Examining the Relationship between Safety Management System Implementation and Safety Culture in Collegiate Flight SchoolsRobertson, Mike 01 May 2017 (has links)
Safety Management Systems (SMS) are becoming the industry standard for safety management throughout the aviation industry. As the Federal Aviation Administration (FAA) continues to mandate SMS for different segments, the assessment of an organization’s safety culture becomes more important. An SMS can facilitate the development of a strong aviation safety culture. This study describes basic principles and components of an SMS and how safety culture and SMS are integrated. Studies focusing on safety culture assessment were identified for other industries as well as for different areas of the aviation industry. The purpose of this study was to examine the relationship between an organization’s safety culture and SMS implementation in collegiate flight schools. The research study was designed to determine (a) relationship between SMS implementation and safety culture, (b) the relationship between safety promotion and safety culture, and (c) the relationship between management commitment and safety culture. The study population consisted of 453 individuals at 13 collegiate flight schools. Data were gathered through an online survey to safety officers at collegiate flight schools within the University Aviation Association to determine the level of SMS implementation; and the Collegiate Aviation Program Safety Culture Survey (CAPSCUS) was used to measure the safety culture at those collegiate flight schools. The results indicated that a relationship existed between SMS implementation and safety culture, safety promotion and safety culture, management commitment and safety culture. The relationship for all three was more prominent within the Formal Safety Program major scale of the CAPSCUS. It is recommended that collegiate flight schools examine their existing level of SMS, management commitment, and their safety promotion and assess safety culture within their institution. Future studies should be done to further examine the relationship between SMS implementation and safety culture so that the collegiate flight training environment would have guidance regarding SMS implementation.

Normal operations safety survey : measuring system performance in air traffic controlHenry, Christopher Steven 17 April 2014 (has links)
The Normal Operations Safety Survey (NOSS) is an observational methodology to collect safety data during normal Air Traffic Control (ATC) operations. It aims to inform organizations about safety matters by using trained ATC staff to take a structured look at everyday operations. By monitoring normal operations through the use of direct over-the-shoulder observations, it is believed that safety deficiencies can be identified in a proactive manner prior to the occurrence of accidents or incidents. NOSS was developed as a collaborative effort between the International Civil Aviation Organization, ATC providers, controller representatives, government regulators, and academics to fill a gap in available ATC safety information. System designers consider three basic assumptions: the technology needed to achieve the system production goals, the training necessary for people to operate the technology, and the regulations that dictate system behavior. These assumptions represent the expected performance. When systems are deployed, however, particularly in realms as complex as ATC, they do not perform quite as designed. NOSS aims to capture the operational drift that invariably occurs upon system deployment. NOSS captures how the ATC system operates in reality, as opposed to how it was intended to operate. NOSS is premised on the Threat and Error Management (TEM) framework. TEM frames human performance in complex and dynamic settings from an operational perspective by simultaneously focusing on the environment and how operators respond to that environment. TEM posits that threats and errors are a part of everyday operations in ATC and must be managed in order to maintain safety margins. This dissertation describes NOSS and its contributions to ATC safety management systems. It addresses the validity and reliability of NOSS data and presents case studies from field trials conducted by a number of ATC providers. / text

Nesse contexto, esta tese apresenta a proposta de um método de avaliação de sistemas de gestão de segurança e saúde no trabalho (MASST), o qual apresenta duas características inovadoras: a) a conciliação das abordagens estrutural (sistema prescrito), operacional (o que está acontecendo na prática) e por desempenho (resultados de indicadores); b) a adoção do enfoque da engenharia de resiliência (ER) sobre a segurança e saúde. O MASST foi desenvolvido a partir das contribuições da literatura, na qual foram identificados quatro princípios da ER (comprometimento da alta direção, flexibilidade, aprendizagem e consciência), bem como de um estudo de caso exploratório realizado em uma empresa de implementos agrícolas. Com base nisso, foram propostos vinte e sete itens distribuídos ao longo de sete critérios. Cada item possui um conjunto de requisitos que são avaliados com base em três fontes de evidências básicas: entrevistas, análise de documentos e observação direta. O MASST foi validado em um estudo de caso em uma empresa da cadeia automotiva. Os principais resultados revelaram que, no estudo de caso, em uma escala de pontuação de 0% a 100%, nove dentre quatorze itens relacionados à ER obtiveram pontuação entre 0% e 10%. Além disso, o MASST possibilitou a identificação dos pontos positivos do SGSST, a identificação das causas sistêmicas da falta de segurança e a identificação das prioridades de ação em termos de SST. The literature review pointed out four major resilience engineering (RE) principles: top management commitment, flexibility, learning and awareness. The MASST was tested in a case study that was carried out in a supplier of the automotive industry. The SEGTRANS contemplates the organisation of a database with information on accidents, traffic conflicts, road system and urban zoning, in electronic spreadsheet programs and geographic information systems; a development of a mixed technique of evaluation of the road safety with data of accidents, traffic conflicts and road environment; a treatment of the data with the production of reports and thematic maps; system for definition of actions to be implemented seeking to reduce the accidents; and a process of evaluation of the adopted measures. Some of the main studies developed in this work included: definition of the traffic conflict technique, selection of variables and formation of a database of accidents with program assembly in Access for introduction of data, a mixed technique of evaluation of the road safety, an evaluation technique for car test and walking test and treatment of data in electronic spreadsheet programs and geographic information systems.

The role of legislation in driving good occupational health and safety management systems: A comparison of prescriptive based legislation.Kim, Jeong - Ah January 2004 (has links)
Countries seek to control exposure to hazardous substances and environments by the enactment of legislation. In the past thirty years, two major different approaches to occupational health and safety legislation have been devleoped by countries around the world. The performance-based legislative approach has been linked with the emergence of occupational health and safety management systems but no research has previously been done to determine whether or not the legislative approach taken by government influences the introduction or form of occupational health and safety management systems used by organisations. Similarly, although the reasons why Australia and other countries have moved to performance-based legislation have been explained in terms of social, political and economic factors that influenced the change, little research has been done on the effectiveness of this approach compared with the prescriptive approach of countries such as Korea. -I- The overall aim of this research is to develop a conprehensive understanding of the management of expusre to heavy metals in selected industries in Korea and Australia. The specific objectives of the study are to determine: The effectiveness of heavy metal exposure management in the fluorescent lamp manufacturing industry in Korea, and an Oral Health Service, and lead-risk workplaces in Queensland, Australia; The management of the legislative arrangements for health surveillance in Korea and Queensland, Australia; The characteristics of the occupational health and safety management systems that are in use in the heavy metal industries in Korea in Australia; and The effectiveness of prescriptive and performance based legislative systems in protecting the health and safety of workers in heavy metal based industries. Secondary analysis of biological monitoring data from 6 fluorescent lamp manufacturing companies (8 workplaces) in Korea was used to examine the extent of mercury exposure and the effectiveness of the health surveillance system in that country. A survey of dental workers in an oral health service in Queensland provided data on the extent of mercury exposure to the workforce and workers' attitudes to the management of occupational risks. The efficiency of the lead health surveillance in Queensland was examined by way of a questionnaire survey of lead designated doctors in the state. A survey of registered lead-risk companies and the oral health servies in Queensland, and 5 of the fluorescent lamp manufacturing companies in Korea provided data on the occupational health and safety management systems in place in these organisations. The health surveillance system for mercury exposed workers in Korea was found to have reduced the incidence of workers with biological levels of mercury above the Baseline Level from 14% in 1994 to 7% in 1999. Bilogical testing of dental workers in Queensland discovered no workers with biological levels of mercury approaching the Baseline Level and air monitoring failed to locate any areas where workers were likely to be exposed to levels approaching the Workplace Exposure Standard. The staff of the Oral Health Service were generally aware of the occupational health and safety management systems in place but only 43% felt that mercury management in the workplace effectively prevented exposure. The lead surveillance system in Queensland was found to be inadequately managed with approximately 37% of registered doctors no longer practicing in the field and their being no way for the government to collect reliable data on the extent of lead exposure in workplaces. The occupational health and safety management systems in the companies surveyed in Queensland and Korea were found to be influenced by the legislative arrangements in place in each of the locations. The Korean systems were more geared to meeting the regulatory requirements whereas the Queensland systems were geared more towards a risk management approach. However substantial differences were also noted depending on the size of the organisation in each case. Legislative arrangements in Korea and Queensland were found to provide reasonable protection from heavy metal exposure to workers however improvements in both systems are needed. The legislation was also found to influence the occupational health and safety management systems in place with performance-based legislation producing systems having a wide risk management focus while a narrower regulatory based focus was noted in Korea where more prescriptive legislation is in force. A confounding factor in the nature of the occupational health and safety management system in place is the size of the organisation and particular attention needs to be paid to this when legislative approaches are considered.

Abstract
The key purpose of this study was to understand and create a model of "safety leadership as a line supervisor's task" based on a wide range of theoretical material. At the same time, an integrated development project examined safety in a target organisation using action-analysis research methods, and this provided new knowledge about the role of the line supervisor as a safety leader. The present study is a scientific report covering the whole research project. The theoretical part of the research examined the background of a zero-accident program and sought to confirm whether it was possible to affect the actions of general management through a systematic and integrated safety program. The conclusion of this study is that the application of the concept of intangible assets is an effective strategy for achieving this end. From this point of view, the zero-accident program can be considered as a project that accumulates intangible assets. In this study special attention was paid to the communication skills of the line supervisor in handling safety management.
The theoretical part of the research posited a development model of total safety management, according to which the safety program was carried out in the form of a three-part project. The model also facilitated effective and authentic communication among the whole staff at the target organisation from the very beginning of the project. Furthermore, the model provided a basis for analysing and interpreting the results that were achieved. The development project focused on improving the level of communication regarding safety so as to support the work of the line supervisor. The basic element of safety management as a line manager's task was crystallised in the communication model created at the beginning of the research process. This model was a vital instrument of communication, committing all managers in the target organisation to a shared vision of the safety program.
The empirical part of the research provided a context bound, in-depth description of the process of realising a zero-accident program in a large organisation (900 employees). On the basis of the traditional lost time injury frequency rate (LTIF) it could be shown that the development project achieved its challenging goal. At the same time, it showed that thoroughly attending to the improvement of safety matters brings about results. On the basis of other ways of measuring safety performance (process-based performance indicators), there was a positive improvement in almost every case, but it was not usually possible to achieve the strict numerical goals.
This study offers new knowledge about the implementation of a long-term development project focused on total safety management, carried out on the basis of a uniquely tailored model. New knowledge was also provided regarding the role of the line supervisor in developing and implementing safety culture. The communication model gathers the basic elements of safety management for which the line supervisor is responsible (risk assessment of one's own work, advanced accident and near-accident investigation, shop floor safety meetings and advanced safety inspection) in a new way. The model clarifies the role of the line supervisor as a safety leader, and at the same time it functions as a concrete tool for enhancing the quality of the line manager's leadership regarding safety. / Tiivistelmä
Tämän väitöskirjatutkimuksen keskeisin tavoite oli lisätä ymmärrystä ja mallintaa ilmiötä "turvallisuuden johtaminen esimiestyönä" laajan teoreettisen aineiston pohjalta. Samalla analysoitiin toiminta-analyyttisen tutkimusotteen keinoin kohdeorganisaatiossa toteutettua kokonaisvaltaista turvallisuuden kehittämishanketta ja tuotettiin uutta tietoa esimiehen turvallisuusroolista turvallisuuden johtamisessa. Tämä väitöskirja on koko tutkimushankkeen tieteellinen raportti. Teoriaosassa haettiin syvempää ymmärrystä nolla-tapaturmaohjelman toteuttamisen taustaksi sekä vahvistusta käsitykselle, että systemaattisella ja kokonaisvaltaisella turvallisuusohjelmalla voidaan parhaimmillaan edistää muutakin johtamistoimintaa. Tarkastelussa päädyttiin soveltamaan aineettoman pääoman -käsitettä, jonka näkökulmasta nolla-tapaturmaohjelmaa voidaan selvästi pitää myös yrityksen aineettoman pääoman kartuttamishankkeena. Tässä tarkastelussa korostui erityisesti kommunikatiivisen osaamisen merkitys esimiehen turvallisuuden johtamisessa.
Teoriaosan kautta luotiin Turvallisuuden johtamisen kehittämismalli, minkä mukaisesti turvallisuusohjelma toteutettiin kolmena toisiaan läheisesti tukevana osaprojekteina. Malli mahdollisti myös ohjelman tehokkaan ja uskottavan viestinnän koko kohdetehtaan henkilöstölle heti kehittämishankkeen käynnistysvaiheesta lähtien. Lisäksi mallin avulla myöhemmin arvioitiin ja tulkittiin toteutuneita tuloksia. Kehittämishankkeessa keskityttiin siis erityisesti esimiestoimintaa tukevaan turvallisuuskommunikoinnin tehostamiseen. Asia kiteytettiin tutkimusprosessin alkuvaiheen aikana turvallisuuden johtamisen esimiestyön peruselementeistä muodostettuun Kommunikointimalliin. Tämä malli toimi kehittämishankkeen tärkeimpänä kommunikaatiovälineenä, jonka avulla turvallisuusohjelman visioon pystyttiin sitouttamaan koko kohdetehtaan esimieskunta.
Empiirinen osa tuotti kontekstisidonnaisen, syvällisen prosessikuvauksen yhden varsin laajan tehtaan (noin 900 henkilöä) nolla-tapaturmaohjelman toteuttamisesta ja siihen liittyvästä analyysistä. Perinteisen tapaturmataajuusmittarin perusteella voidaan todeta kehittämishankeen onnistuneen saavuttaessaan sille asetetun varsin haasteellisen tavoitteen ja samalla osoittaneen, että perusteellinen paneutuminen turvallisuustoiminnan parantamiseen tuottaa tulosta. Muiden mittareiden osalta kehitystä myönteiseen suuntaan tapahtui lähes kaikkien prosessi- ja johtamisjärjestelmäpohjaisten mittareiden osalta, mutta varsinaiset numeraaliset tavoitteet niissä jäivät yleensä saavuttamatta.
Tutkimuksen avulla saatiin uutta tietoa turvallisuuden johtamisen kehittämisen pitkäkestoisen kehittämishankkeen läpiviennistä, johon kehitettiin turvallisuuden johtamisen kehittämismalli. Uutta tietoa saatiin myös esimiehen roolista ja toiminnasta turvallisuuskulttuurin kehittämisessä ja kehittämisprosessin läpiviennissä. Kommunikointimalliin koottiin turvallisuuden johtamisen esimiestyön peruselementit (oman työn riskien arviointi, vaaratilanteiden ja tapaturmien tutkiminen, turvallisuustuokioiden pitämien sekä kehittynyt työpaikkatarkastustoiminta) uudella tavalla. Malli selkeyttää linjaesimiehen roolia turvallisuustyössä ja se toimii samalla konkreettisena työkaluna linjaesimiesten johtajuuden ja turvallisuusjohtajuuden tarkastelussa.

EXPLORING THE STATE OF SMS PRACTICES FOR COMMERCIAL UAS OPERATIONS AT AIRPORTSPratik Jadhav (12456546) 25 April 2022 (has links)
<p>Safety Management Systems (SMS) in the aviation industry is increasingly an essential aspect of identifying hazards and managing the associated risks. While SMS has become commonplace and is often a regulatory requirement for air carriers, it remains voluntary for many other aviation service providers such as airports. Over the past decade, commercial UAS operations have significantly increased, leading to safety and economic challenges for airports. This research studied the current state of SMS and commercial UAS operations at airports. This research utilized a mix of quantitative and qualitative methods, which included an extensive literature review, interviews, and a survey of airport stakeholders. The literature review confirmed an increase in UAS hazards and risks within the airport operating area coupled with immature SMS practices that address these UAS operations. To build on the findings from the review of literature, a survey instrument was developed, distributed to airport stakeholders, and the responses were statistically analyzed. To gain greater insight into these findings, researchers interviewed three airport subject matter experts. The study compared the airports current state of SMS with UAS operations, the airport stakeholder’s level of familiarity with related policies, and their need for additional UAS SMS guidance material or training. Research results suggest a need for further development and adoption of robust SMS practices at airports along with education and training. This study may assist airport stakeholders, UAS operators, and regulators to further develop robust safety and risk management practices that support safe UAS operations within the airport operating area.</p>
