Designing a community reintegration programme for individuals with a traumatic spinal cord injury in the Cape Metropolitan area

Philosophiae Doctor - PhD / Spinal cord injury (SCI) is a devastating condition often affecting young and healthy individuals around the world. This debilitating condition not only creates enormous physical and emotional suffering to individuals but also is a significant financial burden to families and society at large and it affects quality of life. Successful community reintegration following spinal cord injury is considered an important goal of rehabilitation as this has been positively associated with self-esteem, life satisfaction and quality of life. The overall aim of the study was to design a community reintegration programme for individuals who sustained a traumatic spinal cord injury (TSCI) in the Cape Metropolitan Area. The design of this study was a mixed method design including four phases. The first phase of the study was to determine the base line information regarding reintegration into communities after sustaining a TSCI and included 108 participants. Data was collected by use of self-administered/interview administered questionnaire. The results of this phase demonstrated that community reintegration of individuals who sustained a traumatic spinal cord injury was relatively low and employment was found to be a statistically significant variable influencing community reintegration following the injury. The second phase of the study aimed to gain a deeper understanding of how employment and other factors influence reintegration into communities after the injury.

Avaliação da locomoção do paraplégico sob estimulação elétrica neuromuscular / Evaluation of paraplegic locomotion aided by functional electrical stimulation

Este trabalho apresentará um projeto cujo propósito foi definir um protocolo de avaliação para pacientes paraplégicos sob terapia, através do uso de estimulação elétrica neuromuscular, no ambulatório de ortopedia do Hospital das Clínicas da Universidade de Campinas. A análise proposta servirá tanto para o acompanhamento da evolução dos pacientes, quanto, principalmente, para avaliação dos sistemas de reabilitação desenvolvidos pelo laboratório de biocibernética e engenharia de reabilitação na Escola de Engenharia de São Carlos. Neste trabalho o protocolo foi aplicado especificamente para descrever a locomoção do paraplégico usando a estimulação padrão de 4 canais, que realiza a ativação dos quadríceps para extensão das pernas, necessária para o suporte do corpo, e a ativação do reflexo de retirada através da estimulação do nervo fibular para a flexão necessária para a fase de balanço. A análise usou como referência o conhecimento relacionado à descrição das fases e estratégias usadas pela locomoção do ser humano intacto. O protocolo de avaliação usou recursos do laboratório de biomecânica e reabilitação do aparelho locomotor, situado no hospital citado acima, que permitiram a descrição da cinemática dos membros superiores e inferiores, da atividade muscular realizada pelos membros superiores, e das forças de reação do solo. / This work present a case study on the use of the motion analysis laboratory resources (three-dimensional motion capture system, EMG, and force plate) with the purpose of improve the use of surface electrical stimulation to enable the locomotion in paraplegic subjects. Five trials for each of the five patients (thoracic complete spinal cord lesion) using a four channel electrical stimulator was done for synchronously acquisition of: 3D coordinates of 30 markers distributed on the whole body (to evaluate the stride\'s characteristics and three-dimensional measurements of angles of whole-body); EMG signal, on both sides, of triceps brachii long head, pectoralis major, middle deltoideus, extensor carpi radialis; and the vertical component of the ground reaction force. All data were normalized on the right gait cycle. The analysis was done braking the cycle in 11 events related with the swing phase in both sides, because it was focused firstly in the actions related with the swing limb and upper body advance, that represents a critical factor in locomotion aided by electrical stimulation. These events divided the gait cycle in 10 phases, which was used for associate the data evaluated with specifics tasks. The results showed temporal parameters similar to that reported by other authors, specific characteristics of the patient, the mechanism used by the patient for advance the body, and the periods of high activity of the muscles on the cycle.

Les protéases et leurs inhibiteurs sécrétés par la cellule épithéliale : acteurs de l'inflammation et de la douleur / Proteases and their inhibitors secreted by epithelial cell : actors of inflammation and pain

Les protéases sont impliquées dans de nombreux processus biologiques et ont des origines très variées (cellules immunitaires, cellules épithéliales...). Leur activité est régulée par des inhibiteurs de protéases. Cette étude s'intéresse a` la balance entre les protéases épithéliales et leurs inhibiteurs dans le contexte de pathologies affectant l'intégrité´ de l'épithélium intestinal. Les conséquences d'un déséquilibre de la balance protéolytique ont été étudiées dans deux maladies chroniques aux composantes différentes, les Maladies Inflammatoires Chroniques de l'Intestin (MICI) (cycles de poussées inflammatoires) et le Syndrome du Côlon Irritable (SCI) (cycles de douleur). La muqueuse colique de patients atteints de SCI sécrète une activité de type trypsine dont l'origine et les fonctions sont mal définies. Cette étude s'intéresse à la source de cette activité trypsique dans le côlon des patients atteints de SCI, sa nature et son rôle dans l'activation neuronale. L'activité trypsique chez les patients atteints de SCI est augmentée majoritairement dans les cellules épithéliales (CEI). La stimulation de monocouches de cellules épithéliales avec du LPS ou de l'épinéphrine induit une augmentation de la quantité de trypsine-3 et de sa sécrétion au pôle basal de la cellule épithéliale stimulée, en corrélation avec une augmentation de l'activité trypsique. L'hyperactivité de la trypsine-3 au pôle basal entraine une perte de la fonction barrière de l'épithélium, anomalie retrouvée aussi chez les patients atteints de SCI. De plus, nous avons mis en évidence que la trypsine-3 est capable d'activer les neurones humains myentériques et les neurones sensitifs murins. In vivo, son administration intra-rectale à des souris induit une hypersensibilité viscérale dépendante du récepteur PAR2 (Protease Activated Receptor-2). Ainsi, les cellules épithéliales intestinales des patients atteints de SCI produisent et sécrètent de la trypsine-3 spécifiquement au pôle basal. Cette activité trypsique active des neurones sensitifs, participant à l'hypersensibilité viscérale, symptôme majeur dont souffrent les patients atteints de SCI. Les pathologies inflammatoires peuvent aussi être source d'un dérèglement protéolytique. Les patients atteints de MICI présentent une dérégulation de la balance élastolytique colique. Notre équipe a montré que l'apport d'ELAFINE (un inhibiteur d'élastase) par la bactérie génétiquement modifiée L.lactis au contact de la muqueuse inflammatoire protège les souris traitées en réduisant l'inflammation intestinale. Cependant les mécanismes protecteurs médiés par L'Elafine restent encore à déterminer. L'ELAFINE est un inhibiteur d'élastases mais possède aussi des propriétés antimicrobiennes. Afin de mettre en lumière quelles fonctions de L'Elafine portent les propriétés anti-inflammatoires des mutants de L'Elafine ont été générés et insérés dans la bactérie L.lactis: une mutation pour annuler sa fonction inhibiteur de protéase, un second mutant de son domaine antimicrobien et un dernier mutant ayant perdu ces 2 fonctions. Dans des monocouches de cellules intestinales épithéliales, l'apport d'ELAFINE protège de l'inflammation médiée par un déséquilibre de l'activité élastolytique : on observe une restauration de la fonction barrière de l'épithélium et une diminution des cytokines pro-inflammatoires (CXCL8 et IP10). La mutation du domaine antimicrobien n'affecte pas ces propriétés. Cependant, l'absence de la boucle inhibitrice annihile les propriétés anti-inflammatoires de L'Elafine. Ces travaux ont pu mettre en évidence l'importance de l'équilibre protéolytique au sein de la cellule épithéliale dans les pathologies intestinales. La balance entre les protéases et leurs inhibiteurs joue donc un rôle dans l'homéostasie épithéliale et les pathologies inflammatoires. / Proteases are involved in some biologic processes and their origins are variable (immune cells, epithelial cells...). Their activity is regulated by antiproteases. This study investigates the balance between proteases and their inhibitors in pathologies which modify epithelium integrity. Consequences of an unbalance in proteolytic activity was studied in two chronic pathologies with different components: Inflammatory Bowel Disease (IBD) (cycles of inflammatory boost) and Irritable Bowel Syndrome (IBS) (cycles of pain symptoms). Colonic mucosa from IBS patients releases trypsin activity. The origin and the functions of this activity are not well defined. This study investigated the source of this trypsin activity in the côlon of IBS patients, its nature and its role in neuronal activation. Trypsin activity from IBS patients is increased mostly in epithelial cells. Stimulation of epithelial cell monolayers with LPS or epinephrine induces an increase of trypsin-3 quantity and its secretion specifically in the basal side of epithelial cells. This is in correlation with the increase of trypsin activity. Trypsin-3 hyperactivity at the basal side provokes a loss of epithelium barrier function, which is also found in colons of IBS patients. Then, we have highlighted that trypsin-3 is able to activate human myenteric neurons and murine sensitive neurons. In vivo, its intra-rectal administration to mice induces a visceral hypersensitivity dependent of PAR2 (Protease Activated Receptors 2). Thus, intestinal epithelial cells from IBS patients produce and release trypsin-3 specifically on their basal side. This trypsin activity activates sensitive neurons which participate to visceral hypersensitivity, a major symptom of IBS patients. Inflammatory pathologies could be a source of proteolytic malfunction. IBD patients have a dysregulation of elastolytic balance in the colon. Our team has shown that ELAFIN (an elastase inhibitor) delivered by the bacteria genetically modified L.lactis near the inflamed mucosa, protects mice from intestinal inflammation. However, the protective mechanisms induced by ELAFIN need to be investigate. ELAFIN is an elastase inhibitor but have also antimicrobial properties. With the aim to highlight what function of ELAFIN owns anti-inflammatory properties, mutants of ELAFIN have been generated and were insered into L.lactis: a first mutant lacked its antiprotease function, a second lacked antimicrobial properties and a last mutant lacked both properties. In intestinal epithelial monolayers, ELAFIN delivered by L.lactis protects against inflammation: a restauration of epithelial barrier function and a decrease of pro-inflammatory cytokines (CXCL8 and IP10) are observed. Mutation of antimicrobial domain doesn't affected these properties. Nevertheless, the absence of inhibitory loop annihilates anti-inflammatory functions of ELAFIN. This work highlights the importance of proteolytic balance inside the epithelial cell in intestinal pathologies. The balance between proteases and antiproteases plays an important role in epithelial homeostasis.

<p>The design and implementation of a synthesizable model of the Serial Communication Interface and Synchronous Serial Interface, which constitutes the Port C of Motorola DSP56002 is presented in this report. They serves as a full duplex serial interface to other DSPs, processors, codecs, digital-to-analog and analog-to-digital converters and other transducers. The SCI block is able to handle a data rate of 5Mbps in Synchronous mode and 625Kbps in asynchronous mode for a 40MHz clock. It supports five word formats including a multidrop mode for multiprocessor systems. SSI provides a data rate of 10Mbps for the same 40 MHz clock. The design includes a programmable on-chip or external baud rate generator/interrupt timer for the SCI and a clock generator and frame Sync generator for the SSI.</p><p>The thesis focus on arriving at a full functional description of individual blocks included with Port C from the data sheets and product users manual. From this operational description a behavioural model was developed. The structure and implementation is based on the Motorola DSP56002 with additional support for a variable data-width. The model is written completely in behavioural VHDL with a top-down approach and the model was verified and validated.</p>

Color Screening in QCD and Neutrinos from Singlino Dark Matter

Hard diffraction in proton collisions, where the initial state proton emerges from the interaction rather undisturbed despite a hard interaction scale, has been studied for a few decades. First observed in proton-proton collisions, the phenomenon is seen as well in deep inelastic electron-proton scattering (DIS) as a leading final state proton and a rapidity gap-region without final state particles. Although a rather successful description in terms of the exchange of a hadronic color singlet pomeron with a parameterized gluon content exists, it is still an open question whether a theoretically more well-founded description can be obtained based on quantum chromodynamics. The soft color interaction model (SCI) attempts this through additional gluon exchanges at momentum scales below the conventional scale of perturbative QCD and the hadronization scale. Such gluons can lead to an effective color singlet exchange and therefore to diffraction. This thesis explores the phenomenology of the SCI model in diffractive W and photon+jet production. For diffractive deep inelastic scattering, a dynamic color screening model is developed based on a summed amplitude for soft gluon exchanges. The studies of the model within Monte Carlo event simulation show that the additional dynamics improve the description of electron-positron scattering data from HERA. Dijet events in proton-proton collisions with an upper limit on the energy flow between the jets is sensitive to large angle gluon emissions. This thesis applies a resummation method which takes into account also secondary emissions to describe this observable and shows that a good description of data from ATLAS can be achieved. Supersymmetric extensions to the Standard Model provide a possible explanation for dark matter in the universe. The next-to-minimal supersymmetric extension (NMSSM) can contain a dark matter candidate in form of the lightest neutralino with a substantial singlino component. This thesis studies the prospects for indirect detection of dark matter for such viable NMSSM model points via the observation of neutrinos from neutralino annihilation in the sun with IceCube and the future extension PINGU. It is shown that with a few years of data taking large parts of the parameter space can be excluded or a discovery be made.

Plataforma de comunicação tempo real sobre clusters SCI

Devido a sua baixa latência de banda, os clusters equipados com o adaptador SCI são uma alternativa para sistemas de tempo real distribuídos. Esse trabalho apresenta o projeto e implementação de uma plataforma de comunicação de tempo real sobre clusters SCI. O hardware padrão do SCI não se mostra adequado para a transmissão de tráfego de tempo real devido ao problema da contenção de acesso ao meio que causa inversão de prioridade. Por isso uma disciplina de acesso ao meio é implementada como parte da plataforma. Através da arquitetura implementada é possível o estabelecimento de canais de comunicação com garantia de banda. Assim, aplicações multimídias, por exemplo, podem trocar com taxa constante de conunicação. Cada mensagem é enviada somente uma vez. Assim, mensagens som a semântica de eventos podem ser enviadas. Além disso, a ordem e o tamanho das mensagens são garantidos. Além do tráfego com largura de banda garantida, a plataforma possibilita a troca de pacotes IP entre diferentes máquinas do cluster. Esses pacotes são inseridos no campo de dados dos pacotes próprios da plataforma e após são enviados através do uso de pacotes IP. Além disso, essa funcionalidade da plataforma permite também a execução de bibliotecas de comunicação baseadas em TCP/IP como o MPI sobre o cluster SCI. A plataforma de comunicação é implementada como modulos do sistema operacional Linux com a execução de tempo real RTAI. A valiação da plataforma mostrou que mesmo em cenários com muita comunicação entre todos os nodos correndo, a largura de banda reservada para cada canal foi mantida.
