Eficácia da laserterapia de baixa intensidade associada a exercícios em pacientes com osteoartrose de joelho: um estudo randomizado e duplo-cego / Efficacy of low-level laser therapy associated to exercises in knee osteoarthritis: a randomized, double-blind study

Introdução: A osteoartrite (OA) é uma doença degenerativa que atinge as articulações sinoviais, sendo uma das causas mais comuns dalimitação funcional. Objetivo: Investigar os efeitos do Laser de Baixa Intensidade (LBI) associado a exercícios na dor, amplitude de movimento, força muscular, funcionalidade, mobilidade e qualidade de vida de pacientes com osteoartrite de joelhos. Métodos: Quarenta e seis indivíduosde ambos os sexos, entre 50 e 75 anos, com OA de joelho (graus 2-4), foram randomizados em dois grupos: Grupo Laser- LBI ativo com dose de 3J mais exercícios e Grupo Placebo- LBI placebo e exercícios. A avaliação foi realizada antes de iniciar o tratamento (AV1), três semanas após o tratamento com o laser (AV2), oito semanas após o tratamento com exercícios (AV3), três e seis meses após o término do tratamento (AV4 e AV5). A dor foi avaliada com a Escala Visual Analógica (EVA), amplitude de movimento com goniômetro universal, força muscular com dinamômetro, funcionalidade com a Escala AlgofuncionalLequesne, a mobilidade pela TimedGetUpandGo(TGUG) e qualidade de vida com a Western Ontario andMcMasterUniversitiesOsteoarthritis (WOMAC). A intervenção foi realizada três vezes por semana durante 11 semanas, sendo que nas três primeiras foi aplicado o laser e nas demais somente os exercícios. Os dados foram analisados com os testes não paramétricos de Friedman e Mann- Whitney, com nível de significância de 5%.Resultados: Na análise intragrupo, o Grupo Laser apresentou melhora significativa na dor (p<0,001), amplitude de movimento (p=0,032), funcionalidade (p=0,003), mobilidade (p<0,001), WOM-dor (p=0,001), WOM-função (p<0,001) e WOM-total (p<0,001), enquanto o Grupo Placebo apresentou diferença significativa na mobilidade (p=0,013), WOM-função (p=0,017) e WOM-total (p=0,02). Na análise intergrupo foi constatada diferença significativa entre os grupos na AV2 para as variáveis WOM-função (p=0,01) e WOM-total (p=0,02) e na AV3 para as variáveis WOM-dor (p<0,01), WOM-função (p=0,01) e WOM-total (p=0,02), indicando melhora no Grupo Laser. Neste, também foi observado maior ganho relativo em quase todas as variáveis ao longo das avaliações, quando comparado ao Grupo Placebo. Pain was assessed using visual analogical scale (VAS), range of motion with the universal goniometer, muscular strength using a dynamometer, functionality using the LequesneQuestionnaire, mobility using Timed Get Up and Go (TGUG)and quality using the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) of life in five moments: (T1) baseline, (T2) after at the end of laser therapy (3rd week), (T3) after the end of exercises (11th week), (T4) three and six monthsafter thetreatment (T4 and T5). The intervention was performedduring three weeks; patients received laser therapy or placebo three times a week; for the remaining 8 weeks, all patients exercised three times a week.Data was assessed through Friedman and Mann-Whitney non-parametrical tests, with 5% significance. Efeitos clínicos e biomecânicos da utilização de órtese valgizante sob medida na osteoartrite medial do joelho / Immediate clinical and biomechanical effects and after two months of using a customized orthosis in medial knee osteoarthritis

INTRODUÇÃO: A osteoartrite é a maior causa de dor em idosos, o objetivo primário do tratamento é o alívio da dor e evitar a progressão da doença reduzindo o estresse articular. As órteses para correção de varo são uma forma não invasiva e não farmacológica de tratamento desses pacientes. O objetivo deste trabalho foi verificar a eficiência de uma órtese sob medida nos joelhos com osteoartrite medial e deformidade em varo. MÉTODOS: Em um estudo clinico randomizado foram avaliados e tratados com a colocação de órtese valgizante sob medida 51 pacientes com idade entre 60 e 75 anos com osteoartrite medial do joelho divididos em dois grupos: grupo órtese e controle. Foram realizadas três avaliações: momento inicial, imediatamente após a colocação da órtese (somente no grupo órtese) e após dois meses. A avaliação funcional e de dor consistiu em aplicação de questionários (WOMAC, Indice Algofuncional de Lequesne e EVA), exames de radiografia panorâmica para observar a abertura do espaço articular (somente no grupo órtese), avaliação com dinamômetro isocinético para avaliação de força, e avaliação de equilíbrio e marcha com a utilização de plataforma de força. RESULTADOS: Este estudo mostrou que houve diminuição da dor (p=0,001), melhora funcional com diminuição de 50% do valor nos Índices de Lequesne (p=0,010) e WOMAC (p=0,001). Houve abertura do espaço articular medial (p=0,049). Aumento do torque flexor e extensor (p=0,001) evidenciando ganho de força muscular. Diminuição da fase de apoio na marcha (p=0,030). Não houve melhora do equilíbrio em nenhuma das variáveis analisadas. O tempo médio de utilização foi de 6,5 horas. 92,59% sentiram boa estabilidade ao realizar as AVD\'s, apenas 18,18% fizeram uso de medicamento analgésico durante o periodo de utilização, 13,63% apresentaram lesões de pele e 77,77% comprariam a órtese no seu valor de mercado. CONCLUSÕES: A órtese valgizante sob medida promoveu alívio da dor e melhora funcional com ganho de força muscular e melhora do padrão de marcha, não evidenciando interferência no ganho de equilíbrio em indivíduos com osteoartrite medial do joelho. METHODS: In a randomized clinical study, 51 patients aged 60 to 75 years with medial knee osteoarthritis were divided into two groups: bracing and control groups. Three evaluations were performed: initial moment, immediately after the orthosis was placed (only in the orthosis group) and after two months of use. The functional and pain evaluation consisted of questionnaires (WOMAC, Algofunctional Index of Lequesne and VAS), panoramic radiographs to observe the opening of the joint space (only in the orthosis group), evaluation with isokinetic dynamometer for strength evaluation, and balance and gait evaluation with the use of force platform. RESULTS: This study showed that there was a decrease in pain (p = 0.001), functional improvement with a 50% decrease in the value in the Lequesne Indices (p = 0.010) and WOMAC (p = 0.001). There was opening of the medial articular space (p = 0.049). Increased flexor and extensor torque (p = 0.001) evidencing muscular strength gain. Decreased gait support phase (p = 0.030). There was no improvement in the balance in any of the analyzed variables. The mean time of use was 6.5 hours. 92.59% felt good stability when performing ADLs, only 18.18% used analgesic medication during the period of use, 13.63% had skin lesions and 77.77% would buy the orthosis at their market value. CONCLUSIONS: The bespoke valgus orthosis promoted pain relief and functional improvement with gain of muscle strength and gait improvement, not evidencing interference in the gain of balance in individuals with medial osteoarthritis of the knee. The bespoke valgus orthosis is an efficient method for the treatment of medial osteoarthritis of the knee with varus deformity, showing the expected clinical effects, fewer adverse effects, real possibility of daily use and low cost

Introdução: Os corticosteroides intra-articulares (IA) são amplamente utilizados no tratamento da osteoartrite (OA) de joelho, porém é desconhecido qual dentre estes agentes é o mais eficaz. Objetivo: O objetivo do presente estudo foi comparar a eficácia das infiltrações IA de triancinolona hexacetonida (TH) e de acetato de metilprednisolona (AM) na OA de joelho. Pacientes e Métodos: Pacientes com OA sintomática de joelho, graus II ou III de Kellgren-Lawrence, foram randomizados para receber uma única infiltração IA com 40mg de TH ou AM. As avaliações clínicas foram realizadas nas semanas 4, 12 e 24. O desfecho primário do estudo foi a melhora da dor do joelho pelo paciente por escala visual analógica (EVA) da visita basal à semana 4. Os desfechos secundários incluíram a avaliação global da doença pelo paciente e pelo médico, o questionário de osteoartrite Western Ontario and McMaster Universities (WOMAC), o índice de Lequesne e o critério de resposta Outcome Measures in Rheumatology and Osteoarthritis Research Society International (OMERACT-OARSI). Na análise estatística, foram empregadas equações de estimativa generalizada, com estatística de Wald para contrastes do tipo 3 e ajustes de Tukey-Kramer para comparações múltiplas. Resultados: Cem pacientes foram incluídos na população com intenção de tratar, 50 em cada braço do estudo. Uma melhora significativa na dor pela EVA foi observada na semana 4 para ambos os grupos (P < 0,0001), não havendo diferença entre eles (P=0,352). Esta melhora foi sustentada até a semana 24. Uma melhora significativa em relação à avaliação basal foi observada na avaliação global da doença pelo paciente e pelo médico, no questionário WOMAC e no índice de Lequesne, não havendo diferença entre os grupos. A melhora nos desfechos secundários de avaliação foi sustentada durante o estudo, exceto para a avaliação global da doença pelo paciente. O critério de resposta OMERACT-OARSI foi alcançado por 74% e 72% dos pacientes dos grupos TH e AM, respectivamente. Conclusão: TH e AM são igualmente eficazes na OA de joelho, e a melhora na dor e na função física pode ser sustentada por até 24 semanas / Introduction: Intra-articular (IA) corticosteroid injections are broadly used in the treatment of knee osteoarthritis (OA), but it is unknown which of these agents is the most effective. Objective: The aim of the present study was to compare the efficacy of triamcinolone hexacetonide (TH) and methylprednisolone acetate (MA) IA injections in knee OA. Patients and Methods: Patients with symptomatic knee OA, Kellgren-Lawrence grades II or III, were randomized to receive a single IA injection with 40mg of TH or MA. Evaluations were performed at 4, 12 and 24 weeks. The primary outcome of the study was to evaluate the improvement in the patient\'s knee pain by visual analogue scale (VAS) from baseline to week 4. Secondary outcomes included the global assessment of the disease by the patient and the physician, the Western Ontario and McMaster Universities osteoarthritis questionnaire (WOMAC), the Lequesne index and the Outcome Measures in Rheumatology and Osteoarthritis Research Society International (OMERACT-OARSI) criteria of response. Generalized estimating equations with Wald statistics for type 3 contrasts and Tukey-Kramer multiple comparison adjustment were employed in statistical analysis. Results: The intention-to-treat population included one hundred patients; 50 in each study arm. A significant improvement in pain by VAS was observed at week 4 for both groups (P<0.0001), with no difference between them (P=0.352). This improvement was sustained up to week 24. A significant improvement from the baseline was observed for the patients\' and the physicians\' global assessments, WOMAC questionnaire, and Lequesne index, with no differences between the groups. Improvements in the secondary outcomes were sustained during the study, except for the patients\' global assessment of disease. OMERACT-OARSI criteria of response was achieved by 74% and 72% of patients in the TH and the MA groups, respectively. Conclusion: TH and MA are equally effective in knee OA and improvement in pain and physical function can be sustained for up to 24 week

Developing and piloting an exercise programme for older Chinese people with knee osteoarthritis in Hong Kong. / CUHK electronic theses & dissertations collection

A mixed-methods design consisting of two phases was adopted. In Phase I, a purposive sample of 31 older Clnnese people with KOA was invited for a semi-structured interview to explore their perceptions and experiences of exercise. Five major categories emerged: typical living pattern with KOA, self-management of KOA, the practice of exercise, views about exercise, and preferences for learning exercise. The findings were used in the development of an exercise programme for this client group. / Knee osteoarthritis (KOA) is prevalent in the growing older Chinese population in Hong Kong. Therapeutic exercise for KOA has strong empirical evidence of its effectiveness on reducing knee pain and physical disability in people with KOA provided that they persists in practising the recommended exercise regimen. However, few previous studies have incorporated the client's perspectives into the design of an exercise programme for promoting continual practice of exercise. This study aimed to develop a new therapeutic exercise programme and pilot its acceptability to promote continual practice for older Chinese people with KOA. / The Phase II study used both quantitative and qualitative approaches to examine participants' satisfaction with the new exercise programme, adherence to the prescribed exercises, mastering of the exercise movements and health outcomes (knee pain, stiffness and range-of-motion, physical function, muscle strength and endurance of the lower extremities, and quality of life). A convenience sample of 33 older Chinese people with KOA participated in the programme. Seven measurement tools in Chinese versions (if applicable) were used to collect quantitative data from the participants: the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), the 12-item Short Form of the Medical Outcome Study Questionnaire (SF-12), a goniometer, the Timed-Stands Test (TST), a satisfaction questionnaire, an exercise diary, and a return-demonstration performance record sheet. Six of the participants were invited for a semi-structured interview to explore their experience of the programme. / The quantitative results showed that the participants were highly satisfied with the exercise programme with a mean score of 90.15 out of 100 (SD = 8.05). Their average exercise adherence was high with a mean percentage of 91.04% (SD = 14.54) and their overall performance in mastering of the exercise movements was good with a mean score of 76.71 out of 100 (SD = 21.75). Most participants' health outcomes significantly improved at three months after the exercise programme, except for the SF-12 physical health summary score. The qualitative findings revealed four major categories: satisfaction with the exercise programme, mastering of the exercise movements, experience of the exercise's effects, and integration of the exercises into the daily routine. These findings were mainly consistent with the quantitative results of the study. / The results of this study showed that the exercise programme was acceptable to the participants in terms of satisfaction with the programme, adherence to the prescribed exercises and mastering of the exercise movements.
