Management in collaborative and integrated healthcare service systems : concept and practice

This study explores how managers are coping within a changing public healthcare service context and how the role of service managers and the nature of Management Development are being transformed. With the public healthcare sector in the UK facing complex challenges including financial constraint and increasing service demand, it is inevitable that collaborative partnership working and service integration are viewed as a means of addressing such challenges. Using the views and experiences of service managers from Scottish Community Health Care Partnership cases, the study highlights the experiences of managers in relation to partnership working and service integration and explores the potential implications of this for managerial learning, training and development. The research evidence establishes the importance of changing roles, responsibilities and relationships for managers in a changing healthcare service environment and takes on board a Service-Dominant approach and propositions from New Public Governance theory to explain these and to address attendant issues. Specifically, the challenges surrounding the learning, training and development of managers in an increasingly integrated services environment are explored and reconceptualised through a Services-as-Systems approach. The outcomes of this study allow for a better understanding of the changing nature of work that managers do and attempts to reframe Management Development in such a context for the future.

Balance de la investigación académica en regulación de los servicios públicos

El artículo presenta un breve balance de las investigaciones realizadas recientemente en el ámbito de la regulación de los servicios públicos. El análisis se concentra en las industrias de redes (telecomunicaciones,energía, transporte, agua y saneamiento) y se limita a las publicaciones realizadas desde 2004 en adelante, las cuales, de alguna manera, se refieren a la experiencia peruana. Este recuento pone en evidencia la importante producción bibliográfica sobre el tema, destacando en número y calidad los diversos estudios realizados sobre la regulación en los sectores energía y telecomunicaciones. También se exponen las principales contribuciones realizadas fuera del país, referidas al progreso logrado en la identificación de las mejores prácticas de regulación y de los principios básicos que han orientado los sistemas institucionales adoptados en algunos países, lo cual facilita la evaluación en otros contextos nacionales. Finalmente, el balance da cuenta de loscambios sustantivos en la importancia relativa de algunos temas. Así, la privatización ha dejado de ocupar un lugar central en la agenda de políticas públicas y más bien ha pasado a un segundo plano en las prioridades de los investigadores. En contraste, los problemas vinculados a la expansión de las redes y al acceso a los servicios, así como la concesión de proyectos de infraestructura y el diseño de esquemas de cofinanciamiento público-privado, son los temas predominantes en las agendas de investigación y en las políticas públicas, respectivamente.---This paper provides a survey review of the literature on regulation of utilities with a focus on network industries (telecommunications, electricity, transport infrastructure, water and sanitation). It is limited to the publications realized from 2004 onwards which, somehow, refer to the Peruvian experience. The survey provides detailed evidence on the significant bibliographic production on this topic, underscoring the quantity and quality of various studies on regulation, mainly on energy and telecommunications. Also it includes some contributions published abroad, which refer to the progress that has been achieved in identifying the best practices of regulation as well as the principles that have shaped the institutional designs in some countries, providing new benchmarks for evaluation in developing nations. Likewise, the assessment reveals some substantial changes in the relative importance of specific issues. Privatization no longer occupies a central role in the public policy agenda, and has also lost attraction within the research communities. In contrast, network expansion and access to network services as well as the design of public-private partnerships and financial mechanisms for infrastructure development, have turned into the key issues of the agendas for research and public policy, respectively.

Como instrumental de pesquisa, parte de uma revisÃo histÃrica abordando a origem e a evoluÃÃo das agÃncias reguladoras nos Estados Unidos, comparando os padrÃes norte-americano e europeu de regulaÃÃo, representado pelas experiÃncias britÃnica e francesa. Discutindo o conceito da regulaÃÃo de serviÃos pÃblicos, detalha os pressupostos, objetivos e aspectos fundamentais que devem caracterizar as agÃncias reguladoras. Examina como surgiram as agÃncias reguladoras no Brasil, tanto no nÃvel federal, quanto no nÃvel estadual, em um contexto de reforma do Estado empreendido no Governo Fernando Henrique Cardoso, descrevendo as principais caracterÃsticas das agÃncias reguladoras federais brasileiras, bem como detalha o cenÃrio e as condiÃÃes em que foi criada a ARCE â AgÃncia Reguladora de ServiÃos PÃblicos Delegados do Estado do CearÃ. A partir daÃ, a pesquisa discorre sobre a atuaÃÃo da ARCE, analisando o seu perfil institucional, bem como as competÃncias de normatizaÃÃo, fiscalizaÃÃo, mediaÃÃo de conflitos, e anÃlises tarifÃrias realizadas pela AgÃncia. The thesis started with a historic presentation of the origin and evolution of regulatory agencies in the United States, It compared North American and European regulatory patterns represented by the French and the British experiences and discussed the concept of regulation in public services, detailed pretexts, objectives and fundamental aspects that should characterize regulatory agencies. By examining how regulatory agencies emerged in Brazil at the federal as well as at the state level in the context of state reform during the Fernando Henrique Cardoso administration, it described the principal characteristic of Brazilian federal regulatory agencies furthermore it exposed the scenario and the conditions of ARCEÂs creation. From this point on, the thesis described the agencyÂs actions by analyzing its institutional profile, standardization methods, supervision, conflict intermediation and process analysis. It also tackled the role of the agency in formulating public policies and its patterns of negotiations of services rates with civil society through its institutional social control mechanisms. Throughout the thesis, ARCEÂs regulation difficulties were discussed having as a reference international standards while in the conclusion were presented numerous suggestions and propositions to improve regulation of public services in the state of CearÃ.

An investigation of the relationship between qualitative and quantitative standards and the students knowlegde of the use of books and libraries

Library literature and educational writings appraising school libraries imply that if qualitative and quantitative standards of school libraries were sufficiently high, the students would be proficient in the knowledge, of the use, of books and libraries. No evidence, however, is presented to support this assumption. The lack of empirical evidence prompted the present study as an attempt to discover if a positive correlation does exist between qualitative and quantitative standards of school libraries and the students’ knowledge of the use of hooks and libraries. The instrument used to ascertain the qualitative and quantitative standards of nine senior secondary schools in three British Columbia school districts was a closed-question interview questionnaire consisting of four major divisions. The results of the questionnaire were tabulated as numerical scores and placed in rank order for each of the four major divisions and for the total. Two hundred seventy-eight students in nine senior secondary schools were tested under standard procedures using the test A Library Orientation Test for College Freshmen. Intact groups of English 40 or English 91 students were tested in each of the nine schools. Test score results were tabulated for each part of the nine sub-tests for the test as a whole. Means of the results for each of the nine parts of the test were tabulated by school and placed in rank order. Correlations between mean score ranks and interview questionnaire score ranks were computed for each of the nine sub-tests and, the total test. In this study it was found that generally a low positive rank correlation exists. The correlation did not depart significantly from zero. It is suggested that, further studies be conducted, to isolate and to control variables which may have a bearing on the relationship between qualitative and quantitative standards of school libraries and the students' knowledge of the use of hooks and libraries. / Education, Faculty of / Graduate
