O contrato de gestão é um instrumento que visa a obter maior responsabilização por parte da área pública em relação ao cumprimento de objetivos predefinidos. A implantação do contrato de gestão na Secretaria Municipal da Saúde (SMS) de Curitiba surgiu como mais uma ferramenta de gestão para aprimorar o planejamento, a avaliação e o monitoramento local das ações em saúde e, com isso, melhorar a viabilidade técnica, política e financeira dos serviços prestados pela SMS à população de Curitiba. Este estudo objetiva descrever a percepção das autoridades sanitárias locais e dos trabalhadores das equipes de saúde quanto ao contrato interno de gestão (Tercom) e o Plano Operativo Anual da Secretaria Municipal da Saúde de Curitiba. Os dados foram coletados por meio de questionários fechados, elaborados com uso da escala de Likert, encaminhados aos servidores das unidades de saúde básica e unidades de saúde com estratégia de saúde da família, sorteadas aleatoriamente. / The Contract of Management is an instrument that it aims at to get greater responsibility on the part of the public area in relation to the fulfillment of predefined objectives. The implementation of the Contract of Management in the City department of Health of Curitiba appeared as plus a tool of management to improve the planning, evaluation and local checking of the actions in health and with this to improve the viability technique, politics and financier of the services given for the SMS to the population of Curitiba. This objective study to describe the perception of the local and diligent sanitary authorities of the teams in health how much to the Internal Contract of Management (Tercom) and the Annual Operative Plan, from Curitiba Health Department. The data had been collected by means of closed questionnaires, elaborated with use of the scale of Likert, directed to the servers of the Units of Basic Health and Units of Health with Strategy of Health of the Family, drafted randomly.

Desde então, muitas foram as iniciativas implementadas, com destaque para o Pacto pela Saúde, que institucionalizou os Colegiados de Gestão Regional (CGR), espaço de negociação e pactuação visando a gestão regional. O presente trabalho busca analisar este papel dos CGR, frente ao processo de regionalização em Mato Grosso. Método: Pesquisa qualitativa que constou de uma exploração dos 16 CGR constituídos no estado, mediante a utilização de questionário autoaplicado e de estudo de caso de dois CGR, escolhidos por critério de polaridade após análise de dados oriundos daquele questionário. O período de análise referiu-se aos anos de 2006 a 2012. O foco da investigação deu-se na conformação do CGR, no seu funcionamento, sua organização e relações, principalmente à luz da gestão e planejamento, buscando apreender como se dá essa prática no nível regional. Resultados: O incentivo precoce dado à regionalização da saúde em Mato Grosso fortaleceu as instâncias regionais (CIB Regionais e CGR) e a institucionalidade materializou-se, principalmente no que tange aos seus componentes regulatório e normativo. Deram-se avanços também na governança, já que no âmbito do CGR, interesses de sujeitos, governos e organizações são contemplados e pactuados. No entanto, a partir dos questionário autoaplicados, da base documental analisada e do estudo de caso, são evidenciadas as limitações da gestão estadual na coordenação e na indução do processo de regionalização da saúde, comprometendo sobremaneira a gestão do SUS neste estado. / The Brazilian Constitution of 1988 created the National Health System (SUS), adopting decentralization and regionalization as strategies to consolidate this system. Since then, many initiatives have been implemented, especially the Health Pact, which established the Regional Management Boards (CGR), space for negotiation and agreement, aimed at regional management. This paper seeks to analyze the role of CGR within the process of regionalization in Mato Grosso. Method: qualitative research, consisting of the exploration of 16 CRG’s already created in state, through the usage of self-administered questionnaires and the study of two different cases – chosen through a polarity criteria – after the analysis of all the data retrieved from said questionnaire. The period of analysis referred to the years 2006 to 2012. The investigation was mainly focused on the conformation of the CGR, its operation, organization and relations – also considering management and planning, seeking to understand how this practice works at the regional level. Results: The early incentive given to the regionalization of health in Mato Grosso strengthened regional bodies (CIB Regional and CGR), having the institutionalization been materialized mainly in regard to its regulatory and normative components. Advances were also made in governance, since under the CGR, interests of individuals, governments and organizations were contemplated and established. However, from the self-administered questionnaire, the analysis of database and through case studies, the limitations of state management coordination and induction process of regionalization of health were highlighted, proving that it affects and compromises the NHS management on the analyzed state.

It relates the action of the sanitary engineer, Francisco Saturnino de Brito and the town planner engineer Francisco Prestes Maia and their influence in the transformation of the urban space of Campinas. The city, located in the inland area of the state of São Paulo, had his origin from the establishment of troops that followed in the direction of the goldmines. It became a town in the cycle of the sugar cane and since then was in intense process of development, with the consolidation of the coffee complex, when its population was devastated by three big consecutive outbreaks of the epidemic of yellow fever. The paper compares some innovative proposals of Saturnino de Brito, in the condition Chief Sanitary District Commission of the State of São Paulo; and of Prestes Maia, the Urban Improvement Plan. It concludes that the urban planning is fundamental to control the natural growth of the cities, avoiding the major problems. / Aborda as questões relacionadas à implantação do urbanismo sanitarista nas cidades e do planejamento urbano, com intuito de ordenar o crescimento acelerado das cidades. Relata a atuação do engenheiro sanitarista, Francisco Saturnino de Brito e do engenheiro urbanista Francisco Prestes Maia e a influência no processo de transformação do espaço urbano de Campinas. A cidade, localizada no interior do estado de São Paulo, teve sua origem a partir do estabelecimento de tropas que seguiam em direção às minas de ouro. Constituiu-se como vila no ciclo da cana de açúcar e estava em processo intenso de modernização, com a consolidação do complexo cafeeiro, quando teve sua população dizimada por três grandes surtos consecutivos da epidemia de febre amarela. O trabalho compara algumas propostas inovadoras feitas por Saturnino de Brito, na condição de Chefe do Distrito da Comissão Sanitária do Estado de São Paulo; e de Prestes Maia, no Plano de Melhoramentos Urbanos. Conclui que o planejamento urbano é fundamental para direcionar o crescimento natural das cidades, antecipando a resolução dos principais problemas.

In my thesis I deal with the integration of health and social services. I introduced the issue of two/double departmental service management and their financing, there. In addition, I described the various actors involved in the integration of services, mentioned strategic documents and legislation related to the provision of health and social care. In the case study, I mapped the health and social services provided in Louny, through street-level bureaucrats I gained information about the interconnection of services in Louny and about the factors that affect the implementation of integration of services in the town. For successful implementation of integration, it is necessary to specify the users of integrated health and social services, to create a home base for them, to provide a flexible range of services and at the same time to support the informally caring families. The lack of demand for integrated services is linked to other factors such as ignorance of citizens and low participation in caring for close relatives. Street.level bureaucrats in health and social services do not know the integration of services, they do not encounter it in practice, this factor has an impact on the motivation of street-level bureaucrats to make changes in the provision of care.
