Re-evaluation of older antibiotics in the area of resistant mycobacteria / Mycobacterium tuberculosis, resistance, antibiotiques, re-evaluation, Mycobacteries

Chez les patients traités par un régime posologique, La concentration sérique moyenne et l'écart type de la concentration SMX était 161,01 ± 69,154 mg/L et de 5,788 ± 2,74 mg/L pour le TMP. La concentration minimale inhibitrice 90% (CMI 90) était de 10 mg/L pour le cotrimoxazole et la sulfadiazine contre Mycobacterium tuberculosis. Toutes les mycobactéries étaient inhibées par 20 mg/L de cotrimoxazole et de sulfadiazine. Les CMI de l'ivermectine contre 13 souches complexe M. tuberculosis ont varié entre 10 et 40 mg/L. En outre, tous isoler M. tuberculosis étaient résistants à la squalamine avec CMI > 100 mg/L. Dans une autre partie nous avons montré que tous les isolats du complexe Mycobacterium avium étaient résistants au triméthoprime avec une CMI > 200 mg/mL. Le cotrimoxazole, le sulfaméthoxazole et la sulfadiazine ont montré une CMI respectivement de 10 mg/L, 25 mg/L et 20 mg/L, à l'exception de Mycobacterium chimaera qui présentait une CMI de 10 mg/L pour ces molécules. La comparaison de la séquence du gène de la dihydroptéroate synthase de M. intracellulare et M. chimaera a montré seulement quatre changements d'acides aminés. / Firstly, we measured the serum concentration of Sulfamethoxazole (SMX)-Trimethoprim (TMP) in patients treated with high dosage regimen. The mean values and standard deviation for SMX concentration was 161.01± 69.154 mg/L and of 5.788 ± 2.74 mg/L for TMP. Susceptibility testing yielded a minimum inhibitory concentration 90% (MIC90) of 10 mg/L for cotrimoxazole and sulfadiazine. All M. tuberculosis complex mycobacteria (MTC) were inhibited by 20 mg/L cotrimoxazole and sulfadiazine. Also, the MICs of ivermectin varied between 10 and 40 mg/L, against 13 MTC mycobacteria. Moreover, all M. tuberculosis isolate were resistant to squalamine with MIC > 100 mg/L. Also, all Mycobacterium avium complex (MAC) isolates were resistant to trimethoprim with MIC > 200 mg/L. Cotrimoxazole, sulfamethoxazole and sulfadiazine exhibited MIC of 10 mg/L, 25 mg/L and 20 mg/L, respectively against all tested MAC isolates except for Mycobacterium chimaera which exhibited MICs of 10 mg/L for these molecules. Comparing the DHPS gene sequence in M. intracellulare and M. chimaera type strains and clinical isolates yielded only four amino acid changes.

<p><strong>Objectives</strong></p><p>The aim of this study was to compare left ventricular ejection fraction (LVEF) results derived from gated single photon emission computed tomography (SPECT) using Cedars-Sinai quantitative gated SPECT (QGS) processing software with results from 2D echocardiography, both obtained in routine clinical diagnostic use.</p><p> </p><p><strong>Methods</strong></p><p>Data from previously performed tests were obtained from 73 patients who had undergone both 2D echocardiography and gated SPECT within a time span of 6 months and had not had significant events that could influence LVEF. LVEF from 2D echocardiography was reassessed to obtain discrete values and then the data was compared using Bland-Altman analysis.</p><p> </p><p><strong>Results</strong></p><p>The correlation between the tests was shown to be good, but precision lacked. Bland-Altman analysis showed a bias of -0.8 percentage points when gated SPECT compared to mean values and 2 standard deviations (SD) ranged from -20.2 to 18.6.</p><p> </p><p><strong>Conclusions</strong></p><p>LVEF values from the two methods can differ quite a bit and comparisons between them should be done with great caution.</p> / <p><strong>Syfte</strong></p><p>Syftet med studien var att jämföra hur bra ultraljud av hjärta och isotopundersökning av hjärta stämmer överens när det gäller att visa hjärtats pumpförmåga.</p><p> </p><p><strong>Metod</strong></p><p>Data från tidigare utförda undersökningar av 73 patienter jämfördes. Patienter som hade gjort båda undersökningarna inom 6 månader och under perioden mellan undersökningarna inte hade haft hjärtinfarkt eller någon annan händelse som kan påverka hjärtats pumpförmåga valdes till studien.</p><p> </p><p><strong>Resultat</strong></p><p>Utslaget över hela studiepopulationen stämde resultaten från de båda undersökningarna bra överens. Jämförde man däremot resultaten från de båda undersökningarna med varandra patient för patient förekom mycket stora variationer.</p><p> </p><p><strong>Slutsats</strong></p><p>Resultaten angående hjärtats pumpförmåga kan skilja sig mycket från varandra. "Until the crows came to collection their souls": re-vissioning the fantacy hero in selected fiction by Steven Erikson

In the course of this dissertation, I will interrogate traditional representations of the heroic figure within the fantasy genre. I will argue that the tropes established by some of the most renowned fantasy texts are undergoing a process of evolution and that Steven Erikson (who is the special focus of my discussion) seeks to revision the heroic mould through his construction of Coltaine of the Crow Clan in Deadhouse Gates (2001). Deadhouse Gates centres on Coltaine, who is tasked with escorting tens of thousands of refugees across four hundred leagues of hostile territory. This re-evaluation of fantasy modes has significant ramifications for the future development of the genre as a whole. In order to explore how Erikson interrogates heroic representation, I briefly establish some of the distinctive characteristics of fantasy. I then explore how some major fantasy texts represent heroism, before investigating Erikson’s particular response to these traditions. / English Studies / M. A. (English)

Internet-Scale Information Monitoring: A Continual Query Approach

Information monitoring systems are publish-subscribe systems that continuously track information changes and notify users (or programs acting on behalf of humans) of relevant updates according to specified thresholds. Internet-scale information monitoring presents a number of new challenges. First, automated change detection is harder when sources are autonomous and updates are performed asynchronously. Second, information source heterogeneity makes the problem of modelling and representing changes harder than ever. Third, efficient and scalable mechanisms are needed to handle a large and growing number of users and thousands or even millions of monitoring triggers fired at multiple sources. In this dissertation, we model users' monitoring requests using continual queries (CQs) and present a suite of efficient and scalable solutions to large scale information monitoring over structured or semi-structured data sources. A CQ is a standing query that monitors information sources for interesting events (triggers) and notifies users when new information changes meet specified thresholds. In this dissertation, we first present the system level facilities for building an Internet-scale continual query system, including the design and development of two operational CQ monitoring systems OpenCQ and WebCQ, the engineering issues involved, and our solutions. We then describe a number of research challenges that are specific to large-scale information monitoring and the techniques developed in the context of OpenCQ and WebCQ to address these challenges. Example issues include how to efficiently process large number of continual queries, what mechanisms are effective for building a scalable distributed trigger system that is capable of handling tens of thousands of triggers firing at hundreds of data sources, how to effectively disseminate fresh information to the right users at the right time. We have developed a suite of techniques to optimize the processing of continual queries, including an effective CQ grouping scheme, an auxiliary data structure to support group-based indexing of CQs, and a differential CQ evaluation algorithm (DRA). The third contribution is the design of an experimental evaluation model and testbed to validate the solutions. We have engaged our evaluation using both measurements on real systems (OpenCQ/WebCQ) and simulation-based approach. To our knowledge, the research documented in this dissertation is to date the first one to present a focused study of research and engineering issues in building large-scale information monitoring systems using continual queries.

Following text is focused on a revision processing of a burial place that is located on the acropolis of the early medieval settlement in Libice nad Cidlinou. There is one of the most important early medieval necropolises on the territory of the Czech Republic where evidences of elite's burials were found. The church was built during an existence of the cemetery. The necropolis was surveyed between years 1948-1957 and 1967-1973. An existing monograph doesn't fulfill criteria that are required for a modern evaluation in many ways. For this reason there were a complex revision of an extant documentation and archaeological material, consequently a new analysis and interpretation of established facts were made.

A study of students’ mathematical identity was carried out in February 2009 involving participants from two colleges of education, one in Dublin (Republic of Ireland) and one in Belfast (Northern Ireland). All participants were pre-service primary school teachers in the third year of their B.Ed. programme, having chosen to specialize in mathematics. Data was gathered using a questionnaire (with, mainly, open-ended questions) followed by focus groups, involving the same participants, on each campus. This paper considers how students’ exploration of their mathematical identity led them to deepen their insight into learning and teaching mathematics. Recommendations are made for how the methods used in this research might be beneficial on a larger scale, in different environments.

Adat Iban: a living traditional wisdom?

Adat Iban are Indigenous law of the Iban people of Sarawak, Malaysia. This dissertation explores the wisdom of Adat Iban and examines whether the Adat and the underlying wisdom are living Adat. Currently, the interpretation, application, and enforcement of the Adat are basically following the Iban ancestors’ practices and complying with spiritual obligations. This practice provides very little legal reasoning which obscures the rationales and wisdom of Adat. Under such situation, the flux of socio-economic and political developments and social changes that have been taking place in the Iban communities impinges the relevance and significance of Adat in the regulation of social relations of the communities. This study presents a reinterpretation and/or re-evaluation of Adat Iban to elucidate the rationales and wisdom of Adat Iban. This approach could provide legal reasonings which could rejuvenate the relevance of Adat and make Adat to become more comprehensible law of general application to the changing Iban communities. “Wisdom” encompasses Iban autochthonous knowledge, ideas, experiences, judgements, and spiritual principle that evolved into Adat. Wisdom is the quality that enables an Adat to be utilised to bring about beneficial or noble purpose, or justice. The study contends that there is wisdom in most Adat Iban. However, the social changes and modernisation that are taking place in most Iban communities raise the question of whether the Adat and the underlying wisdom are living Adat. A living Adat is one that is dynamic and flexible and constantly adjusting and adapting to the new or changing social situations. This study concludes that most Adat Iban have the potential to be living Adat and continue to be relevant for the regulation of Iban communities.
