Determinants of the Applications to the Institutional Care in Turkey: Darulaceze Example

Although institutional care has started to be outmoded in the developed countries with development of different models of care, it still has a considerable place in the developing countries such as Turkey. This is because, changes in the demographic structure, extended family, and urban development of Turkey has brought about several aging problems leading older adults to end up in institutions. Loneliness was one of the significant reasons given in the Social Inquiry Survey of Applicants of Darulaceze Old-Age Institution and the basis for a micro level analysis in this study. Therefore, the main objective of the study was to determine the predictors of loneliness, including age, the state of living alone, functional independence, education, and gender. Analysis of the results indicated that these predictors have significant effects on the loneliness predominantly defined by social factors rather than medical factors. In addition, the meso and macro level analyses were employed to control the micro level analysis and see a general picture of institutional care. Thus, an academic example of diagnosing the main reasons behind the institutional care was presented to understand the context of aging in Turkey.

The effect of socio-demographic, socio-economic and environmental factors on under-five mortality in South Africa: analysis of the 1998 South African Demographic Health Survey dataset

M.A. University of the Witwatersrand, Faculty of Humanities (Population Studies), 2012 / This study is based on secondary data analysis of the 1998 South African Demographic and Health Survey (SADHS) data set. The aim of the study was to identify socio-demographic, socio-economic and environmental variables that affect the survival of South African children under the age of five years. Descriptive analyses, frequency tables, Pearson’s chi-square tests of association and binary logistic regression analysis were used for data analysis in this study. Mothers who lost an under-five child were predominantly Black and rural. Such mothers were characterized by rural residential areas, relatively large family sizes, relatively poorer socioeconomic status, relatively poorer access to basic health services, relatively more child deliveries at home, and low level of education. The study showed that 269 of the 5, 066 children in the study died before celebrating their fifth birthday (5.31%). At the 5% level of significance, the survival of under-five children is significantly influenced by 2 of the 11 predictor variables found to be significantly associated in the univariate analysis and therefore included in the logistic regression analysis. These 2 predictor variables were: place of delivery of child [OR=0.97; P=0.000; CI = (0.96, 0.98)], and use of modern contraceptives by the mother [OR=0.73; P=0.002; CI = (0.59, 0.89)]. The study found that not using modern contraceptives gives a lower chance on death of a child under 5 as well as delivering at home, in the absence of a trained birth attendant. These findings are unexpected and contrary to what was found in the univariate analysis. No real explanation can be given for these findings and it would be interesting to see if the same results are found with more recent data. In order for the South African National Department of Health to fulfil its United Nations Millennium Development Goals, rural mothers and their under-five children must be provided with improved health as well as socioeconomic services.

Introdução: O perímetro cefálico (PC) é definido como o tamanho da cabeça da criança em sua maior área. O crescimento da caixa craniana está relacionado ao crescimento e volume cerebral e, por isso, deve ser rotineiramente medido e utilizado para seguimento individual, pois é um forte preditor do neurodesenvolvimento. Sabe-se que o PC é relacionado a condições ambientais e de saúde materna, e sua avaliação adquiriu grande importância após a recente epidemia de infecção pelo vírus Zika no Brasil e em outras partes do mundo, que se associou ao aumento da incidência de microcefalia. Objetivo: Avaliar a associação do perímetro cefálico de recém-nascidos com características sociodemográficas e hábitos maternos durante a gestação em duas coortes de nascimentos de Ribeirão Preto, SP, em 1978/79 e 2010. Método: foram analisados dados referentes a dois estudos de coortes de nascimento realizados em Ribeirão Preto, SP, um em 1978/79 e o outro em 2010. Foram incluídos recém-nascidos vivos de parto único hospitalar que tiveram a informação do PC ao nascer. Os dados foram coletados de forma semelhante nos dois momentos, utilizando questionário criado para o estudo e dados coletados nos prontuários. As variáveis utilizadas nas análises foram sexo do RN, restrição do crescimento intrauterino (RCIU), idade gestacional, idade e escolaridade maternas, tabagismo durante a gestação, situação conjugal, paridade, tipo de parto e tipo de hospital. Foram realizadas análises de regressão linear simples e múltipla para testar a associação da variável resposta \"perímetro cefálico\" com as variáveis independentes em cada ano, utilizando o pacote estatístico SAS 9.3. Resultados: Na coorte de 1978/79 participaram 2941 recém-nascidos e em 2010 participaram 7353. A média geral do PC em 1978/79 foi de 34,10 cm, um pouco menor do que a de 2010, de 34,29 cm. Nasceram mais meninos em 1978/79 e mais meninas em 2010. Excetuando a RCIU, as demais variáveis apresentaram mudanças na frequência nesse intervalo de tempo. A escolaridade materna aumentou, a proporção de mães adolescentes ficou estável, em torno de 12% e a de mães >=35 anos aumentou de 8,3% para 12,5%. Tabagismo materno na gestação se reduziu de 28,3% para 11,8% e a taxa de cesáreas mais que dobrou, de 27,5% para 58,6%. Todas as variáveis estudadas se associaram com o PC na análise não ajustada, exceto paridade em 1978/79. Nos dois momentos, as variáveis que foram mais fortes preditoras de maiores valores de PC foram sexo masculino e parto cesárea, e de menores valores de PC o tabagismo materno na gestação e RCIU. A RCIU foi o fator que mais impactou, sendo que essas crianças tiveram medida de PC reduzida em 1,30cm (IC95% -1,43; -1,18) em 1978/79 e em 1,74cm (IC95% - 1,87; -1,61) em 2010, quando comparadas com as nascidas sem restrição. The variables used in the analysis were gender of the newborn, intrauterine growth restriction (IUGR), gestational age, maternal age and schooling, smoking during pregnancy, marital status, parity, type of delivery and type of hospital. Simple and multiple linear regression analyzes were performed to test the association of the response variable \"head circumference\" with the independent variables in each year, using the statistical package SAS 9.3. Results: In the 1978/79 cohort, 2941 newborns participated and 7353 participated in 2010. The general average of the HC in 1978/79 was 34.10 cm, slightly lower than that of 2010, 34.29 cm. More boys were born in 1978/79 and more girls in 2010. Except for IUGR, the other variables showed changes in the frequency in that time interval. Maternal education increased, the proportion of adolescent mothers remained stable at around 12% and that of mothers >=35 years increased from 8.3% to 12.5%. Maternal smoking in pregnancy decreased from 28.3% to 11.8% and the cesarean rate more than doubled, from 27.5% to 58.6%. All variables studied were associated with HC in the unadjusted analysis, except for parity in 1978/79. At the two moments, the variables that were the strongest predictors of higher HC values were male sex and cesarean delivery, and lower values of HC were maternal smoking in pregnancy and IUGR. The IUGR was the factor that most impacted, and these children had a HC reduced by 1.30 cm (95% CI -1.43; -1.18) in 1978/79 and 1.74 cm (CI 95% -1.87; -1.61) in 2010 when compared to those born without restriction. Conclusion: Maternal characteristics during gestation, although they have improved over the years, still need attention. Many women still lack information and prenatal care. Efforts should be made for early detection and intervention to reduce the rates and effects of IUGR.

Also, a general comparison will be made between the results of the present study, based on the 1972 and 1973 surveys, with those from an earlier round of surveys conducted in 1969 and 1970.

Social support, coping, and self-esteem in relation to psychosocial factors : A study of health issues and birth weight in young mothers in Tehran, Iran

Introduction: Generally, pregnancy is considered to be a positive period in life in Iranian culture. For the parents, it is important to have a healthy pregnancy and, as a result, a healthy child. A sufficient birth weight of the infant represents one of the crucial conditions of a healthy development of a child during infancy as well as later in life. Ongoing research has been carried out regarding various medical factors related to birth weight, but there is a gap in knowledge about psychosocial factors such as social support, coping, self-esteem, stress and mother’s mental health, and various socio-demographic factors including domestic violence, which may lead to adverse pregnancy outcomes such as low birth weight. This thesis aims to provide knowledge to fill this gap. Methods: A cross-sectional survey was conducted in Tehran, Iran, including 600 young mothers who had delivered in Akbarabadi hospital, one of the main gynaecological hospitals affiliated with Tehran University of Medical Sciences. The investigation included a self-developed socio-demographic form, the Social Support Questionnaire, the Ways of Coping Checklist, Rosenberg’s Self-Esteem Scale, the General Health Questionnaire-12, and a Life Event Checklist. Results: We could not find a significant association between birth weight and mother’s level of education, and there was no substantial relationship between general mental health and birth weight. Verbal abuse was reported by 26.0% of the young mothers, 4.8% reported physical abuse, 5.5% reported sexual abuse, and 1.3% reported all three types of abuse. The abuse-index was significantly negatively associated with satisfaction with social support and with self-esteem. The higher the abuse-index, by trend, the lower was the infants’ birth weight. Weight before pregnancy, current weight, weight gain during pregnancy, and the number of prenatal care visits were significantly positively associated with the weight of the newborn. Mothers who reported having a history of a low birth-weight (LBW) child or were physically abused during pregnancy had infants with significant lower birth weight. The more the pregnant women were satisfied with their social support and the more often they used positive reappraisal as a way of coping, the higher was their infants’ birth weight. The higher the self-esteem, the less often they used escape avoidance and confrontive coping. Conclusion: The results suggest the importance of relationships between a healthy pregnancy and psychosocial as well as socio-demographic factors. Providing pregnant women with social support is a key component for a healthy pregnancy, especially when faced with stressful situations. The number of people available for support did not provide a significant buffering effect on domestic violence (DV), but the perceived quality of social support did. Higher education in the mother and husband, and women’s employment represented protective conditions against the occurrence of DV. Women who reported physical abuse during pregnancy had infants with lower birth weight. Satisfaction with social support and use of positive reappraisal were significantly associated with higher birth weight.

The Relationship of Breast and Gynecological Cancers with Smoking and Metabolic Syndrome - An Examination of NHANES Data 2001 - 2010

Background: Breast and Gynecological cancers are a major public health problem. Smoking is associated with several chronic diseases including cancer. Other lifestyles of public health predispose many people to dyslipidemia, hypertension and obesity; risk factors for metabolic syndrome, and are associated with cancer. Objectives: The purpose of this study is to find if those who smoke, and have the metabolic syndrome, are more likely to have breast or gynecological cancers, and to find the distribution by education, having health insurance, race/ethnicity and socio-economic status. Methods: A case-control study of females aged 20 years and above who participated in the United States National Health and Nutrition Examination Survey (NHANES) 2001-2010. Results: Females who have smoked more than hundred cigarettes in life and still smoke; a) have a 42 percent less chance of having a breast cancer diagnosis (OR 0.58; 95% CI 0.36 – 0.93, p-value 0.025), and b) are 2.67 times as likely to report a cervical cancer diagnosis as females who have smoked less than hundred cigarettes in life (OR 2.67; 95% CI 1.72 – 4.13, p-value Conclusion: Smoking and metabolic syndrome are very important indicators of reproductive health and needs further study. Smoking cessation interventions should be an integral part of cervical cancer prevention programs especially targeted at younger females and females who live below the federal poverty level.

Pastaruoju metu visuomenėje gana plačiai nagrinėjama psichikos ligomis sergančių žmonių stigmatizacijos ir diskriminacijos problema. Stigmatizuojantis požiūris, susiformavęs istorijos kontekste palaiko stereotipus, kurie neigiamai paveikia psichikos ligomis sergančių asmenų psichosocialinę egzistenciją. Tyrimo tikslas – nustatyti tiriamų asmenų požiūrį į psichikos ligomis sergančius žmones bei įvertinti sąsajas su sociodemografinėmis tiriamųjų charakteristikomis. Tyrime dalyvavo 227 Kauno ir Alytaus miestų gyventojai (65 vyrai ir 162 moterys). Požiūriui į psichikos ligomis sergančius asmenis nustatyti naudota Didžiosios Britanijos Nacionalinio statistikos departamento sudaryta anketa, „Visuomenės požiūrio į psichikos negalią turinčius žmones skalė“ sutrumpinta forma (angl. Community Living Attitudes Scale Mental Retardation – Short Form (CLAS)) bei tyrimo autorės sudaryta anketa. Tyrimo rezultatai atskleidė vyraujančią nuomonę, jog psichikos ligomis sergantys asmenys yra pavojingi, neprognozuojamo elgesio, dažnai jų bijoma. Tačiau šie žmonės nesmerkiami, jiems jaučiama užuojauta ir gailestis. Pusė apklaustųjų manė, kad psichikos ligomis sergantiems asmenims naudinga integruotis į visuomenę, tačiau tik vienas trečdalis tiriamųjų sutiko, kad psichiatrinės ligoninės – pasenęs gydymo būdas. Atsižvelgiant į tiriamųjų lytį bei išsilavinimą, didelių požiūrio skirtumų į psichikos ligomis sergančius asmenis nebuvo nustatyta. Tačiau stebimi reikšmingi požiūrio skirtumai atsižvelgiant... [toliau žr. visą tekstą] / Nowadays one of the widely discussed topics is the stigmatization and discrimination of people with mental illness. Stigmatization is based on the stereotypes and negatively reflects on the psychosocial life of people with mental illness. The goal of this research is to define attitudes towards people with mental illness and find correlations with socio-demografic characteristics. 227 respondents from Kaunas and Alytus participated in this study (65 men and 162 women). The following questionnaires were used to assess the respondents attitudes towards people with mental illness: a questionnaire, taken from National Department Statistics of Great Britain, the Community Living Attitudes Scale Mental Retardation – Short Form (CLAS) and a questionnaire made by the author of this study. The results of this study showed the basic attitude that people with mental illness are dangerous. Their behaviour is regarded as unpredictable and they are feared of. However, people with mental illness can expect sympathy and compassion. Half of the respondents admitted that they thought it would be useful to integrate people with mental illness into society, but, on the other hand, only one third of the respondents agreed that mental hospitals are not quite good for people with mental illness.
