Context-Enriched Conversation Analysis of Relational Hypnotherapy with a Client Diagnosed with a Phobia of Blood and Needles

Although clinical hypnosis has been studied in a variety of ways, most of the research has focused on individual and evidence-based approaches; few have examined relational or systemic models. Influenced by Milton Erickson’s hypnosis methods and Gregory Bateson’s systemic concepts, relational hypnotherapists value the importance of both the intra- and interpersonal context in the treatment of problems, accentuating the significance of the mind and body connection (or relationship) in inviting non-volitional therapeutic change. The author of this research explored how Douglas Flemons, the developer of relational hypnosis, facilitated an enduring non-volitional shift with a client, “Grace,” who desired to have a baby but could not see or talk about blood, needles, or medical procedures without fainting. Using context-enriched conversation analysis (CECA), the author embraced his theoretical understanding of relational hypnosis as a guide to examine multiple sources of data, which included selected audio-recorded excerpts from Douglas and Grace’s hypnotherapeutic sessions; Grace’s descriptions of change in her email correspondence with Douglas; and Douglas’s case notes. Although there were a total of eight sessions, the author’s analysis revealed that the most influential and significant moments occurred during the first two sessions. Douglas’s initial interventions, or as he would say, intraventions, laid the foundation for a shift in Grace’s identity, which helped her embrace a variety of resourceful skills and attributes to overcome her problem. The author also discussed the clinical and research implications for relational hypnosis, brief and family therapy, and psychotherapy in general.

Incorporating Solution-Focused Group Therapy Into a Refugee Resettlement Agency: A Participatory Action Research Project with Stakeholders

Meeting the psychological needs of the culturally and linguistically different has always posed challenges to mental health providers. The Haitian community in South Florida has been one such group for whom mental health services have been less than readily available. Some reasons include a lack of trust from the community, a pejorative cultural framework of mental health services, as well as, a lack of competent Haitian Creole speaking therapists armed with culturally congruent therapeutic skills. These present as challenges for community based-agencies attempting to meet the mental health needs of this population. It is even more problematic for humanitarian voluntary agencies (VOLAG) that sponsor refugees and support them throughout their resettlement efforts. Additionally, the 2010 earthquake has given rise to gender based violence disproportionally impacting women and girls (Amnesty International, 2011) who are now seeking safe haven in the U.S. and particularly South Florida. Tasked with assisting in the resettlement of Haitian refugees/asylees/parolees, many psychologically affected by pre and post migration traumas, these agencies must find unique solutions to help their clients toward the ultimate goal of resettlement, self-sufficiency (Stenning, 1996). This Applied Clinical Project (ACP) showcases a community-university partnership with a VOLAG. Participatory Action Research (PAR) protocol was used as an explorative tool to learn from stakeholders about the efficacy of incorporating Solution-Focused Group Therapy (SFBT) in a resettlement integrative program for Haitians. Results will also contribute to the future development of a toolkit to support family therapists in adapting their western trainings to provide culturally and linguistically competent mental health services.

A Phenomenological Study of Black Fathers in Child Welfare

Each year thousands of children are removed from their homes and placed in foster care where they lose connections with their family, community, and friends. Coakley (2007) points out that children of color are overly represented within the child welfare system, and there is a lack of research on Black fathers and their involvement when their children become a part of the system. Studies have suggested that most families that encounter the child welfare system have adult males who are actively involved with their families, however, child welfare workers do not engage these men (Coady, Hoy, & Cameron, 2013). The purpose of this study was to examine the lived experiences of Black fathers who were previously involved in the child welfare system. Semi-structured interviews were completed with four Black fathers who were previously involved with the child welfare system. Five major themes emerged during the process of data analysis that illuminated the fathers’ experiences and gave meaning to their stories, including case worker attitude, services overload, intergenerational child welfare involvement, feelings of helplessness and willingness to comply. There were also several sub-themes for three of the larger themes. The findings are discussed in light of current literature about father involvement in child welfare, and Black fathers in particular. Implications of the findings are discussed for the field of family therapy, future research and practice. It is my hope that the information obtained from this study will help inform current practices within child welfare and the field of marriage and family therapy to more effectively engage Black fathers in the child welfare system.

An Exploration of the Lived Experience of Women who had Abortions and the Effects of the Abortion Secret on their Relationships: An Interpretive Phenomenological Analysis

The United States Supreme Court decision in the Roe v. Wade (1973) case provides women with a right to privacy and the liberty to make decisions concerning their reproductive lives. With this, women who become pregnant are offered the right to choose between keeping their pregnancy and terminating the pregnancy by way of abortion. Since Roe v. Wade (1973), many women have exercised their reproductive liberties, as evidenced by the termination of over 60 million pregnancies via legal abortion in the United States. Still, secrecy among women with a history of abortion remains a common phenomenon. Studies conducted on abortion and reasons for abortion are innumerable and literature on reasons women keep abortion secret are readily available. However, research lacks in the area of examining the lived experience of women with history of abortion secret. Thus, this research examines this phenomenon from women’s lived experiences and the effects of the secret on their relationship(s) when kept from at least one person(s) with whom the post-abortive women are/were in relationship. Purposive sampling was used to select five female participants for this study. To examine participants’ lived experience with abortion secret history as it relates to its effects on relationship(s), the researcher employed qualitative method Interpretative Phenomenological Analysis (IPA) in conjunction with Bowen Family Systems Theory approach. Seven superordinate themes emerged from the data analysis: self-sacrifice, emotional aftermath of abortion secret; secrecy as protection; dance of anxiety; effects on relationships; generational experiences; and, finding freedom. The collaborative use of IPA and Family Systems Theory provided an understanding of participants’ experiences, the effects of the secret on participants, as well as their emotional systems. The data collected added to the limited research available on this phenomenon providing space for post-abortive women’s secrecy experience to be heard.

Autoethnography of a Whitegirl Marriage and Family Therapist's Experience Working on the Rez

Despite the abundance of literature regarding potentially effective treatment modalities for Native American clients, researchers have been unable to identify an empirically proven effective treatment modality for this population. Common recommendations/considerations for therapists working with Native clients have been identified throughout literature; however, such findings were gathered by insiders (i.e., Native researchers or trained Tribal staff), which has left questions regarding the efficacy of such recommendations when applied by non-Native therapists. Due to Native American history, elaborate IRB requirements were put in place to ensure ethical research with this population but impedes the research process. I conducted an analytic autoethnography to explore my experiences of working on an Indian reservation as an outsider/Whitegirl marriage and family therapist. Experiences were explored contextually and explanatorily through a postmodern epistemology to determine similarities/differences to common recommendations/considerations in literature. Themes that emerged from the analysis of this study reflected common factors of psychotherapy and MFT rather than aspects of a specific MFT modality. This study contributes to the expansion of knowledge regarding effective practices and therapeutic considerations for Native American clients.

A Conversation Analysis of Therapist-Client Interactional Patterns in Single Session Therapy: A Researcher's Interpretation

In response to the growing awareness of the issue of accessibility to mental health services (World Health Organization, 2013), single session therapy (SST) has been implemented in various settings throughout the world. (Hoyt &Talmon, 2014b; Miller, 2008; Miller & Slive, 2004; Talmon, 2014). Although there has been much advancement in the knowledge and application of SST, an understanding of therapist-client interactional patterns that enfold in SST is extremely scarce. In this study, I investigated how therapists collaboratively improved the talk in SST turn by turn in such a way that promoted therapeutic improvement. I utilized conversation analysis (Sacks et al., 1974) to analyze a video-recording of a SST consultation within a single instrumental case study format (Stake, 2005). The findings of this study provide an interactional understanding of the collaborative practice, valued in SST literature (e.g., Campbell, 2012; Miller & Slive, 2004; Slive et al., 2008). Specifically, the therapists’ collaborative manner is exemplified in how the therapists oriented to the moment-to-moment interaction with the client within and across various interactional practices to coordinate their interaction, form and maintain the therapeutic relationship with the client, invite therapeutic change, and negotiate advice with the client. The findings of this study offer SST therapists and supervisors a potential interactional repertoire that they can utilize in their SST consultations and SST trainings. This study also presents a method of psychotherapy research that can address the research-practice gap (Strong & Gale, 2013).

Cooking with Couples: A Grounded Theory Study on the Relational Aspects Found in the Cooking Interactions of Couples

Cooking is a universal activity which all humans can relate to on some level. Historically, cooking has continued to connect people across cultures and time, simultaneously providing nutritive sustenance as well as socio- and psychological benefits. Medical and mental health practitioners only in recent years have utilized cooking in a therapeutic process, and most of those cooking activities available tend to focus on cooking from an occupational and nutrition-education stance. A gap in the literature pertaining to cooking and its therapeutic applications exists around the relational nature underlying the cooking process, especially as it pertains to couples. While marriage and family therapists have used a number of creative experiential modalities in therapy for years, such as art and music therapy, cooking has been especially underutilized in comparison, despite its therapeutic and relational applicability. Therefore, this study was conducted to offer a foundation for understanding how the interactions in a kitchen can highlight relational elements between people. Eight couples (16 participants) who have lived together for at least two years and who cook together often were interviewed in their homes. A constructivist grounded theory methodology was used for this study, and subsequent to data analysis, a three-part theory describing the relational components of couple’s cooking interactions emerged, called The Couple’s Cooking Triad. The theory is made up of Relationship Skills, Emotional Connections, and Languaging. Results from this study, organizing the complex interactions of couples in a kitchen, indicate further use by marriage and family therapists in an experiential therapeutic capacity.

Clinical Director Perspectives on Decision Making of Family Involvement with Clients at Inpatient Substance Abuse Treatment Centers

Literature supports that family members of individuals who abuse substances are significantly influential, whether it be positive or negative (Liddle et al., 2001). Evidence-based family therapy decreases substance use by adolescents (Slesnick et al., 2006). The purpose of this study was to gain the perspectives of clinical directors regarding decision making of family involvement at inpatient substance abuse treatment centers. Clinical directors were the focus of this study due to their experience, credentials, and their ability to oversee all clients and programs in a substance abuse treatment center. Purposeful sampling was utilized to obtain participants. Saturation was reached at three participants. The study used Thematic Analysis to analyze perspectives of clinical directors and identify themes between and among all participants. The data collection utilized were interviews with clinical directors. The importance of family involvement, factors related to choice of model, and evidence-based models preferred were the main themes discovered utilizing Thematic Analysis. Thematic Analysis exhibited all clinical directors in this study perceived family involvement ‛essential’ and all were not directly in control of decision making regarding family involvement with clients at inpatient substance abuse treatment centers.

Addiction and the Family: Substance Use as a Symptom of the Larger Emotional System

Traditional family therapy in the field of addiction primarily focuses on relapse prevention and psychoeducation. The lack of systems thinking in residential treatment facilities led to my desire to apply Bowen Family Therapy to a focus group in a residential treatment center. I used the following Bowen concepts: anxiety, differentiation of self, emotional cutoffs, and triangulation as a means to explore how addiction is a symptom of the larger emotional system of the family. I, co-facilitated a three hour group therapy session over 7 weeks with individuals in a treatment center. I addressed the following questions: RQ 1: What impact, if any did this program have on their life? RQ 2: What were the long-term effects of being in the program? RQ 3: Did participating in the group help to better understand resiliency? RQ 4: How does education on the family system impact an individual's recovery process and relationships in life? Through interviews, I followed up with clients three years later to look at the long-term effects of being in the 7-week program. This Applied Clinical Project focused on understanding resiliency and long-term effects on sobriety through a Bowenian lens. The themes that emerged focused on communication, boundaries, resiliency, relationships, and anxiety. The findings demonstrated that a multigenerational element in the study helped participants develop a way to maintain the Family Dynamics curriculum in their day to day life. The overarching theme is that healthy relationships with open communication lead to better anxiety management, resiliency, and boundaries which shows a foundation of which new approaches to substance abuse treatment can be found.
