Professional health care workers' experiences of care at two Community Day Clinics on the Cape Flats

Magister Artium (Social Work) - MA(SW) / Primary Health Care (PHC) is the cornerstone of health care globally, nationally and locally and, therefore, should be regarded as the foundation of health care provision. In South Africa, Community Day Clinics (CDCs) are part of the bouquet of services that is being offered at a PHC level. There are various factors that generate inconsistency in the provision of care to people accessing these CDCs. The purpose of this study was to identify and explore how these factors impact on the care practices that health care professional’s provide. Research suggests that the majority of health care workers are women, who play a double role as carers in their professional and private lives. Therefore, the political ethics of care, a feminist theoretical approach, was utilized to understand care practices in these health settings. The aim of the study was to develop an in-depth understanding of the care practices of health care workers at two CDC facilities on the Cape Flats. A qualitative research methodology was used to explore and identify the phenomenon under study. The research project followed an explorative and descriptive research design, as the researcher sought to understand the care practices of health care workers and how their values and ethics further influenced care practices at these two CDC settings. The data was gathered using semi-structured one-on-one interviews, and later analysed using qualitative thematic analysis. The research findings were grouped in terms of the values entrenched in the political ethics ofcare, which are attentiveness, responsibility, competence, responsiveness and trust. The research findings identified various aspects that, either negatively or positively, impact on these values. Finally, recommendations were made to management, as well as care workers. These recommendations were in terms of implementing care services that are attentive to service-users and care-workers; providing a service that takes into consideration the value of responsibility; the provision of competent services; and finally creating trusting relationships within the CDC.

Fundamentou-se nos estudos de Claude Dubar e de outros autores que debatem a identidade profissional, tendo como pano de fundo a história da enfermagem e a própria história da enfermagem no hospital. Estudo qualitativo, descritivo e exploratório, realizado no Hospital Universitário, no município do Rio Grande-RS. Fizeram parte da pesquisa dez enfermeiros: todos os primeiros enfermeiros da Fundação de Apoio ao Hospital de Ensino, em 1987, que também foram selecionados no concurso de 1994 e ainda permanecem no hospital (Grupo 1); dois enfermeiros que ingressaram no hospital por meio do concurso de1994 (Grupo 2); dois enfermeiros concursados em 2004 (Grupo 3) e dois concursados em 2008 (Grupo 4). A coleta de dados ocorreu entre os mesesde setembro e outubro de 2011, por meio da História Oral Temática. O tratamento dos dados foi por meio da Análise Temática proposta por Bardin, resultandonas seguintes categorias e subcategorias: 1.Traços identitários da enfermagem do Hospital Universitário Dr. Miguel Riet Corrêa Jr.: fortalezas nas trajetórias profissionais de enfermeiros; 1.1 O mundo vivido no trabalho do HU:a construção e organização do hospital; 1.2 O mundo vivido no trabalho do HU: a auto-organização e a interação da equipe de enfermagem; 2 Traços identitários da enfermagem do Hospital Universitário Dr. Miguel Riet Corrêa Jr.: fragilidades nas trajetórias profissionais de enfermeiros; 2.1 Omundo vivido no trabalho do HU: o afastamento docente-assistencial na enfermagem; 2.2 O mundo vivido no trabalho do HU: (des)estruturas e (des)organizações físicas, materiais e humanas. Considera-se que os aspectos fortalecedores e fragilizadores revelam que a identidade profissional do enfermeiroé construída continuamente no tempo e no espaço do cotidiano do ambiente hospitalar do indivíduo, como um processo dinâmico e transitório expresso ao longo da trajetória profissional de cada membro do grupo profissional, alicerçada por alguns traços explorados nesta pesquisa. Para harmonizar o mundo vivido no trabalho do Hospital devem-se explorar os aspectos fortalecedores dos profissionais, motivando e aproximando-os da realidade, e converter os fragilizadores em ações coletivas que modifiquem e tonifiquem o grupo diante da organização, potencializando características desafiadoras favoráveis e otimizando as iniciativas que identificam os profissionais com a instituição e a profissão. / The study aimed to reveal identity features of nursing at the University Hospital Dr. Miguel Riet Correa Jr (HU). It was based on studies of Claude Dubar and other authors who discuss professional identity, with the backdrop of the history of nursing and the history of nursing in the hospital. Qualitative, descriptive and exploratory study, conducted at University Hospitalin Rio Grande-RS. The participants were ten nurses: all the first nurses from Foundation to Support Teaching Hospital in 1987, which were also selectedin the 1994 contest and still remain in the hospital (Group 1); two nurses who started working in the hospital applying for position in 1994 (Group 2), two other nurses applied in 2004 (Group 3) and two applied for the position in 2008 (Group 4). Data gathering occurred between the months of September and October 2011, through the thematic oral history. The data were through thematic analysis proposed by Bardin, resulting in the following categories and subcategories: 1.Identity Features of nursing at the University Hospital Dr. Miguel Riet Correa Jr.: strongholds in the career paths of nurses; 1.1The work of the world lived in HU: the construction and organization of the hospital; 1.2 The work of the world lived in HU: The self-organization and interaction of the nursing staff; 2 Identity Features of nursing at the University Hospital Dr. Miguel Riet Correa Jr.: weaknesses in career paths of nurses; 2.1 The work of the world lived in HU: remoteness Teaching care in nursing; 2.2 The work of the world lived in HU: (de) and structures (dis) organizations physical, material and human. It is considered that the embrittlement and empowering aspects reveal that the nurse's professional identity is constructed continuously in time and space of everyday life of the individual hospitals, as a dynamic and transient expressed along the career path of each member of the professional group , founded by some traits explored in this research. To harmonizethe world experienced in the work of the Hospital should be explored aspectsof professionals empowering, motivating and bringing them closer to the reality, and convert the embrittlement into collective actions that modify and invigorate the group on the organization, leveraging and optimizing features challenging favorable initiatives that identify professionals with the institution and the profession. / El estudio tuvo como objetivo desvelar trazos identitarios de la enfermería del Hospital Universitario Dr. Miguel Riet Corrêa Jr (HU). Se fundamentó en los estudios de Claude Dubar y de otros autores que debaten la identidad profesional, teniendo como segundo plano la historia de la enfermería y la propia historia de la enfermería en el hospital. Estudio cualitativo, descriptivo y exploratorio, realizado en el Hospital Universitario, en el municipio de Río Grande-RS. Formaron parte de la investigación diez enfermeros: todos los primeros enfermeros de la Fundación de Apoyo al Hospital de Enseñanza, en 1987, que también fueron seleccionados en el concurso de 1994 y aún permanecen en el hospital (Grupo 1); dos enfermerosque ingresaron en el hospital por medio del concurso de 1994 (Grupo 2); dos enfermeros concursados en 2004 (Grupo 3) y dos concursados en 2008 (Grupo 4). La colecta de las informaciones ocurrió entre los meses de septiembre y octubre de 2011, por medio de la Historia Oral Temática.
