration och hjälplöshet kan uppstå. Behov av mer tid till personcentrering och kunskap ses som avgörande för att sjuksköterskorna ska kunna ge en god omvårdnad i mötet med kvinnor utsatta för våld i nära relation. / Background Intimate partner violence is a global problem. Method A literature study that follows Polit and Beck’s nine-step model with qualitative content analysis. Results Reported in two categories Complexity in the care meeting and the impact of the Work Environment. Complexity in the care meeting is described in two subcategories Feeling ambivalent, Emotional reactions, The impact of the work environment is described in three different subcategories: Challenges in the daily work, Need for time and Need of more knowledge. Conclusion Nurses feel inadequate in their professional role, frustration and helplessness arise. The need for more time and knowledge is seen as crucial for the nurses to be able to provide good nursing care in the meeting with women exposed to intimate partner violence.

Bakgrund: En av tre kvinnor utsätts för våld i en nära relation, detta har kommit att bli ett stort samhällsproblem inte bara i Sverige utan i hela världen. Då sjuksköterskan oftast har det första mötet med patienten är det av stor vikt att sjuksköterskan besitter relevant kunskap kring omvårdnad av kvinnor utsatta för våld i nära relation.Syfte: Syftet med litteraturöversikten är att beskriva sjuksköterskors kunskap och attityder kring omvårdnad av kvinnor utsatta för våld i nära relation.Metod: Studien har genomförts som en litteraturöversikt av sexton artiklar från hela världen. De databaser som används för att hitta artiklarna var CINAHL och PubMed.Resultat: Resultatet visade att sjuksköterskor upplever en brist på kunskap gällande omhändertagandet av våldsutsatta kvinnor samt att det saknades möjligheter till klinisk övning på att kunna vårda dessa kvinnor på ett adekvat sätt. Bristen på behandlingsriktlinjer från klinikens organisation var bidragande till bristfälliga omvårdnadsinsatser. Ännu en barriär som framkom ur resultatet var bristande beredskap inför mötet med kvinnor som utsatts för våld i nära relationer. Detta baseras på sexton vetenskapliga artiklar vilka sammanfattades i tre huvudteman; Attityder, förhållningsätt och stigmatisering¹, Brist på kunskap², Brist på utbildning och behandlingsriktlinjer³.Slutsats: Utifrån studiens resultat dras slutsatsen att sjuksköterskor behöver mer utbildning, kontinuerlig övning och tydliga riktlinjer för att på ett adekvat sätt vårda kvinnor som utsatts för våld i nära relationer. / Background: One out of three women are exposed to some type of intimate partner violence during a relationship. Since the nurse usually is the one having the first meeting with the patient it is out of great importance that the nurse has relevant knowledge about the care given to women exposed to intimate partner violence.Aim: The aim of this literature review is to describe nurses´ knowledge and attitudes of caring for women exposed to intimate partner violence (IVP).Methods: The method of this study were a literature review of sixteen articles from around the world. The databases used to find the articles were CINAHL and PubMed.Results: The result showed that nurses experience a lack of knowledge regarding the care of abused women and that there are no opportunities for clinical practice in being able to care for these women in an adequate manner. The lack of treatment guidelines from the clinic's organization contributed to inadequate nursing efforts. One more barrier emerged from the result were lack of preparedness for the meeting with women who are exposed to intimate partner violence. This is based on sixteen scientific articles which were summarized in three main themes: Attitudes, approaches and stigma¹, Lack of knowledge², Lack of education and guidelines³.Conclusions: Based on the results of the study, it is concluded that nurses need more training, continuous practice and clear guidelines to adequately care for women who have been subjected to intimate partner violence.
