Cardiovascular diseases in the Czech Republic represent the main cause of death and significantly contribute to the sickness rate and premature disability. Possibilities of treatment of cardiovascular diseases keep developing increasingly these days. An important part is the follow up spa treatment as well as sufficiently performed effective cardio rehabilitation. The theoretical part characterizes functional examination of the lungs and stress tests in cardiology. Simultaneously it describes early spa treatment and defines quality of life. The aim of the diploma thesis, which deals with the vital capacity of the lungs, was to prove positive effect of early spa treatment on the vital capacity of the lungs and on improvement of quality of life, specifically with respect to positive perception of one´s overall physical health. Hypotheses - H1: An early spa therapy positively affects spirometry values. H2: Patients with an early spa therapy better perceive their overall physical health. A form of quantitative research was chosen, in order to verify the determined aims and hypotheses of the diploma thesis. A standardized international Short Form SF - 36 questionnaires on quality of life was used for the data collection. Spirometry was utilized for ascertaining objective functional parameters. Values of spirometry examination were used for comparison. 32 patients after cardiovascular surgery were included in the research, who were transferred to Lázně Poděbrady on the 6th - 8th day after the surgery, 22 of them being men (69%) and 10 women (31 %). The average age of patients under research was 66,06 + 11,48 years. The check group consisted of 10 healthy volunteers. 8 women (80 %) and 2 men (20 %) at the average age of 37,1 + 13,3 years were included in the research. It follows from the spirometry results measured before the heart surgery at IKEM cardio center that a difference in results between the second and third spirometry occurred. The second and third spirometrical examination is the period of time, when the respondent undergoes the early spa treatment. After the heart surgery spirometry values worsen and improve in the course of the spa therapy. The research showed that the perception of overall physical health after the heart surgery is subject to perception of pain, which negatively affected sense of overall perception of physical health. H1 was confirmed and H2 was not confirmed. In order to improve current situation, it would be suitable to devote not only to cardio rehabilitation but also to improvement of perception of pain, e.g. by means of psychotherapy (art therapy, music therapy?). It is also important to widen a possibility of outpatient cardio rehabilitation in each cardio center for patients after heart surgery.

Contribution to the molecular epidemiology of methicillin-resistant staphylococcus aureus (MRSA) in Belgian populations

Staphylococcus aureus est une bactérie commensale et pathogène qui s’est rapidement adaptée à la pression sélective des antibiotiques entraînant la diffusion de souches résistantes à la méticilline (MRSA). En Belgique, des souches appelées Healthcareassociated (HA)-MRSA sont devenues endémiques dans les hôpitaux, causant de nombreuses épidémies durant les années 90.<p>Parallèlement, des cas d’infections communautaires par des souches appelées Community-associated (CA)-MRSA produisant la leucocidine de Panton-Valentine (PVL) ont été rapportées en Europe, en Australie et aux USA, chez des patients jeunes sans contact avec les institutions de soins. Depuis 2005, une prévalence élevée de portage de MRSA a été observée chez les porcs, les éleveurs et les vétérinaires aux Pays-Bas et dans les pays voisins. Ces souches dites Livestock-associated (LA)-MRSA montrent une origine génétique distincte des souches humaines précédemment décrites.<p>Nos travaux de recherche ont porté sur l’épidémiologie et la caractérisation moléculaire de la colonisation et l’infection par S. aureus dans diverses populations humaines en Belgique afin d’élucider :1) l’évolution de la distribution spatio-temporelle des clones épidémiques de MRSA parmi les patients hospitalisés au cours de la période de 1995 à 2003 ;2) les mécanismes moléculaires de résistance aux antibiotiques associés à ces clones ;3) les relations phylogénétiques entre souches de S. aureus sensibles et résistantes à la méticilline parmi les patients hospitalisés afin d’identifier l’origine probable des clones épidémiques ;4) la prévalence et les facteurs de risque de portage de MRSA parmi les résidents des maisons de repos et l’origine de ces souches ;5) l’origine des souches de CA-MRSA et les profils cliniques des infections associées ;6) la prévalence et les facteurs de risque associés au portage de MRSA et la diffusion des souches de MRSA dans les fermes d’élevage porcin.<p>Des enquêtes multicentriques nous ont permis de caractériser les souches de S. aureus résistantes et sensibles à la méticilline affectant les patients hospitalisés (4 enquêtes, 1995 -2003), les patients ambulants (1 enquête, 2002-2004), les résidents dans des maisons de repos et de soins (1 enquête en 2005) et les habitants des fermes d’élevage porcin (1 enquête en 2007). Ces souches ont été caractérisées par détermination du type de cassette mec (SCCmec typing), séquençage d’un gène hypervariable (spa typing) et de 7 gènes de ménage (multi-locus sequence typing, MLST), combinées à l’analyse par électrophorèse en champs pulsé (PFGE) du profil de acrorestriction génomique. Les gènes codant pour trois classes d’antibiotiques et les toxines, PVL, TSST-1 et exfoliatines, ont été recherchés par PCR.<p>L’étude des souches de MRSA a montré une diversification importante des clones dans les hôpitaux, avec le remplacement d’un clone multi-résistant par des clones plus sensibles aux antibiotiques. La distribution des gènes de résistance ainsi que du gène TSST-1 était fortement liée au génotype. Les S. aureus sensibles à la méticilline (MSSA) montraient une plus grande diversité génotypique que les MRSA. La majorité des MRSA épidémiques appartient à des génotypes endémiques de MSSA, suggérant la possibilité d’émergence locale de MRSA par acquisition récente de l’élément mobile SCCmec.<p>D’autres souches de génotypes pandémiques pourraient avoir été importées en Belgique. L’enquête dans les maisons de repos et de soins a montré une prévalence élevée de résidents porteurs de MRSA. L’exposition aux antibiotiques, les lésions cutanées, la perte de mobilité, l’absence de formulaire thérapeutique et de procédures d’hygiène pour le contrôle du MRSA, associée à un nombre élevé de médecins, augmentaient significativement le risque de portage. La présence des mêmes génotypes dans les hôpitaux et les maisons de repos d’une même province suggère des transferts locaux de MRSA entre patients résidant dans et circulant entre ces secteurs de soins.<p>Les souches de CA-MRSA productrices de toxine PVL importées ou acquises en Belgique appartiennent à divers génotypes. La présence de MRSA de même lignée clonale mais présentant des profils de virulence différents clonale dans les institutions de soins et dans la population générale suggère que ces souches ont évolué de manière divergente dans des environnements différents.<p>Nous avons documenté une prévalence très élevée de porteurs de MRSA de génotype ST398 chez les éleveurs de porcs. Le réservoir de ce clone est très probablement d’origine animale, la transmission à l’homme ayant lieu par contact avec des animaux d’élevage ou de compagnie et potentiellement, par voie alimentaire.<p>En conclusion, S. aureus est un pathogène capable de s’adapter dans des environnements très différents. Les souches épidémiques résistantes aux antibiotiques, qu’elles soient d’origine hospitalière, communautaire ou animale, appartiennent à un nombre limité de génotypes bien établis dans chaque écosystème au niveau local ou régional. L’étude approfondie de la dynamique de transmission des MRSA, et de leur profil de résistance dans la communauté, les secteurs des soins aigus et chroniques et l’élevage, est indispensable pour définir les stratégies cliniques et de santé publique pour adapter les schémas thérapeutiques et endiguer l’endémie d’infections à MRSA.<p> / Doctorat en Sciences médicales / info:eu-repo/semantics/nonPublished

The epidemiology of work-related musculoskeletal disorders in beauty therapists working within the hotel spa industry in the eThekwini municipality

Treatments include massage, waxing, pedicures and facials. These techniques involve vigorous and repetitive movements that may lead to overuse and strain on the musculoskeletal system, resulting in injury. Work-related activities together with factors such as socio-demographic, psychosocial, environmental and anthropometric can influence the prevalence of work-related musculoskeletal disorders (WRMDs). There has been little investigation into the WRMDs of beauty therapists both locally and internationally. Aim The aim of this study was to determine the prevalence, selected risk factors and impact of self-reported work related musculoskeletal disorders (WRMDs) in beauty therapists working within the hotel spa industry in the eThekwini municipality. Method A cross sectional epidemiological design was used to survey 254 beauty therapists working in the hotel spa industry in the eThekwini municipality of Durban. A pre-validated questionnaire was hand delivered to beauty therapists working at all hotel spas that agreed to participate. Informed consent was obtained. The survey contained questions related to musculoskeletal pain, socio-demographic, psycho-social, lifestyle and occupational factors. Results A response rate of 70% was obtained (n=178). The majority of the respondents were female (94.3%; n=165), the mean age of the respondents was 27.74 (SD±4.83) years. The respondents had been working as beauty therapists for an average of 5.91 (SD±4.19; n=176) years and performed on average 27.89 (SD±13.33; n=170) treatments per week, working 47.38 (SD±13.36; n=175) hours per week. Those working overtime worked on average 5.43 (SD±2.94; n=59) hours of overtime per week. There was a high rate of reported WRMDs with the lifetime, current and 12 month period prevalence being 86% (n=153), 85% (n=151) and 83% (n=148), respectively. The area most affected were the low back followed by the neck, the hand and wrist. Ninety percent of the respondents indicated that their pain was mild to moderate in nature, with half expressing an inability to cope with the pain, and 78% reporting that it interfered with their ability to work. Mental exhaustion (p=0.032), suffering from a concomitant co-morbidity (p=0.031), years worked as a beauty therapist (p˂0.001) and treating clients after hours (p=0.007) were significantly associated with the presence of musculoskeletal pain. Performing manual massage (p=0.043) and reporting feelings of exhaustion whilst performing specialised massage techniques (p=0.014) and applying makeup/eyelashes (p=0.022) were associated with the presence of WRMDs. Those who reported having co-worker (p=0.006) and managerial support (p˂0.001) were less likely to experience musculoskeletal pain. In contrast, feeling frustrated by work (p=0.007), being under pressure (p=0.005) and over worked (p=0.009) increased the chances of experiencing WRMDs. Working with the back in an awkward position (p˂0.001), standing for prolonged periods (p˂0.001), lifting heavy loads (p=0.019) and working in a hot and humid environment (p<0.001) increased the risk of WRMDs. Conclusion WRMDs have a significant impact on beauty therapists’ ability to work. Effective strategies to prevent and manage WRMDs in hotel spa beauty therapists is required, along with governmental regulation. / M

The intellectual property concept of 'communication to the public of the broadcast works by radio or television receivers' in Article 23 of the Czech copyright act is a current issue especially in a relation to the exception to this right in favour of health establishments that has been called in a question in front of both Czech courts and CJEU. The most important criteria of communication to the public -- laid down by CJEU -- suggest that the concept should be interpreted broadly, covering all transmissions, regardless of the means used and the nature of the place for the communication. A public refers to an indeterminate but fairly high number of people who constitute so called new public. In order to fall within the scope of a lawful exception the criteria of the so called 'three-step test' must be met as well as the formal criteria stated in the abovementioned Article. The most recent cases discussed in this work dealt with the transmission of the protected works by means of television sets located in the bedrooms of the guests of spa establishments. Although it was held by CJEU that this exception does not comply with EU law, it is still part of the Czech law and Czech courts must cope with it.

Implementación VLSI del algoritmo de proyecciones sucesivas para detección de sistemas MIMO

[EN] The insatiable demand for bandwidth of communication on the part of end-users, linked to the lowering the price of the terminals and in telecommunication services have led to a spectacular growth of the wireless communications market in recent years. Those entities that are responsible, at the international level, of the technological standardization have known to guide this growth writing standards as LTE (Long Term Evolution), IEEE 802.11 (WiFi) and IEEE 802.16 (WiMax) or 3G networks or 4GPP. They all share a common denominator, for the improvement of the spectral efficiency, the use of MIMO technologies, which uses multiple antennas on transmitter and receiver, and the use of high modulation schemes as 256QAM, introduced in revision 12 of the standard 3GPP-LTE. Under this perspective of great gains in the spectral efficiency, it is not surprising that MIMO technology has been incorporated into the standards mentioned above. However, achieving these gains is not trivial, to the extent that the VLSI implementation of this technology has become a challenge. In this thesis has undertaken a comprehensive study of different MIMO detectors, studying those belonging to the two families that show best features for being implemented in VLSI technology: successive interference cancellation (VBLAST detector) and based on a search in tree (KBest detector). Although initially the benefits achieved by the seconds (KBest) are far superior to those of the first (VBLAST), the recent appearance in the specialized literature of the Successive Projections Algorithm (SPA) opens the door to the development of a new detector, belonging to the family of the detectors of Successive Interference Cancellations (SIC), which will be able to compete in performance with the KBest detectors. This work provides the necessary algorithmic keys that make viable and competitive the hardware implementation of the SPA algorithm. In particular, two mechanisms of control of repetitions have been developed: Simplified-ESPA (SESPA) and Table-ESPA (TESPA), and the mechanisms for obtaining hard and soft output, existing in the literature, have been adapted to this algorithm. It has designed the first VLSI architecture for the SPA algorithm, being highly flexible, in the sense that adapts to different conditions of transmission and complies with the latest published specifications in the WiMAX and LTE standards. The flexibility of the architecture allows you to select different configurations of antennas in transmission and reception, from 2x2 to 4x4, different modulation schemes from QPSK until 256QAM, controls the balance between transmission rate and the benefits BER/FER and offers the soft output and hard output decisions. Finally, with this architecture has been implemented the SESPA and TESPA detectors, with soft output and hard output, in FPGA and ASIC technology. These detectors have been evaluated and compared to the best published in the specialized literature, achieving a peak rate of 465 Mbps for the detector SESPA 4x4 256QAM, with an area of 3.83 mm2 with a 90 nm technology. The detectors implemented offer as added value, in addition to the high configurability, the ability to decode 256-QAM without increasing the area. This feature is highly competitive with the non-linear detectors based on KBest, which are very sensitive, in regard to decoding rate and area, with the selected modulation scheme. In addition, the detectors based on ESPA reach a FER performance (soft output) clearly competitive with KBest detectors, due to a higher quality of the LLR generated by the ESPA. The comparison with other flexible architectures selected shows that the SESPA and TESPA detectors offer the greater configurability of transmission parameters and the best balance between area, BER performance and detection rate. / [ES] La insaciable demanda de ancho de banda de comunicación por parte de los usuarios finales, unido al abaratamiento de los terminales y de los servicios de telecomunicación han provocado un crecimiento espectacular del mercado de las comunicaciones inalámbricas en estos últimos años. Las entidades responsables, a nivel internacional, de la estandarización tecnológica han sabido acompañar y guiar este crecimiento redactando normas como LTE (Long Term Evolution), IEEE 802.11 (WiFi) e IEEE 802.16 (WiMax) o las redes 3G o 4GPP. Todas ellas comparten como denominador común, para la mejora de la eficiencia espectral, el uso de las tecnologías MIMO, que utiliza múltiples antenas en emisor y receptor, y el uso de esquemas de modulación elevados como 256QAM, introducido en la revisión 12 del estándar 3GPP-LTE. Bajo esta perspectiva de grandes ganancias en la eficiencia espectral, no es de extrañar que la tecnología MIMO haya sido incorporada en los estándares mencionados anteriormente. No obstante, conseguir estas ganancias no es trivial, hasta el punto de que la implementación VLSI de esta tecnología se ha convertido en un reto. En esta tesis se ha realizado un estudio exhaustivo de diferentes detectores MIMO, fijando el punto de mira en aquellos pertenecientes a las dos familias que muestran mejores características para su implementación VLSI: cancelación sucesiva de interferencias (detector VBLAST) y basados en búsqueda en árbol (detector KBest). Aunque inicialmente las prestaciones alcanzadas por los segundos (KBest) son muy superiores a las de los primeros (VBLAST), la reciente aparición en la literatura especializada del algoritmo de proyecciones sucesivas (SPA) abre la puerta al desarrollo de un nuevo detector, que pueda competir en prestaciones con los detectores KBest. La tesis aporta las claves algorítmicas necesarias que hacen viable y competitiva la implementación hardware del algoritmo SPA. En particular, se han desarrollado dos mecanismos de control de repeticiones: Simplified-ESPA (SESPA) y Table-ESPA (TESPA), y se han adaptado los mecanismos de obtención de salidas hard output y soft output, existentes en la literatura, a este algoritmo. Se ha diseñado la primera arquitectura VLSI para el algoritmo SPA, siendo ésta altamente flexible, en el sentido de que se adapta a diferentes condiciones de transmisión y cumple con las últimas especificaciones publicadas en los estándares WiMAX y LTE. La flexibilidad de la arquitectura permite seleccionar diferentes configuraciones de antenas en transmisión y recepción, desde 2x2 hasta 4x4, diferentes esquemas de modulación desde QPSK hasta 256QAM, controla el balance entre tasa de transmisión y las prestaciones BER/FER y ofrece las decisiones soft output y hard output. Finalmente, con esta arquitectura se ha realizado la implementación de los detectores SESPA y TESPA, con salidas soft output y hard output, en los dispositivos FPGA y ASIC. Estos detectores han sido evaluados y comparados con los mejores publicados en la literatura especializada, consiguiendo la tasa de pico máxima de 465 Mbps para el detector SESPA 4x4 256QAM, en un área de 3.83 mm2 con una tecnología de 90 nm. Los detectores implementados ofrecen como valor añadido, además de la alta configurabilidad, la posibilidad de decodificar 256QAM sin incrementar el área. Esta característica es altamente competitiva con los detectores no lineales basados en KBest, que son muy sensibles, en cuanto a tasa de decodificación y área se refiere, con el esquema de modulación seleccionado. Además, los detectores basados en ESPA alcanzan unas prestaciones FER (soft output) claramente competitivas con los detectores KBest, debido a la mayor calidad del LLR generado por el ESPA. No obstant això, aconseguir aquests guanys no és trivial, fins l'extrem que la implementació VLSI d'aquesta tecnologia s'ha convertit en un repte. En aquesta tesi s'ha realitzat un estudi exhaustiu de diferents detectors MIMO, fixant el punt de mira en aquells que pertanyen a les dos famílies que mostren millors característiques per a la seua implementació VLSI: cancel-lació successiva d'interferències (detector VBLAST) i els basats en recerca en arbre (detector KBest). Encara que inicialment les prestacions aconseguides pels segons (KBest) són molt superiors a les dels primers (VBLAST), la recent aparició en la literatura especialitzada de l'algoritme de projeccions successives (SPA) permet el desenvolupament d'un nou detector, que puga competir en prestacions amb els detectors KBest. Este treball aporta les claus algorítmiques necessàries que fan viable i competitiva la implementació hardware de l'algoritme SPA. En particular, s'han desenvolupat dos mecanismes de control de repeticions: Simplified-ESPA (SESPA) i Table-ESPA (TESPA), i s'han adaptat els mecanismes d'obtenció d'eixides hard-output i soft-output, existents en la literatura, a aquest algoritme. S'ha dissenyat la primera arquitectura VLSI per a l'algoritme SPA, sent aquesta altament flexible, en el sentit de que s'adapta a diferents condicions de transmissió i acompleix les últimes especificacions publicades en els estàndards WiMax i LTE. La flexibilitat de l'arquitectura permet seleccionar diferents configuracions d'antenes en transmissió i recepció, des de 2x2 fins 4x4, diferents esquemes de modulació des de QPSK fins 256QAM, controla el balanç entre taxa de transmissió i les prestacions BER/FER i ofereix les decisions hard output i soft output. Finalment, amb l'arquitectura proposta s'ha realitzat la implementació dels detectors SESPA i TESPA, amb eixides hard output i soft output, en els dispositius FPGA i en ASIC. Aquests detectors han segut valorats i comparats amb els millors publicats en la literatura especialitzada, i s'ha aconseguint la taxa de pic màxim de 465 Mbps per al detector SESPA 4x4 256QAM, dins una àrea de 3.83 mm2 en una tecnologia de 90 nm. Els detectors implementats ofereixen com a valor afegit, a més de l'alta configurabilitat, la possibilitat de decodificar 256QAM sense incrementar l'àrea. Esta característica és altament competitiva en els detectors no lineals basats en KBest, que són molt sensibles, en relació a taxa de decodificació i a l'àrea del circuit, a l'esquema de modulació seleccionada. A més a més, els detectors basats en ESPA aconsegueixen unes prestacions FER (soft output) clarament competitives amb els detectors KBEST, degut a la major qualitat del LLR generat per l'ESPA. The Subject of this diploma thesis is design documentation of Relax sports facility with accommodation in Suchdol and Odrou. Mixed-use building is three-storeyed with partial basement. Roofing is flat single-skinned roof over the entire building. From the material point of view, the basement is made from lost formwork, all aboveground floors are bricked from ceramic blocks. The facade is designed as ventilated. Ceilings will be made from cast-in-place reinforced slabs and the sports ground will be covered with glued lamellar beams. The basement part of the building is relaxing - there are various types of saunas, cooler, whirlpool and massage room. On the first floor there is a restaurant with backgrounds and kitchen, a café, and sports activities in the form of badminton, squash and gym. Two more floors are used mainly to accommodate guests in the hotel. All operations are functionally connected by the reception hall.

The topic of this thesis is project and documentation for montane recreative komplex, which is situated in Loučná nad Desnou. The object is located in land-registered area Kouty nad Desnou. In this region is famous ski slope and from the hotel is nice view on this ski slope. The montane hotel respects criteria of protected landscape area Jeseníky. Architectonic plan is designed with respect to mountain scenery. In the hotel are 18 rooms with possibility 9 extra beds for 51 guests. There is a option to visit restaurant all-day. This restaurant is part of the hotel. Recreative zone is situated in north part of the hotel and this zone is devided into two parts. First part, is public and serves for all guests. This zone includes pool with whirpool bath, sauna and steam bath. Second zone is private. This zone provides privacy in massage rooms or in rooms for aromatic bath. The object is devided into four floors. Three floors are overground and one floor is underground. In the second and thirds floors are hotels rooms and rooms for clean-up and employees. First overground floor containes restauration with kitchen, coffee-bar, main hall with reception and all recreation zone, which passes to the second floor. Basement is consists of air handling units, technical rooms and housed mass garages with 21 places. Main entrance to the hotel is from south-eastern side and it is open space which passes to gallery. Type of constructional system is reinforcement framework. It is made of monolithic pillars placed on reinforcement column base with beam PEIKKO DELTABEAM. On this beam are located reinforcement ceilings elements SPIROLL. Fulfil brickwork is SILKA covered by combination of heating EPS with MW, ISOVER TWINNER with 200 millimetre thikness. Roof is combination of double cover flatbed roofs and single cover flatbed roofs. In the second floor is roof with posibility to walk to a terrace. Project was process in program ArchiCAD like BIM. The subject of the Diploma thesis was to design and prepare a documentation for the construction of a spa hotel. The building is situated in the village of Lipno nad Vltavou. This area is famous for a ski resort and the Lipno Dam which can be seen from the hotel. The hotel consists of three operating parts - spa, restaurant, accommodation and recreation area. The whole complex is designed as two rectangular plans. The spa part is a one-storey building with a flat green roof. The hotel with restaurant is a three-storey building with a pitched roof and metal roofing. The main entrance is oriented to the north. In front of the building there is a parking space for hotel guests. The spa structural system is a mix of a reinforced concrete frame with beam and a wall system made of clay blocks Porotherm. The hotel and restaurant is a wall structural system made of clay blocks Porotherm. The floor structure is designed as reinforced concrete slabs. The whole building is insulated to meet all the requirements of energy consumption of buildings.
