Sistemas de informação e avaliação de desempenho hospitalar: a integração e interoperabilidade entre fontes de dados hospitalares / Systems of information and evaluation of hospital performance : the integration and interoperability among of hospital data source

Com o avanço tecnológico, a valorização da informação, o ritmo acelerado das mudanças e a globalização, características que fazem com que os hospitais (sejam eles, filantrópicos, governamentais ou privados), aliada às exigências cada vez maiores dos pacientes, aumentem a busca pela qualidade na prestação dos serviços. Sobre este pretexto, os hospitais integrantes do Sistema Único de Saúde (SUS), devem elaborar seus respectivos Censos Hospitalares e apresentar seus dados estatísticos ao Ministério da Saúde, baseado nas definições da Portaria no. 312 de 02 de maio de 2002. Estes indicadores não são os únicos necessários ou importantes para a gestão hospitalar, entretanto são considerados como informaçãoes básicas em uma instituição desta natureza. Contudo, a disponibilidade destas informações de forma contínua, sustentável e confiável não tem se mostrado uma tarefa trivial, principalmente devido alguns fatores: falta de registro, registro inadequado e incompleto, falta de padronização na aplicação do vocabulário médico e mudanças constantes nas rotinas administrativas. Em hospitais considerados de grande porte, este problema pode tomar uma extensão ainda maior. Por ser bastante complexo o Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo HCFMUSP, considerado o maior complexo hospitalar da América Latina, com aproximadamente 2.200 leitos e atendendo em média 6.000 pacientes ambulatoriais por dia, constitui um bom objeto para estudo de caso, pois permite que vários desafios em relação ao tratamento da informação, como por exemplo: compartilhamento, conectividade, interoperabilidade e integração, possam surgir de maneira mais acentuada a outros hospitais de menor complexidade. Segundo esta abordagem, o trabalho também pretende explorar o nível de contribuição dos diversos Sistemas de Informação Hospitalar utilizados pelo hospital na obtenção de variáveis, para a composição de informações que serão utilizadas como base para as ações administrativas e avaliação de desempenho. O alto grau de heterogeneidade presente nas soluções no domínio da saúde, distribuídos nos diferentes sistemas de informação, apontam para a v necessidade de compartilhar e troca informações entre ambientes heterogêneos. Neste contexto, a interoperabilidade tem um papel fundamental, pois permite a comunicação de forma transparente entre sistemas e ambientes heterogêneos, autônomos e distribuídos. Foram utilizados neste estudo documentos oficiais relativos aos sistemas de informação assistencial e sua gestão no HC-FMUSP, além de atas de Reuniões do Comitê de Tecnologia da Informação do hospital (CTI). Para a coleta de dados foi aplicada a técnica de entrevista semi-estruturada aos sujeitos responsáveis pelo Serviço de Arquivo Médico SAME de áreas estratégicas do hospital e do Núcleo de Informação em Saúde NIS. A crescente complexidade da assistência à saúde torna premente a necessidade de integração dos sistemas corporativos, bem como a adoção de padrões de registro e procedimentos, porém, não basta resolver as questões somente do ponto de vista tecnológico, o desafio é trabalhar estes problemas considerando toda a sua complexidade e articulando diferentes áreas, em busca de resultados efetivos / With the technological advance, the valuation of the information, the sped up rhythm of the changes and the globalization, characteristics that make with that the hospitals (they are they, philanthropy, governmental or private), allied to the bigger requirements each time of the patients, increase the search for the quality in the installment of the services. On this excuse, the integrant hospitals of the Only System of Health (SUS), must elaborate its respective Hospital Censuses and present its statistical data to the Health department, based on the definitions of governmental decree no. 312 of 02 of May of 2002. These pointers are not only the necessary ones or important for the hospital management, however they are considered as basic information in an institution of this nature. However, the availability of these information of continuous, sustainable and trustworthy form if has not shown a trivial task, mainly had some factors: lack of register, inadequate and incomplete register, constant lack of standardization in the application of the medical vocabulary and changes in the administrative routines. In considered hospitals of great transport, this problem can take an extension still bigger. Of to be sufficiently complex the Hospital of the Clinics of the College of Medicine of the University of Sao Paulo - HCFMUSP, considered the hospital complex greater of Latin America, with approximately 2,200 stream beds and taking care of in average 6,000 patients per day in the ambulatory, constitutes a good object for case study, therefore it allows that some challenges in relation to the treatment of the information, as for example: sharing, connectivity, interoperability and integration, can appear more of accented way to other hospitals of lesser complexity. According to this boarding, the work also intends to explore the level of contribution of the diverse systems of hospital information used by the hospital in the attainment of the variables, for the composition of information that will be used as base for the administrative cases and evaluation of performance. The high degree of present heterogeneity in the solutions in the domain of health distributed in the different systems of information, points with respect to the necessity to share and changes information between heterogeneous environments. In this vii context, the interoperability has a basic paper, therefore it allows to the communication of transparent form between systems and heterogeneous, independent and distributed environments. Official documents to the systems of health care information and its management in the HCFMUSP had been used in this study relative, beyond acts of meetings of the Committee of Technology of the Information of the hospital (CTI). For the collection of data the technique of interview half-structuralized to the responsible citizens for the Service of Medical Archive was applied - SAME of strategical areas of the hospital and the Nucleus of Information in Health - NIS. The increasing complexity of the health care becomes pressing the necessity of integration of the corporative systems, as well as the adoption of register standards and procedures, however, are not enough to only decide the questions of the technological point of view, the challenge are to work these problems considering all its complexity and articulating different areas, in search of effective results

Uma infraestrutura orientada a serviços para a modelagem de nicho ecológico. / A service-oriented infrastructure for ecological niche modelling.

A modelagem de nicho ecológico combina dados ambientais com informações sobre a ocorrência de uma espécie para obter modelos que descrevem a sua distribuição probabilística em uma região. Algoritmos computacionais geram modelos baseados na hipótese de que, se uma espécie foi encontrada sob as condições que definem o seu nicho, então ela pode sobreviver em qualquer lugar que ofereça as mesmas condições. A técnica de modelagem normalmente utiliza dados obtidos de diferentes provedores, em grandes quantidades e em formatos diferentes, e os resultados são exportados para outras ferramentas de análise e interpretação, como os SIGs, Sistemas de Informação Geográfica. Os sistemas de informação disponíveis para modelagem de nicho ecológico são monolíticos e, embora efetivamente implementem algoritmos para gerar modelos, não foram projetados para atender todos os requisitos da modelagem. Este trabalho define uma solução mais abrangente, na forma de uma infraestrutura orientada a serviços para a modelagem de nicho ecológico. A opção pela orientação a serviços está diretamente relacionada com a integração de sistemas. A infraestrutura especifica um barramento de serviços para gerenciá-los, com funcionalidades como roteamento e garantia de entrega, e para tratar questões de interoperabilidade. Antes de construir a infraestrutura, foi formalizado o processo de modelagem de nicho ecológico. Ele permitiu definir o domínio do problema, identificar falhas e potenciais evoluções, e acelerar a curva de aprendizado de pesquisadores iniciantes. Definida a infraestrutura, foram abordados outros aspectos da modelagem, resultando em soluções integradas para alguns dos problemas identificados. Em algoritmos para geração de modelos, foi apresentada uma solução usando tecnologia adaptativa, com resultados promissores. Em computação de alto desempenho, o Garp, um dos algoritmos mais utilizados para modelagem, foi paralelizado. Além de resultados positivos em relação ao desempenho, esta iniciativa mostrou como se evoluir neste sentido. Para integração com outros provedores e disponibilização de recursos oferecidos por outras ferramentas de software, foi proposta a integração com serviços de SIG, utilizando padrões estabelecidos para serviços geoespaciais. Em conjunto, estas soluções mostram a capacidade da infraestrutura de atender os requisitos já identificados e evoluir para incorporar novas características que a evolução natural das técnicas de modelagem venham a desenvolver. Incorporando as soluções propostas à infraestrutura, foi desenvolvido um portal de modelagem para atender aos usuários desta técnica. A modelagem de nicho ecológico apresenta desafios em diferentes áreas do conhecimento e a abordagem adotada permitiu formalizá-los. Trabalhos em andamento incluem a avaliação de modelos, a evolução do barramento de serviços, o desenvolvimento de ontologias e a incorporação de sistemas legados, com potenciais benefícios para a modelagem de nicho ecológico. Como resultado adicional, o próprio desenvolvimento do trabalho mostrou que a solução é integrável com outros domínios, comprovadamente os de agricultura de precisão e polinizadores, mostrando a versatilidade e o potencial da infraestrutura de serviços apresentada. / Ecological niche modelling combines environmental data and information about the occurrence of a species to obtain models to describe its probabilistic distribution in a region. Computational algorithms generate models based on the hypothesis that, if a species was found under the conditions which define its niche, then it may survive in any place which offers the same conditions. The modelling technique usually applies data obtained from different providers, in large quantities and different formats, and the results are exported to other analysis and interpretation tools, such as GIS, Geographic Information Systems. Information systems available for ecological niche modelling are monolithic and, although they effectively implement algorithms for model generation, they were not designed to meet all modelling requirements. This work defines a more comprehensive solution, a service-oriented infrastructure for ecological niche modelling. The choice of a service-oriented solution is directly related to system integrations requirements. The infrastructure specifies a service bus to manage services with many features, such as routing and delivery, and to treat interoperability issues. Before building the infrastructure, the ecological niche modelling process was formalized. It allows defining the problem domain, identifying failures, gaps and potential evolutions, and increasing the learning curve of beginners. Having the infrastructure defined, other modelling aspects were addressed, resulting in integrated solutions for some of the identified problems of the technique. In algorithms for model generation, a solution using adaptive technology was introduced, with promising results. In high performance computing, the Garp, one of the algorithms most applied for modelling, became parallel. Besides positive results related to performance, this initiative showed how to evolve in this sense. Related to integration with other providers and availability of resources offered by other software tools, a proposal to integrate GIS services was presented, applying patterns for geospatial services. Together, these solutions show the ability of the infrastructure to meet all identified requirements and to evolve in order to incorporate new features that the natural evolution of modelling techniques may develop in the future. Incorporating the proposed solutions to the infrastructure, a portal of ecological niche modelling was developed to serve the users of this technique. Ecological niche modelling presents challenges in different areas of knowledge and the adopted approach allowed formalizing them. Research work is ongoing and includes model evaluation, bus services evolution, ontologies development and incorporation of legacy systems, with several potential benefits for ecological niche modelling. As an additional result, the development of this work indicates that the solution may be extended to other domains, such as precision agriculture and pollinators, showing the versatility and the potential of the presented services infrastructure.

Power supply noise analysis for 3D ICs using through-silicon-vias

3D design is being recognized widely as the next BIG thing in system integration. However, design and analysis tools for 3D are still in infancy stage. Power supply noise analysis is one of the critical aspects of a design. Hence, the area of noise analysis for 3D designs is a key area for future development. The following research presents a new parasitic RLC modeling technique for 3D chips containing TSVs as well as a novel optimization algorithm for power-ground network of a 3D chip with the aim of minimizing noise in the network. The following work also looks into an existing commercial IR drop analysis tool and presents a way to modify it with the aim of handling 3D designs containing TSVs.

Design and Testing of a Flight Control System for Unstable Subscale Aircraft

The primary objective of this thesis was to study, implement, and test low-cost electronic flight control systems (FCS) in remotely piloted subscale research aircraft with relaxed static longitudinal stability. Even though this implementation was carried out in small, simplified test-bed aircraft, it was designed with the aim of being installed later in more complex demonstrator aircraft such as the Generic Future Fighter concept demonstrator project. The recent boom of the unmanned aircraft market has led to the appearance of numerous electronic FCS designed for small-scale vehicles and even hobbyist-type model aircraft. Therefore, the purpose was not to develop a new FCS from scratch, but rather to take advantage of the available technology and to examine the performance of different commercial off-the-shelf (COTS) low-cost systems in statically unstable aircraft models. Two different systems were integrated, calibrated and tested: a simple, gyroscope-based, single-axis controller, and an advanced flight controller with a complete suite of sensors, including a specifically manufactured angle-of-attack transducer. A flight testing methodology and appropriate flight-test data analysis tools were also developed. The satisfactory results are discussed for different flight control laws, and the controller tuning procedure is described. On the other hand, the different test-bed aircraft were analysed from a theoretical point of view by using common aircraft-design methods and conventional preliminary-design tools. The theoretical models were integrated into a flight dynamics simulator, which was compared with flight-test data obtaining a reasonable qualitative correlation. Possible FCS modifications are discussed and some future implementations are proposed, such as the integration of the angle-of-attack in the control laws.

Towards an Integrated Infrastructure: Using Architecture to Celebrate a Canadian National Park Town

This thesis proposes an architecturally integrated stormwater system and research facility in the town of Wasagaming, Riding Mountain National Park (RMNP), Manitoba. The design proposal provides four-season, interior and exterior space for the integration of resource management operations and park visitor experiences. Visible integration of infrastructure, building and landscape cultivate destination-based travel to RMNP while reducing human impacts on the ecosystem. Aging infrastructure and diminishing federal funding make responsible ecological and cultural management of parks increasingly difficult. Integration of research and tourism as a component of visitor experience at parks is one way of addressing cost-effective co-location of programme, ensuring future funding can be generated for resource management.

Manufacturing industry has made extensive use of simulation as a means of trying to model the impact of variability on manufacturing system behavior and to explore various ways of coping with change and uncertainty. Simulation helps find optimal solutions to a number of problems at both design and application stages of Flexible Manufacturing Systems (FMS&rsquo / s) serving to improve the &ldquo / flexibility&rdquo / level The flexibility requirement of FMS necessitates the dissemination of every activity that concerns production, throughout all the levels of a company forcing almost every level of employee face with simulation software, either in terms of preparing models, modifying runs or evaluating results. This problem of inadequate capability of personnel to utilize simulation effectively can be overcome through the design of custom interfaces and integration of simulation software with everyday-use programs. This research mainly focuses on realizing the modeling and simulation of FMS&rsquo / s by the use of the existing system in Middle East Technical University, Mechanical Engineering Department, Computer Integrated Manufacturing Laboratory as a test-bed. Additionally, the means of integration of simulation with auxiliary programs is demonstrated. The models developed throughout the study using ARENA&reg / are used to come up with different scenarios of production. Sample results and decisions about production issues that can be attained through the use of simulation are provided. The described model creation procedures, the generated models, and result assessments are expected to act as a guideline for other simulations regarding FMS&rsquo / s.

No âmbito industrial, o custo empregado na manutenção de equipamentos ainda representa uma grande parcela dos investimentos. Dessa forma, o desenvolvimento de técnicas de manutenção, e o seu correto planejamento, cada vez mais estão assumindo papeis de grande importância nesse setor, visto que impactam diretamente no fator econômico das empresas. Neste sentido, o trabalho em questão apresenta a proposta de uma arquitetura orientada a serviços para um sistema de manutenção inteligente, a fim de possibilitar a integração de forma facilitada entre equipamentos e as ferramentas de análise de degradação. A arquitetura é composta por diferentes entidades, cada uma responsável por determinada tarefa para o funcionamento do sistema. No trabalho, as entidades são implementadas e experimentos são realizados a fim de validar a solução proposta. / In the industrial field, the costs associated with equipment’s maintenace still represents a large portion of the financial resources available to a company. Therefore, new researches in maintenance systems, and the correct task planning, are growing, since they impact directly on the economic side of the companies. Thus, current work presents a service-oriented architecture for maintenance systems integration. The proposed architecture intends to facilitate the integration of equipments and degradation analysis tools. The architecture is comprised of several entities, where each one is responsible for executing a given task in order to keep the system running properly. In this work, all the entities are implemented and experiments are performed in order to validate the proposed system.
