Orientador: Rui Seabra Ferreira Junior / Resumo: Introdução: A dor lombar crônica representa uma doença que gera grandes custos sociais e financeiros. A síndrome facetária é uma possível etiologia para a dor lombar, porém de difícil diagnóstico e tratamento. Objetivo: Avaliar se determinados critérios clínicos e de exames de imagem foram capazes de identificar pacientes com a síndrome facetária e confirmar o diagnóstico por meio da infiltração realizada sob escopia em centro cirúrgico. Pacientes e métodos: 38 pacientes de ambos os sexos atendidos no Ambulatório de Cirurgia da Coluna – Ortopedia do Hospital das Clínicas da Faculdade de Medicina de Botucatu, tiveram o exame físico específico avaliado, e seus dados gerados a partir da Escala Visual Analógica da Dor; Questionário de Incapacidade de Oswestry; Qualidade de Vida EuroQoL-5D-5L e SF-36; e DRAM (Método de Avaliação de Sofrimento e Risco) no dia 1 e 2 meses após a infiltração. Trinta pacientes tiveram seus exames radiográficos avaliados; vinte e quatro pacientes tiveram seus exames de ressonância magnética nuclear avaliados; e sete pacientes tiveram seus exames de tomografia computadorizada avaliados. O processo degenerativo das estruturas da coluna foram classificados. Todos os pacientes foram submetidos a infiltração facetária com 1 mL de levobupivacaina 0,5% e 50 mg de metilprednisolona (1 mL). The facet syndrome is one of the possible etiologies for low back pain, but its diagnostic and treatment remains intricate. Objective: To evaluate whether certain clinical and imaging criteria were able to identify patients with facet syndrome and confirm the diagnosis with injections performed in the operation room. Patients and methods: 38 patients of both genders attended at the Spine Surgery Ambulatory from Botucatu Medical School Clinical Hospital, had a specific physical examination evaluated, and were assessed using the Visual Analog Pain Scale; Oswestry Disability Index; EuroQoL-5D-5L and SF-36 Health Survey; and DRAM (The Distress and Risk Assessment Method) on day 1 and after 2 months. Thirty patients had their computed radiographs evaluated; twenty-four patients had their magnetic resonance imaging evaluated; and seven patients had their CT scans evaluated. The degenerative processes of the spinal structures were graded. All patients underwent facetary injections with 1 mL 0.5% levobupivacaine and 50 mg methylprednisolone (1 mL). Lesões geriátricas típicas acomentem quadril, joelho e coluna. Procedimentos cirúrgicos nesta população são de alto risco de morbimortalidade e há falta de dados nacionais sobre esses desfechos. O objetivo primário deste estudo foi descrever e analisar os eventos adversos associados ao tratamento cirúrgico para correção de fratura de fêmur, para coxartrose, para doenças da coluna vertebral e para artroplastia total de joelho, com particular atenção para taxas e causas específicas de mortalidade. Métodos: foi realizado estudo prospectivo, observacional, para avaliar as complicações perioperatórias em pacientes submetidos à cirurgia para correção de doenças de quadril, joelho e coluna. Patients undergoing these surgical procedures are at high risk of postoperative complications. At present, however, we lack robust data on postoperative outcomes for the most frequent performed musculoskeletal surgical procedures in Brazil. Accordingly, we aimed to describe and analyze adverse events associated with musculoskeletal surgical procedures, with focus on cause-specific mortality. Methods: we propose a prospective observational clinical trial using data from a preoperative questionnaire and from the electronic medical records of the Clinics Hospital of Botucatu Medical School, Botucatu, Sao Paulo, Brazil. Patients admitted to the hospital for surgical procedures performed for hip fracture, and for hip, knee, and spinal surgeries were enrolled in the study. Measures of health care use associated with the principal surgical procedure included length of hospital stay, readmission to hospital, and mortality. Postoperative complications were evaluated at moments within the first postoperative year (one week, one month, four months, and one year after surgery). As análises estatísticas foram realizadas por meio do software IBM SPSS STATISTICS V.21. Resultados: foram arrolados 400 pacientes candidatos a participar do estudo. Contudo, 38 pacientes foram excluídos por diferentes razões. Entre os 362 pacientes incluídos, 54 foram submetidos ao tratamento cirúrgico de coxartrose, 210 à cirurgia para correção de fratura de fêmur (FF), 10 à artroplastia total de joelho (ATJ) e 88 foram submetidos à cirurgia de coluna. A maioria dos pacientes submetidos à ATJ eram do sexo feminino (80,0%), apresentaram média de idade de 69,0±9,9 anos, 90% foram classificados como ASA I ou II e 60% são hipertensos. Em relação às complicações pós-operatórias, um paciente apresentou distúrbio hidroeletrolítico, lesão renal aguda (LRA) e pneumonia e outro apresentou depressão no acompanhamento de um ano de pós-operatório. Não houve óbito durante o seguimento. Dos pacientes submetidos à cirurgia de coluna, 52,3% eram do sexo masculino. A média de idade foi de 62,5±10,3 anos e 79,5% eram ASA III ou IV. Em relação às comorbidades prévias à cirurgia, 45,5% eram hipertensos e 25% diabéticos. A complicação pós-operatória mais frequente neste grupo foi distúrbio hidroeletrolítico (25,0%). A taxa de mortalidade foi de 13,6% em 1 ano, tendo com principal causa de óbito infecção seguida de choque séptico (41,7%). Pacientes submetidos à cirurgia de coluna que apresentaram ASA ≥ III cursaram com menor sobrevida em um ano. Metade dos pacientes submetidos à cirurgia para coxartrose eram do sexo feminino. A média de idade foi de 66,3 ± 8,3 anos e 88,9% eram ASA I ou II. Em relação às comorbidades prévias à cirurgia, 64,9% hipertensos, 22,2% hipotireoideos e 14,8% diabéticos. As complicações pósoperatórias mais frequentes foram depressão (5,6%), distúrbio hidroeletrolítico (5,6%) e pneumonia (5,6%). Nenhum paciente faleceu no seguimento de um ano. Em relação à cirurgia para correção de FF, 73,3% dos pacientes eram do sexo feminino. A média de idade foi de 75,2 ± 12,5 anos, 50,5% dos pacientes eram ASA III ou IV. Em relação às comorbidades, 42,4% eram hipertensos e 17,6% eram diabéticos. As complicações pós-operatórias mais frequentes neste grupo foram alterações eletrolíticas (18,6%), alterações cognitivas (12,4%), pneumonia (10,5%) e infecção do trato urinário (10,0%). Sessenta indivíduos do grupo de pacientes submetidos à cirurgia para correção de FF foram a óbito (taxa de mortalidade = 28,6%), tendo como causa principal a infecção seguida de choque séptico (31,7%). Pacientes submetidos à cirurgia para tratamento de FF que apresentaram idade ≥ 78 anos, ASA ≥ III, delirium hipoativo pré-operatório, alteração cognitiva pós-operatória, distúrbio hidroeletrolítico pós-operatório e LRA pós-operatória cursaram com menor sobrevida em um ano. Após análise de regressão logística, obteve-se que, com o aumento de cada ano na idade, há aumento correspondente de 4,1% da razão de chanches de óbito nos pacientes com FF. Pacientes com FF classificados como ASA III ou IV apresentaram razão de chances de óbito aumentada em 2,0 vezes quando comparados aos ASA I ou II. A presença de delirium hipoativo no pré-operatório aumentou em dezoito vezes a razão de chances de óbito nestes pacientes, assim como a presença de distúrbio hidroeletrolítico pós-operatório aumentou em 8,4 vezes a razão de chances de óbito. Conclusões: as cirurgias de quadril, joelho e coluna na população estudada relacionaram-se com alta prevalência de complicações pósoperatórias. Não houve óbitos relacionados à cirurgia de ATJ ou ao tratamento de coxartrose. As complicações perioperatórias mais frequentes em todos os grupos foram os distúrbios hidroeletrolíticos. A taxa de mortalidade encontrada na população de FF no primeiro ano de seguimento foi de 28,6%, enquanto a taxa de mortalidade nos pacientes submetidos à cirurgia de coluna foi de 13,6%. Pacientes submetidos à cirurgia de coluna classificados como ASA ≥ III apresentaram menor sobrevida em um ano. Idade avançada, ASA ≥ III, delirium hipoativo pré-operatório, alteração cognitiva pós-operatória, distúrbio hidroeletrolítico pós-operatório e LRA pós-operatória foram preditores de menor taxa de sobrevida em um ano no pós-operatório de pacientes submetidos à cirurgia para correção de FF. Results: through the electronic medical record system, 400 patients were enrolled to participate in the study (97 of spine, 10 of total knee joint arthroplasty (TKA), 54 of coxarthrosis and 239 of femoral fracture (FF). However, 9 individuals of the spine were excluded from the study due to preoperative death (1) and conservative treatment (8), and 29 patients from FF were excluded from preoperative death (17), non-surgical conservative treatment (1), they underwent pelvic fracture correction surgery (5), they did not accept to participate in the study (4) or the surgery was performed in another service (2). Among the 362 patients, 54 were submitted to surgical treatment of coxarthrosis, 210 to surgery for correction of femoral fracture (FF), 10 to total knee arthroplasty due to arthrosis in the joint (TKA) and 88 underwent spinal surgery. The majority of patients submitted to TKA were female (80.0%). Mean age of 69.0 ± 9.9 years, 90.0% are ASA I or II, 60.0% are hypertensive. 1 patient presented hydroelectrolytic disturbance, acute renal injury (AKI) anda pneumonia and 1 presented depression at 1-year post-operative follow-up. There was no death during the follow-up. Of the patients in the spine, 52.3% were male. The mean age was 62.5 ± 10.3 years, 79.5% were ASA III or IV. 45.5% hypertensive, 25.0% diabetic. The most frequent postoperative complication was hydroelectrolytic disturbance (25.0%). The mortality rate was 13.6% at 1 year, with the main cause of death being infection followed by septic shock (41.7%). Patients submitted to spine surgery who presented ASA ≥ III had a shorter survival in one year. Half of the patients undergoing surgery for coxarthrosis were female. The mean age was 66.3 ± 8.3 years, 88.9% ASA I or II, 64.9% hypertensive, 22.2% hypothyroid, 14.8% diabetic. The most frequent postoperative complications were depression (5.6%), hydroelectrolytic disturbance (5.6%), and pneumonia (5.6%). No patient died in 1 year. 73.3% of the patients submitted to FF correction surgery were female. The mean age was 75.2 ± 12.5 years, 50.5% ASA III or IV, 42.4% hypertensive and 17.6% diabetic. The most frequent postoperative complications in FF patients were electrolytic alterations (18.6), cognitive alterations (12.4%), pneumonia (10.5%) and urinary tract infection (10.0%). Sixty patients from the group of patients submitted to FF correction surgery were submitted to death (mortality rate = 28.6%). The main causes were undetermined (40.0%) and infection followed by septic shock (31.7%). Patients with ASA ≥ III, aged ≥ 78 years, with preoperative hypoactive delirium, with postoperative cognitive alteration, with postoperative hydroelectrolytic disturbance and acute postoperative renal injury were found to be the lowest survival rate in one year of patients undergoing FF. After performing a logistic regression analysis with the data of the patient’s submitted to surgical treatment for FF correction, it was obtained that with the increase of each year in age, there is a corresponding increase of 4.1% of the death rate ratio. Patients classified as ASA III or IV presents the odds ratio for death increased by 2.0 times when compared to ASA I or II. The presence of hypoactive delirium preoperatively increases the odds ratio of death by eighteen times. The presence of a postoperative hydroelectrolytic disorder increases the odds of death by 8.4 times. Conclusions: the hip, knee and spine surgeries in the study population were related to a high prevalence of postoperative complications. There were no deaths related to TKA or to treatment of coxarthrosis. The most frequent perioperative complications in all groups were hydroelectrolytic disorders. The mortality rate found in the FF population in the first year of follow-up was 28.6%, while the mortality rate in patients undergoing spinal surgery was 13.6%. Advanced age, ASA ≥ III, preoperative hypoactive delirium, postoperative cognitive alteration, postoperative hydroelectrolytic disturbance and acute postoperative renal injury were predictors of lower postoperative survival rate of FF. The increase of each year in age, patients classified as ASA III or IV, the presence of hypoactive delirium in the preoperative period and the presence of hydroelectrolytic postoperative disorder increase the odds ratio of death for patients undergoing surgery for correction of FF.

Previous issue date: 2012 / Resumo: Modelo de Estudo: Estudo diagnóstico Transversal de caso controle. Objetivos: comparar pacientes com estenose do canal lombar com indicação de cirurgia e um grupo controle, assintomáticos para esta doença, levando em consideração imagens de ressonância magnética (RM) e o questionário de Oswestry. Material e Método: Vinte e três pacientes com estenose do canal lombar foram comparados com um grupo controle de 17 indivíduos. Todos foram submetidos a RM e responderam ao questionário de Oswestry. Medidas qualitativas foram descritas segundo os grupos com uso de frequências absolutas e relativas e, verificada a existência de associação dessas medidas com uso do teste exato de Fisher. Medidas quantitativas foram descritas e comparadas entre os grupos com uso do teste Mann-Whitney. O teste de Spearman foi utilizado para avaliar a correlação entre os grupos. Resultados: Lombalgia foi a queixa mais frequente nos dois grupos. O índice de Oswestry mostrou média de percentual de invalidez de 45,69% no Grupo Estenose e 11,60% no Grupo Controle. A RM mostrou que a área de secção transversa do saco dural, o diâmetro do canal e a avaliação dos forames e recesso lateral estavam alterados em ambos os grupos. Conclusões: Não houve relação entre o grau de estenose do canal lombar mensurado em exame de RM com o índice de Oswestry em ambos os grupos / Abstract: Study design: Transversal Case-control, diagnostic study. Objectives: To verify the relationship between the degree of lumbar spinal canal stenosis (LCS), as seen in magnetic resonance imaging (MRI), and the severity of disability as seen in the Oswestry Index, in patients with LCS compared to controls without a diagnosed LCS. Methods: Twenty-three patients with a previous diagnosis of LCS were compared with a control group of 17 volunteers. All participants underwent MRI and answered the Oswestry questionnaire. Qualitative data were described according to the groups with the use of absolute and relative frequencies, and the association of these measures was checked by using the Fisher exact test. Quantitative measures were described and compared between groups using the Mann-Whitney test. Spearman's test was used as well. Results: Low back pain was the most frequent complaint in both groups. The Oswestry index showed average percentage of disability of 45.69% in patients with stenosis and 11.60% in the control group. MRI revealed that the dural sac cross-sectional area (DCSA), the diameter of the canal and the evaluation of lateral recesses and foramina were equally changed in both groups. T1rho MRI in brain aging, lumbar disc degeneration, and liver fibrosis: clinical and experimental studies.January 2013 (has links)
T1rho弛豫是旋轉坐標系中的自旋晶格弛豫,它決定橫向磁化向量在存有自旋鎖定射頻脈衝情況下的衰減,自旋鎖定脈衝與橫向磁化向量同向。T1rho磁共振成像對於低頻運動過程敏感,故可研究水與其周大分子物質環境間的交互作用,有鑒別組織內早期生化改變的潛力。 / 衰老與慢性高血壓是常見腦退行性疾病的兩個主要危險因素。但是正常腦衰老過程及慢性高血壓兩個因素與腦組織T1rho是否有相關性,尚缺乏研究。序貫性測量SD老鼠自5至15月齡、WKY(血壓正常)和SHR(患有自發性高血壓)老鼠自6至12月齡的雙側丘腦、海馬、和皮質的腦組織T1rho值。發現三組老鼠的丘腦、海馬及皮質的T1rho均隨年齡增長而增高;且SHR的顯著高於WKY老鼠。 / T1rho值與椎間盤退變等級的相關性已有報導。但相比T2值,T1rho在評價椎間盤退變方面是否優於或如何優於T2值尚缺乏研究。將椎間盤髓核及纖維環的T1rho和T2值與5級和8級椎間盤退變等級系統做比較;發現髓核的T1rho及T2與椎間盤退變等級的相關性均呈二次函數降低,且無顯著差別(P=0.40)。纖維環的T1rho及T2與椎間盤退變等級的相關性呈線性函數降低,T2降低的斜率明顯比T1rho降低的斜率要平坦(P<0.001)。故T1rho值比T2值更加適合評價纖維環退變,而兩者在評價髓核時相似。 / 肝纖維化是幾乎所有慢性肝病的常見特徵,包括大分子物質在細胞外基質的沉積。選用四氯化碳CCl4腹腔注射6周來製造肝纖維化模型。肝臟T1rho在注射後的第二天輕度上升,然後持續上升,直到注射六周後T1rho達最高值,此後T1rho隨CCl4注射停止而降低。顯示T1rho磁共振成像對於監測慢性注射CCl4誘導的肝纖維化及肝損傷有價值。當沒有明顯肝纖維化時,肝T1rho輕微受水腫及急性炎症的影響。 / 為將肝臟T1rho磁共振成像轉化到臨床使用,我們研究了其可行性,以及正常志願者肝臟T1rho值分佈範圍。發現採用六個自旋鎖定時間來測量健康志願者肝T1rho,結果有較高的可重複性和一致性,肝T1rho平均值為42.5ms,分佈範圍為38.8到46.5ms。採用三個自鎖鎖定時間點掃描,可以減少一半掃描時間,且可以得到可信的肝T1rho值,但採用兩個自旋鎖定時間點則不行。 / T1rho relaxation is spin-lattice relaxation in the rotating frame. T1rho MRI is sensitive to low frequency motional processes, so it can be used to investigate the interaction between water molecules and their macromolecular environment. T1rho imaging is suggested to have the potential to identify early biochemical changes in tissues. / Aging and chronic hypertension are two major risk factors for common neurodegenerative disease. However, whether normal brain aging and chronic spontaneous hypertensive are associated with brain T1rho values changes were not reported. We longitudinally measured the T1rho value in rat brain of Sprague-Dawley (SD) rats from 5-month to 15-month, and spontaneous hypertensive rats (SHR) with Wistar Kyoto (WKY) rats from 6-month to 12-month. The T1rho values in three brain regions of thalamus, hippocampus, and cortices increased with aging process, and were significantly higher in SHR than WKY rats. / For intervertebral disc, the correlation between T1rho and degenerative grade has been reported. However, whether and how T1rho specifically offer better evaluation of disc degeneration compared with T2 was not studied previously. T1rho and T2 value of nucleus pulposus (NP) and annulus fibrosus (AF) was compared with reference to the five-level and eight-level semi-quantitative disc degeneration grading systems. For NP, T1rho and T2 decreased quadratically with disc degeneration grades and had no significant trend difference (P=0.40). In NP, T1rho and T2 decrease in a similar pattern following disc degeneration. For AF, T1rho and T2 decreased linearly and the slopes of T2 were significantly flatter than those of T1rho (P<0.001). Therefore, the T1rho is better suited for evaluating AF in degenerated disc than T2. / Liver fibrosis, a common feature of almost all causes of chronic liver disease, involves macromolecules accumulated within the extracellular matrix. Male Sprague-Dawley rats received intraperitoneal injection of 2 ml/kg CCl4 twice weekly for up to 6 weeks. Then CCl4 was withdrawn for recovery. The liver T1rho values increased slightly on day 2, then increased further and were highest at week 6 post CCl4 insults, and decreased upon the withdrawal of the CCl4 insult. This study demonstrated that T1rho MRI is a valuable imaging biomarker for liver injury and fibrosis induced by CCl4. Liver T1rho value was only mildly affected by edema and acute inflammation when there was no apparent fibrosis. / To translate liver T1rho MRI to clinical application, the technical feasibility of T1rho MRI in human liver was explored and the normal range of T1rho values in healthy volunteers was determined. We found it is feasible to obtain consistent liver T1rho measurement for healthy human liver with six spin-lock time (SLT) points of 1, 10, 20, 30, 40, and 50ms; the mean liver T1rho value of the healthy subjects was 42.5ms, with a range of 38.8-46.5ms. Adopting 3-SLT points of 1, 20, and 50ms for T1rho measurement could provide reliable measurement and reduce the scanning time, while 2-SLT points of 1 and 50ms do not provide reliable measurement. / Previous issue date: 2006-02-13 / Adjacent segment disease is defined as an abnormal process developing in the adjacent level above and/or bellow the arthrodesis of the segment. It is considered a late complication of lumbar spine arthrodesis. It has been very important due to the procedures in the last years. Objective: To evaluate the risk factors associated with the disease at adjacent level as well as its relationship with the natural history of degenerative disease of the lumbar spine. Material and Methods: This was a retrospective study, from January 2000 to December 2002.Thirty-eight patients undergoing arthrodesis in the lumbosacral spine using the pedicle screw fixation participated in the study. Results: These patients had a mean follow-up of 30 months; 10 patients presented adjacent segment disease, 7 disc degeneration, 2 spinal stenosis and one discal spine hernia. The patients´ mean age was 48.5 years; female sex was the majority. Degeneration occurred in 4 patients with stenosis; 4 with spondylolisthesis, and 1 with post disc hernia; all these had been submitted to arthrodesis of lumbosacral spine. The majority was asymptomatic, after being treated by arthrodesis in multiple levels. There was no statistical difference when the above factors were related. Conclusion: Adjacent segment disease is a late complication in the arthrodesis of lumbosacral spine with no relationship of risk factors presented in this study. Therefore, its origin could be related with the natural history of the degenerative disease of lumbar spine. / A doença do nível adjacente é definida como um processo anormal que se desenvolve no nível adjacente, acima e/ou abaixo do segmento artrodesado. Considerada como uma complicação tardia da artrodese da coluna vertebral, tem-se tornado muito importante em decorrência do aumento dos procedimentos nos últimos anos. Objetivo: Avaliar os fatores de risco associados à ocorrência da doença do nível adjacente e sua relação com a história natural da doença degenerativa da coluna vertebral. Material e métodos: Análise retrospectiva de janeiro de 2000 a dezembro de 2002, realizada em 38 pacientes submetidos a artrodese de coluna lombosacra com a utilização de parafuso pedicular. Resultado: Os pacientes analisados apresentavam follow up médio de 30 meses, com a presença de 10 pacientes com doença do nível adjacente; sendo 7 com degeneração discal, 2 com estenose de canal vertebral e 1 com hérnia de disco. A idade média dos pacientes foi 48,5 anos, com predomínio no sexo feminino. A degeneração ocorreu em 4 pacientes com estenose de canal, em 4 pacientes com espondilolistese, em 1 paciente com escoliose e em 1 paciente pós hérnia de disco, os quais tinham sido submetidos a artrodese da coluna lombosacra. A maioria dos pacientes foram submetidos a artrodese em múltiplos níveis e apresentavam-se assintomáticos. Nenhuma diferença estatística foi evidenciada quando relacionados os fatores acima. Conclusão: A doença do nível adjacente é uma complicação tardia existente na artrodese de coluna lombosacra, não relacionada aos fatores de risco apresentados; portanto o seu aparecimento estaria relacionado à história natural da doença degenerativa da coluna lombar.

A dor na coluna vertebral é a principal causa de incapacidade no mundo, com altas taxas de prevalência global. A partir de estudos sobre a fisiologia da dor e suas relações com estados psicológicos, tornou-se essencial a avaliação psicológica dos indivíduos com quadros dolorosos, para selecionar os perfis mais favoráveis às diferentes formas de tratamento. O questionário DRAM (Distress Risk Assessment Method) foi desenvolvido como instrumento de triagem para portadores de dor na coluna vertebral subclassificando os indivíduos em quatro grupos distintos (normal, sob risco, somático e depressivo), conforme a pontuação dos dois questionários que compõem o DRAM (MSPQ e Zung). O objetivo desse estudo é traduzir e adaptar o DRAM para o português brasileiro da versão original em inglês, além de analisar a confiabilidade da versão traduzida e adaptada. Segundo a metodologia IQOLA, consagrada em inúmeras publicações, foi desenvolvida uma versão em português brasileiro que foi aplicada a uma amostra inicial de 30 pacientes e a seguir à amostra final de 85 indivíduos dos três centros participantes portadores de dor lombar. Os resultados comprovaram a confiabilidade e reprodutibilidade da versão traduzida e adaptada do questionário DRAM com índice de Cronbach alfa de 0,815 para o MSPQ e 0,794 para o Zung e coeficiente de correlação intraclasse de 0,688 para o MSPQ e 0,659 para o Zung. The objective of this study is to translate and culturally adapt DRAM to Brazilian Portuguese language, and determine the final version reliability. As proposed by IQOLA method, a Brazilian Portuguese version of DRAM has been applied to an initial sample of 30 patients and finally to a 85 individuals from three participant centers.
