Improving Workflow at the Point of Care Using the Electronic Health Record

The electronic health record (EHR) is an important part of the effort to improve health care and reduce costs in the United States. Primary care providers, among the largest group of caregivers in the nation, often experience difficulty with implementation and utilization of EHRs. Efforts to enhance the provider's effectiveness in the use of the EHR should result in improved patient outcomes as well as decreasing the overall cost of health care. Guided by the diffusion of innovation theory, this project was initiated to develop a plan for improved usage of the EHR in a primary care setting. A survey and observations were used to better understand how the providers and staff were using the EHR. Observations and a survey of 11 participants were completed. The observations utilizing a mock patient revealed issues related to the usability of screen information, information availability, and user preference for documentation. The mock patient scenario took 25-35 minutes, on average, to complete. All participants stated they had stayed late to input information on actual clinic patients or to clarify their documentation. The same 11 participants completed the Primary Care Information Project (PCIP) Post-Electronic Health Record Implementation: Survey of Providers responses. Descriptive statistics were used to analyze the results. Most participants indicated that the screen font was difficult to read (72.7%), they had difficulty using the EHR (72.8%) and were not satisfied with its use (63.6%). The project recommendations include working with the vendor to improve information access and ongoing training. Improvements to the EHR should support social change by improving access to information at the point of care, enhancing quality treatment and improving patient care outcomes.

Analyzing the Effect of Moving Resonance on Seismic Response of Structures using Wavelet Transforms

Nonlinear structures, when subjected to multiple ground motion records that are scaled to consistent ground motion intensity show significant variation in their response. This effect of ground motion randomness on the variation of structural response is defined as Record-to-Record (RTR) Variability. Ground motion characteristics that contribute to this variability in response includes the variation of signal composition (frequency content) with time (spectral nonstationarity).The phenomenon of moving resonance which occurs when the frequency content of the ground motion shifts in a similar manner as the natural frequencies of the structural response, is likely a contributor to variability. This brings the need to further understand the sources of variability due to moving resonance. The present study was carried out to develop a method to analyze the time-frequency content of a ground motion to assess the occurrence of moving resonance and to quantify its potential in effecting the structural systems. Bilinear elastic and elastoplastic hysteretic behavior was considered. Detailed analysis is done to quantify the effect of moving resonance on structural systems due to 22 far field ground motion records. The wavelet coefficient plots gave very good detail of the characteristics of the ground motions that were not clear from the acceleration time histories and response spectra plots. Instances of moving resonance were found out to be significant. Amplification due to moving resonance was found to be quite large. One instance studied in detail (accelerogram of Northridge earthquake at Beverly Hills) had peak displacement amplified by 6 times compared to the amount of peak displacement expected if the system did not exhibit moving resonance. Based on the analyses results, the characteristics of the ground motion records that don't cause significant moving resonance effect on structural systems were observed. Similarly, the characteristics of the ground motions that do cause moving resonance effect on structural systems were examined. / Master of Science

Making it work for me: beliefs about making a personal health record relevant and useable

Yes / Background: A Personal Health Record (PHR) is an electronic record that individuals use to manage and share their health information, e.g. data from their medical records and data collected by apps. However, engagement with their record can be low if people do not find it beneficial to their health, wellbeing or interactions with health and other services. We have explored the beliefs potential users have about a PHR, how it could be made personally relevant, and barriers to its use. Methods: A qualitative design comprising eight focus groups, each with 6–8 participants. Groups included adults with long-term health conditions, young people, physically active adults, data experts, and members of the voluntary sector. Each group lasted 60–90 min, was audio recorded and transcribed verbatim. We analysed the data using thematic analysis to address the question “What are people’s beliefs about making a Personal Health Record have relevance and impact?” Results: We found four themes. Making it work for me is about how to encourage individuals to actively engage with their PHR. I control my information is about individuals deciding what to share and who to share it with. My concerns is about individuals’ concerns about information security and if and how their information will be acted upon. Potential impact shows the potential benefits of a PHR such as increasing self-efficacy, uptake of health-protective behaviours, and professionals taking a more holistic approach to providing care and facilitating behaviour change. Conclusions: Our research shows the functionality that a PHR requires in order for people to engage with it. Interactive functions and integration with lifestyle and health apps are particularly important. A PHR could increase the effectiveness of behaviour change apps by specifying evidence-based behaviour change techniques that apps should incorporate. A PHR has the potential to increase health-protective behaviours and facilitate a more person-driven health and social care system. It could support patients to take responsibility for self-managing their health and treatment regimens, as well as helping patients to play a more active role when care transfers across boundaries of responsibility. / Leeds Informatics Board

A Proposed Ground Motion Selection And Scaling Procedure For Structural Systems

This study presents a ground-motion selection and scaling procedure that preserves the inherent uncertainty in the modified recordings. The proposed procedure provides a set of scaled ground-motion records to be used in the response estimation of structural systems for a pre-defined earthquake hazard level. Given a relatively larger ground-motion dataset, the methodology constrains the selection and scaling of the accelerograms to the differences between individual records and corresponding estimations from a representative ground-motion predictive model. The procedure precisely calculates the distribution parameters of linear structural systems whereas it provides estimations of these parameters for nonlinear structural response. Thus this method is not only useful for ground-motion selection and scaling but also for probability based performance assessment studies. The proposed procedure is also capable of matching with a pre-defined target elastic response spectrum and corresponding variance over a period range. Case studies that compare the performance of the proposed procedure with some other record selection and scaling methods suggest its usefulness for the accurate verification of structural systems and rapid loss estimation studies.

This paper deals with medical data extraction from electronic patient record (EPR) system. Most of the medical data are stored in patient record systems, and data that are much valuable for medical research. If a researcher wants to extract medical information today, it has to be done manually because the data are stored in unstructured textual format in a system created by hospital staff. There is no way of extracting data in structure way. This paper is going to introduce an information extraction application for EPR system that allows the researcher to set up a study with inclusion and parameters for extraction for retrospective surveys in a webuser-interface environment. Inclusion is what the researcher would like to study (a defined category or criteria) and parameters specify the characteristics of inclusion the criteria. Result of this application provides an extracted clinical data that is used for retrospective surveys, downloadable to an MS-Excel file.

Records of rights and other data in the Land Registry should be considered as very actual topic because of the new Act No. 89/2012 Sb., Civil Code. This Act contrary to the previous legislation brought out a lot of changes, which had to be integrated in to the legislation of the Land Registry. Changes consits from different definition of the immovable thing, and from raising of various rights newly registered in the Land Registry including rights with the character of the obligation. Major change is in the reinforcement of the principle of publicity material of the Land Registry, which make it possible to acquire the right of ownership and other rights in rem from somebody, who is not really an owner, if the conditions of the law are met. With the reinforcement of the principle of publicity material the Act No. 89/2012 Sb., Civil Code, brought out also tools for real owners and other entitled persons from others rights in rem to defend themselves before the principle of publicity material, which was also necessary to project in to legislation of the Land Registry. After numerous review and modification cycles, the document is printed and distributed, often through intermediaries. Finally, it reaches the hands and eyes of perhaps thousands. This contrasts dramatically with the Internet where, within minutes of completion, text can be seen by millions. The Internet offers enormous research power. With a PC and a phone line, one can locate a recipe for delicious meringue or deadly ricin; can research a thesis or the step-by-step fabrication of a thermonuclear device. Recognizing the potential for misuse as well as for informing the public, the Department of Defense charged each of its agencies with the responsibility of policing content and form of that agency's publicly accessible websites. As the United States Navy command responsible for this daunting assignment, FIWC faces a job that grows in complexity and size by the day. Taking on this problem manually would result, at best, in unitary growth of dedicated resources and a similar increase in potential for error, both of oversight and of inappropriate action. This thesis provides one approach to automating FIWC's website monitoring and enforcement activities. The approach it advocates is focused on reducing manpower and increasing accuracy. Thesis advisor: James Hamm / In this thesis I discuss the Cuban American National Foundation (CANF) and its unique success as an ethnic interest group. I explain how the group came to be. I then address a number of reasons that scholars give as to why the CANF still exerts so much influence today. These reasons include but are not limited to: government favoritism during the cold war, the group's influence as a voting bloc, and campaign donations to prominent politicians. I discuss each of these theories and conclude that they all contain a grain of truth, yet none of them is significant enough to fully explain the situation.
