The master’s thesis describes IMS (IP Multimedia Subsystem) in terms of IMS core ele- ments (functional description, different implementation, signaling etc.) Communication protocols SIP and DIAMETER, together with SNMP protocol, which is used for collecting data are briefly described in this thesis.Thesis is also describing various IMS projects to- gether with Open IMS project, for whom was this surveillance system designed. Next part deals with architecture design of surveillance system along with management options implemented in surveillance system for users and administrators. The main part of master’s thesis deals with the description of the surveillance system for the experimental school Open IMS network and describes the remote configuration of core elements and monitoring of network traffic, together with the monitoring servers performance. The most of the data in the designed surveillance system are processed into graphs, which are regularly updated. The final part of master’s thesis describes the configuration and implementation of monitoring tools MRTG and NfSen that were used in created web based surveillance system.

Police Officers' Perceptions of Body-Worn Camera Technology

In the past several years, police-community relations have received enormous scrutiny based on several high-profile incidents involving the use of deadly force. Politicians, civil societies, and victims' families have called for law enforcement agencies to equip local officers with body-worn cameras to increase transparency and accountability. The purpose of the study was to investigate how law enforcement officers in a Sheriff's office in the Southern United States perceived ease of use and usefulness of body-worn camera technology and to identify if gender and years of service related to police officers' acceptance of body-worn cameras as a component of their regular uniform. The theoretical foundation for this study was based on the Technology Acceptance Model (TAM) developed by Davis in 1989. Paper survey using TAM instrument was used to collect data from officers at the training center. A hierarchical multiple regression analysis was conducted to determine which independent variables predicted the frequency of use of body-worn cameras. Analysis of data collected from 88 officers found that their perceptions of the ease of use of body-worn cameras were moderately and positively correlated with their perceptions of the cameras' usefulness and their attitudes toward the camera. The relationship between usefulness and years of service was negative, indicating that as officers' length of service increased, their perceptions of body-worn cameras usefulness decreased. However, officers' attitudes toward using body-worn cameras were a predictor of their reported frequency of use. Findings from the study could contribute to positive social change by providing policymakers with new tools to craft training policies to enhance police-community relations.

Trends and Determinants of Up-to-date Status with Colorectal Cancer Screening in Tennessee, 2002-2008

BACKGROUND: Screening rates for colorectal cancer (CRC) are increasing nationwide including Tennessee (TN); however, their up-to-date status is unknown. The objective of this study is to determine the trends and characteristics of TN adults who are up-to-date status with CRC screening during 2002-2008. METHODS: We examined data from the TN Behavioral Risk Factor Surveillance System for 2002, 2004, 2006 and 2008 to estimate the proportion of respondents aged 50 years and above who were up-to-date status with CRC screening, defined as an annual home fecal occult blood test and/or sigmoidoscopy or colonoscopy in the past 5 years. We identified trends in up-to-status in all eligible respondents. Using multivariable logistic regression models, we delineated key characteristics of respondents who were up-to-date status. RESULTS: During 2002-2008, the proportion of respondents with up-to-date status for CRC screening increased from 49% in 2002- 55% in 2006 and then decreased to 46% in 2008. The screening rates were higher among adults aged 65-74 years, those with some college education, those with annual household income ≥$35,000 and those with health-care access. In 2008, the respondents who were not up-to-date status with CRC screening included those with no health-care coverage (adjusted odds ratio [OR] 0.46, 95% confidence interval [CI] 0.33-0.63), those aged 50-54 years (OR 0.62, 95% CI 0.46-0.82) and those with annual household income CONCLUSIONS: TN adults who are up-to-date status with CRC screening are increasing, but not across all socio-demographic subgroups. The results identified specific subgroups to be targeted by screening programs, along with continued efforts to educate public and providers about the importance of CRC screening.

Association Between Heavy Alcohol Consumption and Coronary Heart Disease Among U.S. Adults: Using the 2015 BRFSS Annual Survey Data

Background: Significant evidence exists about J-shaped relationship between alcohol consumption and total or cardiovascular disease (CVD)-specific mortality in US middleaged adults. Epidemiologic investigations presume that the J-shaped distribution is the sum of the detrimental effect of high levels of consumption on other causes of death and the protective effect on coronary heart disease (CHD) morbidity and mortality. Several studies demonstrated that moderate alcohol consumption reduces the risk of CHD. However, results have been inconsistent among heavy drinkers. In this study, we investigated the association of heavy alcohol consumption with CHD among adults aged 18-years or older in the US. Methods: Data from the 2015 Behavioral Risk Factor Surveillance System (BRFSS) were used to conduct this study. BRFSS is an annual cross-sectional survey administered to >400,000 adults in all 50 states to collect information about their health-related risk behaviors, chronic health conditions and the use of preventive services. Self-reported responses to BRFSS questionnaire were used to define study variables. Heavy alcohol consumption was defined as greater than 14 drinks (1 drink =12 ounces of beer) per week for men and 7 drinks per week for women. Logistic regression analysis was conducted to determine the association between history of coronary heart disease or angina and heavy alcohol consumption. The model was also adjusted for demographics (age, sex, and race), behaviors (exposure to tobacco smoking, physical activity, fruit consumption), other chronic conditions such as high blood pressure (ever been told having high blood pressure or not), high cholesterol (ever been told having high blood cholesterol or not) and overweight or obese. Results: Approximately 6% of study population reported history of CHD and 5% reported heavy alcohol consumption. The odds of having coronary heart disease or angina among heavy alcohol drinkers was 43% less than those who were not heavy alcohol drinkers (adjusted odds ratio: 0.57, 95% confidence interval: 0.52-0.62, pConclusion: The study findings demonstrate that heavy alcohol consumption is a protective factor for CHD morbidity. Future observational studies should be conducted to determine the overall benefits of heavy alcohol consumption as it relates to coronary heart diseases.

Dying to make a fresh start : mortality and health transition in a new South Africa

Rationale: Vital registration is lacking in developing settings where health and development problems are most pressing. Policy-makers confront an “information paradox”: the critical need for information on which to base priorities and monitor progress, and the profound shortage of such information. Aims: To better understand the dynamics of mortality transition in rural South Africa over a decade of profound socio-political change coupled with emerging HIV/AIDS. Thereby to inform health and development programming, policy formulation, and the research agenda; and contribute to debate on the nature of the ‘health transition’. Methods: The Agincourt health and demographic surveillance system is based on continuous monitoring of the Agincourt sub-district population in rural north-east South Africa. This involves annual recording of all vital events, specifically deaths, births and migrations in 11,700 households comprising some 70,000 persons. A “verbal autopsy” is conducted on every death, and special modules provide additional data. Key findings: A major health transition has occurred over the past decade, with marked changes in population structure and rapidly escalating mortality particularly among children and younger adults. A quadruple burden of disease is evident with persisting infectious disease and malnutrition in children, emerging non-communicable disease in the middle-aged and older, high levels of violence in an apparently peaceful community, and rapidly escalating HIV/AIDS and tuberculosis. There is evidence of sex differences and socio-economic differentials in mortality; vulnerable sub-groups include the children of Mozambican immigrants and recently returned labour migrants. Implications: With respect to health transition, empirical data demonstrate a marked “counter transition” with mortality increasing in children and young adults; “epidemiologic polarization” is evident with the most vulnerable experiencing a higher mortality burden; and a “protracted transition” is reflected in the co-existence of persisting infectious disease and malnutrition, emerging HIV/AIDS, and increasing chronic non-communicable disease. With respect to health policy and practice there is urgent need to: strengthen HIV/AIDS prevention, treatment and care; offer effective long-term care to control the rising burden of chronic illness and related risk; maintain and improve maternal and child health services; and address differential access to care. This poses a substantial challenge to a severely stretched health system.

Patient age, number and type of clinical encounters, and provider advice to quit smoking. BRFSS 2000

The purpose of this study was to determine how often smoking patients receive quit advice and if patient age, and number and type of clinical encounters are associated with odds of receipt. Behavioral Risk Factor Surveillance System (BRFSS) 2000 data were used to study 10,582 smokers (aged ³ 18) having ³ 1 of three types of clinical encounters in the past year: routine checkups, other physician encounters, or dental visits. Multivariate-adjusted odds ratios (ORs) for quit advice by patient age, encounter type, and number of doctor's visits were calculated. Almost 55% of patients were advised to quit smoking. There was a 4-23% chance of receiving quit advice at any given doctor's visit. Odds of receiving advice did not increase with increasing number of visits. With advancing age, men were more likely, women less likely, to receive quit advicebut only significantly for White men. Compared to those having dental visits, ORs for receiving quit advice for patients having checkups and other physician encounters were 3.35 (95% CI 2.ll, 5.31) and 3.03 (95%CI 1.32, 6.97) respectively. These cross-sectional data suggest that whereas a small majority of smoking patients are advised to quit at some clinical encounter, smoking patients are not advised to quit at the majority of encounters. Being young and male, or seeing dentists rather than doctors made patients less likely to receive quit adviceas did having lower education or BMI, no insurance or coverage other than military or private, not having asthma, or not having breast exams or follow-up Papanicolaou smears if female. Based on a previously-reported absolute quit difference of 1.9%, if smoking patients received quit advice just once at any of their encounters with physicians in a year, at least 800,000 more U.S. smokers would quit at an economic savings of $2.4 billion.

In this context, surveillance activities should be systematic, timely and simple to produce adequate data. According to the Center for Disease Prevention and Control surveillance systems present essential attributes to perform properly, being timeliness one of these attributes. This work aims to evaluate the timeliness of the surveillance systems for dengue, meningitis, exanthematic diseases and tuberculosis based on the data available from two versions of the National Notifiable Diseases Information System (Sinan) between 2005 and 2008. The analysis of notification timeliness for tuberculosis cases has shown that more than 50% of the cases were notified on the same day of the diagnostic, reaching 90% after 40 dias. For the other diseases evaluated, the median time for notification was up to 3 days, with 90% of the cases being reported within 13 to 17 days. There was no difference in the timeliness of notification according to sex. The analysis of investigation timeliness highlighted that 90% of the cases initiated this process on the same day of the notification. The analysis of the interval for data entry in Sinan presented a significant loss of timeliness with a median of 10 to 14 days after the notification for the acute diseases and 25 to 31 days for tuberculosis. The analysis of treatment timeliness for tuberculosis showed that 70% of the cases started their treatment at the day of diagnostic. The timeliness of case closing was adequate for all evaluated diseases, except for the dengue and the exanthematic cases in 2007 and 2008. The national surveillance system presented adequate timeliness of notification and investigation for all the diseases evaluated in this study, regardless of the version of Sinan. However, it is necessary to improve the timeliness of data entry, treatment and completion of the investigation of the cases. / A vigilância epidemiológica tem entre seus objetivos o fornecimento de informações para a execução de ações para controle de doenças e agravos. Nesse contexto, é fundamental que o sistema de vigilância seja sistemático, oportuno, simples e corretamente alimentado para produzir dados com maior qualidade. De acordo com o Centro de Prevenção e Controle de Doenças, os sistemas de Vigilância possuem atributos essenciais para o funcionamento adequado sendo um destes a oportunidade. O objetivo deste trabalho foi avaliar a oportunidade do sistema de vigilância epidemiológica da dengue, meningites, doenças exantemáticas e tuberculose a partir dos dados registrados em duas versões do Sistema de Informação de Agravos de Notificação (Sinan) entre 2005 e 2008. A análise da oportunidade de notificação para os casos de tuberculose observou que mais de 50% dos casos notificados no mesmo dia do diagnóstico, atingindo 90% após 40 dias. Para os demais agravos, a mediana de tempo para a notificação foi de até 03 dias, com 90% dos casos sendo notificados entre 13-17 dias. A oportunidade da notificação foi semelhante de acordo com o sexo. Na análise da oportunidade de investigação observamos uma excelente oportunidade do sistema, com 90% dos casos apresentando o início desse processo no mesmo dia da notificação. Para o intervalo de digitação, ocorreu uma perda de oportunidade significante com mediana dos casos entre 10-14 dias para eventos agudos e 25 e 31 dias para tuberculose. Na análise da oportunidade de tratamento cerca de 70% dos casos de tuberculose iniciaram o tratamento no mesmo dia do diagnóstico. Na avaliação da oportunidade de encerramento da investigação, apenas os casos de dengue e doenças exantemáticas não apresentaram encerramento oportuno nos anos de 2007 e 2008. O sistema de vigilância nacional apresentou oportunidade adequada para a notificação e investigação de todos os agravos do estudo, independente da versão do Sinan. Entretanto, são necessárias melhorias na oportunidade de digitação, tratamento e encerramento dos casos.

An ontological approach for monitoring and surveillance systems in unregulated markets

Ontologies are a key factor of Information management as they provide a common representation to any domain. Historically, finance domain has suffered from a lack of efficiency in managing vast amounts of financial data, a lack of communication and knowledge sharing between analysts. Particularly, with the growth of fraud in financial markets, cases are challenging, complex, and involve a huge volume of information. Gathering facts and evidence is often complex. Thus, the impetus for building a financial fraud ontology arises from the continuous improvement and development of financial market surveillance systems with high analytical capabilities to capture frauds which is essential to guarantee and preserve an efficient market.This thesis proposes an ontology-based approach for financial market surveillance systems. The proposed ontology acts as a semantic representation of mining concepts from unstructured resources and other internet sources (corpus). The ontology contains a comprehensive concept system that can act as a semantically rich knowledge base for a market monitoring system. This could help fraud analysts to understand financial fraud practices, assist open investigation by managing relevant facts gathered for case investigations, providing early detection techniques of fraudulent activities, developing prevention practices, and sharing manipulation patterns from prosecuted cases with investigators and relevant users. The usefulness of the ontology will be evaluated through three case studies, which not only help to explain how manipulation in markets works, but will also demonstrate how the ontology can be used as a framework for the extraction process and capturing information related to financial fraud, to improve the performance of surveillance systems in fraud monitoring. Given that most manipulation cases occur in the unregulated markets, this thesis uses a sample of fraud cases from the unregulated markets. On the empirical side, the thesis presents examples of novel applications of text-mining tools and data-processing components, developing off-line surveillance systems that are fully working prototypes which could train the ontology in the most recent manipulation techniques.
