Markéta Pasdiorová The study of bacterial microflora at patients with spontaneous early delivery Diploma thesis Charles University in Prague, Faculty of Pharmacy in Hradec Králové Healthcare Bioanalytics - Specialist in Laboratory Methods Backround: Premature delivery and premature rupture of membranes have been associated with maternal genital tract infections or with altered vaginal microflora during pregnancy. This work discuses knowledge about vaginal microbiome, premature delivery and analysis options of vaginal microbiome using next-generation sequencing. The aim of the work was to evaluate the composition of the bacterial microflora at patients with spontaneous preterm delivery and compared with each other of vaginal and cervical samples. Methods: The study group consisted of 7 patients with diagnosed spontaneous preterm delivery. Samples were obtained on the basis of cooperation with the Department of Obstetrics and Gynecology of University Hospital Hradec Králové. The evaluation of the vaginal microbiome was performed by pyrosequencing method using Genome Sequencer FLX+. Results: We have observed all typical patterns of the vaginal microflora in the samples tested. This thesis is concerned with the legislature of the Czech Republic, Bundesrepublik Deutschland and the Republic of France, as regards the anonymous abandonment of children in babyboxes, anonymous births and births with the identity of the mo- ther classified. It presents a complex overview of the legal framework in which the above mentioned institutions are functioning, from the point of view of the current legislation and factual state of affairs, set in the background of historical evolution of the individual institutes in individual states. The first chapter of the thesis de- als with the terminology, which in these questions is often somewhat confused and confusing, due to laymen influences and translations between languages. The se- cond chapter presents the current legal framework of the individual states, together with the umbrella framework of the international Agreements which are binding to all three of the countries. The third chapter deals with babyboxes, describing their functioning in Germany and in the Czech Republic. The childbirth in the medical discourse is still viewed as a proces of high risk. The birth at home is then in such a discourse considered to be hazardous practice which can threaten the life of the unborn child. This paper tries to contribute to the discussions about the freedom of choice of the place of birth. The birth is then viewed from the perspective of gender as a constructed phenomenon culturaly and socialy biased. The main focus is on the research of the experience of women who gave birth at home. Their experience is to be considered as constructed as well, therefore I investigate the leverages that influence their interpretation of the birth experience. I am also interested in researching how their experience influenced their futher life and what meaning they asign to it in the context of their life experience as such. Further interest of this paper is to research the knowledge that the women, who gave birth at home, used during the proces of childbirth. The main focus is to elicit the auhoritative knowledge based on which they make decisions and how this knowledge is constituted.

Socioeconomic status and its influence on the process of pregnancy. The dissertation deals with the problems of the influence of the socioeconomic status on the development of pregnancy pathologies. The dissertation is divided into practical and theoretical parts. The aim of the theoretical part is to inform about problems of pregnancy from the medical standpoint and from the social point of view. There is also a treatise about socioeconomic status and its attributes, about fertility and about natality. In the dissertation there is also a chapter that with the problems of family. Research in the practical part was concentraced on the analysis of socioeconomic status and the pregnancy complications of women that gave birth to a child in Havlíčkův Brod Hospitál in a certain period of time. Complex of women was divided into two groups according to the presence or absence of pregnancy and obstetrical complications. Both groups were compared. As the instrument for the data acquisition and questionnaire, the quantitative research strategy was used. The aim of the dissertation was to find some factors refering to the socioeconomic status that might influence pregnancy complications. As the attributes of socioeconomic status that influence pregnancy complications following factors were marked: the highest finished education of woman and profession of woman before maternity leave.

This thesis deals with disagreement between midwives and doctors about the fact where and in which conditions mothers should give birth to a child. While the doctors support the present way when most children are born in hospitals under doctor's control, midwives argue for so called natural childbirth when women deliver the child surrounded by familiar environment and with midwife's assistance. The description of both groups behaviour is based on the theory of Peirre Bourdieu. Midwives are focused on the change of their status in the process of childbirth while doctors want to preserve the present situation. Both groups use different discourses while creating their professional position providing different services. Doctors' way is more technological and clinical, while midwives create their professional position in more psychological way. There is a competitive fight in which both group use strategy of their own professionalization and disprofessionalization of the rival group and different interpretation of the same data. Because of disadvantageous position of midwives in the process of childbirth, they create their own strategies against doctors.
