Associações entre dieta, composição corporal e densidade mineral óssea em mulheres na pós-menopausa

A menopausa é definida como a ausência permanente das menstruações, em decorrência da diminuição da função folicular ovariana ou remoção cirúrgica dos ovários. O declínio da produção endógena de estrogênio durante a transição menopáusica tem sido associado à perda de densidade mineral óssea (DMO) e de massa muscular esquelética. Além disso, não só o status menopáusico, mas também o envelhecimento promove outras modificações na composição corporal, como o aumento e redistribuição da massa de gordura, resultando em um padrão de acumulação de gordura central ou de distribuição androide. Essas alterações de composição corporal levam a efeitos deletérios na saúde dessas mulheres, como aumento do risco de doenças cardiovasculares, fraturas e mortalidade. Estudos de intervenção na população em geral tem demonstrado que dietas hiperproteicas em comparação com dietas de baixo teor de proteínas levam a maior perda de circunferência de cintura e gordura abdominal durante o seu seguimento. Ainda não foram publicadas revisões sobre o efeito de dietas hiperproteicas sobre gordura abdominal ou adiposidade central em mulheres de meia-idade. Portanto, o objetivo da introdução, sendo o capítulo 1 desta tese, foi conduzir uma revisão sistemática de ensaios clínicos randomizados para identificar os efeitos de dietas hiperproteicas comparadas a dietas controles sobre gordura abdominal em mulheres de meia-idade. Devido ao número limitado de evidências identificadas, não foi possível a realização de metanálise. Não foram encontradas diferenças na redução da circunferência da cintura e da massa de gordura abdominal entre as dietas para a maioria dos estudos. No entanto, dietas com objetivo de redução de peso corporal, independentemente da composição nutricional podem diminuir a massa de gordura abdominal em mulheres de meia-idade. As recomendações de ingestão (DRI) de proteína para idosos, homens e mulheres, continuam sendo as mesmas de adultos jovens saudáveis. A estimativa média de requerimento (EAR) e a recomendação diária de ingestão (RDA) são de 0,66 e 0,8 g/kg de peso corporal, respectivamente, sendo que a RDA teoricamente satisfaz as necessidades de 97,5 % da população. No entanto, em dois estudos recentes que avaliaram as recomendações de proteína para mulheres com mais de 65 e 80 anos de idade, encontraram uma EAR de 0,85 e 0,96 e uma RDA de 1,15 g e 1,29 g/kg de peso corporal, respectivamente. Dessa forma, o objetivo do segundo estudo foi investigar a associação entre massa muscular esquelética e ingestão de proteína, atividade física habitual, composição corporal e variáveis metabólicas em mulheres na pós-menopausa, encaminhadas através de chamamento na mídia e atendidas pelo grupo de endocrinologia ginecológica do Hospital de Clínicas de Porto Alegre. A prevalência de baixa massa muscular foi de apenas 7%, visto que nossas mulheres eram jovens e sem doenças clínicas evidentes. O índice de massa muscular foi positivamente associado com ingestão de proteína e negativamente associado com % gordura corporal. As participantes com ingestão proteica no tercil superior apresentaram melhor perfil metabólico e padrão alimentar mais saudável. Este grupo também apresentou menor IMC, circunferência da cintura e massa de gordura do tronco. O declínio nos níveis de 17-β-estradiol está associado com o aumento na perda de DMO. Esse declínio de DMO também pode ser atribuído a diversos outros fatores, incluindo: idade, carga genética, nutrição, estilo de vida e uso prolongado de algumas medicações. Portanto, o objetivo do terceiro trabalho foi investigar como a composição corporal, padrão alimentar e atividade física habitual são associados com DMO de acordo com o tempo de menopausa em mulheres do sul do Brasil, sem doença clínica evidente. Nesse estudo transversal, o tempo de menopausa, menor massa magra e de gordura, além de ingestão de vitamina A < 700 μg/dia foram associados com baixa massa óssea. Ingestão calórica e de macronutrientes, bem como atividade física habitual, não interferiram na DMO, no entanto a maior parte das participantes eram sedentárias. / Menopause is defined as the permanent absence of periods, due to the decline in ovarian follicular function or ovaries surgical removal. The decline in endogenous estrogen production during the menopausal transition has been associated with loss of bone mineral density (BMD) and skeletal muscle mass. Furthermore, not only the menopausal status, but also aging promotes other changes in body composition, resulting in increases visceral fat mass. These body composition changes have potential impact on health status in women, increasing the risk of cardiovascular disease, fracture and mortality. Intervention studies in the general population have provided evidence that high protein (HP) compared with low protein diets improve loss of waist circumference and abdominal fat during follow-up. The Nurses' Health Study observed a 26% reduction in the risk of cardiovascular disease in the group of women with higher protein intake compared to the lowest intake group. Specifically in women, there are no reviews on the effect of HP diets on abdominal obesity. Therefore, the aim of introduction and Chapter 1 of this thesis was to conduct a systematic review of randomized clinical trials to identify the effects of HP diets compared to control diets on abdominal obesity in middle-aged women. The limited evidence identified in this systematic review does not show a HP diet conferring benefit over control diet in middle-aged women. In conclusion, a diet aimed at reducing weight improved abdominal fat in middle-aged women regardless of the diet composition. The dietary recommend intake (DRI) of protein for the elderly, men and women, are still the same for healthy young adults. The estimated average requirement (EAR) and the recommended daily intake (RDA) are 0.66 and 0.8 g/ kg of body weight, respectively, and the RDA theoretically meets the needs of 97.5% of the population. However, two recent studies evaluating the protein recommendations for women aged 65 to 80, found a RDA of 0.85 and 0.96 and a EAR of 1.15 and 1.29 g / kg of body weight, respectively. Thus, the purpose of the second study was to investigate the associations between skeletal muscle mass and protein intake, habitual physical activity, body composition and metabolic variables in postmenopausal women, attended by gynecological endocrinology group from a Porto Alegre University Hospital. Probably because of high mean protein intake, along with the mean age (55.2 ± 4.9 years) of our postmenopausal women, the prevalence of low muscle mass in our sample was only 7%. The body mass index was positively associated with protein intake and negatively associated with percentage body fat. Participants with protein intake in the highest tertile had a better metabolic profile and healthier eating pattern. This group also had lower BMI, waist and trunk fat mass. Falling levels of 17-β-estradiol are thought to accelerate the decline in BMD, which remains the single best predictor of primary osteoporotic fracture. This decline can also be attributed to a number of factors: age, genetics, nutrition, lifestyle factors, or the prolonged use of certain medication. Therefore, the aim of the third study was to investigate whether body composition, dietary pattern and habitual physical activity are associated with BMD according to time since menopause in women from Southern Brazil with no clinical evidence of disease. In this cross-sectional study with postmenopausal women with no clinical evidence of disease, time since menopause, low lean and fat mass were associated with low bone mass. Calories and macronutrients intake as well as habitual physical activity did not interfere with BMD, but participants were mostly sedentary.

This research aimed to identify the factors related to inadequate practice of Pap smear for women examination on a city in the interior of Ceará. Thus, a cross-sectional study was conducted using a quantitative approach. The collection was from June to October 2013 in the health district V of Juazeiro do Norte-CE. The sample was calculated using the formula for finite populations, totaling a value of 240 women who were included based on the following criteria: age between 20 and 59 years; have initiated sexual life and accepting participate. Excluded were those who were not in full physical or mental condition and were not at the clinic at the time of data collection. Was used as an instrument of data collection, the household survey-type Knowledge, Attitude, Practice, and subsequently applied by the researcher classified according to criteria of adequacy. Data were stored and analyzed using STATA software, version 12.0. The data showed that all respondents reported having heard of the Pap smear, but in 72.9% of the knowledge was assessed as inappropriate. Improper attitude also showed high percentage (73.3%). The practice was classified as inadequate in 39.2% of the sample, in which 13.3% reported never having undergone the examination, and 23.6% have done it for over three years. After the adjusted Odds Ratio test, the following variables showed statistical significance for appropriate practice: age between 20 and 29 years (OR (CI) = 2.25), single marital status (OR (CI) = 3.18) and inadequate knowledge (OR (CI) = 2.90). The difficulties encountered in the realization of the Pap smear in the health unit, the variables of higher percentages for inadequate practice were lack of material (68.1%), shame the professional (27.6%) and not like the professional who performs the examination (20.8%). It was realized from this research, the importance of clarifying the population and effective communication among professionals of the teams of the Family Health Strategy about the issues related to cervical cancer and its prevention, as well as warranty and support continuity of care by managers. / No Brasil, o exame citológico de Papanicolaou é a principal estratégia recomendada para a prevenção do câncer de colo do útero (CCU), orientado para as mulheres de 25 a 64 anos, realizado a cada três anos, após dois exames normais consecutivos realizados com um intervalo de um ano. Esta pesquisa objetivou identificar os fatores relacionados com a prática inadequada do exame Papanicolaou pelas mulheres em um município do interior do Ceará. Para tanto, foi realizado um estudo transversal com abordagem quantitativa. A coleta foi no período de junho à outubro de 2013 no distrito sanitário V do município de Juazeiro do Norte-CE. A amostra foi calculada através da fórmula para populações finitas, perfazendo um valor de 240 mulheres, as quais foram inclusas a partir dos seguintes critérios: ter idade compreendida entre 20 e 59 anos; ter iniciado vida sexual e aceitar participar da pesquisa. Eram excluídas aquelas que não estavam em plena condição física ou mental e não estavam na unidade de saúde no momento da coleta de dados. Utilizou-se como instrumento de coleta de dados, o inquérito domiciliar do tipo Conhecimento, Atitude e Prática, aplicado e posteriormente classificado pela pesquisadora segundo critérios de adequação. Os dados obtidos foram armazenados e analisados pelo software STATA, versão 12.0. Os dados evidenciaram que todas as entrevistadas informaram ter ouvido falar do exame Papanicolaou, mas em 72,9% destas o conhecimento foi avaliado como inadequado. A atitude inadequada também apresentou percentuais elevados (73,3%). A prática foi classificada como inadequada em 39,2% da amostra, na qual 13,3% afirmaram nunca ter se submetido ao exame, e 23,6% o realizaram há mais de três anos. Após o teste de Odds Ratio ajustado, as seguintes variáveis revelaram relevância estatística para a prática inadequada: idade entre 20 e 29 anos (OR(IC)=2.25), estado civil solteira (OR(IC)=3.18) e conhecimento inadequado (OR(IC)=2.90). As dificuldades encontradas para a realização do exame Papanicolaou na unidade de saúde, as variáveis de maiores percentuais para a prática inadequada foram: falta de material (68,1%), vergonha do profissional (27,6%) e não gostar do profissional que realiza o exame (20,8%). Percebeu-se a partir desta pesquisa, a importância do esclarecimento à população e a comunicação efetiva por parte dos profissionais das equipes da Estratégia Saúde da Família acerca da problemática relacionada ao CCU e sua prevenção, assim como a garantia e apoio à continuidade do cuidado por parte dos gestores.

The simple pronunciation of the word cancer, already generates in the feminine population feeling of fear, sadness, anxiety and negation, which had to the raised index of morbid-mortality and also of decurrent mutilation of the illness, that always compromises the auto-esteem, the social interaction, the affective relationships and the professional life of who it is under attack. It enters the familiar ones of the patients with cancer, is not different. The feelings and reactions generated ahead of the confirmation of if having a relative with mammary neoplasms, cause a series of feelings that many times modify all the familiar dynamics. The doubts, questionings, changes of papers and alterations in the routine of the family, are frequent and many times, the psychological unpreparedness to deal with the new condition of the dear person, bring doubts and frustrations. Therefore, to know of that way these families deal with the fact of if having a with mammary neoplasms, it was the fidget that motivated the accomplishment of this study, that had as objectives to discovering the meanings that the families of mastectomized women construct and that they guide its action when taking care of it and to present a theoretical model concerning the care given for the families to the mastectomized woman. One is about a descriptive study with qualitative boarding that it used as review theoretician the Symbolic Interactionism and the Theory Based on data Grounded Teory as frameworks metodological. The collection of data happened in the month of July of 2008, in an institution of reference in the treatment of the cancer. We use as strategy for the collection of data, the interview with eleven family caregivers ones of mastectomized women. We got then, the data that after the analysis, had allowed the creation of the central theory: Meaning of the familiary care to the mastectomized woman, which consists of distinct phenomena that if across: Understanding of the family on the mastectomy, that it is characterized disclosing as the family if it feels ahead of a family caregivers one with cancer and when taking care of a mastectomized woman; The difficulties of the family to take care of, whose purpose is to demonstrate the occured changes in the family, are they, financial or in the modified familiar social paper; Understanding the diagnosis of cancer in the family, where it is perceived dimension of the impact of the diagnosis of the breast cancer, reaching not only the woman, but all the familial sinus and the familiar care after the surgery, that emphasizes as woman and family searchs to surpass the limitations while people interacting in the familiar group under attack by the breast cancer. In our opinion, the care in family requires the true and affectionate envolvement, to become involved themselves and to believe that its presence is so important how much the accomplishment of procedures technician. As we stand out, nor always the objective knowledge technician function so well ahead of situations of stress as the subjective knowledge that if disclose in the familiar dynamics. / O câncer de mama costuma ser motivo de receio entre as mulheres e para a sociedade, em geral. A simples pronúncia da palavra câncer gera na população feminina sentimentos de medo, tristeza, ansiedade e negação, devido ao elevado índice de morbimortalidade e também de mutilação decorrente da doença, que sempre compromete a auto-estima, a interação social, os relacionamentos afetivos e a vida profissional de quem é acometido. Portanto, conhecer de que modo lidam com o fato de terem um familiar com neoplasia mamária, foi a inquietação que motivou a realização deste estudo, que teve como objetivos desvelar os significados que as famílias de mulheres mastectomizadas constroem e que orientam suas ações ao cuidar delas e apresentar um modelo teórico acerca do cuidado prestado pelas famílias à mulher mastectomizada. Trata-se de um estudo descritivo com abordagem qualitativa que utilizou como referencial teórico o Interacionismo Simbólico e a Teoria Fundamentada em dados Grounded Teory como referencial metodológico. A coleta de dados aconteceu no mês de julho de 2008, em uma instituição de referência no tratamento do câncer. Utilizamos como estratégia para a coleta de dados, a entrevista com onze familiares de mulheres mastectomizadas. Obtivemos, então, os dados que após a análise, permitiram a criação da teoria central: Significado do cuidado familiar à mulher mastectomizada, a qual constitui-se de fenômenos distintos que se inter-relacionam: Compreensão da família sobre a mastectomia, que se caracteriza revelando como a família se sente diante de um familiar com câncer e ao cuidar de uma mulher mastectomizada; as dificuldades da família para cuidar, cuja finalidade é demonstrar as mudanças ocorridas na família, sejam elas, financeiras ou no papel social familiar modificado; compreendendo o diagnóstico de câncer na família, em que se percebe a dimensão do impacto do diagnóstico do câncer de mama, atingindo não somente a mulher, mas todo o seio familiar e o cuidado familiar após a cirurgia, que enfatiza como mulher e família buscam superar as limitações enquanto pessoas interagindo no grupo familiar acometido pelo câncer de mama. Em nossa opinião, o cuidado em família requer o envolvimento verdadeiro e afetuoso, envolver-se e acreditar que sua presença é tão importante quanto à realização de procedimentos técnicos. Como ressaltamos, nem sempre os conhecimentos técnicos objetivos funcionam tão bem diante de situações de estresse como os conhecimentos subjetivos que se revelam na dinâmica familiar.

Probably because of high mean protein intake, along with the mean age (55.2 ± 4.9 years) of our postmenopausal women, the prevalence of low muscle mass in our sample was only 7%. The body mass index was positively associated with protein intake and negatively associated with percentage body fat. Participants with protein intake in the highest tertile had a better metabolic profile and healthier eating pattern. This group also had lower BMI, waist and trunk fat mass. Falling levels of 17-β-estradiol are thought to accelerate the decline in BMD, which remains the single best predictor of primary osteoporotic fracture. This decline can also be attributed to a number of factors: age, genetics, nutrition, lifestyle factors, or the prolonged use of certain medication. Therefore, the aim of the third study was to investigate whether body composition, dietary pattern and habitual physical activity are associated with BMD according to time since menopause in women from Southern Brazil with no clinical evidence of disease. In this cross-sectional study with postmenopausal women with no clinical evidence of disease, time since menopause, low lean and fat mass were associated with low bone mass. Calories and macronutrients intake as well as habitual physical activity did not interfere with BMD, but participants were mostly sedentary.

Associações entre dieta, composição corporal e densidade mineral óssea em mulheres na pós-menopausa

A menopausa é definida como a ausência permanente das menstruações, em decorrência da diminuição da função folicular ovariana ou remoção cirúrgica dos ovários. O declínio da produção endógena de estrogênio durante a transição menopáusica tem sido associado à perda de densidade mineral óssea (DMO) e de massa muscular esquelética. Além disso, não só o status menopáusico, mas também o envelhecimento promove outras modificações na composição corporal, como o aumento e redistribuição da massa de gordura, resultando em um padrão de acumulação de gordura central ou de distribuição androide. Essas alterações de composição corporal levam a efeitos deletérios na saúde dessas mulheres, como aumento do risco de doenças cardiovasculares, fraturas e mortalidade. Estudos de intervenção na população em geral tem demonstrado que dietas hiperproteicas em comparação com dietas de baixo teor de proteínas levam a maior perda de circunferência de cintura e gordura abdominal durante o seu seguimento. Ainda não foram publicadas revisões sobre o efeito de dietas hiperproteicas sobre gordura abdominal ou adiposidade central em mulheres de meia-idade. Portanto, o objetivo da introdução, sendo o capítulo 1 desta tese, foi conduzir uma revisão sistemática de ensaios clínicos randomizados para identificar os efeitos de dietas hiperproteicas comparadas a dietas controles sobre gordura abdominal em mulheres de meia-idade. Devido ao número limitado de evidências identificadas, não foi possível a realização de metanálise. Não foram encontradas diferenças na redução da circunferência da cintura e da massa de gordura abdominal entre as dietas para a maioria dos estudos. No entanto, dietas com objetivo de redução de peso corporal, independentemente da composição nutricional podem diminuir a massa de gordura abdominal em mulheres de meia-idade. As recomendações de ingestão (DRI) de proteína para idosos, homens e mulheres, continuam sendo as mesmas de adultos jovens saudáveis. A estimativa média de requerimento (EAR) e a recomendação diária de ingestão (RDA) são de 0,66 e 0,8 g/kg de peso corporal, respectivamente, sendo que a RDA teoricamente satisfaz as necessidades de 97,5 % da população. No entanto, em dois estudos recentes que avaliaram as recomendações de proteína para mulheres com mais de 65 e 80 anos de idade, encontraram uma EAR de 0,85 e 0,96 e uma RDA de 1,15 g e 1,29 g/kg de peso corporal, respectivamente. Dessa forma, o objetivo do segundo estudo foi investigar a associação entre massa muscular esquelética e ingestão de proteína, atividade física habitual, composição corporal e variáveis metabólicas em mulheres na pós-menopausa, encaminhadas através de chamamento na mídia e atendidas pelo grupo de endocrinologia ginecológica do Hospital de Clínicas de Porto Alegre. A prevalência de baixa massa muscular foi de apenas 7%, visto que nossas mulheres eram jovens e sem doenças clínicas evidentes. O índice de massa muscular foi positivamente associado com ingestão de proteína e negativamente associado com % gordura corporal. As participantes com ingestão proteica no tercil superior apresentaram melhor perfil metabólico e padrão alimentar mais saudável. Este grupo também apresentou menor IMC, circunferência da cintura e massa de gordura do tronco. O declínio nos níveis de 17-β-estradiol está associado com o aumento na perda de DMO. Esse declínio de DMO também pode ser atribuído a diversos outros fatores, incluindo: idade, carga genética, nutrição, estilo de vida e uso prolongado de algumas medicações. Portanto, o objetivo do terceiro trabalho foi investigar como a composição corporal, padrão alimentar e atividade física habitual são associados com DMO de acordo com o tempo de menopausa em mulheres do sul do Brasil, sem doença clínica evidente. Nesse estudo transversal, o tempo de menopausa, menor massa magra e de gordura, além de ingestão de vitamina A < 700 μg/dia foram associados com baixa massa óssea. Ingestão calórica e de macronutrientes, bem como atividade física habitual, não interferiram na DMO, no entanto a maior parte das participantes eram sedentárias. / Menopause is defined as the permanent absence of periods, due to the decline in ovarian follicular function or ovaries surgical removal. The decline in endogenous estrogen production during the menopausal transition has been associated with loss of bone mineral density (BMD) and skeletal muscle mass. Furthermore, not only the menopausal status, but also aging promotes other changes in body composition, resulting in increases visceral fat mass. These body composition changes have potential impact on health status in women, increasing the risk of cardiovascular disease, fracture and mortality. Intervention studies in the general population have provided evidence that high protein (HP) compared with low protein diets improve loss of waist circumference and abdominal fat during follow-up. The Nurses' Health Study observed a 26% reduction in the risk of cardiovascular disease in the group of women with higher protein intake compared to the lowest intake group. Specifically in women, there are no reviews on the effect of HP diets on abdominal obesity. Therefore, the aim of introduction and Chapter 1 of this thesis was to conduct a systematic review of randomized clinical trials to identify the effects of HP diets compared to control diets on abdominal obesity in middle-aged women. The limited evidence identified in this systematic review does not show a HP diet conferring benefit over control diet in middle-aged women. In conclusion, a diet aimed at reducing weight improved abdominal fat in middle-aged women regardless of the diet composition. The dietary recommend intake (DRI) of protein for the elderly, men and women, are still the same for healthy young adults. The estimated average requirement (EAR) and the recommended daily intake (RDA) are 0.66 and 0.8 g/ kg of body weight, respectively, and the RDA theoretically meets the needs of 97.5% of the population. However, two recent studies evaluating the protein recommendations for women aged 65 to 80, found a RDA of 0.85 and 0.96 and a EAR of 1.15 and 1.29 g / kg of body weight, respectively. Adesão da mulher hipertensa ao estilo de vida saudável : uma tecnologia em saúde

Although there is much evidence regarding the general behaviors that make up the healthy lifestyle, there are not only one but several types of healthy lifestyles that change according to the context in which the individual is inserted and their specific needs. Participant research with the objective of analyzing the adherence of hypertensive women to the healthy lifestyle with the application of a health technology based on the Theory of Change performed with fourteen hypertensive women embased in the Basic Health Unit in Eusébio-CE, during the period from july to september 2016. Data were collected through interviews. The women stood out in the age group of 37 to 59 years (twelve), brown (ten), married (six), evangelical religion (eight), Eusébio-CE naturalness (thirteen), without individual monthly income (thirteen) (R$ 880.00) (nine), occupancy of household activities (twelve), and elementary school (eleven). As to previous knowledge about healthy lifestyle behaviors, the following stand out: predominance of white meat and vegetables; abstention from tobacco; stress management; regular use of the medicine; regular physical exercise; adequate intake of coffee; proper use of salt; and use of dietary sweetener. In relation to the adherence at the conducts of control of arterial hypertension (AH), there was a predominance of adequate use of salt and vegetable fat, abstention of alcohol and tobacco, adequate intake of coffee and predominance of white meat. With the application of Health Technology (HT), it enabled the adherence of women to the conducts of control of AH. After one month of HT application, we verified the continuity of the change process in the majority of the group, with the evolution to the Maintenance Stage. At this stage the participants had already adhered to healthy lifestyle behaviors, and efforts were focused on maintaining the changes achieved and preventing relapse. Therefore, with the application of HT, we observed an increase in the knowledge about the AH and its treatment, and the evolution of the participants in the Stages of change. Thus, HT was effective in promoting changes in inappropriate life habits, in order to prevent AH. / Estilo de vida pode ser conceituado como um conjunto de decisões e atitudes individuais, passíveis de algum nível de controle, que exercem influência sobre a saúde. Embora existam muitas evidências a respeito das condutas gerais que compõem o estilo de vida saudável, não existe apenas um, mas vários tipos de estilos de vida salubres, que se modificam de acordo com o contexto em que o indivíduo se encontra inserido e suas necessidades específicas. Pesquisa participante com o objetivo de analisar a adesão de mulheres hipertensas ao estilo de vida saudável com a aplicação de uma tecnologia em saúde embasada na Teoria Estágio de Mudanças, realizada com catorze mulheres hipertensas inscritas na Unidade Básica de Saúde em Eusébio-CE, no período de julho a setembro de 2016. Os dados foram coletados por meio de entrevistas. As mulheres se destacaram na faixa etária de 37 a 59 anos (doze), cor parda (dez), casadas (seis), religião evangélica (oito), naturalidade Eusébio-CE (treze), sem renda mensal individual (treze), renda familiar mensal inferior a um salário mínimo vigente (R$ 880,00) (nove), ocupação de atividades do lar (doze), e ensino fundamental (onze). Quanto ao conhecimento prévio acerca das condutas de estilo de vida saudável, destacaram-se: predomínio de carnes brancas e de vegetais; abstenção do tabaco; gerenciamento do estresse; uso regular do medicamento; exercício físico regular; ingesta adequada de café; uso adequado do sal; e uso de adoçante dietético. Em relação à adesão às condutas de controle da hipertensão arterial sistêmica (HAS), sobressaíram-se: predomínio do uso adequado do sal, uso de gordura vegetal, abstenção do álcool, abstenção do tabaco, ingesta adequada de café e predomínio de carnes brancas. A aplicação da Tecnologia em Saúde (TS) possibilitou a adesão das mulheres às condutas de controle da HAS. Após um mês da aplicação da TS, verificamos a continuidade do processo de mudança na maioria do grupo, com a evolução para o Estágio de Manutenção. Nesta etapa, as participantes já haviam aderido as condutas de estilo de vida saudável, e os esforços estavam direcionados à manutenção das mudanças alcançadas e prevenção de recaídas. Logo, com a aplicação da TS, observamos incremento do conhecimento sobre a HAS e seu tratamento, e evolução das participantes nos Estágios de mudança. Assim, a TS se mostrou efetiva para promoção de mudanças de hábitos de vida inadequados, com vista à prevenção da HAS.

Proposta de tecnologia educativa na prevenção do risco da hipertensão na gravidez : uma construção coletiva

Since the medic-alization of childbirth in the eighteenth century, the policies related to maternal and child health carne intensifying to cover positive health outcomes, from conception to pregnancy and the postpartum period, which are physiological phenomena naturally expected in the development of the human species , but subject to unfavorable complications. In addition to being a tool for recording data during gravídicopuerperal cycle, the card Pregnant (CG) , added to an Educational Technology (ET) could act in promoting women's health through the prevention and / or control of the factors of risk of gestational hypertension (GR ) and other health problems. Upon the issue of maternal and perinatal morbidity and mortality associated with hypertensive syndrome, the need for educational interventions for the prevention and / or control of the HG risk factors , and the use of CG only as data logging instrument, it was decided to this study with the goal of building a proposal of educational technology (PTE ) in the prevention and / or control the risk of hypertension in pregnancy. Methodological study conducted in Primary Health Care Units of the Regional Executive Secretary VI , in Fortaleza, with 90 professionals of the Family Health Teams (EqSF) (68 nurses and 22 doctors) , who accompanied the APN from 2 years . There was a predominance of nurses (75.5 % ) , female ( 82.0 % ) , aged up to 39 years ( 72.2 % ) , academic training and professional practice time up to 9 years ( 52.2 % ) , acting in primary care and APN 02-07 years is ( 66.6 % ) . In relation to healthy eating, doctors fully agree with the pipes as a percentage change of 63.6 % to 100.0 %. However, the nurses expressed the same opinion in a range of 66.1 % to 98.0 %, except for the "use of dietary sweeteners" (35.2% ) . As for the "regular exercise", a concordance of physicians (100.0%) and 67 (98.5 %) nurses. The EqSF recommended the dissemination, implementation of PTE, including attached to the GC and the electronic medical record. The construction of PTE aroused in professional the power of creativity, expressiveness and especially persistence, through health education scenario, which is the focus of the changes that empower people in the quest for health promotion and quality of life. / A assistência pré - natal (APN) tem sido foco de investigação no Brasil e no mundo. Desde a medicalização do parto no século XVIII, as políticas relacionadas à saúde matemo-infantil vieram se intensificando para abranger desfechos positivos em saúde, desde a concepção até a gestação e o puerpério, que são fenômenos fisiológicos naturalmente esperados no desenvolvimento da espécie humana, mas sujeitos a intercorrências desfavoráveis. Além de ser um instrumento para o registro de dados durante o ciclo gravídicopuerperal, o Cartão da Gestante (CG), agregado a uma Tecnologia Educativa (TE) poderia atuar na promoção da saúde da mulher por meio da prevenção e/ou controle dos fatores de risco da hipertensão gestacional (HG) e de outros agravos. Mediante a problemática da morbimortalidade materna e perinatal associada à síndrome hipertensiva, à necessidade de intervenções educativas com vista à prevenção e/ou controle dos fatores de risco da HG, e à utilização do CG somente como instrumento de registro de informações, optou-se por este estudo com o objetivo de construir urna proposta de tecnologia educativa (PTE) na prevenção e/ou controle do risco da hipertensão na gravidez. Estudo metodológico, realizado nas Unidades de Atenção Primária à Saúde da Secretaria Executiva Regional VI, em Fortaleza-CE, com 90 profissionais das Equipes Saúde da Família (EqSF) (68 enfermeiros e 22 médicos), que acompanhavam o APN a partir de 2 anos. Houve a predominância de enfermeiros (75,5%), sexo feminino (82,0%), faixa etária de até 39 anos (72,2%), tempo de formação acadêmica e de exercício profissional até 9 anos (52,2%), de atuação na atenção básica e na APN de 02 a 07 anos é de (66,6%). Em relação à alimentação saudável, os médicos concordavam totalmente com as condutas em variação percentual de 63,6% a 100,0%. Todavia, os enfermeiros manifestaram a mesma opinião em uma faixa de 66,1% a 98,0%, excetuando o "uso de adoçantes dietéticos" (35,2%). Quanto ao "exercício físico regular", houve concordância dos médicos (100,0%) e 67 (98,5%) enfermeiros. A EqSF recomendou a divulgação, implantação da PTE, inclusive anexada ao CG e no prontuário eletrônico. A construção da PTE despertou nos profissionais o poder da criatividade, expressividade e sobretudo persistência, mediante o cenário da educação em saúde, que é o foco das transformações, que empoderam as pessoa na busca pela promoção da saúde e qualidade de vida.

O Objetivo central dessa pesquisa é analisar, pela ótica do docente o Ensino da Saúde da Mulher, em cursos de Enfermagem da cidade de São Paulo. Quanto ao referencial teórico adotado, privilegiou-se um diálogo com autores que discutiam a problemática do Ensino da Enfermagem e as especificidades da Saúde da Mulher no Brasil. A população desse estudo foi constituída por 17 docentes responsáveis e/ou coordenadores das disciplinas que abordam conteúdos relativos à Saúde da Mulher, tendo-se coletado os dados por meio da aplicação de um questionário. A análise e discussão dos resultados obtidos permitem destacar, nesta pesquisa, que a maior parte dos docentes são mestres e doutores, com diferentes experiências no campo da formação docente. No tocante às práticas pedagógicas, os docentes apontaram os laboratórios de enfermagem e os ambulatórios, porém, privilegiaram a sala de aula como espaço para o desenvolvimento de atividades. Nos diversos ambientes de aprendizagem, os professores utilizam a aula expositiva como uma estratégia fundamental, mas percebe-se a introdução de metodologias inovadoras no processo ensinoaprendizagem. No âmbito das práticas avaliativas, a prova escrita foi citada como o instrumento mais utilizado pelos docentes, sendo também referido o levantamento bibliográfico. As aproximações ao perfil dos docentes e a busca de um conhecimento mais amplo sobre o Ensino da Saúde da Mulher em Enfermagem, revelaram a necessidade de trabalhos acadêmicos que enfatizem o saber fazer e o saber aprender, valorizando as trocas de experiências e o estabelecimento de processos pedagógicos que abram novas possibilidades de aprendizagem. Construir uma investigação que privilegiou a ótica docente implicou, além do esforço em responder às questões orientadoras da pesquisa, no reconhecimento do lugar que o professor ocupa na formação em saúde, delineando perspectivas de uma ação pedagógica que seja produtiva, significativa e contribua para a formação de outros sujeitos, pois que participar da formação do outro é, também, continuar a se formar. / The great importance to extend knowledge on issues related to woman’s health begins not only as a need to find solutions for the women’s daily problems but also aiming to extend health education to undergraduating female students in the nursing area. The major purpose of this study is to analyze, from the professor’s point of view, Women’s Health Education in the nursing courses of the city of São Paulo. Regarding the adopted theoretical referential, the authors allowed a dialog where the problem of the nursing teaching and specificities of the women’s health in Brazil were discussed. The population of this study consisted of 17 academic professors in charge of and/or coordinators of the disciplines on women’s health related issues. Data was collected by means of a questionnaire. In this research, analysis and discussion of the obtained results showed that most of the faculty members had Master and Ph.D. levels, with diversified experiences in the academic area. Regarding pedagogical practices, the academic professors mentioned nursing laboratories and outclinics but the greatest emphasis was given to the classroom as a space to develop activities. In the several learning environments, professors used the expositive class format as a fundamental strategy but innovative methodologies were also introduced in the teachinglearning process. Regarding evaluation practices, the writing test was the instrument most used by the academic members followed by the bibliographic survey. The approach to the academic professor’s profile and the search for an extended knowledge on the women’s Health Education in Nursing revealed the need of further academic studies emphasizing how to do and how to learn, giving emphasis to the exchange of experiences and the establishment of pedagogical processes bringing new possibilities of learning. To build an investigation favoring the academic point of view also involves the attempt to answer the orienting questions of the research, as an acknowledgment for the professor’s role in the health education, delineating perspectives of a pedagogical action which could be productive, significant and that could contribute to bring education to other individuals since to take part in one’s education is also a continuing process of graduating. / BV UNIFESP: Teses e dissertações

It investigates how such considerations are elaborated and the way the public accept them, taking/adopting them as truth and as reference. It’s been used articles on the subjects health, beauty and nutrition of twenty four editions of the magazines “ Boa Forma” and “Corpo a Corponas” from August 2011 to July 2012. In order to achieve this goal a critical discourse analysis was undertaken aiming denaturalize constructions on health, beauty and youth, showing the complexity of an engine where the key is the production of a thin body to support the media and medical aesthetic speeches. Through this analysis was possible to observe the disguised machinery of power/knowledge that gets stronger working oiled by the consumer’s (body) desires. The idea of the body as built, the propagated idea of a body that worked out, of a body that eats only light food and that consumes goods and services that will make it be more and more synthetic , plastic, hygienic. A body without wrinkles to show time’s passing. Above all a body keeps inciting discipline and control, begging to work out and the use of technological procedures. Thus it’s among the “healthy” ways of life and amid production of an identification aesthetic through consumption that thin bodies as an ideal are justified. The conclusion is that what’s done to take care of the body and the health propaganda made in name of quality of life are announced by women’s magazines to the common consumers have impact on them in a subjective way making them want those things and identifying perfect images relating them to happiness and body perfection. In addition, there are elaborations of ways of behave and of relating to others that reinforce models based on bodies without wrinkles legitimating the constant confection of beauty and health production based on youth. / Nosso estudo buscou explorar e compreender as articulações entre os discursos midiático e médico estético acerca das construções corporais em revistas de beleza feminina. Interrogamos as formas como tais elaborações se constroem e são tomadas/adotadas como verdades e referencias para modos de viver e cuidar do corpo. Trabalhamos com as revistas Boa Forma e Corpo a Corpo nas várias matérias sobre saúde, beleza e nutrição, nas edições de agosto de 2011 a julho de 2012, no total de vinte e quatro fascículos. Lançamos mão da análise critica de discurso visando desnaturalizar construções sobre saúde, beleza e juventude, apontando a complexidade de uma engrenagem que se movimenta produzindo o corpo magro como peça central de sustentação da mídia e do discurso médico estético. Foi possível visualizar um maquinário sorrateiro de poder/saber que se solidifica lubrificado em suas partes pelo desejo dos (corpos) consumidores. O corpo confeccionado, difundido como trabalhado, consumidor de alimentos leves, de produtos e de serviços que o deixam mais e mais sintético, plastificado, higiênico, sem sinais ou marcas do tempo é, sobretudo, aquele que não cessa de incitar disciplina, controle, prática de exercícios e muitos procedimentos com tecnologias. Assim, é entre os modos de viver a “saúde”, de produção de uma estética da identificação, pela via do consumo, que corpos magros como ideal, se justificam. Concluímos que os cuidados com o corpo e o discurso da saúde em nome da qualidade de vida são propagados e anunciados pelas revistas para as mulheres comuns consumidoras, com impacto em processos de subjetivação pela produção de objetos de desejo, de consumo e de identificações com imagens perfeitas, indicadoras de felicidade e perfeição corporal. São ainda construídos modos de existir e se relacionar que engendram e reforçam modelos pautados em corpos sem marcas, autorizando a confecção constante da produção de beleza e saúde baseadas na juventude.

Within the passing years we’ve learned a lot about ways of transmission, more efficient medical treatments, more accurate laboratory results, immunity, and so on. It’s still evident, however, the stigma experienced with the find out of the disease. That signalizes that the person is stamped and rejected. To carry a stigma means to carry a characteristic not accepted by society. Based on this fact, people live many prejudice situation and their chances of living naturally are reduced. That way, when a woman faces the positive HIV diagnostic she experiences uncertainty feelings and insecurity, those feelings lead her to a crisis moment. Taking all this into account, this work’s aim is to find out the stigmas that surround the women with HIV infection. This investigation is descriptive and exploratory and its nature is qualitative. The scenario is an infirmary of a specialized hospital in Fortaleza – CE .The research period was from December 2004 to March 2005. Ten women with positive HIV participated. The research method used was the Thematic Oral. It was used recorded semi-structured interview to collect data. The women’s reports were analyzed by creating four categories: 1) facing the diagnostic; 2) changes imposed by the disease; 3) guilt; and 4) living in social exclusion. The women were young, with scarce financial income; low scholar level and they were infected by heterosexual way. This profile coincides with the profile of the majority of infected women in Brazil. Many forms of stigma were identified. Most of them were related to the difficulty of dealing with the diagnostic, changes in the women’s lives because they tried to live better, guilt, social exclusion’s evidence and lack of family’s support. When they remembered the experience of receiving the diagnostic news, they expressed fear of death, shame, and concern with their family, abandonment, solitude, sadness and fault. Besides, they had to keep on trying not to loose their jobs. The different stigma that surround their lives don’t let them live naturally and free of any type of prejudice, and this is a right that has to be respected when we talk about women’s infected with the HIV, because they already suffer bad feelings caused by the disease. This work supported changes in the global assistance offered to infected women, emphasizing that the treatment shouldn’t concern only about biological-opportunists diseases, but should also consider the conflicts experienced by the women. That way, we search for an improvement in these women’s life quality. To defeat the stigmas and the prejudice is a task for all the professionals evolved with these women, this can be done by cultural or legal means. / Há mais de duas décadas, o mundo convive com a aids. Com o passar dos anos, muito se descobriu sobre formas de transmissão, terapias medicamentosas mais eficazes, marcadores laboratoriais mais precisos, imunogenicidade. Ainda é evidente, entretanto, o estigma vivenciado em decorrência da descoberta da doença. Este é o sinete ou impressão a sinalizar que a pessoa está marcada e rejeitada; assim, portar um estigma, implica possuir uma característica não aceita pela sociedade. Com base neste fato, vivenciam-se vários tipos de discriminações, mediante os quais, efetivamente, reduzem-se as chances de se viver plenamente. Desta forma, diante da descoberta do diagnóstico, a mulher com HIV/aids experimenta sentimentos de incerteza e insegurança, levando-a a vivenciar um momento de crise. Nesta perspectiva, teve-se como objetivo apreender os estigmas que as mulheres portadoras de HIV/aids vivenciam em decorrência da sua infecção. Investigação descritiva e exploratória de natureza qualitativa, cujo cenário foi uma enfermaria de um hospital especializado em Fortaleza-CE, no período de dezembro de 2004 a março de 2005. Participaram dez mulheres com aids. Utilizou-se como modalidade de pesquisa a História Oral Temática, recorrendo-se, para coleta de dados, à entrevista semi-estruturada gravada. Para análise dos depoimentos, empregou-se a técnica de análise de conteúdo, elaborando-se quatro categorias: 1. enfrentando o diagnóstico; 2. mudanças impostas pela doença; 3. sentimento de culpa; e 4. vivenciando a exclusão social. As mulheres eram jovens, com escassos rendimentos financeiros, apresentavam pouca escolaridade e foram contaminadas pela via heterossexual, perfil que coincide com a maioria das brasileiras infectadas pelo HIV. Foram apreendidas diferentes formas de estigma, as quais estavam relacionadas, principalmente, às dificuldades no enfrentamento do diagnóstico, mudanças no cotidiano decorrentes de imposições para se viver melhor em face da doença, sentimentos de culpa e evidências de exclusão, além da ausência e apoio dos familiares. Ao reviverem a descoberta do diagnóstico da doença, as mulheres expressaram o medo da morte, vergonha, preocupação com a família, abandono, solidão, tristeza e culpa, além da tentativa constante de manutenção do emprego. Os diferentes estigmas vivenciados impediam as mulheres de conduzir sua vida naturalmente, livres de qualquer tipo de discriminação, pois é um direito que deve ser respeitado, principalmente, se tratando de uma portadora de HIV/aids, que já enfrenta uma gama de sentimentos negativos decorrentes da doença. Este estudo permitiu, ao longo de sua descrição, fornecer subsídios para reelaborar a assistência global oferecida a essa clientela, destacando que não devem ser apenas direcionados às preocupações para a doença biológica-oportunistas, mas destinando a essas mulheres uma atenção voltada aos conflitos que estão vivenciando, sejam eles decorrentes dos estigmas da aids ou não; buscando, assim, a melhoria da qualidade de vida das mulheres que vivem com HIV/aids. Combater, por meio de ações culturais ou através de meios legais, o estigma e a discriminação é uma tarefa de todos os profissionais que as assistem.
