Estudo de parâmetros de estresse oxidativo na síndrome pós-parada cardiorrespiratória

A Parada Cardiorrespiratória (PCR) é caracterizada pela parada abrupta da ventilação e circulação espontâneas. A Síndrome Pós-parada Cardiorrespiratória (SPCR) surge após a PCR, destacando-se por acarretar disfunção do miocárdio, lesão cerebral, resposta sistêmica inflamatória devido à isquemia/reperfusão, possuindo importante causa de mortalidade em todo o mundo. O processo de isquemia/reperfusão gerado pela PCR contribui para o dano tecidual, em parte pela geração de espécies reativas. A formação de espécies reativas pode tanto gerar dano direto em biomoléculas como lipídeos, DNA e proteínas quanto alterar a composição celular de antioxidantes enzimáticos e não enzimáticos, componentes de diferentes vias celulares como apoptose e necrose e ativar inflamação. O objetivo do estudo (observacional, coorte prospectivo) foi analisar, em humanos, o perfil do processo de estresse oxidativo na SPCR acompanhando os pacientes durante as diferentes fases da síndrome. Foi coletado sangue total de 30 pacientes pós-PCR 6, 12, 36 e 72 horas após a ressuscitação bem sucedida, os quais se encontravam internados na Unidade de Terapia Intensiva (UTI) do Hospital Nossa Senhora da Conceição – Grupo Hospitalar Conceição (HNSC-GHC) de Porto Alegre (entre maio de 2011 a novembro de 2012). Excluíram-se pacientes menores de 18 anos, vítimas de trauma, pós-operatório imediato (<7 dias), gestantes e pacientes terminais. O dano em biomoléculas foi analisado através da dosagem de grupos carbonil (proteínas), malondialdeído (MDA) (lipídeos) e nitritos/nitratos, além da dosagem da proteína S100B como um marcador de dano neuronal; os antioxidantes enzimáticos através da atividade das enzimas superóxido dismutase (SOD), glutationa S-tranferase (GsT) e consumo de H2O2; antioxidantes não-enzimáticos através da concentração de glutationa (total (tGSH), reduzida (GSH) e oxidada (GSSG)) e vitamina C. Além disso, dados clínicos e bioquímicos foram fornecidos pelo hospital, como pH, glicose, lactato, hemoglobina, hematócrito, proteína-C reativa e eletrólitos. Dos 30 pacientes, 60% eram homens e apenas 18 sobreviveram durante todo o período de coleta (72h pós-PCR). A análise estatística foi realizada através do teste The Generalized Estimation Equations (GEE). Os pacientes apresentaram em três dos quatro momentos de coleta médias correspondentes à acidose e lactato com valores superiores aos considerados normais em todos os momentos. Os níveis de hemoglobina diminuiram significativamente do primeiro dia para o último. Os níveis de proteína-C reativa aumentaram significativamente do primeiro dia para o último e os valores médios de todos os momentos foram altos comparados com valores normais. Os níveis de nitritos/nitratos em eritrócitos encontraram-se significativamente mais elevados no último momento do que no penúltimo. Os níveis de tGSH, GSH e GSSG diminuíram significativamente do primeiro momento para o terceiro. Também foram realizadas análises desses parâmetros comparando homens e mulheres. A atividade de GsT teve aumento das 6 para 36 h pós-PCR. Mulheres apresentaram variações significativas nos níveis de glicose (relacionado a hiperglicemia), com o valor mais alto nas 72h pós-PCR. Homens tiveram variação nos níveis de GSSG, tendo seu menor valor 36h pós-PCR, bem como diminuição do consumo de H2O2 de 6h para 36h pós-PCR. Concluise que o estresse oxidativo não influencia no processo da SPCR somente nos momentos iniciais deste, mas continua sua influência ao longo de todas as fases. Além disso, homens e mulheres possuem resposta fisiológica diferente à SPCR. Enfatizando que este é o primeiro trabalho comparando as diferenças entre os sexos nesta síndrome em humanos com um número maior de parâmetros analisados. / Cardiac arrest (CA) is characterized by the abrupt stop of spontaneous ventilation and circulation. The process of ischemia/reperfusion generated by CA contributes to tissue injury, in part by generating reactive species. The formation of reactive species can generate both direct damage to biomolecules such as lipids, DNA and proteins and alter the cellular composition of enzymatic and non-enzymatic antioxidants, components of different cellular pathways such as apoptosis and necrosis and activate inflammation. The objective of the study (observational, prospective cohort study) was to analyze, in humans, the profile of the process of oxidative stress in SPCR accompanying patients during different stages of the syndrome. Whole blood of 30 patients post-CA 6, 12, 36 and 72 hours were collected after successful resuscitation, which were admitted to the Intensive Care Unit (ICU) of the Hospital Nossa Senhora da Conceição - Conceição Hospital Group (HNSC - GHC) of Porto Alegre (from May 2011 to November 2012). We excluded patients younger than 18 years, victims of trauma, immediate postoperative period (< 7 days), pregnant and terminally ill patients. The damage to biomolecules was examined through measurement of carbonyl groups (proteins), malondialdehyde (MDA) (lipids) and nitrites/nitrates, dosage of S100B protein as a marker of neuronal damage, through enzymatic activity of antioxidant enzymes superoxide dismutase (SOD), glutathione S-transferase (GST) and consumption of H2O2; non-enzymatic antioxidants through the concentration of glutathione (total (tGSH) , reduced (GSH) and oxidized (GSSG)) and vitamin C. In addition, biochemical and clinical data were provided by the hospital, such as pH, glucose, lactate, hemoglobin, C-reactive protein and electrolytes. Of the 30 patients, 60 % were men and only 18 survived throughout the collection period (72h post- CA). Statistical analysis was performed using Generalized Estimation Equations test (GEE). Patients reported in three of four times average collection corresponding to acidosis and lactate values over that considered normal at all times. Hemoglobin decreased significantly from the first day to the last. The C-reactive protein increased significantly from the first day to the last, and the averages of all times were high compared with normal values. Nitrites/nitrates found in erythrocytes were significantly higher than the last time in the penultimate. tGSH, GSH and GSSG decreased significantly from the first moment to the third. We also did the analysis of these parameters comparing men and women. Women showed significant variations in glucose levels (hyperglycemia associated) with the highest value at 72h post-CA. GsT activity had decreased from 6 to 36 h post-CA. Men had a variation in the levels of GSSG, having its lowest value 36h post-CA as well as reducing the consumption of H2O2 6h for 36h post-CA. We conclude that oxidative stress does not influence the process of SPCR only in the early moments of this, but his influence continues throughout all stages. In addition, men and women have different physiological response to Post-CA Syndrome. Emphasizing this is the first study to compare differences between genders in this syndrome in humans with a greater number of parameters analyzed. Estresse oxidativo e hipotermia terapêutica durante a síndrome pós-parada cardiorrespiratória

A síndrome pós-parada cardiorrespiratória é o estado fisiopatológico pós-reanimação em pacientes que sofreram parada cardíaca, consequente do estresse oxidativo gerado pela isquemia-reperfusão. A hipotermia terapêutica leve pode reduzir o dano aos tecidos sofrido pelos pacientes pós-parada. Porém, os mecanismos pelos quais a hipotermia pode reduzir estes danos ainda precisam ser elucidados. Este estudo foi realizado em duas partes. Síndrome pós-parada cardiorrespiratória em modelo animal: 26 ratos sofreram 6 min de parada cardíaca em fibrilação ventricular e foram depois reanimados. Os animais reanimados foram eutanasiados 2, 4 e 72-h pós-parada, e comparados ao grupo controle. O volume de ejeção do ventrículo esquerdo foi reduzido 2 e 4 h pós-reanimação, e recuperado após 72 h. Os níveis de troponina-T cardíaca altamente sensível, isoprostanos e 8-hidroxiguanosina no tecido miocárdico aumentaram 2 e 4 h pós-reanimação, e foram recuperados após 72 h. No miocárdio, isoprostanos estão correlacionados positivamente aos níveis de troponina-T e inversamente com o volume de ejeção do ventrículo esquerdo. Os níveis de 8-hidroxiguanosina no miocárdio também estão correlacionados positivamente aos níveis de troponina-T e inversamente ao volume de ejeção do ventrículo esquerdo. Os resultados evidenciam que a peroxidação e o dano oxidativo ao DNA no tecido miocárdico podem estar relacionados ao dano e à disfunção do miocárdio durante as primeiras horas pós-parada cardíaca. Hipotermia terapêutica em pacientes durante a síndrome pós-parada cardiorrespiratória: Foram incluídos pacientes vítimas de parada cardíaca intra- e extra-hospitalar, internados em unidade de terapia intensiva de 2011 e 2012, para receber o tratamento padrão (n= 31) ou hipotermia terapêutica (n= 11). A hipotermia leve (32-34°C) foi induzida 4-5 h pós-reanimação durante 24 h. Dados clínicos e amostras de sangue venoso foram coletadas 6, 12, 36 e 72 h pós-parada. Os níveis séricos de malondialdeído (marcador de peroxidação lipídica), e os níveis de grupos carbonila (marcador de oxidativo em proteínas) no plasma, foram significativamente reduzidos pela hipotermia terapêutica. A atividade antioxidante das enzimas superóxido dismutase, glutationa peroxidase e glutationa-S-transferase está significativamente aumentada em eritrócitos no grupo hipotérmico. A atividade da paraoxonase está diminuída no soro de pacientes tratados com hipotermia. A atividade da enzima geradora do radical ânion superóxido xantina oxidase está diminuída 36 e 72 h pós-parada no grupo hipotérmico. Os dados mostram que a hipotermia reduz significativamente o dano oxidativo pós-parada cardiorrespiratória e, ao menos em parte, restaurou a redução das defesas antioxidantes durante a síndrome pós-parada cardiorrespiratória. / The post-cardiac resuscitation syndrome is a pathophysiologic state after the successful cardiopulmonary resuscitation in cardiac arrest patients, consequent of oxidative stress after ischemia-reperfusion. The mild hypothermia treatment may improve the tissue damage in post-cardiac arrest patients. The mechanisms involving cardiac arrest pathophysiology and hypothermia treatment are not well elucidated. The study presents two parts. Post-cardiac resuscitation syndrome in animal model: Untreated ventricular fibrillation for 6 min was induced in 26 rats followed by cardiopulmonary resuscitation. Resuscitated animals were sacrificed at 2, 4 and 72 h, and were compared to a control group. Left ventricular ejection fraction was reduced at 2 and 4 h following resuscitation, while it was similar at 72 h. Inversely, high sensitivity cardiac troponin T, isoprostanes and 8-hydroxyguanosine levels increased at 2 and 4 h post-resuscitation, while returned to baseline 72 h later. Myocardial isoprostanes were directly related to high sensitivity cardiac troponin T levels and inversely related to left ventricular ejection fraction. Myocardial 8-hydroxyguanosine, biomarker of DNA damage, were also directly related to high sensitivity cardiac troponin T levels and inversely related to left ventricular ejection fraction. The results provide evidence that lipid peroxidation and DNA oxidative damage in myocardial tissue are closely related to myocardial injury and dysfunction during the initial hours following cardiac arrest. Hypothermia treatment for post-cardiac resuscitation syndrome patients: Intensive care unit patients admitted in 2011 and 2012, victims of in-hospital or out-of-hospital cardiac arrest were screened for the study. Patients were assigned for standard care (n= 31) or therapeutic hypothermia (n= 11). Mild hypothermia (32-34°C) was induced 4-5h after successful cardiopulmonary resuscitation for 24 h. Clinical data and venous blood samples were collected 6, 12, 36 and 72 h post-cardiac arrest. Serum malondialdehyde levels (lipid peroxidation biomarker) and plasma carbonyl levels (oxidative protein damage biomarker) were decreased in hypothermic group at all time-points. The enzymatic antioxidant activities of superoxide dismutase, glutathione-S-transferase and glutathione peroxidase were increased in the hypothermic group at all time-points. Differently, the paraoxonase activity, a high-density lipoproteins antioxidant enzyme, was decreased in hypothermic group. Presented data show hypothermia treatment decreased oxidative damage and, at least in part, reduced the impairment of antioxidant defenses during post-cardiac resuscitation syndrome.

A hipotermia terapêutica leve pode reduzir o dano aos tecidos sofrido pelos pacientes pós-parada. Porém, os mecanismos pelos quais a hipotermia pode reduzir estes danos ainda precisam ser elucidados. Este estudo foi realizado em duas partes. Síndrome pós-parada cardiorrespiratória em modelo animal: 26 ratos sofreram 6 min de parada cardíaca em fibrilação ventricular e foram depois reanimados. Os animais reanimados foram eutanasiados 2, 4 e 72-h pós-parada, e comparados ao grupo controle. O volume de ejeção do ventrículo esquerdo foi reduzido 2 e 4 h pós-reanimação, e recuperado após 72 h. Os níveis de troponina-T cardíaca altamente sensível, isoprostanos e 8-hidroxiguanosina no tecido miocárdico aumentaram 2 e 4 h pós-reanimação, e foram recuperados após 72 h. No miocárdio, isoprostanos estão correlacionados positivamente aos níveis de troponina-T e inversamente com o volume de ejeção do ventrículo esquerdo. Os níveis de 8-hidroxiguanosina no miocárdio também estão correlacionados positivamente aos níveis de troponina-T e inversamente ao volume de ejeção do ventrículo esquerdo. Os resultados evidenciam que a peroxidação e o dano oxidativo ao DNA no tecido miocárdico podem estar relacionados ao dano e à disfunção do miocárdio durante as primeiras horas pós-parada cardíaca. A hipotermia leve (32-34°C) foi induzida 4-5 h pós-reanimação durante 24 h. Dados clínicos e amostras de sangue venoso foram coletadas 6, 12, 36 e 72 h pós-parada. Os níveis séricos de malondialdeído (marcador de peroxidação lipídica), e os níveis de grupos carbonila (marcador de oxidativo em proteínas) no plasma, foram significativamente reduzidos pela hipotermia terapêutica. A atividade antioxidante das enzimas superóxido dismutase, glutationa peroxidase e glutationa-S-transferase está significativamente aumentada em eritrócitos no grupo hipotérmico. A atividade da paraoxonase está diminuída no soro de pacientes tratados com hipotermia. A atividade da enzima geradora do radical ânion superóxido xantina oxidase está diminuída 36 e 72 h pós-parada no grupo hipotérmico. O objetivo dessa pesquisa foi avaliar a incidência, causas e desfechos das PCs que ocorreram no perioperatório nos pacientes com trauma em hospital terciário de ensino de janeiro de 1996 a dezembro de 2009. Após aprovação do Comitê de Ética e Pesquisa da Faculdade de Medicina de Botucatu, UNESP, iniciou-se a pesquisa sobre a incidência de PC durante a anestesia em pacientes com ou sem trauma, prospectivamente identificada a partir de um banco de dados. Houve 90.909 anestesias durante o período estudado. Os dados coletados incluíram características demográficas dos pacientes, procedimento cirúrgico (eletivo, urgência ou emergência), classificação do estado físico segundo a ASA (American Society of Anesthesiologists), informações sobre o procedimento anestésico, o tipo de cirurgia, a clínica cirúrgica e o desfecho. Todas as PCs no trauma foram revisadas e agrupadas segundo o fator causal em quatro categorias: totalmente relacionadas à anestesia, parcialmente relacionadas à anestesia, totalmente relacionadas à cirurgia e totalmente relacionadas à doença e/ou condição do paciente. Ocorreram nos pacientes com trauma 58 PCs (6,4 por 10.000 anestesias) e 47 óbitos (5,2 por 10.000 anestesias). O maior risco de PC nos pacientes com trauma ocorreu na faixa etária de 18 a 35 anos (p=0,04), no sexo masculino (p<0,0001), no estado físico ASA III ou pior (p=0,04), nas cirurgias de emergência (p=0,04), nas clínicas cirúrgicas multiclínicas e torácica e nos pacientes gravemente enfermos que receberam cuidados de monitorização e suporte hemodinâmico. This survey evaluated the incidence, causes, and outcomes of perioperative cardiac arrests in trauma patients in a Brazilian tertiary general teaching hospital between 1996 and 2009. After institutional review board approval (UNESP, School of Medicine, Botucatu, Brazil), the incidence of cardiac arrest during anesthesia in patients with and without trauma was prospectively identified from an anesthesia database. There were 90,909 anesthetics during the study period. The data collected included patient demographics, surgical procedures (elective, urgent or emergency), ASA (American Society of Anesthesiologists) physical status classification, anesthesia provider information, type of surgery, surgical areas, and outcome. All of the cardiac arrests in trauma patients were reviewed and grouped by cause of cardiac arrest into one of four groups: totally anesthesia-related, partially anesthesia-related, totally surgery-related and totally trauma patient condition-related. Fifty-eight cardiac arrests (6.4 per 10,000 anesthetics) and 47 deaths (5.2 per 10,000) had occurred in the trauma patients. The major risk factors for cardiac arrest in the trauma patients were age (18 to 35 yr, p=0.04), male sex (p<0.0001) with ASA physical status III or poorer (p=0.04), emergency surgery (p=0.04) in multiclinical or thoracic surgery and monitored anesthesia care in very injured patients (p=0.04). Uncontrolled hemorrhage and head injury were the most significant causes of cardiac arrest and mortality. The majority of the intraoperative cardiac arrests and deaths in the trauma were patients condition-related.
