The nurse as generalist needs the additio~al support of a Clinical Nurse Specialist with advanced knowledge and nursing skills. The Clinical Nurse Specialist must be able to assist the general nurse in giving effective and quality nursing care. A exploratory and descriptive survey was undertaken by means of a interview, questionnaires and a patient acuity classification system. Data was obtained from Senior Nursing Service Managers, Nursing Service Managers, Chief Professional Nurses, senior and junior registered nurses. The purpose of the study was to determine the desirability of and functions of a Clinical Nurse Specialist in a postacute care unit. Analysis of the data obtained indicated that there is a need for. a Clinical Nurse Specialist to assist with specialized nursing care and to assure continuity of care when a patient is moved from a critical care unit to a postacute care unit.

Reporting on radiographic images in after-hours trauma units :Experiences of radiographers and medical practitioners

Globally there is a lack of radiologists, which results in unreported radiographic examinations, or a delay in reporting on radiographic images even in emergency situations. In order to mitigate and alleviate the situation, and optimise the utilisation of radiographers a red dot system was introduced in the United Kingdom, which later aided in the transformation of the role of radiographers in terms of formal reporting of various radiographic examinations. Although there is a shortage of medical practitioners and radiologists in South Africa the extended role of radiographers has not been yet realised for radiographers. At present, radiographers and medical practitioners work in collaboration to interpret and report on radiographic examinations informally, to facilitate effective and efficient patient management, but this is done illegally because the regulations defining the scope of the profession of radiography does not allow for such practice, putting radiographers and organisations at risk of litigation. In order to gain an in-depth knowledge of the phenomena, to enable the researcher to provide recommendations to the Professional Board of Radiography and Clinical Technology (PBRCT) of the Health Professions Council of South Africa (HPCSA), a qualitative, exploratory, descriptive, and contextual research study was undertaken. Radiographers and medical practitioners were interviewed in order to elicit rich descriptions of their experiences regarding reporting of trauma related radiographic images in the after-hours trauma units. Data were gathered using in-depth semi-structured interviews, and the data were analysed using kesch’s method of thematic synthesis. Three themes emerged from the data, namely the challenges radiographers and medical practitioners face in the after-hours trauma units respectively, with regards to reporting of trauma related adiographs, and suggestions were proposed to optimize the participation of radiographers with regard to trauma related radiographs in these units. A thick description and literature control was done using quotes from participants. Measures to ensure trustworthiness and ethical research practices were also implemented. Thereafter, recommendations were put forward for the PBRCT of the HPCSA, using current literature and inferences made from the findings of the study.

Leadership competences of professional nurses in the eThekwini health district during the first year of registration with the South African Nursing Council

Submitted in accordance with the requirements of the Degree In Masters in Health Sciences (Nursing), Durban University of Technology, Durban, South Africa, 2018. / This qualitative research study was conducted to explore and describe leadership competencies of the newly registered professional (NRP) nurses during the first year of registration with the South African Nursing Council (SANC) in selected provincial hospitals in the eThekwini health district. A qualitative, exploratory, descriptive study methodology was used. Semi-structured interviews were used to collect data; open- ended questions provided participants with an opportunity to provide rich and detailed information about selected experiences as qualified professional nurses. The main objective of the study was to build on prior work in order to explore and describe selected leadership competencies of the NRP nurses, particularly those related to inability to manage conflict, uncertainty and fear of having to delegate, and observing unethical practice. Benner’s model of Novice to Expert nurse was used as the organising framework. The purposive sample comprised eight R425 trained professional nurses in their first year of registration with the SANC, who were working in the selected regional hospitals of eThekwini health district. All participants had completed one year of practice and reported believing that they were at Stage 3: Competent of Benner’s Stages of Clinical Competence model – from Novice to Expert. Six themes emerged, namely: support; uncertainty and fear of having to delegate; competence; transition to professional nurse; observed unethical practices; fear of victimisation; and difficult relationships. From the above stated themes fifteen sub-themes emerged. The results found that nurses are continuing to transition into Benner’s Stage 5: which requires continued support and integration as they evolve in their roles as professional nurses. / M

The study aimed to explore and describe factors which impacted on the satisfaction of patients using the curative component of primary health care in the Ekurhuleni Metropolitan area. A qualitative, explorative and contextual design was followed in this study. Focus group interviews were used to collect data from clients, nurse clinicians and community health committee members. Findings indicated that there was lack of facilities, resources and supplies, lack of safety and security measures, negative attitudes of nurse clinicians, lack of community involvement and lack of clinic management involvement. Suggestions were made by the groups on how to improve the curative primary health care service and intervention strategies were identified from the suggestions made. The aim of the study was to explore the perceptions of nurses regarding communication with nurse managers in the workplace. The design of the study was a generic qualitative. The methodology of choice was qualitative, explorative method. The sample was non-probability and the approach or technique used was purposive sampling method. It comprised of thirty nurses, ten of each category. The category was a component of professional nurses, enrolled nurses and enrolled nursing auxiliary nurses. The data collection method used in the study was in-depth interviews using a self-designed interview guide. Face to face interviews was conducted in a quiet room within the hospital ward as a natural setting. Data was collected using a voice recorder for the sake of protecting the missing of information which may be important. The data analysis was with the help of employing transcribing and coding of voice recorded data and observation noted during the collection of data. The data collection method used in the study was in-depth interviews using a self-designed interview guide. Face to face interviews was conducted in a quiet room within the hospital ward as a natural setting. Data was collected using a voice recorder for the sake of protecting the missing of information which may be important. The data analysis was with the help of employing transcribing and coding of voice recorded data and observation noted during the collection of data. The findings showed that there were dynamics in communication from all nurse categories and that also indicated that there was a need to conduct a research so that the root cause may be identified and suggestions to be put in place to curb the challenges. Student nurses’ perceptions of professional nurses as role models in the clinical learning environment

Text in English / This study explores and describes the perceptions of student nurses regarding professional nurses as role models in the clinical learning environment. An exploratory-descriptive qualitative approach was followed. The population for this study was the final year students in the programme: Bridging course for enrolled nurses leading to registration as a general nurse. Nonprobability, quota sampling was done. The study was conducted at two Gauteng campuses of a private nursing education institution. Two “World Café” conversations were conducted where after data saturation was achieved. The three major themes that emerged were professionalism of the professional nurse, the need for student support and the teaching and learning environment. From the students’ feedback it was evident that professional nurses should be aware of the important role they play in the professional and personal development of students. Professional nurses as role models should portray the professional behaviours and attitudes required by the profession. Students have a vision of how they expect to be when they qualify as professional nurses. Recommendations from this study relate to the three themes identified and clearly indicate that students have a realistic expectation of and a need for visible role models in the clinical learning environment. / Health Studies / M.A. (Health Studies)

Perceptions of professional nurses on the impact of shortage of resources for quality patient care in a public hospital: Limpopo Province

Text in English / Professional nurses are regarded as the backbone of the health care system and quality patient care is linked to patient safety.The purpose of the study was to describe and explore the perceptions of professional nurses on impact of shortage of resources for quality patient care. The resources include health professionals, equipment and drugs. The study was conducted in Mankweng hospital which is a public hospital in the Capricorn district in Limpopo Province. Qualitative descriptive exploratory design was used which provided the researcher with in-depth information regarding phenomena under study. Data was collected from ten (10) professional nurses who have 5 to 20 years of nursing experience allocated in medical and surgical wards. Unstructured face-to-face interview was conducted using field notes and audio tape. Data was analysed following Creswell (2014) Tesch method. Five themes and eighteen subthemes emerged from data.
