The Meaning of School Body Mass Index (BMI) Screening and Referral to the Parents/Guardians of First, Third, and Sixth Grade Students

The purpose of this study was to discover the meaning of school body mass index (BMI) screening and referral to parents. The goal of school BMI screening and referral is to provide information to compel parents to change their child's diet and activity levels when overweight and obesity are identified. Measuring BMI in schools and alerting parents to findings above what is considered normal is one intervention to reduce overweight and obesity that has been utilized since 2001 in Florida. To determine the meaning of BMI screening and referral to parents a mixed methods approach was utilized. Voluntary interviews of 20 parents who had received BMI referrals for their children were conducted. Analysis of the interviews was guided by phenomenology, as delineated by van Manen (1997). A cross sectional survey developed by Ruggieri (2012), was distributed to measure parent beliefs and opinions regarding the BMI screening and their re action to referrals. Findings from interviews indicated that parents ascribe different meaning to school BMI screening and referral. Overarching themes of changing and reflecting were discovered. The themes were manifested as dichotomies; some parents reflected on their role as parent and were more satisfied with the process; they reported change of diet and activity for their families upon receipt of a BMI referral. Others reflected on the role of the school in their child's life. They were more dissatisfied with the school screening and recommended changes in the screening and referral process. Suggested changes for school screening and referrals included subthemes: sensitivity, accuracy, privacy, and notification. Parents responded in the survey that they would change diet and activity for their families if they were told by the school that their child had a weight issue. Actions taken upon receipt of a BMI referral were not limited to changing diet and activity levels but also included discussing weight with their child and others. Parents denied they would be offended by a BMI referral. School BMI screening and referral is a valuable and effective intervention to address child overweight and obesity, especially if the process is accomplished with characteristics that parents deem caring. / Includes bibliography. / Dissertation (Ph.D.)--Florida Atlantic University, 2015. / FAU Electronic Theses and Dissertations Collection

Coronary artery disease progression and calcification in metabolic syndrome

Indiana University-Purdue University Indianapolis (IUPUI) / For years, the leading killer of Americans has been coronary artery disease (CAD), which has a strong correlation to the U.S. obesity epidemic. Obesity, along with the presence of other risk factors including hyperglycemia, hypercholesterolemia, dyslipidemia, and high blood pressure, comprise of the diagnosis of metabolic syndrome (MetS). The presentation of multiple MetS risk factors increases a patients risk for adverse cardiovascular events. CAD is a complex progressive disease. We utilized the superb model of CAD and MetS, the Ossabaw miniature swine, to investigate underlying mechanisms of CAD progression. We studied the influence of coronary epicardial adipose tissue (cEAT) and coronary smooth muscle cell (CSM) intracellular Ca2+ regulation on CAD progression. By surgical excision of cEAT from MetS Ossabaw, we observed an attenuation of CAD progression. This finding provides evidence for a link between local cEAT and CAD progression. Intracellular Ca2+ is a tightly regulated messenger in CSM that initiates contraction, translation, proliferation and migration. When regulation is lost, CSM dedifferentiate from their mature, contractile phenotype found in the healthy vascular wall to a synthetic, proliferative phenotype. Synthetic CSM are found in intimal plaque of CAD patients. We investigated the changes in intracellular Ca2+ signaling in enzymatically isolated CSM from Ossabaw swine with varying stages of CAD using the fluorescent Ca2+ indicator, fura-2. This time course study revealed heightened Ca2+ signaling in early CAD followed by a significant drop off in late stage calcified plaque. Coronary artery calcification (CAC) is a result of dedifferentiation into an osteogenic CSM that secretes hydroxyapatite in the extracellular matrix. CAC is clinically detected by computed tomography (CT). Microcalcifications have been linked to plaque instability/rupture and cannot be detected by CT. We used 18F-NaF positron emission tomography (PET) to detect CAC in Ossabaw swine with early stage CAD shown by mild neointimal thickening. This study validated 18F-NaF PET as a diagnostic tool for early, molecular CAC at a stage prior to lesions detectable by CT. This is the first report showing non-invasive PET resolution of CAC and CSMC Ca2+ dysfunction at an early stage previously only characterized by invasive cellular Ca2+ imaging.

Affective traits and adiposity : a prospective, bidirectional analysis of the African American Health study data

Indiana University-Purdue University Indianapolis (IUPUI) / Research indicates that negative affective traits (e.g., depression) are predictors and consequences of excess adiposity. Given that racial minorities and positive affective traits have been underrepresented in past investigations, more prospective studies are needed which examine multiple affective traits in relation to obesity in these populations. The objective of the current study was to investigate the prospective, bidirectional associations between multiple affective traits and multiple adiposity indicators in African Americans using data from the African American Health (AAH) study. The AAH study is a prospective cohort study of African Americans aged 49-65 years at baseline (N = 998). The longest follow-up period in the current study was 9 years (N = 579). Self-reported and measured body mass index (BMI; kg/m2) and body fat percent (BF%) were used as adiposity indicators. Depressive symptoms were assessed with the 11-item Center for Epidemiologic Studies-Depression Scale (CES-D), and anxiety was assessed using the Generalized Anxiety Disorder-2 (GAD-2) scale. Positive affective traits were assessed with the Vitality subscale of the Short Form-36 and Positive Affect subscale from the CES-D. Latent variable path analysis, a structural equation modeling technique, was conducted. Although fit statistics indicated that the models fit the data (RMSEA < .06), examination of the structural paths revealed that the CES-D and GAD-2 were not predictors or consequences of self-reported BMI, measured BMI, or BF% (ps > .05). Likewise, Vitality and CES-D Positive Affect were not related to any adiposity indicator (ps > .05). The results of this prospective cohort study suggest that affective traits are not predictors or consequences of adiposity in middle-aged African Americans and that this group may require obesity prevention or intervention programs with little to no emphasis on affective traits. Possible explanations for the current results include ethnic differences in the mechanistic pathways between affective traits and adiposity.
