Post-Deployment Health Assessment in United States Service Members after Iraq Deployment: A Dissertation

Collins, Sean T. 01 January 2009 (has links)
The purpose of this study was to identify health and emotional-related issues of service members after a deployment to Iraq. Secondary data analysis and a cross-sectional descriptive design, were used to analyze data from the Department of Defense Post Deployment Health Assessment (PDHA) database. The cognitive appraisal model of stress and coping (Lazarus and Folkman, 1984) guided this study. Several statistical techniques were used including: frequency distributions cross tab evaluations, factor analysis, reliability calculations, regression analysis and tests for mediation. The study sample included 510, 352 service members (49,998 females, 460,349 males) with a mean age of 29 years. The sample represented all components and branches of the military. Of the total sample, 51.9% (n=264,777) saw wounded, killed or dead individuals and 22.1% (n=112,620) discharged their weapon in combat. Environmental exposures were an important source of stress. Exposures to sand and dust were the largest complaint (89.8% of the sample). Multiple physical symptoms were identified and 40% of the sample reported four or more symptoms (e.g. diarrhea, back pain, headache, fatigue). PTSD symptoms were identified in 11.8% (n = 60,200) and depressive symptoms in 26.5%, (n=123,808) of participants. Results of the study indicated that age, gender, rank, race/ethnicity, military component and branch were important predictors of emotional and health-related concerns in this sample. Appraisal variables (danger of being killed and exposure concerns) mediated the relationship between immediate (physical and depressive symptoms) and long term outcomes (health perception, PTSD symptoms) for the majority of the analyses; supporting the study hypothesis. However, length of deployment did not have a significant impact on stress-related outcomes in this study. Implications for practice, policy and future research are discussed.

Serotonina e sensibilidade a estímulos relacionados ao trauma no transtorno de estresse pós-traumático remitido com inibidores seletivos de recaptura de serotonina / Serotonin and sensibility to trauma-related stimuli in selective serotonin reuptake inhibitors posttraumatic stress disorder

Corchs, Felipe D\'Alessandro Ferreira 15 December 2008 (has links)
As serotonin (5HT) helps regulate stress responses in other anxiety disorders, the acute tryptophan depletion (aTD) technique was used to lower brain 5HT in SSRSs-remitted PTSD patients. METHODS: Ten patients with PTSD (Mini-International Neuropsychiatric Interview diagnosed) who had made a full recovery on SSRIs (Clinical Global Impressions Improvement Scale 1-2 for at least 3 months) were enrolled in the experiment. Patients were tested on 2 separate occasions a week apart - each session they received a drink containing large neutral amino acids either with (Sham Depletion [SD]; control day) or no tryptophan (aTD day). Self reports of anxiety and mood, as well as cardiovascular measures, were obtained throughout the tests. At 5.5 hours after the drink subjects were reexposed to their trauma using a modification of Pitmans imagery guided method. RESULTS: These procedures provoked elevated ratings on both days, with significantly more marked responses on the aTD day according to Visual Analogue Scales (aTD 47.57 [21.75] -v- SD 20.71 [18.4]; p=0.006), Davidson Trauma Scale (29.4 [12.7] -v- 15.7 [7.79]; p=0.001), Spielberger State Anxiety Inventory (28.9 [11.03] -v- 18.5 [10.13]; p=0.066, and Profile of Mood States (p<0.001). CONCLUSIONS: These data are the first to show that 5HT depletion worsens the subjective responses to reliving traumatic memories in PTSD and suggest that that SSRI-induced increases in 5HT function restrains PTSD symptoms, especially under provocation, i.e. 5HT helps mediate resilience to stress. As well as giving insights into how SSRIs work in PTSD, these data may also offer a translational approach to potential new treatments for this disorder

The battlefield at home: the meaning of homelessness from the female veteran’s perspective

Miller, Chiquita January 1900 (has links)
Doctor of Philosophy / School of Family Studies and Human Services / Charlotte Shoup Olsen / Farrell J. Webb / Homelessness has become an enduring fixture of contemporary United States society. Female veterans face a host of unique challenges; females often carry the burden of serving in the armed forces, while balancing marriages, motherhood, and care giving responsibilities in their home lives. As the veterans return to their lives as civilians, the females who served in the military must deal with the possibility of sexual harassment, sexual assault, and rape while in the armed services. Female service members are twice as likely to have Post Traumatic Stress Disorder (PSTD) than their male service members and are three to four times more likely to become homeless. Understanding this view of homelessness from the female veteran’s perspective is limited due to small sample sizes in previous research efforts. However, with the increasing numbers of homeless female veterans it is imperative to understand the risk factors. A qualitative descriptive study was conducted using a modified framework for studying vulnerable populations. The study was designed to explore the meaning of homelessness from the female homeless veteran’s perspective. Second, risk factors were examined for homelessness and the services necessary for the female veteran to exit the homeless cycle. Third, the data were coded and analyzed to identify patterns and commonalities of multiple psycho social factors such as unstable family support, domestic violence, job loss, affordable housing options, substance abuse, mental and physical health issues. These factors were cited as the leading risk factors contributing to the homeless state of this sample of female veterans. The data collection consisted of ten homeless female veterans participating in a private, audio taped interview using a semi-structure interview tool. Resources listed as a necessity to end homelessness consisted of affordable housing, job security, earning a living wage income, transportation, remaining drug free, and being awarded disability. The pathway to homelessness varied for each participant, but they all demonstrated a tremendous amount of resiliency.

Développement socio-affectif des enfants exposés à la violence conjugale : une approche de la sécurité émotionnelle : étude des conduites intériorisées et extériorisées et des symptômes de stress post-traumatique de 46 enfants âgés de 5 ans et demi à 12 ans, et analyse de leurs représentations de la violence et des relations familiales / Socio-emotional development of children exposed to domestic violence : an approach of emotional security : study internalizing and externalizing problems and post-traumatic stress symptoms of 46 children aged 5 and a half to 12, and analysis of their representations of violence and family relationship

Paul, Olivia 12 October 2015 (has links)
The aim of this research is to study the influence of specific and combined context of domestic violence on socio-emotional development of children aged between 5 and a half to 12 and to consider their representations of violence and their family relationships. Our study is based on the model of emotional security Cummings and Davies (1994). With this in mind, our sample consists of 32 mothers and 46 children aged 5 and a half to 12, living in a context of domestic violence. The mothers have filled in three questionnaires assessing first, the intensity of domestic violence using the « Conflict Tactic Scale II » (Cyr, Fortin & Chénier, 1997), the socio-emotional adaptation of their children with the questionnaire « Children Behavior Checklist » (Achenbach, 1991) and then, a general information questionnaire. As for the children, they have filled in four questionnaires that allow to understand their perceptions of violence with « Children's Perception of Interparental Conflict » (Cyr & Fortin, 2001), the process of parentification through the « Parentification Questionnaire Youth » (Fortin 2001), loyalty conflicts with the « loyalty conflicts questionnaire » (Fortin, 2005) and post-traumatic stress symptoms with « Trauma Symptom Checklist for Children » (Briere, 1989). Finally, the « Systemic Analysis of Group Affiliation » (Compagnone, 2009) has been used with the children to understand their representations of cohesion and power within their families. The main results show that half of the children have externalizing problems and 82.6% internalizing problems. The degree of exposure to child abuse influences his representations of family cohesion, his feeling of threat and blames, his loyalty conflicts and the presence of certain symptoms of PTSD. The children’s representations of violence and family relationships, such as parentification, the feeling of blame, loyalty conflicts and the representations of cohesion and power in the family, are predictive of symptoms of post-traumatic stress disorders and social-emotional adjustment. The modulating effect of the representation of power and family cohesion, as well as depictions of violence and conflicts of loyalty have been highlighted. Finally, more than half of the children in this sample, have been exposed to domestic violence but also to direct abuse, that’s why the consequences underlined are all the more important. All these results allow us to propose research perspectives and methods of interventions.
