An evaluation of health care of prisoners at selected institutions : a nursing perspective

Thesis (MCur) -- University of Stellenbosch, 2005. / ENGLISH ABSTRACT: A non-experimental descriptive study was conducted in four prisons in the Western Cape. The research focussed on the standard of care within primary health care settings in the purposively selected prisons. No official written standards existed to measure the quality of care. After an in-depth literature study, structure, process and outcome standards were formulated and validated. The main findings of the study indicated that: • Standards in these three dimensions of care did not meet the pre-set level of performance as determined by the researcher; • The clients were generally satisfied with the hygiene in the clinics; • Aspects of concern were the lack of explanation offered to the clients before and after consultations and nursing care interventions. The researcher recommends the implementation of a comprehensive quality care model for the health care of the prisoners with adequate human and nonhuman resources Core words: Qualtiy care, Correctional services health care, Nursing care in prisons. / AFRIKAANSE OPSOMMING: "n Nie-eksperimentele beskrywende studie is in vier gevangenisse in die Wes- Kaap uitgevoer. Die navorsing het gefokus op die gehalte van sorg binne primere gesondheidsorgomgewings in die doelbewuste geselekteerde omgewings. Geen amptelike geskrewe standaarde om die gehalte van sorg te meet, het bestaan nie. Na 'n in-diepte literatuurstudie is struktuur-, proses- en uitkomsstandaarde geformuleer en gevalideer. Die belangrikste bevindings van die studie het aangedui dat: • Standaarde in hierdie drie dimensies van sorg het nie aan die voorafbepaalde vlak van sorg voldoen het nie soos deur die navorser bepaal is; • Die kliente was oor die algemeen tevrede met die higiene in die klinieke; • Kommerwekkende aspekte het ingesluit die gebrek aan voldoende verduidelikings aan kliente voor en na konsultasies en verpleegintervensies. Die navorser bevel aan dat In omvattende gehalteversekeringsmodel vir die gesondheidsorg van gevangenes ingestel word met voldoende beskikbare mensen ander hulpbronne. Exploring the experiences of adult offenders living with HIV on pre-antiretroviral therapy program at the Losperfontein Correctional Centre

Text in English / The aim of the study was to explore the experiences of adult offenders living with HIV (OLWHIV) not qualifying for antiretroviral therapy (ART). Such offenders are put on the pre-antiretroviral therapy (pre-ART) program after HIV diagnosis. Follow up of OLWHIV is done every six months to ensure prompt treatment. Research objectives include exploration of experiences of OLWHIV on the pre-ART program, the accessibility of the program and the challenges thereof. An exploratory, qualitative study with face-to-face interviews was conducted. Purposive sampling of the eight participants was done to conduct the study. Seven out of eight participants accessed the pre-ART program well but had little knowledge of the pre-ART program. Furthermore, participants experienced little support from partners and health care workers. The study showed institutional constraints such as poor diet, shortage of staff and humiliation from Correctional officers. Participants portrayed commitment in the support group irrespective of the challenges experienced. The study further showed that the self-care theory could enhance the pre-ART program but that institutional constraints deterred the progress. Participants made recommendations such as strengthening of partnerships for support groups, good diet, and an increase of staff capacity. Harm reduction in state prisons

Risk taking behaviours such as drug use, sexual activities and tattooing are prevalent in the correctional institutions, including those in South Africa. Such behaviours pose a serious challenge as regards health care of inmates. In particular, these behaviours contribute to the transmission of HIV/AIDS which results in morbidity and mortality. Harm reduction components are employed as effective measure to curb the spread of the pandemic. These components are lauded owing to their considerable impact. They consist of needle exchange programmes, substitution therapy, condom provision and education. Various developing (Morocco, Brazil and Egypt) and developed (Scotland and Canada) countries make use of such components to address risk taking behaviours in correctional institutions. Although condom provision and education have been implemented by the South African Department of Correctional Services in state institutions, there is an urgent need to enhance the efforts. This study investigates the extent of risk taking behaviours amongst inmates at the Leeuwkop Correctional Centre. It also determines the level of knowledge of inmates and staff regarding HIV/AIDS. The researcher employed a qualitative form of methodology, collecting data by means of a structured questionnaire. The data was coded and analysed by means of the Statistical Package for the Social Sciences (SPSS) software. The findings reveal that the risk taking behaviours are prevalent at the Leeuwkop Correctional Centre. Furthermore, the analysis of the knowledge items regarding HIV/AIDS indicates that there are certain deficits that require attention. They are also notable differences in the primary sources of HIV information for inmates and staff. It was found that a dire need exists to enhance and expand current harm reduction initiatives in correctional institutions in order to offer health care services that are compliant with international conventions such as the Dublin Declaration on HIV/AIDS as well as the South African Constitution. Reluctance to do so is tantamount to housing inmates in „de facto‟ death chambers. Hence the augmentation of such initiatives is strongly recommended. / Penology / D. Litt. et Phil. (Penology)
