Ošetřovatelská péče o dítě s onemocněním srdce na jednotce intenzivní péče / Nursing care for a child with heart disease at the intensive care unit

Children heart diseases can be divided into congenital and acquired heart defects. The acquired heart defects can be further divided into acquired heart defects, heart rhythm disorders, including hypertension, inflammatory heart diseases and heart failure. Children heart diseases can occur at any age, even if it is a congenital heart defect that is less severe. The diploma thesis should bring nurses closer to the problems of nursing care of children with heart diseases hospitalized at the ICU. The goal of this thesis was to find out the specifics of nursing care of children with heart diseases who are hospitalized at the ICU and to find out the most common nursing diagnoses according to NANDA II taxonomy, and their treatment in case of children with heart diseases who are hospitalized at the ICU. During the realization of the research part, a qualitative research survey was used using semi-structured interviews with the nurses involved in the observation. And a summarization that provides a more comprehensive view of the composition of children with heart diseases hospitalized at the ICU. The interviews were conducted with the nurses at the children's ICU, where the participant observation took place as well. In the research section the most common nursing interventions in case of these children were described and how much they differ from interventions in case of children with other illnesses that are also hospitalized at the ICU. Further, we have also found out the most common nursing diagnoses of children with heart diseases hospitalized at the ICU.

Background: In recent years mechanical circulatory assist devices became an established option in bridging patients with refractory heart failure to heart transplantation. One of the alleged limitations of mechanical devices is a high degree of antibody production with possible deleterious effect on subsequent heart transplantation outcome. Aim: The main goal of this study is to assess the role of antibodies on the outcome of surgical treatment of patients with end- stage heart failure. Method: Firstly, we present a literature review on the current state of knowledge of possible immunologic mechanisms involved in antibody production in left ventricular assist device (LVAD) recipients, new methods of antibody detection, desensitization strategies and overview of published evidence assessing the impact of sensitization on post-transplantation outcome. In the experimental part of our study we prospectively evaluated the presence of anti-Angiotensin II Type 1 Receptor (AT1R) antibodies in 83 Heart Mate II (HMII) recipients who were implanted at our institution between 2008 and 2012 and survived the first 60 days. The aim of this thesis was to clarify the influence of the stress on the adrenergic and muscarinic receptors in the heart and in the lungs. Research was perform on rat hearts and lungs and on the hearts and lungs of the CRH KO mice. First, we assessed mRNA levels of all α- and β-adrenergic receptor and muscarinic receptor subtypes. Subsequently, we performed the radioligand-binding studies to determine densities of these receptors. We identified all three α1-adrenergic receptor subtypes in the rat lungs. In the lungs of WT mice, we found that the amount of α1-adrenergic and muscarinic receptors was sex-dependent. Densities of the former were higher in females and those of the latter were higher in males. There was no difference between males and females in β-adrenergic receptor density. As for CRH KO mice, the basal densities of studied receptors were lower than in CRH WT mice (except β1-adrenergic receptors in females). The main purpose of the thesis was to detect the immobilization-induced changes in the studied receptors in the kontrol (WT) and CRH KO mice. Short-term and long-term immobilization caused decrease in all α1-adrenergic receptor subtypes in females, whereas only α1A-adrenergic receptors decreased in males. In the past years, the cardiac surgery performance was very demanding. The limited range of medical performances, imperfect procedures and techniques resulted in frequent postoperative complications and high postoperative mortality. Over time, surgical techniques, heart replacements, extracorporeal circulation and postoperative care have improved. This makes it possible today to operate on much older patients, where surgeries previously were nor feasible. The diploma thesis on the topic Cardiac surgical patient in the RES room and the approach of a nurse to nursing care is dedicated to the patient after heart surgery. It deals with meeting the patient's needs in the RES room and what the patient perceives while staying in the resuscitation unit and what he /she needs from a nurse. In the empirical part of this thesis two objectives were stated. To determine what a cardiac surgical patient perceives and what he /she needs in the RES room, and to find out whether it is possible to improve the quality of patient-oriented care. Based on these objectives, hypotheses and research questions were established. The objectives were successfully achieved, the hypothesis was confirmed and the questions answered. The research was conducted using qualitative quantitative research methods in the RES units of the cardiac surgery department, where the patients with heart disease are operated on. For the quantitative research the questionnaire interview method was selected. The qualitative research method was used to carry out semi-conducted interviews with the patients.

This diploma thesis is devoted to segmentation of ECG signal based on its quality and compression of quality segments suitable for diagnostics (in telemedicine). A completely new approach is to use accelerometer data to estimate ECG signal quality. This is possible thanks to the Bittium Faros mobile recorder. It records both the ECG signal motion – accelerometric data. A total of 34 features were extracted from accelerometric data. Using these features the predictive model was taught to classify the ECG signal into 3 quality groups according to the level of noise. Quality segments were compressed. The wavelet transform in combination with high-frequency bands zeroing and length encoding was used as a compression method.

Analýza srdce myšího embrya prostřednictvím rentgenové počítačové tomografie / X-ray computed tomography analysis of mouse embryonic heart

Rentgenová počítačová tomografie je univerzální technika umožňující nedestruktivní trojrozměrné (3D) zobrazování. Je široce používána v průmyslu pro metrologii a charakterizaci materiálů. V poslední době našla svůj význam také v biologii a vývojové vědě, kde 3D rekonstruovaná data poskytují komplexnější informace o vzorku než konvenční 2D zobrazovací metody. Kromě toho může vytvářet vysoce kvalitní snímky různých biologických vzorků. Překážkou při zobrazování biologických vzorků může být velmi podobný koeficient útlumu měkkých tkání, a proto je nutné použít barvení chemickou látkou. Tato práce si klade za cíl pomocí mikropočítačové tomografie vizualizovat a kvalitativně vyhodnotit srdce embryonálních myší E17.5 a představit nejlepší barvící protokol. Dále byla zavedena metodika pro zvýraznění rozdílů mezi myší divokého typu a mutantem. Rigaku nano 3DX byl použit pro experimenty s mezi-komorovou přepážkou myšího srdce a po nastavení specifických parametrů zařízení byl použit algoritmus pro zvýraznění fáze. Nakonec byla provedena validace $\mu$ CT dat se snímkami z konfokálního mikroskopu.

Cardiac energy metabolism is the one of the most complex system in the body. To sustain life, but also to respond quickly to any sudden changes (e.g. running, emotional stress), the heart has developed a unique ability and has become a metabolic "omnivore". At physiological conditions, long chain fatty acids (LCFAs) present the major energetic source for the adult myocardium. However, the cardiac energy metabolism may be compromised during pathophysiological states. One of the most dangerous is, undoubtedly, ischaemia-reperfusion injury with its acute form, myocardial infarction. However, the adaptation to chronic hypoxia has been known for decades for its cardioprotective effect against I/R. Changes of cardiac energy metabolism induced by the adaptation have not been fully explored and the system conceals still too many secrets. This thesis has aimed to determine how adaptation to chronic hypoxia affects the cardiac metabolism of the rat LVs in the following set-ups: 1. This work focuses on the regression analysis of morphological parameters calculated from the ECG for estimating the position of the heart to the measuring electrodes. It consists of a theoretical analysis of the problems of ECG recording and description of the data obtained from experiments on isolated animal hearts. On the theoretical part is followed by a description of the calculation parameters suitable for regression analysis and their application in the training and testing of the following regression models to estimate the position of the heart to the measuring electrode.
