Dental implications of inherited connective tissue disorders in South Africa

The prevalence of Osteogenesis imperfecta type III (OI III) as a category of the inherited connective tissue disorders in South Africa is of paramount importance. Although worldwide, autosomal recessive(AR) OI is rare, it had emerged that the frequency of OI III is relatively high in the indigenous Black African population of South Africa. A review of the literature revealed a paucity of information regarding the dental and craniofacial manifestations of the disorder in this ethnic group. For these reasons, the central theme of this thesis is the identification, documentation and analysis of these features in individuals with OI III in the Black African population of SA. Osteogenesis imperfecta type III (OMIM 259420) is a severe autosomal recessive disorder in which frequent fractures and progressive limb and spinal deformity result in profound physical disability. The condition is heterogeneous and dentin genesis imperfecta (DI) is an important syndromic component of some types of OI III. Other maxillofacial and dental manifestations also have significant implications in terms of management.

O objetivo do presente trabalho foi desenvolver e verificar a funcionalidade de um sistema de telediagnóstico de doenças orais no estado de Pernambuco. Métodos: Quinze computadores e a mesma quantidade de câmeras digitais foram distribuídos em centros secundários de saúde onde Cirurgiões-Dentistas foram encorajados a visitarem um website e enviarem imagens dos seus pacientes que apresentassem lesões orais para avaliação à distância por dois profissionais com experiência em estomatologia. Um formulário de feedback com opiniões dos dentistas e pacientes também foi enviado. Resultados: Entre 325 pacientes, 313 (96,31%) acharam o procedimento de tirarem fotos de suas bocas bastante confortável, 265 (81,5%) ficaram satisfeitos em ver o interior de suas bocas e 288 (88,62%) acharam o procedimento útil no entendimento dos seus problemas clínicos. Foi estabelecida uma concordância entre os avaliadores e um correto diagnóstico final em 195 (60%) casos. Em 98 (30,16%) casos somente um avaliador acertou a hipótese diagnóstica sugerida à distância. Em 26 (8%) dos casos nenhum avaliador acertou o diagnóstico. 94,44% dos cirurgiões-dentistas consideraram que o auxílio do centro de referência contribuiu para os seus conhecimentos em relação à desordem/doença e acreditaram que o processo de telediagnóstico evita um encaminhamento desnecessário do paciente Conclusão: o telediagnóstico de doenças orais provou ser um modo viável de se evitar algum encaminhamento desnecessário de pacientes. / Objective: Pernambuco, located in Northern Brazil currently offers few oral medicine specialized units. Based upon this, the aim of the present study was to develop and verify the functionality of a telediagnosis system of oral diseases in Pernambuco state. Methods: fifteen computers and the same amount of digital cameras were distributed to secondary health care centers. Dentists were then encouraged to visit a website and send pictures of their patients who presented oral lesions. A feedback form with dentists and patients opinions was also sent. Results: Among 325 patients, 313 (94.3%) found the procedure of recording images of their mouth very comfortable, were happy to view the inside of their mouths 265 (81,5%), and found the procedure generally useful in understanding their clinical problem (288 out of 325 individuals (88%). In 195 (60%) of the instances an agreement between examiners and a correct final diagnosis was established; only one consultant made a correct diagnosis in 104 of the cases (32%); and neither consultants established a correct diagnosis in only 26 cases (8%). 94,44% of dentists considered that the help of a specialized center contributed to the improvement of their knowledge concerning oral diseases and believed that the telediagnosis system avoided unnecessary referrals. Conclusion: telediagnosis of oral diseases proved a feasible way of avoiding some unnecessary referrals.

Antimicrobial effectiveness of electro-chemically activated water as an endodontic irrigation solution

The use of sodium hypochlorite (NaOCI) as an endodontic irrigation solution is effective in eliminating microorganisms from the root canals of human teeth. The purpose of this in vitro study was to evaluate the antimicrobial effectiveness of electro-chemically activated water (ECA) as an endodontic irrigation solution. ECA is a solution comprising many different ions, molecules and radicals. Sixty caries-free, single root, adult, maxillary, anterior human teeth were used. The root canals were instrumented and irrigated with NaGCI in a similar method to that employed for in vivo root canal treatment. The external root surface of each tooth was sealed and the access cavity kept patent so that root canals could be inoculated with a suspension containing four bacterial. The teeth were randomly divided into four groups (n=15). Each group was irrigated ultrasonically, using solutions of distilled water (control), NaOCI (3.5%), and ECA, the latter at pH's 7.0 and 9.0 respectively. Antimicrobial effectiveness was established directly after irrigation and again seven days later, by counting colony forming units on blood agar plates and by spectrophotometric analysis. The surfaces of the root canals were visually examined by means of scanning electron microscopy. Large numbers of bacteria were present in the canals of teeth irrigated with distilled water. No bacteria were observed following irrigation with NaGCI. Neither of the ECA solutions were found to be effective against all the bacteria. Although some reduction in the number of bacteria was evident in the ECA groups, this was not statistically significant (p>0.05). Dentre estas, a leucoplasia oral (LO) é a principal condição dentre as DPMOs com uma taxa de incidência igual a 3,4% e uma taxa de transformação maligna que varia de 0,2% até 17,5%. Uma forma menos reconhecida e ainda pouco compreendida de LO denominada leucoplasia verrucosa proliferativa (LVP), representa uma variante de comportamento persistente e progressivo para malignidade, com uma taxa de transformação maligna ocorrendo em mais de 70% dos casos. O diagnóstico da LVP atualmente só é possível através da observação temporal e individual de cada paciente, com a demonstração de progressão clínica e histológica das lesões para um CEC. A dificuldade no diagnóstico precoce da LVP decorre da sobreposição de características clínicas e histopatológicas, especialmente em casos de LO multifocal convencional. Portanto, foi proposto inicialmente realizar a classificação de casos multifocais de LO encontrados no Serviço de Medicina Bucal da Faculdade de Odontologia de Araraquara - UNESP, durante os anos de 1995 até 2015, segundo descrição da Organização Mundial da Saúde (OMS, 2005) e critérios diagnósticos para LVP atualmente propostos por Cerero-Lapiedra (2010) e Carrard (2013). Posteriormente, foi realizada uma análise comparativa entre as características clínicas e microscópicas de casos de LVP e LO. Os resultados deste estudo revelaram que dentre 86 casos de LO, 45 casos apresentaram o envolvimento de dois ou mais sítios orais configurando estes como casos de LO multifocal. Segundo avaliação realizada por dois observadores experientes...(Resumo completo, clicar acesso eletrônico abaixo) / The squamous cell carcinoma (SCC) represent more than 95% of all the malignant neoplasia of the oral cavity and most of times these tumors are preceded by clinical conditions that have a potential for malignant transformation called oral potentially malignant disorders (OPMD). Among these, the oral leukoplakia (OL) is the most important OPMD, with an incidence rate of 3.4% and a malignant transformation from 0.2 to 17.5%. A form less recognized and poorly understood of leukoplakia is the proliferative verrucous leukoplakia (PVL) which represents a variant with a persistent and progressive to malignancy behavior, with a malignant transformation ratio up to 70% of cases. Currently, the diagnosis of PVL is only made by the temporal and individual observation of each patient, with the demonstration of clinical and histopathological progression to an SCC. The difficult to stablish an early diagnosis of PVL comes from the overlap of clinical and histopathological features with conventional multifocal OL cases. Therefore, was initially proposed to perform a classification of multifocal OL cases found in the Oral Medicine Service from the Araraquara Dental School - UNESP, between 1995 and 2015, according to the WHO definition (WHO, 2005) and the current suggested diagnostic criteria for PVL by Cerero-Lapiedra (2010) and Carrard (2013). Lately, a comparative analysis was conducted between the clinical and microscopic features from cases of PVL and OL. The results of this study revealed that between 86 cases of OL, 45 presented involvement of two or more setting this as multifocal OL cases. According to the evaluation by two experts, 14/45 were considered true cases of PVL. The classification of the same 45 cases by the WHO definition revealed 21/45 cases as PVL, by the Cerero-Lapiedra 27/45 cases were PVL and 4/45 cases were PVL according to Carrard.
