The future role of the health record administrator : a Delphi-survey

The voluntary North American accreditation movement, born at the beginning of this century with the intent to improve the standards of medical care and teaching, decreed that the clinical records should reflect the care given to the patients. As a consequence, the occupation of Medical Record Librarian, recently renamed Health Record Administrator, has grown rapidly over the past few decades. In Canada, this growth was more numerical than substantive, and the types and quality of health record administration services—discussed later, did not meet the needs of the health care system. Within the occupation, there is serious concern about its continued viability. For these various reasons, the adaptability of the health record administrator to the scientific, technological and social changes taking place in the health field is investigated here. To study the question, the health record administrator occupation was examined in the context of the changing status of health information within the Canadian health care delivery system and the jostling professionalization of the health occupations. These two major forces are believed to have great impact upon the health record administrator occupation in its quest for survival. The Delphi-method developed by the Rand Corporation was used to elicit the predictions of some members of the health occupations as to the potential development or regression of the role of the Health Record Administrator in the future. The findings of this study show that the health occupations, and particularly the medical profession have acknowledged needs for health information and health information management services. They have forecast a strong administrative role for the future Health Record Administrator, while giving equal importance to a participative role as collaborator providing the health occupations with the information services that they require for the performance of their own duties. Use of XRunner for Automation

International Telemetering Conference Proceedings / October 28-31, 1996 / Town and Country Hotel and Convention Center, San Diego, California / XRunner, a UNIX, client-server based, automated record-replay test tool developed by Mercury Interactive Corporation, was used by a project at NASA's Goddard Space Flight Center to automate intensive GUI/window-driven satellite operations functions. This work was part of the Integrated Monitoring, Analysis and Control COTS System (IMACCS), a COTS integration prototype development effort. XRunner, running in its object-oriented context sensitive mode, recorded the window/push-button images of significant events in spacecraft operations with sequential steps and generated test script language (TSL) for subsequent replay or test verification. The recorded TSL was optimized such that scenario replay timing, sequencing, grouping, and UNIX interactions became simple, easily-automated tasks instead of manually intensive, error-prone operations. This feature of the XRunner tool is expected to greatly enhance operations and testing.

Signals of astronomical climate forcing in the exposure topography of the North Polar Layered Deposits of Mars

Using high-resolution topography, we link the stratigraphy of layered ice deposits at the north pole of Mars to astronomically driven climate variability. Observations of trough exposures within these deposits are used to construct virtual ice cores at 16 sites, to which we apply wavelet analysis to identify periodicities in layer properties. To confidently relate these periodicities to climatic forcing, we identify overlapping dominant stratigraphic wavelengths and compare their ratios to that of the two dominant modes of insolation variability. The average ratio of stratigraphic wavelengths in the profiles is 1.90.1, lower than the ratio of 2.3 between dominant insolation periodicities. A similar analysis of synthetic stratigraphic profiles created with a climate-driven model of ice and dust accumulation shows that this lower stratigraphic ratio is a natural consequence of time-variable ice accumulation rates.

The author examined the errors by content, frequency, type and MARC field location to study the effect on both copy cataloging activities and library patrons’ access to an online catalog record. Finally, a number of recommendations are proposed for future research and error decreasing measures.

In the literature study section, EHR background, development history and service condition are introduced. The paper focuses on the sharing of medical information in different users, data safety and privacy. The adjunctions of computer science, technologies are used to solve the medical informatics’ problems. In the implementation section, based on the study of the current EHR systems, the design and implement of a shared EHR system are presented, which can be accessed by different doctors and patients. Access control function and cryptography protections are included in this system. The system test and evaluation are also given.

Exploring record-keeping and the use of micros in the nursery school and class

The investigations which follow are exploratory in the sense that the topic is new and not that the methodology itself is innovatory. The new research topic relates to innovation in both in-service education and nursery practice with the starting point being that micro-based systems of database management and word-processing may contribute to nursery record-keeping in the 1990's. Such a micro-based system was introduced as part of an experimental intervention with a sample of classrooms. This could be seen as pre-feasibility work towards the development of an "expert system" to help nursery staff with future curriculum development, implementation, and record-keeping. The research question is: how might nursery record-keeping develop in the future, and in what conditions, and with what support? Factors associated with most successful outcomes in the experimental intervention were small nurseries, attendance of a full age-range of children, good quality curriculum, clearly differentiated staff roles, and prior record-keeping. (All classrooms were able to complete many procedures and were to some extent "successful".) A comparison was made of nursery records developed, by staff, during the micro experiment with those obtained from a national survey: there was a difference in that the micro-based records were more detailed. Analysis of individual participant responses also indicated that change in record-keeping practice took place during the intervention. A survey of the views of the experimental participants and a comparison group of nursery staff found that both groups expressed a willingness to work outside set hours to aid the introduction of micro-based record-keeping. However, the experimental group had given more thought to uses for micros in their classrooms. When the responses of nursery assistants and teachers were analysed separately, assistants were found to be less in favour of record-keeping and using micros in nurseries. Investigations, in the study as a whole, encompass a survey of under fives record-keeping throughout Britain and a detailed study of the keeping of official records in two contrasting LEA's. There are also critical reviews of relevant literature, the methods used for the investigations, and a final discussion of key themes.

A Multichannel Single Fiber EMG Recording System

A multichannel single fiber EMG recording system has been developed and tested. This system is based on a multichannel needle electrode which has the ability to record EMG signals from multiple sites within the muscle under study without reinsertion. Therefore, not only is the recording volume of the electrode. increased but also enhancement of the single channel electrodes is possible through spatial filtering. The recording system, currently implemented on a PDP-11/34 computer, amplifies, filters and digitally stores the multichannel EMG signals for further signal processing. Signal processing techniques were developed to detect and extract motor unit action potentials from the EMG signal. Due to the large amount of data recorded by the multichannel electrode, slower sampling rates for digitizing the action potentials were required. Even with near Nyquist sampled signals the accuracy of this technique was shown to be nearly equivalent to conventional extraction of action potential features samples at four times the Nyquist rate. Finally, the problem of superpositioning of action potentials was addressed. By modifying the interpolation routine an algorithm for resolving superpositions was developed. / Thesis / Master of Engineering (ME)

BXE2E: a bidirectional transformation approach for medical record exchange

Modern health care systems are information dense and increasingly relying on computer-based information systems. Regrettably, many of these information systems behave only as an information repository, and the interoperability between different systems remains a challenge even with decades of investment in health information exchange standards. Medical records are complex data models and developing medical data import / export functions a is difficult, prone to error and hard to maintain process. Bidirectional transformations (bx) theories have been developed within the last decade in the fields of software engineering, programming languages and databases as a mechanism for relating different data models and keeping them consistent with each other. Current bx theories and tools have been applied to hand-picked, small-size problems outside of the health care sector. However, we believe that medical record exchange is a promising industrial application case for applying bx theories and may resolve some of the interoperability challenges in this domain. We introduce BXE2E, a proof-of-concept framework which frames the medical record interoperability challenge as a bx problem and provides a real world application of bx theories. During our experiments, BXE2E was able to reliably import / export medical records correctly and with reasonable performance. By applying bx theories to the medical document exchange problem, we are able to demonstrate a method of reducing the difficulty of creating and maintaining such a system as well as reducing the number of errors that may result.
