The purpose of this study was to describe critical care nurses' perceptions of their experiences with nursing quality assurance activities. Using an exploratory, descriptive design, data were collected in a survey, utilizing a self-administered questionnaire. A convenience sample of critical care nurses, who are members of the Canadian Association of Critical Care Nurses, was used. The results showed that these particular nurses knew what comprised the components of a nursing quality assurance program, however, their participation in these activities was low. In addition, the majority identified that the primary purpose of nursing quality assurance activities was to meet the accreditation requirements of the hospital. An assessment of the management of sexually transmitted diseases (STDs) in a rural district health ward of Northern KwazuluMcCoy, David 03 May 2017 (has links)
This study is an assessment of the quality of sexually transmitted disease (STD) management and control in a rural district of South Africa. A semi-structured questionnaire was administered to 5 nurses from public sector primary health care clinics, 5 doctors from the public district hospital, 5 private general practitioners, 6 traditional healers and 7 STD patients. A patient simulation exercise involving 6 nurses and 6 general practitioners was also conducted. Using routine data collection forms, the spectrum of STD syndromes and the contact tracing rate were assessed. The private sector treated nearly a third of the STDs even though they charge about ten times the price of the public sector services. In general, the clinical skills of all providers were poor. While hypothetical patient histories produced reasonable responses on STD management during the interviews, the patient simulation results showed that health service providers provided STD management that was much poorer than the questionnaires indicated. The private general practitioners did not practice syndromic STD management and often did not use laboratory tests appropriately resulting in incorrect diagnosis and inappropriate treatment for STDs. All health service providers did not counsel, promote condoms or encourage contact notification adequately. All health service providers were keen to participate in continuing medical education that better equip them to manage STDs. Any attempts at improving the quality of care in the district must therefore include private general practitioners as an important and central component of STD policy and planning. Interviews with traditional healers and patients showed the importance of using non-biomedical constructs of health and illness in developing health promotion strategies. There is an urgent need to improve STD management at district level in an attempt to meet the first milestone of ensuring that a patient presenting with an STD to a health service is correctly managed. This can be done through the design of simple quality assurance methods as demonstrated in this paper.

Introdução: A dor da criança é uma das queixas de maior procura pelos serviços de urgência e de emergência e seu manejo tem sido um motivo de preocupação pelos profissionais de saúde. Para avaliar os cuidados prestados aos pacientes, são utilizados indicadores de qualidade, dentre eles, a satisfação. Objetivo: Compreender a satisfação da criança e de seu familiar acerca do manejo da dor no pronto-socorro infantil. Metodologia: Estudo qualitativo exploratório e descritivo. Para a discussão dos dados foi utilizado o modelo de Donabedian. Os participantes deste estudo foram constituídos por crianças entre seis e 12 anos de idade e seus familiares. As entrevistas foram gravadas e os dados analisados à luz da análise temática. Resultados: Foram entrevistados 19 crianças e 19 familiares entre outubro de 2016 a maio de 2017. Os resultados sobre a percepção da satisfação acerca do manejo da dor no pronto-socorro infantil compuseram o Tema 1: A satisfação percebida pelas crianças e familiares, composto por três subtemas: Atendimento prioritário, alívio rápido da dor e Acolhimento pela equipe A satisfação percebida pelas crianças composto por um subtema: Necessidades básicas atendidas. A satisfação percebida pelos familiares, composto por um subtema: Investigação dos sintomas. E a insatisfação percebida acerca do manejo da dor no pronto-socorro infantil compuseram o Tema 2: A insatisfação percebida pelas crianças e familiares, composto por um subtema: Demora em aliviar a dor A insatisfação percebida pelas crianças composto por dois subtemas: Procedimentos dolorosos e Ambiente desconfortável. A insatisfação percebida pelos familiares composto por um subtema: Demora pelos resultados dos exames\". Conclusão: Observou-se que a satisfação da criança e de seus familiares foi muito mais do que o alívio da dor, ou o atendimento prioritário. Englobou também o ambiente e o acolhimento pela equipe. É recomendado que o cuidado do profissional de saúde seja direcionado às singularidades das crianças e de suas famílias. / Introduction: Child pain is one of the most pressing complaints of emergency services and its management has been a cause of concern for health professionals. To assess the care provided to patients, quality indicators are used, among them, the satisfaction. Objective: To understand the satisfaction of the child and his/her family with pain management in the emergency room. Methodology: A qualitative, exploratory and descriptive study. To discuss our data was used the Donabedian model. The study population consisted of children between six and 12 years age and their family. Interviews were recorded and data analyzed in the light of the thematic analysis. Results: We interviewed 19 children and 19 family members between october 2016 and may 2017. The results on the perception of satisfaction with pain management in the emergency room constituted theme 1: Children\'s and their families perceived satisfaction, composed of three sub-themes: \"Priority in care\", \"quick pain relief\" and \"warm welcome at the hospital\". Children\'s perceived satisfaction, composed of one sub-theme: \"Basic needs met\". Families perceived satisfaction, composed of one sub-theme: \"Investigation of symptoms\". And the results on the perception of dissatisfaction with pain management in the emergency room constituted theme 2: Children\'s and their families perceived satisfaction, composed of one sub-theme: \"Delays in relieving pain\". Children\'s perceived dissatisfaction composed of with two sub-themes: Painful Procedures \"and\" Uncomfortable Environment\". Families perceived dissatisfaction composed of one sub-theme: \"Delays in the results of the exams\". Conclusion: We observed that the satisfaction of the child and his/her family with pain management was beyond pain relief or priority care. Also included the environment and the warm welcome given by the healthcare team. We recommend that the care provided by the health professional be directed to the singularities of each child and their family.

A avaliação de qualidade das instituições de saúde evidencia uma contribuição expressiva das ações de enfermagem nos processos assistenciais. A técnica de observação com base em um check list pode se constituir em uma ferramenta valiosa para tal. A gestão do serviço de enfermagem pode ser facilitada com a adoção de indicadores assistenciais que permitem revisar os processos vigentes. Assim, este estudo objetivou avaliar a qualidade da assistência de enfermagem relacionando-a ao perfil de cuidados identificado. Estudo observacional, longitudinal, relacional, quantitativo, realizado em um hospital regional, acreditado de Minas Gerais. A amostra foi composta pelos pacientes internados nas Unidades de Clínica Cirúrgica (UCC) e Clínica Médica (UCM). Foram utilizados dois instrumentos para coleta de dados: o registro de busca ativa, baseado em 15 indicadores de qualidade, que classifica a assistência em segura, desejável, adequada, limítrofe ou insuficiente e o instrumento de caracterização do grau de dependência por meio do Sistema de Classificação de Pacientes de Perroca. Os dados foram coletados e organizados em planilhas com dupla digitação e analisados por meio estatística descritiva e teste qui quadrado de Pearson, realizadas por meio do programa STATA. Durante os 14 dias de coleta, 185 pacientes foram avaliados, totalizando 650 observações, 296 na UCC e 354 na UCM. Os resultados apontam a prevalência de homens internados nas clínicas e com idade mediana de 56 anos na UCC e 67 anos na UCM, sendo a maioria dos pacientes avaliados com perfil de cuidados mínimos nas duas unidades. Os pacientes em cuidados mínimos (CM) prevaleceram nas duas unidades, em seguida pacientes em cuidados intermediários (CI); presença de cuidados semi-intensivos (CSI) apenas na UCM, e nenhum paciente categorizado em cuidados intensivos. Na associação entre o perfil assistencial dos pacientes aos indicadores avaliados, contatou que não houve diferença na qualidade prestada, quando comparados CM e CI na UCC e UCM, e entre pacientes em CI e CSI na UCM. No tocante a adequação ao padrão de qualidade, o percentual de positividade obtido ficou abaixo do esperado, considerado insuficiente, com valores de 58% na UCC e 55% na UCM. Ao se estabelecer a associação entre o perfil de cuidados e o percentual de adequação de cada indicador nas unidades avaliadas verificou-se que não houve diferença estatística na qualidade assistencial oferecida. Apesar das características diversas das clínicas bem como de perfis de pacientes e da equipe de enfermagem identificou-se semelhança nas avaliações. Os resultados encontrados trazem implicações para a qualidade assistencial ofertada pela instituição que tem investido na melhoria da qualidade e poderão subsidiar uma discussão mais consistente sobre os processos de trabalho / The evaluation of quality of the health institutions makes evident the significant contribution of the nursing actions on the assisting processes. The observational technique based in a check list can be a valuable tool for such an action. The management of the nursing services can be facilitated with the adoption of the assistential indicators that allow to review the current processes. Therefore this study aimed to evaluate/assess the quality of the assistance in nursing according to the profile of patients of an accredited hospital in Minas Gerais. It is an observational, longitudinal and relational study with quantitative approach; accomplished in a regional hospital with inpatients receiving care in a Surgical Clinic Unit (SCU) and at a Medical Clinic Unit (MCU). Two instruments were used to collect the data: the Register of Active Search based on the 15 quality indicators that assess the care assistance in safe, desirable, adequate, limited or insufficient; the second instrument, provided by the institution determines the patients according to the degree of dependency by the Perroca\'s Classification System of Patients, after being exposed to the opinion of the Committee of Ethics in Research. The data were collected and organized in double typed spread sheets. And was used simple descriptive statistic for analysis and the Pearson\'s Chi-Squared test by the STATA program. During the 14 collecting days 185 patients were evaluated totalizing 650 observations, 296 at SCU and 354 at MCU. It was observed a predominance in men admitted to the hospital receiving care in the health center with an average age of 56 years old at SCU and 67 years old at MCU. The patients in Minimal Care (MC) prevailed on the two units, followed by the patients receiving Intermediate Care (IC). With Semi Intensive Care (SIC) only at MCU and no one patient categorized in Intensive Care. By associating the assistential profile of the patients to the assessed indicators, it was verified that there wasn\'t difference in the quality offered when comparing MC and IC at SCU and MCU, and between patients in IC and SIC at MCU. Concerning to the adequacy of the quality pattern, the percentage of positivity obtained was below expected, considered insufficient with values of 58% at SCU and 55% at MCU. By establishing the association between the profile of caring and the percentage of adequacy to pattern of quality of each indicator at the evaluated units, it was verified that there wasn\'t statistic difference in the quality offered. Although the different characteristic in each clinic as well as the profile of the patients and of the nursing staff it was identified similarity on the evaluations. The results found point out implications to the assistential quality of the assistance offered by the institution that has invested on the improvement of the quality and can contribute in a more effective discussion about the working process
