Estado nutricional de crianças assistidas em creches / Nutritional status of children attending day care centers

Methodology: literature search was performed in MEDLINE, LILACS and SciELO articles reviewing the nutritional status of children attending day care centers in Brazil, published from 1990 to 2009. Search strategy: (nutritional status OR anthropometry OR malnutrition OR overweight) AND day care centers, and its counterpart in Portuguese. In the MEDLINE search the termBrazil was also used. In cross-sectional study the nutritional status of infants, the food security situation within the family, socioeconomic conditions, the characteristics of the mother and the child attending day care centers in Joao Pessoa were evaluated. Results: children attending day care centers in Brazil showed prevalence of chronic malnutrition ranged from 0.5 to 55%. Overweight has fluctuated between 0.7 and 8.6%. The most citedfactors related to malnutrition in the reviews were child's age, number of siblings, per capitafamily income and frequency of day care attendance. Among children attending day care centers in Joao Pessoa, chronic malnutrition was present in 7.6% and overweight in 6.4%. Factors associated with malnutrition in this sample were short maternal stature and incomplete vaccination scheme. The overweight in children was associated with maternal overweight. Furthermore, 59.6% of the families had food and nutritionalinsecurity, prevailing its milder form (32.4%). Conclusion: overweight and stunting, both in children attending day care centers inJoão Pessoa, as in many Brazilian cities, were the most common nutritional disorders, constituting priorities for public policy today. / Objetivos: analisar o estado nutricional da população pré-escolar brasileira assistida em creches e seus fatores associados; examinar fatores associados a excesso de peso, baixa estatura e déficit de peso em crianças usuárias de creches de João Pessoa e descrever a situação de (in)segurança alimentar de suas famílias. Metodologia: realizou-se busca bibliográfica nas bases MEDLINE, LILACS e SciELO, revisando os artigos sobre estado nutricional de crianças assistidas em creches no Brasil, publicados entre 1990 a 2009. Estratégia de busca: (estado nutricional OR antropometria OR desnutrição OR sobrepeso) AND creches, e seus correspondentes em inglês. No caso da busca na MEDLINE o descritor Brazil também foi usado. Em estudo transversal avaliou-se o estado nutricional infantil, a situação de segurança alimentar intrafamiliar, as condições socioeconômicas, as características da mãe e da criança assistida em creches de João Pessoa. Resultados: Crianças brasileiras usuárias de creches apresentaram prevalência de desnutrição crônica variando entre 0,5 e 55%. O excesso de peso oscilou entre 0,7 e 8,6%. Os fatores associados à desnutrição crônica mais citados na revisão foram idade da criança, número de irmãos, renda familiar per capita e tempo de frequência à creche. Entre crianças usuárias de creches de João Pessoa, a desnutrição crônica foi presente em 7,6% e o excesso de peso em 6,4%. Os fatores associados à desnutrição nesta amostra foram a baixa estatura materna e esquema vacinal incompleto. O excesso de peso infantil apresentou associação com o excesso de peso materno. Ademais, 59,6% das famílias destas crianças apresentaram situação de insegurança alimentar e nutricional, prevalecendo sua forma mais leve (32,4%). Conclusão: excesso de peso e baixa estatura, tanto em crianças assistidas por creches de João Pessoa, quanto em diversas localidades brasileiras, foram os distúrbios nutricionais mais frequentes, constituindo prioridades para políticas públicas atuais.

Changes in these different aspects of the executive functions have shown to be related to various disorders, such as the Attention Deficit Hyperactivity Disorder (ADHD), and they can be identified fairly early on, in preschool age. The executive functions can be observed in different environments, such as at home and at school, however, parents and teachers tend to have differing reports about the performance and/or behavior of certain child. Thus, the objective of this research was to verify the relationship between performance in tests of selective attention/inhibitory control and parents and teachers reports about symptoms of inattention/hyperactivity and executive functioning. As well as verifying the progression of these measures with the school grade increase.137 children studying in nursery, preschool, elementary and junior high school in Grande São Paulo , as well as their parents and teachers have participated. In order to assess the inhibitory control and the selective attention abilities, the Computerized Stroop Test has been applied to preschoolers. Their parents and teachers have answered the Behavior Rating Inventory of Executive Function (BRIEF) and the MTA-SNAP-IV. Kruskal-Wallis analysis have shown that the children s performances have increased with the school progression. The correlation between the reports of parents and teachers has tended to be significant, but low, suggesting trend in difference between such reports, once the parents have tended to point to more symptoms assessed by the MTA-SNAP-IV and the teachers more difficulties in the BRIEF abilities. In general, the StroopTest performances were more related to the teachers than to the parents reports. Such study corroborated the importance of listening to different informants by working with small children, and it suggests that, in this specific case, the teachers reports were closer to the child performance than the parents reports. / As funções executivas, que agregam os processos de memória de trabalho, flexibilidade cognitiva, planejamento, controle inibitório e atenção seletiva, são cruciais para a adaptação do indivíduo às rotinas do cotidiano, sendo também a base para o desenvolvimento de novas habilidades. Alterações nesses diferentes aspectos das funções executivas têm se mostrado relacionadas a vários transtornos, como o Transtorno do Déficit de Atenção e Hiperatividade (TDAH), e podem ser identificadas precocemente, ainda em idade pré-escolar. As funções executivas podem ser observadas em diferentes ambientes, como em casa e na escola, entretanto pais e professores tendem a ter relatos divergentes acerca do comportamento de determinada criança. Sendo assim, o objetivo dessa pesquisa foi verificar a relação entre o desempenho de crianças em testes de atenção seletiva / controle inibitório e os relatos de pais e professores sobre sinais de desatenção/hiperatividade e sobre funcionamento executivo, bem como verificar a progressão dessas medidas com o aumento das séries. Participaram 137 crianças do Ensino Infantil e Fundamental I da grande São Paulo, bem como seus pais e professores. Para avaliar as habilidades de controle inibitório e atenção seletiva, foi aplicado o Teste de Stroop versão computadorizada para pré-escolares. Os pais e professores responderam o Inventário de Funcionamento Executivo Infantil (IFEI) e o MTA-SNAP-IV. Análises de Kruskal-Wallis mostraram que os desempenhos das crianças aumentaram com a progressão da escolaridade. A correlação entre os relatos de pais e de professores tendeu a ser significativa, porém baixa, sugerindo tendência de divergência entre tais relatos, sendo que os pais tenderam a apontar mais sintomas avaliados pelo MTA-SNAP-IV e os professores mais dificuldades nas habilidades do IFEI. De modo geral, os desempenhos do Teste de Stroop estiveram mais relacionados aos relatos dos professores do que ao relato dos pais. Tal estudo corroborou a importância de ouvir diferentes informantes ao trabalhar com crianças pequenas e sugere que, nesse caso específico, o relato dos professores foi mais próximo ao desempenho da criança do que o relato dos pais.

1 ABSTRACT The diploma work titled "The parents', teachers' and day care center staff view on possibility of placing two-year-old children in nursery schools" analyzes the specifics and problems of today's families with young children. It defines the concept of early childhood, describes a child's development at toddler age, and also characterizes today's toddler. The work is dedicated to the maturity of children for entry into nursery schools. Furthermore the diploma work describes the history and current problems of the care for children younger than three years in the Czech Republic in comparison with representative European countries.
